Aim: The aim of the study was to investigate the influence of dietary intake of commercial hydrolyzed collagen (Gelatine Royal ®) on bone remodeling in pre-pubertal children. Methods: A randomized double-blind study was carried out in 60 children (9.42 ± 1.31 years) divided into three groups according to the amount of partially hydrolyzed collagen taken daily for 4 months: placebo (G-I, n = 18), collagen (G-II, n = 20) and collagen + calcium (G-III, n = 22) groups. Analyses of the following biochemical markers were carried out: total and bone alkaline phosphatase (tALP and bALP), osteocalcin, tartrate-resistant acid phosphatase (TRAP), type I collagen carboxy terminal telopeptide, lipids, calcium, 25-hydroxyvitamin D, insulin-like growth factor 1 (IGF-1), thyroid-stimulating hormone, free thyroxin and intact parathormone. Results: There was a significantly greater increase in serum IGF-1 in G-III than in G II (p < 0.01) or G-I (p < 0.05) during the study period, and a significantly greater increase in plasma tALP in G-III than in G-I (p < 0.05). Serum bALP behavior significantly (p < 0.05) differed between G-II (increase) and G-I (decrease). Plasma TRAP behavior significantly differed between G-II and G-I (p < 0.01) and between G-III and G-II (p < 0.05). Conclusion: Daily dietary intake of hydrolyzed collagen seems to have a potential role in enhancing bone remodeling at key stages of growth and development.

Contexte¦Le VIH reste une des préoccupations majeures de santé publique dans le monde. Le nombre de patients infectés en Europe continue de croître et s'élève, en 2008, à 2.3 millions (1). De plus, environ 30 % des personnes séropositives ignorent leur statut et, de ce fait, contribuent à la propagation de l'épidémie. Ces patients sont responsables de la moitié des nouveaux cas du VIH (2) ; ils transmettent, en effet, 3.5 fois plus l'infection que les patients dont le diagnostic est connu (3).¦Aux USA, en raison de l'épidémiologie actuelle du VIH, les Centers for Disease Control and Prevention (CDC) ont, en septembre 2006, mis le point sur la nécessité d'étendre drastiquement les tests et, de ce fait, ont publié de nouvelles recommandations. Non seulement, le test devra dépasser les groupes à risque dans les zones à grande prévalence mais aussi, être répandu à toute la population adulte de 13 à 64 ans sauf si la prévalence du VIH est en dessous de 0.1 % (4). Cette démarche est appelée routine opt-out HIV screening et plusieurs arguments parlent en faveur d'un dépistage systématique. Cette maladie rempli tout d'abord les 4 critères pour l'introduction d'un dépistage systématique : une maladie grave pouvant être mise en évidence avant l'apparition des symptômes, son diagnostic améliore la survie par une progression moins rapide et diminution de la mortalité, des tests de dépistage sensibles et spécifiques sont disponibles et les coûts sont moindres en comparaison aux bénéfices (5). Aux USA, 73 % des patients diagnostiqués à un stade avancé de l'infection VIH entre 2001 et 2005 avaient eu recours à l'utilisation des systèmes de soins au moins une fois dans les 8 ans précédant le diagnostic (6). Ces occasions manquées font aussi partie des arguments en faveur d'un dépistage systématique. En règle générale, le médecin se basant uniquement sur les symptômes et signes, ainsi que sur l'anamnèse sexuelle sous-estime la population à tester. Ce problème de sélection des candidats n'a plus lieu d'être lors d'un tel screening. Après cette publication des recommandations du CDC, qui introduit le dépistage systématique, il a été constaté que seulement 1/3 du personnel soignant interrogé connaissait les nouvelles directives et seulement 20 % offrait un dépistage de routine à tous les patients concernés (7). Cette étude nous montre alors qu'il est impératif de vérifier le niveau de connaissances des médecins après la publication de nouvelles recommandations.¦Devant le problème de l'épidémie du VIH, la Suisse opte pour une stratégie différente à celle des Etats-Unis. La Commission d'experts clinique et thérapie VIH et SIDA (CCT) de l'OFSP a tout d'abord publié, en 2007, des recommandations destinées à diminuer le nombre d'infections VIH non diagnostiquées, grâce à un dépistage initié par le médecin (8). Cette approche, appelée provider initiated counselling and testing (PICT), complétait alors celle du voluntary counselling and testing (VCT) qui préconisait un dépistage sur la demande du patient. Malheureusement, le taux d'infections diagnostiquées à un stade avancé a stagné aux environs de 30 % jusqu'en 2008 (9), raison pour laquelle l'OFSP apporte, en 2010, des modifications du PICT. Ces modifications exposent différentes situations où le test du VIH devrait être envisagé et apportent des précisions quant à la manière de le proposer. En effet, lors d'une suspicion de primo-infection, le médecin doit expliquer au patient qu'un dépistage du VIH est indiqué, un entretien conseil est réalisé avec des informations concernant la contagiosité élevée du virus à ce stade de l'infection. Si le patient présente un tableau clinique qui s'inscrit dans le diagnostic différentiel d'une infection VIH, le médecin propose le test de manière systématique. Il doit alors informer le patient qu'un tel test sera effectué dans le cadre d'une démarche diagnostique, sauf si celui-ci s'y oppose. Enfin, dans d'autres situations telles que sur la demande du patient ou si celui-ci fait partie d'un groupe de population à grande prévalence d'infection VIH, le médecin procède à une anamnèse sexuelle, suivie d'un entretien conseil et du test si l'accord explicite du patient a été obtenu (10).¦Nous pouvons donc constater les différentes stratégies face à l'épidémie du VIH entre les USA et la Suisse. Il est nécessaire d'évaluer les conséquences de ces applications afin d'adopter la conduite la plus efficace en terme de dépistage, pour amener à une diminution des transmissions, une baisse de la morbidité et mortalité. Aux USA, des études ont été faites afin d'évaluer l'impact de l'approche opt-out qui montrent que le screening augmente la probabilité d'être diagnostiqué (11). En revanche, en Suisse, aucune étude de ce type n'a été entreprise à l'heure actuelle. Nous savons également qu'il existe un hiatus entre la publication de nouvelles recommandations et l'application de celles-ci en pratique. Le 1er obstacle à la mise en oeuvre des guidelines étant leur méconnaissance (12), il est alors pertinent de tester les connaissances des médecins des urgences d'Hôpitaux de Suisse au sujet des nouvelles recommandations sur le dépistage du VIH de l'OFSP de mars 2010.¦Objectifs¦Montrer que les recommandations de l'OFSP de mars 2010 ne sont pas connues des médecins suisses.¦Méthodes¦Nous testerons la connaissance des médecins concernant ces recommandations via un questionnaire qui sera distribué lors d'un colloque organisé à cet effet avec tous les médecins du service des urgences d'un même établissement. Il n'y aura qu'une séance afin d'éviter d'éventuels biais (transmission d'informations d'un groupe à un autre). Ils recevront tout d'abord une lettre informative, accompagnée d'un formulaire de consentement pour l'utilisation des données de manière anonyme. La feuille d'information est rédigée de façon à ne pas influencer les candidats pour les réponses aux questions. Le questionnaire comprend deux parties, une première qui comprend divers cas cliniques. Les candidats devront dire si ces situations se trouvent, selon eux, dans les nouvelles recommandations de l'OFSP en termes de dépistage du VIH et indiquer la probabilité d'effectuer le test en pratique. La deuxième partie interrogera sur la manière de proposer le test au patient. La durée nécessaire pour remplir le questionnaire est estimée à 15 minutes.¦Le questionnaire élaboré avec la collaboration de Mme Dubois de l'UMSP à Lausanne et vont être testés par une vingtaine de médecins de premier recours de Vidy Med et Vidy Source, deux centres d'urgences lausannois.¦Réstulats escomptés¦Les médecins suisse ne sont pas au courant des nouvelles recommandations concernant le dépistage du VIH.¦Plus-value escomptée¦Après le passage du questionnaire, nous ferons une succincte présentation afin d'informer les médecins au sujet de ces recommandations. Aussi, l'analyse des résultats du questionnaire nous permettra d'agir au bon niveau pour que les nouvelles recommandations de l'OFSP de mars 2010 soient connues et appliquées, tout en ayant comme objectif l'amélioration du dépistage du VIH.

Also known as ferroxidase ceruloplasmin, belongs to the family of inflammation-sensitive proteins, and its main function to transport copper in the blood. Although, in addition to this transport function, at present, there are numerous studies that have attempted to use the determination of serum concentrations as a predictive indicator of cardiovascular risk in patients who are overweight or obese. The results of this study confirm the existence of a significant correlation between serum ceruloplasmin and nutritional status of the subjects, which means that for the population of students assessed, serum levels of this protein are an important predictor the risk of cardiovascular disease.

INTRODUCTION Human host immune response following infection with the new variant of A/H1N1 pandemic influenza virus (nvH1N1) is poorly understood. We utilize here systemic cytokine and antibody levels in evaluating differences in early immune response in both mild and severe patients infected with nvH1N1. METHODS We profiled 29 cytokines and chemokines and evaluated the haemagglutination inhibition activity as quantitative and qualitative measurements of host immune responses in serum obtained during the first five days after symptoms onset, in two cohorts of nvH1N1 infected patients. Severe patients required hospitalization (n = 20), due to respiratory insufficiency (10 of them were admitted to the intensive care unit), while mild patients had exclusively flu-like symptoms (n = 15). A group of healthy donors was included as control (n = 15). Differences in levels of mediators between groups were assessed by using the non parametric U-Mann Whitney test. Association between variables was determined by calculating the Spearman correlation coefficient. Viral load was performed in serum by using real-time PCR targeting the neuraminidase gene. RESULTS Increased levels of innate-immunity mediators (IP-10, MCP-1, MIP-1beta), and the absence of anti-nvH1N1 antibodies, characterized the early response to nvH1N1 infection in both hospitalized and mild patients. High systemic levels of type-II interferon (IFN-gamma) and also of a group of mediators involved in the development of T-helper 17 (IL-8, IL-9, IL-17, IL-6) and T-helper 1 (TNF-alpha, IL-15, IL-12p70) responses were exclusively found in hospitalized patients. IL-15, IL-12p70, IL-6 constituted a hallmark of critical illness in our study. A significant inverse association was found between IL-6, IL-8 and PaO2 in critical patients. CONCLUSIONS While infection with the nvH1N1 induces a typical innate response in both mild and severe patients, severe disease with respiratory involvement is characterized by early secretion of Th17 and Th1 cytokines usually associated with cell mediated immunity but also commonly linked to the pathogenesis of autoimmune/inflammatory diseases. The exact role of Th1 and Th17 mediators in the evolution of nvH1N1 mild and severe disease merits further investigation as to the detrimental or beneficial role these cytokines play in severe illness.

La thrombocytopénie immune primaire (ITP) est une affection auto-immune acquise avec diminution de la survie des plaquettes et perturbation de la production plaquettaire. Il n'existe aucun test clinique simple permettant de prouver la nature auto-immune de l'affection. Pour cette raison, il s'agit presque toujours d'un diagnostic par exclusion d'autres causes. Bien que les plaquettes soient souvent inférieures à 10 x 109/l lors de la présentation initiale, la tendance hémorragique est étonnamment modérée chez la majorité des patients. Le traitement initial fait toujours appel aux corticostéroïdes, combinés à des immunoglobulines intraveineuses et à des transfusions de plaquettes dans les formes compliquées avec hémorragies significatives. Chez l'enfant, la maladie est souvent induite par des infections virales et son évolution est bénigne et spontanément régressive dans la majorité des cas. Chez l'adulte, la maladie est plus souvent persistante ou chroniquement récidivante, et le taux de plaquettes se situe souvent à un taux suffisant pour prévenir des hémorragies spontanées. Seule une faible proportion de patients souffre d'une thrombocytopénie sévère prolongée accompagnée de saignements réguliers avec risque d'hémorragies potentiellement fatales. C'est probablement ce groupe de patients restreint qui tirera surtout profit des nouvelles options thérapeutiques telles que les agonistes du récepteur de la thrombopoïétine. A la lumière de ces nouvelles possibilités, un groupe d'hématologues suisses s'est réuni pour élaborer des directives concernant la prise en charge de l'ITP conformément aux besoins et aux habitudes de notre pays.

BACKGROUND The randomized placebo-controlled IFIGENIA-trial demonstrated that therapy with high-dose N-acetylcysteine (NAC) given for one year, added to prednisone and azathioprine, significantly ameliorates (i.e. slows down) disease progression in terms of vital capacity (VC) (+9%) and diffusing capacity (DLco) (+24%) in idiopathic pulmonary fibrosis (IPF). To better understand the clinical implications of these findings we performed additional, explorative analyses of the IFGENIA data set. METHODS We analysed effects of NAC on VC, DLco, a composite physiologic index (CPI), and mortality in the 155 study-patients. RESULTS In trial completers the functional indices did not change significantly with NAC, whereas most indices deteriorated with placebo; in non-completers the majority of indices worsened but decline was generally less pronounced in most indices with NAC than with placebo. Most categorical analyses of VC, DLco and CPI also showed favourable changes with NAC. The effects of NAC on VC, DLco and CPI were significantly better if the baseline CPI was 50 points or lower. CONCLUSION This descriptive analysis confirms and extends the favourable effects of NAC on lung function in IPF and emphasizes the usefulness of VC, DLco, and the CPI for the evaluation of a therapeutic effect. Most importantly, less progressed disease as indicated by a CPI of 50 points or lower at baseline was more responsive to therapy in this study.

BACKGROUND Clinical predictors for fatal pulmonary embolism (PE) in patients with venous thromboembolism have never been studied. METHODS AND RESULTS Using data from the international prospective Registro Informatizado de la Enfermedad TromboEmbolica venosa (RIETE) registry about patients with objectively confirmed symptomatic acute venous thromboembolism, we determined independent predictive factors for fatal PE. Between March 2001 and July 2006, 15520 consecutive patients (mean age+/-SD, 66.3+/-16.9 years; 49.7% men) with acute venous thromboembolism were included. Symptomatic deep-vein thrombosis without symptomatic PE was observed in 58.0% (n=9008) of patients, symptomatic nonmassive PE in 40.4% (n=6264), and symptomatic massive PE in 1.6% (n=248). At 3 months, the cumulative rates of overall mortality and fatal PE were 8.65% and 1.68%, respectively. On multivariable analysis, patients with symptomatic nonmassive PE at presentation exhibited a 5.42-fold higher risk of fatal PE compared with patients with deep-vein thrombosis without symptomatic PE (P<0.001). The risk of fatal PE was multiplied by 17.5 in patients presenting with a symptomatic massive PE. Other clinical factors independently associated with an increased risk of fatal PE were immobilization for neurological disease, age >75 years, and cancer. CONCLUSIONS PE remains a potentially fatal disease. The clinical predictors identified in the present study should be included in any clinical risk stratification scheme to optimally adapt the treatment of PE to the risk of the fatal outcome.

Background: Chest pain (CP) represents about 5% of admissions to emergency departments (ED), even in young people. Acute coronary syndrome (ACS) and myocarditis are among the most important diagnoses to rule out. Clinical and ECG findings are not specific for either condition and separating both diagnoses is a challenge. Aim of the study: To evaluate the prevalence of ACS and myocarditis in young patients presenting with CP and elevated cardiac biomarkers to the ED and to determinate the differences in their clinical presentation. Methods: Retrospective study of all consecutive patients < 40 years old admitted to our ED from January 2009 to June 2011 for CP with elevated serum troponin concentration. All clinical, angiographic and cardiac magnetic resonance (CMR) data from the local database was reviewed. Clinical follow-up was obtained to assess all cause mortality, myocardial infarction and re-hospitalisation for CP. Results: 1588 patients < 40 years old were admitted to the ED with chest pain. 49 (3%) patients presenting with an elevated troponin I (> 0.09ug/l) were included in the study. 32.7% (16/49) were diagnosed with ACS (11 STEMI and 5 NSTEMI) and 59.2% (29/49) with myocarditis. Among the 29 patients with myocarditis, 17 presented with typical subepicardial late enhancement on CMR and 12 were diagnosed based on clinical presentation (6 had no complementary workup, 3 normal coronary angiogram and 3 inconclusive CMR). 8.1% (4/49) of patients had other diagnoses. Compared to patients with myocarditis, ACS patients were older (34.1±3.9 vs 26.9±6.4, p=0.0002) with significantly more cardiovascular risk factors (mean 2.06 vs 0.69, p<0.0001). Diabetes (18.8% vs 0%, p=0.004), dyslipidemia (56.3% vs 3.4%, p=0.0001) and family history of coronary artery disease (CAD) (37.5% vs 10.3%, p=0.050) were significantly associated with ACS. No significant association was found for smoking, hypertension and obesity. Fever (>38°C) or recent viral illness were present in 75.9% (22/29) of patients with myocarditis, and in 0% of ACS patients. During follow-up (mean 19.9 months ± 8.6), only 2 patients with myocarditis were re-admitted for chest pain. Conclusions: In this study, 32.7% of patients < 40 year old admitted to an ED with CP and elevated troponin had an ACS. Key clinical factors include diabetes, dyslipidemia, family history of CAD, fever or recent viral illness, and may help to differentiate ACS from myocarditis.

BACKGROUND This study was realized thanks to the collaboration of children and adolescents who had been resected from cerebellar tumors. The medulloblastoma group (CE+, n = 7) in addition to surgery received radiation and chemotherapy. The astrocytoma group (CE, n = 13) did not receive additional treatments. Each clinical group was compared in their executive functioning with a paired control group (n = 12). The performances of the clinical groups with respect to controls were compared considering the tumor's localization (vermis or hemisphere) and the affectation (or not) of the dentate nucleus. Executive variables were correlated with the age at surgery, the time between surgery-evaluation and the resected volume. METHODS The executive functioning was assessed by means of WCST, Complex Rey Figure, Controlled Oral Word Association Test (letter and animal categories), Digits span (WISC-R verbal scale) and Stroop test. These tests are very sensitive to dorsolateral PFC and/or to medial frontal cortex functions. The scores for the non-verbal Raven IQ were also obtained. Direct scores were corrected by age and transformed in standard scores using normative data. The neuropsychological evaluation was made at 3.25 (SD = 2.74) years from surgery in CE group and at 6.47 (SD = 2.77) in CE+ group. RESULTS The Medulloblastoma group showed severe executive deficit (tests, the most severe occurring in vermal patients. The Astrocytoma group also showed executive deficits in digits span, semantic fluency (animal category) and moderate to slight deficit in Stroop (word and colour) tests. In the astrocytoma group, the tumor's localization and dentate affectation showed different profile and level of impairment: moderate to slight for vermal and hemispheric patients respectively. The resected volume, age at surgery and the time between surgery-evaluation correlated with some neuropsychological executive variables. CONCLUSION Results suggest a differential prefrontal-like deficit due to cerebellar lesions and/or cerebellar-frontal diaschisis, as indicate the results in astrocytoma group (without treatments), that also can be generated and/or increased by treatments in the medulloblastoma group. The need for differential rehabilitation strategies for specific clinical groups is remarked. The results are also discussed in the context of the Cerebellar Cognitive Affective Syndrome.

Introduction.- La bérylliose chronique est une granulomatose pulmonaire pour laquelle le diagnostic est rarement posé en raison d'une méconnaissance du risque d'exposition professionnelle au béryllium.En outre, le tableau clinique, les signes radiologiques et les symptômes ressemblent a` une sarcoïdose. Ainsi, la fréquence de la maladie est probablement sous-estimée malgré l'utilisation de béryllium dans la fabrication de nombreux produits industriels et techniques en Suisse, tels que ressorts, prothèses dentaires, micro-électronique... Comme aide au diagnostic, un test de prolifération lymphocytaire au béryllium (BeLPT) a été développé pour évaluer la réponse immunocellulaire spécifique au béryllium. Mais celui-ci présente des inconvénients : utilisation de produits radioactifs, variabilité interlaboratoire importante, valeur prédictive contesteé. L'utilisation de l'Elispot, évaluant également la réponse lymphocytaire pourrait être une alternative au BeLPT. Objectifs.- Les objectifs de l'étude sont, d'une part, de déceler parmi les cas de sarcoïdose du registre SIOLD (Swiss Group for Interstitial and Orphan Lung Diseases) des cas de bérylliose sous-diagnostiqués et, d'autre part, d'améliorer les tests de de´pistage (questionnaire et test immunologique) de sensibilisation et de diagnostic. Méthode.- Cent soixante autoquestionnaires évaluant l'exposition professionnelle au béryllium ont été adressés à des patients référencés dans le registre SIOLD, souffrant de sarcoïdose. Les patients relatant dans l'autoquestionnaire un contact professionnel avec le béryllium ont bénéficié d'une visite médicale à l'Institut universitaire romand de santé au travail (IST) comportant une anamnèse professionnelle, un questionnaire spécifique à l'exposition au béryllium (questionnaire de Cherry) et une prise de sang. Par la suite, le sang a été analysé par Elispot pour évaluer la possibilité d'une sensibilisation au béryllium. En parallèle, des patients déjà diagnostiqués pour une bérylliose par un BeLPT ont servi de cas contrôle positifs pour la mise en place du test. Résultats.- Cinquante pour cents des autoquestionnaires adressés aux patients du registre SIOLD ont été renvoyés. Parmi ceux-ci, 33 %(26 personnes) rapportaient une exposition professionnelle possible au béryllium et ont été reçus a` l'IST pour compléter l'anamnèse professionnelle et répondre au questionnaire adapté de Cherry. Il ressort de ce questionnaire que 3 personnes ont effectivement travaillé avec du béryllium au cours de leur carrière professionnelle et que 4 ont potentiellement été exposées dans le contexte de leur environnement professionnel. Tous les patients ont été testés par Elispot, mais aucun n'a montré de prolifération évoquant une sensibilisation au béryllium. Des résultats supplémentaires devraient être disponibles dès février. Conclusion.- Au vu de ces résultats où seules 7 personnes sur les 80 ayant répondu au questionnaire (8 %) sont suspectées d'avoir été exposées directement ou indirectement au béryllium au cours de leur activité professionnelle, il est possible que le risque d'exposition au béryllium soit anecdotique, malgré une utilisation importante de ce métal dans certaines régions de Suisse. Une hypothèse envisageable est que la population de l'étude ne soit pas représentative de la population exposée. En outre, le fait que les tests lymphocytaires effectués sur les 7 personnes ayant rapporté une exposition au béryllium soient revenus négatifs peut être expliqué de 2 manières : soit que chez ces personnes, la maladie pulmonaire n'est pas due au béryllium, même si l'anamnèse professionnelle est positive et qu'il s'agit bien d'une sarcoïdose, soit que la prise de corticoïdes modifie la réponse immunitaire, soit que le test n'est pas suffisamment sensible.

Leptospirosis is a zoonotic disease caused by the pathogenic Leptospira spp. The clinical presentations are diverse, ranging from undifferentiated fever to fulminant disease including meningeal forms. The neurological leptospirosis forms are usually neglected. The aim of this study was to investigate leptospirosis as the cause of aseptic meningitis using different diagnostic techniques including the polymerase chain reaction (PCR). Thirty-nine cerebrospinal fluid (CSF) samples from patients presenting with meningeal abnormalities, predominance of lymphocytes and negative results by traditional microbiological tests were processed by leptospiral culture, anti-leptospiral antibody response and PCR. Leptospira spp DNA was detected in 23 (58.97%) of the CSF samples. Anti-leptospiral antibodies were found in 13 (33.33%) CSF samples. Twelve CSF samples were positive by PCR assay and negative by microscopic agglutination test (MAT) assay. Two CSF samples were positive by MAT and negative by PCR. The positive and negative agreement between both tests was 11 and 14, respectively. CSF samples from six cases of unknown diagnosis were positive by PCR assay. Eight cases showed positive results using PCR and MAT. Leptospirosis could be detected by PCR assay from the 3rd-26th day after illness onset. The sensitivity of the PCR was assessed with confirmed cases of leptospirosis (by MAT) and found to be 89.5%. All CSFs were negative by culture. PCR was found to be a powerful tool for diagnosing meningitis cases of leptospirosis. We recommend that it may be used as a supplementary diagnostic tool, especially in the early stages of the disease, when other diagnostic techniques such as serology are not sensitive.

Compared to total allergenic extracts, recombinant allergens available for specific IgE measurement represent an important advance in the diagnosis and treatment of IgE-mediated allergies. Recombinant allergens lead to define the sensitization profile of allergic patients, to identify markers of sensitization and to understand better polysensitivities related to cross-reactions and markers of severity of allergic reactions. They also contribute to the decision to establish tolerance induction (allergen specific immunotherapy) and the optimal selection of the allergenic composition of the vaccine.

Chlamydia are obligate intracellular bacteria. Three species are considered human pathogens. Chlamydophila pneumoniae is one of the most common agents of atypical community-acquired pneumonia. Chlamydophila psittaci causes psittacosis, a severe zoonotic pneumonia transmitted by birds. Finally, Chlamydia trachomatis is the etiologic agent of trachoma and urogenital infections. The latter are commonly asymptomatic or paucisymptomatic. Thus, they may remain undiagnosed for years, leading to serious late complications such as salpingitis, ectopic pregnancy and infertility. Currently, the diagnosis of chlamydial infections is essentially based on molecular methods. Treatment should use an antibiotic with good intracellular bioavailability such as tetracycline, macrolides and new generation fluoroquinolones.

BACKGROUND. Ritonavir-boosted saquinavir (SQVr) is nowadays regarded as an alternative antiretroviral drug probably due to several drawbacks, such as its high pill burden, twice daily dosing and the requirement of 200 mg ritonavir when given at the current standard 1000/100 mg bid dosing. Several once-daily SQVr dosing schemes have been studied with the 200 mg SQV old formulations, trying to overcome some of these disadvantages. SQV 500 mg strength tablets became available at the end of 2005, thus facilitating a once-daily regimen with fewer pills, although there is very limited experience with this formulation yet. METHODS. Prospective, multicentre study in which efficacy, safety and pharmacokinetics of a regimen of once-daily SQVr 1500/100 mg plus 2 NRTIs were evaluated under routine clinical care conditions in either antiretroviral-naïve patients or in those with no previous history of antiretroviral treatments and/or genotypic resistance tests suggesting SQV resistance. Plasma SQV trough levels were measured by HPLV-UV. RESULTS. Five hundred and fourteen caucasian patients were included (47.2% coinfected with hepatitis C and/or B virus; 7.8% with cirrhosis). Efficacy at 52 weeks (plasma RNA-HIV <50 copies/ml) was 67.7% (CI95: 63.6 - 71.7%) by intention-to-treat, and 92.2% (CI95: 89.8 - 94.6%) by on-treatment analysis. The reasons for failure were: dropout or loss to follow-up (18.4%), virological failure (7.8%), adverse events (3.1%), and other reasons (4.6%). The high rate of dropout may be explained by an enrollment and follow-up under routine clinical care condition, and a population with a significant number of drug users. The median SQV Cmin (n = 49) was 295 ng/ml (range, 53-2172). The only variable associated with virological failure in the multivariate analysis was adherence (OR: 3.36; CI95, 1.51-7.46, p = 0.003). CONCLUSIONS. Our results suggests that SQVr (1500/100 mg) once-daily plus 2 NRTIs is an effective regimen, without severe clinical adverse events or hepatotoxicity, scarce lipid changes, and no interactions with methadone. All these factors and its once-daily administration suggest this regimen as an appropriate option in patients with no SQV resistance-associated mutations.

INTRODUCTION According to several series, hospital hyponutrition involves 30-50% of hospitalized patients. The high prevalence justifies the need for early detection from admission. There several classical screening tools that show important limitations in their systematic application in daily clinical practice. OBJECTIVES To analyze the relationship between hyponutrition, detected by our screening method, and mortality, hospital stay, or re-admissions. To analyze, as well, the relationship between hyponutrition and prescription of nutritional support. To compare different nutritional screening methods at admission on a random sample of hospitalized patients. Validation of the INFORNUT method for nutritional screening. MATERIAL AND METHODS In a previous phase from the study design, a retrospective analysis with data from the year 2003 was carried out in order to know the situation of hyponutrition in Virgen de la Victoria Hospital, at Malaga, gathering data from the MBDS (Minimal Basic Data Set), laboratory analysis of nutritional risk (FILNUT filter), and prescription of nutritional support. In the experimental phase, a cross-sectional cohort study was done with a random sample of 255 patients, on May of 2004. Anthropometrical study, Subjective Global Assessment (SGA), Mini-Nutritional Assessment (MNA), Nutritional Risk Screening (NRS), Gassull's method, CONUT and INFORNUT were done. The settings of the INFORNUT filter were: albumin < 3.5 g/dL, and/or total proteins <5 g/dL, and/or prealbumin <18 mg/dL, with or without total lymphocyte count < 1.600 cells/mm3 and/or total cholesterol <180 mg/dL. In order to compare the different methods, a gold standard is created based on the recommendations of the SENPE on anthropometrical and laboratory data. The statistical association analysis was done by the chi-squared test (a: 0.05) and agreement by the k index. RESULTS In the study performed in the previous phase, it is observed that the prevalence of hospital hyponutrition is 53.9%. One thousand six hundred and forty four patients received nutritional support, of which 66.9% suffered from hyponutrition. We also observed that hyponutrition is one of the factors favoring the increase in mortality (hyponourished patients 15.19% vs. non-hyponourished 2.58%), hospital stay (hyponourished patients 20.95 days vs. non-hyponourished 8.75 days), and re-admissions (hyponourished patients 14.30% vs. non-hyponourished 6%). The results from the experimental study are as follows: the prevalence of hyponutrition obtained by the gold standard was 61%, INFORNUT 60%. Agreement levels between INFORNUT, CONUT, and GASSULL are good or very good between them (k: 0.67 INFORNUT with CONUT, and k: 0.94 INFORNUT and GASSULL) and wit the gold standard (k: 0.83; k: 0.64 CONUT; k: 0.89 GASSULL). However, structured tests (SGA, MNA, NRS) show low agreement indexes with the gold standard and laboratory or mixed tests (Gassull), although they show a low to intermediate level of agreement when compared one to each other (k: 0.489 NRS with SGA). INFORNUT shows sensitivity of 92.3%, a positive predictive value of 94.1%, and specificity of 91.2%. After the filer phase, a preliminary report is sent, on which anthropometrical and intake data are added and a Nutritional Risk Report is done. CONCLUSIONS Hyponutrition prevalence in our study (60%) is similar to that found by other authors. Hyponutrition is associated to increased mortality, hospital stay, and re-admission rate. There are no tools that have proven to be effective to show early hyponutrition at the hospital setting without important applicability limitations. FILNUT, as the first phase of the filter process of INFORNUT represents a valid tool: it has sensitivity and specificity for nutritional screening at admission. The main advantages of the process would be early detection of patients with risk for hyponutrition, having a teaching and sensitization function to health care staff implicating them in nutritional assessment of their patients, and doing a hyponutrition diagnosis and nutritional support need in the discharge report that would be registered by the Clinical Documentation Department. Therefore, INFORNUT would be a universal screening method with a good cost-effectiveness ratio.
