In order to evaluate the potential allergenicity of Blomia tropicalis (Bt) antigen, IgE production of both specific and non-specific for Bt antigen was monitored in BALB/c mice after exposure to the antigen by nasal route. It was evidenced that B. tropicalis contains a functional allergen in its components. The allergenic components, however, when administered intranasally without any adjuvant, did not function to induce IgE response within a short period. On the other hand, intranasal inoculation of Bt antigens augmented serum IgE responses in mice pretreated by a subcutaneous priming injection of the same antigens. Inoculation of Bt antigen without subcutaneous priming injections induced IgE antibody production only when the antigen was continuously administered for a long period of over 24 weeks. Even when the priming injection was absent, the Bt antigen inoculated with cholera toxin (CT) as a mucosal adjuvant also significantly augmented the Bt antigen-specific IgE responses depending on the dose of CT co-administered. The present study also demonstrated that Bt antigen/CT-inoculated mice showed increased non-specific serum IgE level and peripheral blood eosinophil rates without noticeable elevations of the total leukocyte counts. The immunoblot analysis demonstrated 5 main antigenic components reactive to IgE antibodies induced. These components at about 44-64 kDa position were considered to be an important candidate antigen for diagnosis of the mite-related allergy.

Bartonellosis or Carrion's disease is endemic in some regions of Peru, classically found in the inter-Andean valleys located between 500 and 3200 meters above sea level. We report the case of a 43 year-old male patient, farmer, who was born in the Pichanaki district (Chanchamayo, Junin), located in the High Forest of Peru. He presented with disseminated, raised, erythematous cutaneous lesions, some of which bled. The distribution of these lesions included the nasal mucosa and penile region. Additionally subcutaneous nodules were distributed over the trunk and extremities. Hematologic exams showed a moderate anemia. Serologic studies for HIV and Treponema pallidum were negative. The histopathologic results of two biopsies were compatible with Peruvian wart. Oral treatment with ciprofloxacin (500 mg BID) was begun. Over 10 days, the patient showed clinical improvement. This is the first report of a confirmed case of bartonellosis in the eruptive phase originating from the Peruvian High Forest, showing the geographical expansion of the Carrion's disease.

Endophyte-assisted phytoremediation has recently been suggested as a successful approach for ecological restoration of metal contaminated soils, however little information is available on the influence of endophytic bacteria on the phytoextraction capacity of metal hyperaccumulating plants in multi-metal polluted soils. The aims of our study were to isolate and characterize metal-resistant and 1-aminocyclopropane-1-carboxylate (ACC) utilizing endophytic bacteria from tissues of the newly discovered Zn/Cd hyperaccumulator Sedum plumbizincicola and to examine if these endophytic bacterial strains could improve the efficiency of phytoextraction of multi-metal contaminated soils. Among a collection of 42 metal resistant bacterial strains isolated from the tissues of S. plumbizincicola grown on Pb/Zn mine tailings, five plant growth promoting endophytic bacterial strains (PGPE) were selected due to their ability to promote plant growth and to utilize ACC as the sole nitrogen source. The five isolates were identified as Bacillus pumilus E2S2, Bacillus sp. E1S2, Bacillus sp. E4S1, Achromobacter sp. E4L5 and Stenotrophomonas sp. E1L and subsequent testing revealed that they all exhibited traits associated with plant growth promotion, such as production of indole-3-acetic acid and siderophores and solubilization of phosphorus. These five strains showed high resistance to heavy metals (Cd, Zn and Pb) and various antibiotics. Further, inoculation of these ACC utilizing strains significantly increased the concentrations of water extractable Cd and Zn in soil. Moreover, a pot experiment was conducted to elucidate the effects of inoculating metal-resistant ACC utilizing strains on the growth of S. plumbizincicola and its uptake of Cd, Zn and Pb in multi-metal contaminated soils. Out of the five strains, B. pumilus E2S2 significantly increased root (146%) and shoot (17%) length, fresh (37%) and dry biomass (32%) of S. plumbizincicola as well as plant Cd uptake (43%), whereas Bacillus sp. E1S2 significantly enhanced the accumulation of Zn (18%) in plants compared with non-inoculated controls. The inoculated strains also showed high levels of colonization in rhizosphere and plant tissues. Results demonstrate the potential to improve phytoextraction of soils contaminated with multiple heavy metals by inoculating metal hyperaccumulating plants with their own selected functional endophytic bacterial strains.

Envolvido na necessidade emergente de associar a Saúde à Educação e da importância e proeminência que a literatura tem manifestado sobre a respiração e as suas implicações na aprendizagem, este estudo pretende explorar e identificar as principais características anátomo-fisiológicas de alunos com RN e RO, apresentar as alterações do comportamento em alunos com RN e alunos com RO, expôr as modificações das funções executivas em alunos com RN e alunos com RO e a relação entre as funções executivas e o comportamento em alunos com RN e alunos com RO. Foram avaliados 169 alunos do 2º ciclo de escolaridade da Escola Eb2/3 de São João da Madeira na respiração, no comportamento e funções executivas – com recurso à adaptação da Avaliação Miofuncional Orofacial – MBGR, do Questionário de Autoavaliação para Jovens (Youth Self Report) e do Inventário de Classificação Comportamental de Funções Executivas – ICCFE-C/A (versão para crianças/adolescentes). Usou-se a avaliação miofuncional orofacial para se obter a caraterização do modo respiratório e das alterações miofuncionais na amostra; e o questionário de autoavaliação comportamental e o inventário de classificação comportamental de funções executivas para verificar a existência de relação entre o desempenho comportamental e funções executivas com o modo respiratório. Os alunos com modo respiratório predominantemente oral apresentam maior incidência de alterações miofuncionais e de modificações comportamentais e nas funções executivas em comparação de alunos com respiração predominantemente nasal.

A Granulomatose de Wegener é uma doença sistémica de etiologia desconhecida, caracterizada pela tríade clínica: envolvimento da mucosa nasal e dos seios perinasais, infiltração e cavitação pulmonar e doença renal com hematúria. A Granulomatose de Wegener afecta normalmente adultos jovens e de meia idade e embora seja incomum nas crianças, pode surgir em qualquer estrato etário. Apresentamos o caso clínico de uma adolescente de 14 anos, com sintomas de epistaxis, cuja investigação posterior mostrou insuficiência renal e múltiplos nódulos pulmonares. O teste positivo para o anticorpo antineutrofílico citoplasmático suportou o diagnóstico de Granulomatose de Wegener, necessitando de terapêutica com hemodiálise e imunossupressão.

Even though Chagas disease is rare in the Brazilian Amazon, the conditions for the establishment of domiciliated cycles prevail in many areas where triatomines are of frequent occurrence. In Roraima, a previous serological and entomological survey in three agricultural settlements showed the existence of all transmission cycle elements, i.e., individuals infected by Trypanosoma cruzi, triatomine species previously found harboring T. cruzi in the broader Amazon region of neighboring countries and, domicile/ peridomicile conditions favorable to triatomine colonization. Triatoma maculata was the most frequent species, found in chicken houses in the peridomicile and sporadically within residences. Aiming to investigate the possibility of T. maculata to possess the potentiality to transmit T. cruzi in the area, bionomic characteristics were studied under laboratory conditions. These were feeding frequency, time for defecation after a blood meal, time elapsed in voluntary fasting pre- and pos-ecdysis, moulting time periods, pre-oviposition and oviposition periods and index of oviposition, incubation period, egg viability, longevity and mortality rate. Results show that the Passarão population of T. maculata should be considered a potential vector of T. cruzi since it shows a capacity to infest artificial ecotopes in the peridomicile, to carry out large number of meals during the nymphal cycle, to have a relatively short developmental cycle capable of producing 2.9 generations/year, to blood source eclecticism, to defecate immediately after the blood meal while still on the host and to the fact that has been previously found naturally infected by T.cruzi.

As pericardites bacterianas, apesar da sua baixa incidência e das terapêuticas actuais, apresentam um prognóstico desfavorável, sobretudo quando causadas por Staphylococcus aureus meticilino resistente (MRSA). O Tamponamento cardíaco é uma complicação potencialmente letal nos doentes com pericardites por este agente. Numa Unidade de Cuidados Intensivos, para além da imunossupressão, constituem factores predisponentes para este tipo de infecção, a elevada taxa de colonização nasal e cutânea, assim como a utilização de técnicas invasivas (1) entre as quais a simples colocação de catéteres intravenosos. Relatam-se dois casos clínicos de tamponamento cardíaco em doentes jovens, no contexto de imunossupressão de diferentes etiologias (infecção HIV e pós transplante hepático). Os internamentos foram complicados de quadros sépticos importantes com isolamento de MRSA nos líquidos biológicos e desenvolvimento de pericardite bacteriana e subsequente tamponamento cardíaco. Os autores salientam a importância dos quadros clínicos infecciosos em doentes imunodeprimidos, que constituem uma população cada vez mais numerosa, e a importância da monitorização ecocardiográfica na evolução prognóstica das pericardites bacterianas no contexto de sépsis.

Despite the importance of understanding the epidemiology of agents responsible for infectious diarrhea in human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) population, the number of articles about this subject is relatively few. The current article summarizes published data on bacterial, fungal, viral and parasitic enteropathogens in the HIV/AIDS seropositive subjects in different countries, regions and localities. In general, there is a great difference in the frequencies of etiological agents due to factors which include immune status, geographical location, climate and socioeconomic conditions. It is important to stress that a great prevalence of infection by emergent agents has been reported in the more advanced stages of AIDS. Therefore, to establish specific treatment depends directly on knowledge of these agents and risk factors associated to their distribution. Moreover, the colonization by potential pathogenic agents verified in these individuals is high thus implicating that they act as carriers. Finally, public health measures of control and prevention must take into consideration the regional previously identified enteropathogens, especially in areas where HIV prevalence is high.

1.Pre-assessment data of the patient A 2-year-old boy, weighing 15 kg was admitted with a history of limited mouth opening(inter-incisor distance of 6 mm), hypoplastic and retrognathic mandible (bird face deformity) and facial asymmetry from left temporomandibular joint ankylosis (TMJA). He was born at term, after an uneventful pregnancy, and there was no report of trauma during caesarean section. No other possible aetiologies were identified. He was scheduled for mandibular osteotomy. Preoperative ENT examination revealed adenotonsillar hypertrophy. 2. Anaesthetic Plan A fiberoptic nasal intubation was performed under deep inhalation anaesthesia with sevoflurane, with the patient breathing spontaneously. Midazolam (0.05 mg.kg-1) and alfentanil (0.03 mg.kg-1) were given and anaesthesia was maintained with O2/air and sevoflurane. No neuromuscular blocking agent was administered since the surgical team needed facial nerve monitoring. 3. Description of incident During surgery an accidental extubation occurred and an attempt was made to reintubate the trachea by direct laryngoscopy. Although the osteotomy was nearly completed, the vocal cords could not be visualized (Cormack-Lehane grade IV laryngoscopic view). 4. Solving the problem Re-intubation was finally accomplished with the flexible fiberscope and the procedure was concluded without any more incidents. Extubation was performed 24 hours postoperatively with the patient fully awake. After surgery mouth opening improved to inter-incisor gap of 15 mm. 5. Lessons learned and take home message Two airways issues present in this case can lead to difficultventilation and intubation: TMJA and adenotonsillar hypertrophy. These difficulties were anticipated and managed accordingly. The accidental extubation brought to our attention the fact that, even after surgical correction, this airway remains challenging. Even with intensive jaw stretchingexercises there is a high incidence of re-ankylosis, especially in younger patients. One should bear that in mind when anaesthetizing patients with TMJA.

Introduction: Rhinoscleroma is a rare, chronic, granulomatous disease that most frequently affects the upper respiratory tract, especially the nasal cavity and sometimes extends through the lower respiratory tract. Is associated with Klebsiella rhinoscleromatis, which is endemic in certain geographic regions namely Central America. The pathogenesis and risk factors remain unclear. Clinical case: We report a five years Old Portuguese boy, previously healthy, brought to the Emergency Department with epistaxis, without other accompanying signs or symptoms. The Otorhinolaryngologist (ORL) performed rhinoscopy and identified an intranasal bleeding mass. The MRI revealed an intranasal mass with extension to the ethmoid bone sinus, and performed biopsy. The histopathology was vital, making the diagnosis of Rhinoscleroma. The child had traveled abroad for the first time on vacations a year before to Dominican Republic. The bacteriologic exam identified a Klebsiella spp. sensible to the association of amoxicillin and clavulanic acid. Blood test performed excluded association of immunodeficiency. Since it’s a rare disease genetic study are under course. Monthly evaluation by ORL and pediatrician was performed which documented progressive reduction until total disappearing of the macro and microscopic lesion, and negative bacteriologic exam. Six months of antibiotic therapy were completed without any known secondary effects. The child remained asymptomatic up to the last visit, 3 months following treatment and has shown no evidence of recurrence. Conclusion: Globalization and free transit of people to areas far from origin countries here some rare diseases are endemic brings a new challenge to modern medicine. Sometimes vacations bring more than memories.

The geographical distribution and intra-domiciliary capture of sylvatic triatomines in three districts of the province of La Convención, Cusco, Peru are presented. In the district of Vilcabamba, eight adults of Rhodnius pictipes and five adults of Panstrongylus geniculatus were found. In the district of Ocobamba, 19 adults, 14 nymphs, and eggs of P. rufotuberculatus were found. In the district of Echarate, six adults and 10 nymphs of Eratyrus mucronatus, an adult of R. pictipes and P. geniculatus, and a nymph of P. rufotuberculatus were also found. The geographical distribution of E. mucronatus has extended to Cusco. This is the first report in Peru of household colonization by this triatomine.

RESUMO: A permeabilidade nasal (nasal patency) é um conceito importante na avaliação da função nasal. Pode ser explicado como uma medida do quanto a cavidade nasal está “aberta” e é determinada por fatores dependentes da sua estrutura anatómica que se relacionam com o fluxo e a resistência nasais e a existência de um fenómeno denominado ciclo nasal. Tradicionalmente, a avaliação da permeabilidade nasal baseia-se em indicadores subjetivos, nomeadamente Escalas Visuais Analógicas ou Scores de Sintomas, embora a sensação subjetiva de obstrução nasal seja difícil de quantificar e frequentemente não se correlacione com métodos mais objetivos de avaliação. A Rinometria Acústica (RA) tem adquirido um papel cada vez mais relevante na avaliação das alterações da permeabilidade nasal, pelo seu carácter não-invasivo, rápido, de fácil execução e por ser bem tolerada pelos pacientes. Contudo, não foram ainda publicados valores de referência para a população normal. Com o presente trabalho pretendeu-se determinar áreas transversais (AT) e volumes (Vol) da cavidade nasal em adultos saudáveis através de RA, em condições basais e após a administração de um fármaco descongestionante tópico nasal, relacionando estes parâmetros com variáveis demográficas e antropométricas. MATERIAL E MÉTODOS: O estudo foi realizado em 32 adultos jovens, de ambos os sexos (18 mulheres e 14 homens), com idade igual ou superior a 18 anos, não fumadores, sem patologias crónicas ou infeção recente das vias aéreas superiores. Para a execução do exame de RA foi utilizado o equipamento SRE2000 com Software RhinoScan Interacoustics, Dinamarca). O rinograma foi analisado em condições basais e após descongestionamento até aos 5 cm de distância da narina, obtendo-se os valores médios entre cavidade nasal direita e esquerda das áreas transversais (AT) dos entalhes registados (AT1 e AT2), a sua localização e volumes nasais a diferentes distâncias (Vol.0-2; Vol.2-5 e Vol.0-5). RESULTADOS: Os indivíduos do género masculino apresentaram valores significativamente superiores aos do género feminino em todos os parâmetros estudados. A administração do fármaco descongestionante tópico nasal provocou um aumento generalizado das AT e volumes nasais, traduzindo uma diminuição da congestão da mucosa nasal, mais acentuada posteriormente ao nível dos cornetos. A comparação entre fossas nasais (direita versus esquerda) após descongestionamento, eliminando o efeito do ciclo nasal, revelou simetria significativa. De uma forma geral, os parâmetros rinométricos estudados revelaram correlação positiva significativa com a altura. CONCLUSÃO: O estudo foi pioneiro para a população portuguesa e representa uma contribuição para a obtenção de valores de referência em adultos jovens. Estudos futuros em amostras de maior dimensão e representatividade são necessários para a obtenção de valores de referência com utilidade clínica e em investigação.--------------ABSTRACT: Nasal patency is an important concept in the evaluation of nasal function. It may be explained as a measure of how open the nasal cavity is, and it depends of anatomical aspects, that relate to nasal airflow, resistance and nasal cycle. Nasal patency evaluation is based in subjective indicators like visual analogical scales or symptom scores, but subjective sensation of nasal obstruction is difficult to quantify and usually doesn’t correlate with more objective methods of evaluation. Acoustic rhinometry has gained an increasingly relevant role in assessing changes in nasal patency, because it is noninvasive, fast and easy to perform and well tolerated by patients However, reference values for the normal population have not yet been published. The aim of the present study was to determine nasal cross-sectional areas and volumes in healthy adults by acoustic rhinometry, before and after administration of a topical nasal decongestant drug, correlating these parameters with demographic and anthropometric variables. MATERIAL E METHODS: The study was performed in 32 young adults (18 females and 14 males), aged 18 years old or more, non-smokers, without chronic diseases or recent airway infection. Acoustic rhinometry evaluation was performed using SRE2000 with RhinoScan software (Interacoustics, Denmark). Nasal cavities were assessed up to 5 cm distance from the nostril, before and after administration of the topical nasal decongestant drug, and mean cross-sectional areas of the notches, their location and mean volumes at different distances from nostril were measured. RESULTS: Males presented significant higher values than females for all rhinometric parameters studied. The decongestant drug, significantly increased all the rhinometric parameters evaluated, in particular those corresponding to the more posterior regions. Comparison between nasal cavities after nasal decongestion showed no significant asymmetry. Globally,a positive correlation was found between rhinometric parameters and height . CONCLUSION: This study was pioneer in the Portuguese population and represents a contribution to acoustic rhinometry reference values in young adults . Further studies, on larger and representative samples, are necessary in order to obtain reference values, useful both to clinical and research applications of this technique.

Mycetoma is a pathological process in which eumycotic (fungal) or actinomycotic causative agents from exogenous source produce grains. It is a localized chronic and deforming infectious disease of subcutaneous tissue, skin and bones. We report the first case of eumycetoma of the oral cavity in world literature. CASE REPORT: A 43-year-old male patient, complaining of swelling and fistula in the hard palate. On examination, swelling of the anterior and middle hard palate, with fistula draining a dark liquid was observed. The panoramic radiograph showed extensive radiolucent area involving the region of teeth 21-26 and the computerized tomography showed communication with the nasal cavity, suggesting the diagnosis of periapical cyst. Surgery was performed to remove the lesion. Histopathological examination revealed purulent material with characteristic grain. Gram staining for bacteria was negative and Grocott-Gomori staining for the detection of fungi was positive, concluding the diagnosis of eumycetoma. The patient was treated with ketoconazole for nine months, and was considered cured at the end of treatment. CONCLUSION: Histopathological examination, using histochemical staining, and direct microscopic grains examination can provide the distinction between eumycetoma and actinomycetoma accurately.

Background: This is the first prospective, randomized, doubleblind, placebo-controlled study showing statistical improvement of an H1-antihistamine in children with seasonal allergic rhinitis in all symptoms throughout the entire treatment period. Objective: This randomized, placebo-controlled, parallelgroup,double-blind study was performed to assess the efficacy and safety of fexofenadine in children with seasonal allergic rhinitis. Methods: This study was conducted at 148 centers in 15 countries. Nine hundred thirty-five children (aged 6-11 years) were randomized and treated with either fexofenadine HCl 30 mg (n = 464) or placebo (n = 471) tablets twice a day for 14 days. Individual symptoms (sneezing; rhinorrhea; itchy nose, mouth, throat, and/or ears; itchy, watery, and/or red eyes; and nasal congestion) were assessed at baseline and then daily at 7:00 AM and 7:00 PM (±1 hour) during the double-blind treatment period. Each total symptom score was the sum of all symptoms, excluding nasal congestion. The primary efficacy variable was the change from baseline in the average of the daily 12-hour evening reflective total symptom scores throughout the double-blind treatment. Safety was evaluated from adverse-event reporting, vital signs, physical examinations, and clinical laboratory data at screening and study end point.
