The "4-stage approach" has been widely accepted for practical skill training replacing the traditional 2 stages ("see one, do one"). However, the superior effectiveness of the 4-stage approach was never proved.

Objective To determine if clinical guidelines recommending therapeutic exercise for people with hip osteoarthritis (OA) are supported by rigorous scientific evidence. Methods A meta-analysis of randomized controlled trials (RCTs) recruiting people with hip OA and comparing some form of land-based exercise program (as opposed to exercises conducted in the water) with a non-exercise group in terms of hip pain and/or self-reported physical function. Results Thirty-two RCTs were identified, but only five met the inclusion criteria. Only one of the five included RCTs restricted recruitment to people with hip OA, the other four RCTs also recruiting participants with knee OA. The five included studies provided data on 204 and 187 hip OA participants for pain and physical function, respectively. Combining the results of the five included RCTs using a fixed-effects model demonstrated a small treatment effect for pain (standardized mean difference (SMD) −0.38; 95% confidence interval (CI) −0.67 to −0.09). No significant benefit in terms of improved self-reported physical function was detected (SMD −0.02; 95% CI −0.31 to 0.28). Conclusion Currently there is only silver level evidence (one small RCT) supporting the benefit of land-based therapeutic exercise for people with symptomatic hip OA in terms of reduced pain and improved physical function. The limited number and small sample size of the included RCTs restricts the confidence that can be attributed to these results.

Woody Guthrie’s song, “The 1913 Massacre,” written around 1940-41, has become something of a folk anthem for progressives, leftists, and labor supporters. It depicts the Italian Hall Disaster of December 24, 1913, in a plainspoken and colorful way, but has been (rightfully) described as “deeply flawed historically.” Much like Guthrie’s English-language folk songs, Finnish immigrant Santeri Mäkelä had a major impact on capturing the working-world around him. Mäkelä’s lyrics for the “Kaivantomiehen Laulu (The Miners’ Song)” were first published in Hancock, 1909, in “Uusi Työväen Laulukirja (The New Workers’ Songbook),” and was probably sung widely by Finnish strikers during the 1913-14 Michigan Copper Strike. Leading up to, and during this Strike Centennial year, there have been renewed performances of the song, both in Finland and the United States—but only in the original Finnish language. This presentation will delve into the accuracy, history, and lyrics of these two important, but historically problematic labor songs.

OBJECTIVE: Apoptosis of pancreatic beta-cells is critical in both diabetes development and failure of islet transplantation. The role in these processes of pro- and antiapoptotic Bcl-2 family proteins, which regulate apoptosis by controlling mitochondrial integrity, remains poorly understood. We investigated the role of the BH3-only protein Bid and the multi-BH domain proapoptotic Bax and Bak, as well as prosurvival Bcl-2, in beta-cell apoptosis. RESEARCH DESIGN AND METHODS: We isolated islets from mice lacking Bid, Bax, or Bak and those overexpressing Bcl-2 and exposed them to Fas ligand, tumor necrosis factor (TNF)-alpha, and proinflammatory cytokines or cytotoxic stimuli that activate the mitochondrial apoptotic pathway (staurosporine, etoposide, gamma-radiation, tunicamycin, and thapsigargin). Nuclear fragmentation was measured by flow cytometry. RESULTS: Development and function of islets were not affected by loss of Bid, and Bid-deficient islets were as susceptible as wild-type islets to cytotoxic stimuli that cause apoptosis via the mitochondrial pathway. In contrast, Bid-deficient islets and those overexpressing antiapoptotic Bcl-2 were protected from Fas ligand-induced apoptosis. Bid-deficient islets were also resistant to apoptosis induced by TNF-alpha plus cycloheximide and were partially resistant to proinflammatory cytokine-induced death. Loss of the multi-BH domain proapoptotic Bax or Bak protected islets partially from death receptor-induced apoptosis. CONCLUSIONS: These results demonstrate that Bid is essential for death receptor-induced apoptosis of islets, similar to its demonstrated role in hepatocytes. This indicates that blocking Bid activity may be useful for protection of islets from immune-mediated attack and possibly also in other pathological states in which beta-cells are destroyed.

Clouds are one of the most influential elements of weather on the earth system, yet they are also one of the least understood. Understanding their composition and behavior at small scales is critical to understanding and predicting larger scale feedbacks. Currently, the best method to study clouds on the microscale is through airborne in situ measurements using optical instruments capable of resolving clouds on the individual particle level. However, current instruments are unable to sufficiently resolve the scales important to cloud evolution and behavior. The Holodec is a new generation of optical cloud instrument which uses digital inline holography to overcome many of the limitations of conventional instruments. However, its performance and reliability was limited due to several deficiencies in its original design. These deficiencies were addressed and corrected to advance the instrument from the prototype stage to an operational instrument. In addition, the processing software used to reconstruct and analyze digitally recorded holograms was improved upon to increase robustness and ease of use.

Die 1700 erschienene Fürstliche Bau-Lust des Herzogs Heinrich von Sachsen-Römhild (1650-1710) dokumentiert in Wort und Bild systematisch die von 1692 bis 1698 in der Residenz und ihrem näheren Umfeld errichteten Festarchitekturen und Lustbauten des Herzogs. Es ist bislang das einzig bekannt gewordene Werk seiner Art, da es sowohl auf Veranlassung des Bauherrn erschien, der zudem als Autor fungiert, als auch ausschließlich die ephemere Architektur und nicht auf das Fest selbst fokussiert. Die Bandbreite der Festbauten reicht von floralen Raumausstattungen bis hin zu mehrere Gebäude umfassenden eigenständigen architektonischen Komplexen. Architektur und Ausstattung werden ausführlich beschrieben und in Kupferstichen gezeigt. Dabei wird klar, inwiefern Festarchitekturen formal von Festanlässen abhängig sind. Ebenfalls wird deutlich, dass nicht die Architektur selbst, sondern ihre Ausstattung entscheidend für eine spezifische Aussage ist. Die einzelnen Elemente werden dabei je nach Anlass abgewandelt und immer wieder aufs Neue verwendet.

OBJECTIVES The aim was to study the impact of the defect size of endodontically treated incisors compared to dental implants as abutments on the survival of zirconia two-unit anterior cantilever-fixed partial dentures (2U-FPDs) during 10-year simulation. MATERIALS AND METHODS Human maxillary central incisors were endodontically treated and divided into three groups (n = 24): I, access cavities rebuilt with composite core; II, teeth decoronated and restored with composite; and III as II supported by fiber posts. In group IV, implants with individual zirconia abutments were used. Specimens were restored with zirconia 2U-FPDs and exposed to two sequences of thermal cycling and mechanical loading. Statistics: Kaplan-Meier; log-rank tests. RESULTS During TCML in group I two tooth fractures and two debondings with chipping were found. Solely chippings occurred in groups II (2×), IV (2×), and III (1×). No significant different survival was found for the different abutments (p = 0.085) or FPDs (p = 0.526). Load capability differed significantly between groups I (176 N) and III (670 N), and III and IV (324 N) (p < 0.024). CONCLUSION Within the limitations of an in vitro study, it can be concluded that zirconia-framework 2U-FPDs on decoronated teeth with/without post showed comparable in vitro reliability as restorations on implants. The results indicated that restorations on teeth with only access cavity perform worse in survival and linear loading. CLINICAL RELEVANCE Even severe defects do not justify per se a replacement of this particular tooth by a dental implant from load capability point of view.

Der Aufsatz beschäftigt sich mit der 'improvisierten Choreographie' You've Changed des Schweizer Choreographen Thomas Hauert und seiner in Brüssel angesiedelten Kompanie ZOO. Mittels der Denkfigur des Schwarms werden die Funktions- und Organisationsprinzipien der schwärmenden Konstellationen, welche die Tanzenden immer wieder bilden, beschrieben und analysiert. Zusätzlich werden die kinästhetischen Übertragungsprozesse erläutert, die sich in dieser Live-Improvisation zwischen den Tanzenden ereignen. Mittels Überlegungen, die der französische Philosoph Jean-Luc Nancy in seiner Schrift singulär plural sein entwickelt hat, wird anschließend diskutiert, wie in dieser 'improvisierten Choerographie' das gemeinsame Sein beständig ausgehandelt wird, wobei eine heterogene und zugleich integrierte Gemeinschaft entsteht.

Matrix metalloproteinase (MMP)-9 is produced by the central nervous system and inflammatory cells in a variety of inflammatory conditions in both animals and humans. MMP-9 promotes inflammation, breakdown of the blood-brain barrier, and vasculitis. Because vasculitis is seen frequently in patients with coccidioidal meningitis (CM), this study evaluated the presence of MMP-9 within the cerebrospinal fluid (CSF) of rabbits infected intracisternally with Coccidioides immitis arthroconidia. Infected rabbits demonstrated systemic and neurological sequelae to infection, including CSF pleocytosis. Levels of MMP-9 within CSF were assayed by use of zymography and compared with MMP-2 levels, which served as an internal control. Elevated levels of MMP-9 were detectable by day 3, continued to increase through day 10, and declined by day 15 after infection. MMP-9 may contribute to inflammation and vasculitis in this animal model. Future work can focus on evaluation of MMP inhibitors, to gain a better perspective of the role of this MMP in CM.

OBJECTIVES Cotrimoxazole prophylactic treatment (CPT) prevents opportunistic infections in HIV-infected or HIV-exposed children, but estimates of the effectiveness in preventing malaria vary. We reviewed studies that examined the effect of CPT on incidence of malaria in children in sub-Saharan Africa. METHODS We searched PubMed and EMBASE for randomised controlled trials (RCTs) and cohort studies on the effect of CPT on incidence of malaria and mortality in children and extracted data on the prevalence of sulphadoxine-pyrimethamine resistance-conferring point mutations. Incidence rate ratios (IRR) from individual studies were combined using random effects meta-analysis; confounder-adjusted estimates were used for cohort studies. The importance of resistance was examined in meta-regression analyses. RESULTS Three RCTs and four cohort studies with 5039 children (1692 HIV-exposed; 2800 HIV-uninfected; 1486 HIV-infected) were included. Children on CPT were less likely to develop clinical malaria episodes than those without prophylaxis (combined IRR 0.37, 95% confidence interval: 0.21-0.66), but there was substantial between-study heterogeneity (I-squared = 94%, P < 0.001). The protective efficacy of CPT was highest in an RCT from Mali, where the prevalence of antifolate resistant plasmodia was low. In meta-regression analyses, there was some evidence that the efficacy of CPT declined with increasing levels of resistance. Mortality was reduced with CPT in an RCT from Zambia, but not in a cohort study from Côte d'Ivoire. CONCLUSIONS Cotrimoxazole prophylactic treatment reduces incidence of malaria and mortality in children in sub-Saharan Africa, but study designs, settings and results were heterogeneous. CPT appears to be beneficial for HIV-infected and HIV-exposed as well as HIV-uninfected children.

Objectives: The final goal in the successful treatment of schizophrenia patients is defined in improved functional recovery. Thus the integration of social cognitive tasks within a comprehensive treatment concept should offer significant advantages in generalization and transfer of therapy effects. Recent therapy outcome research supports these advantages. Empirical modeling identified social cognition as a mediating factor between neurocognition and functional recovery. Regarding this, we first developed the Integrated Psychological Therapy Program (IPT). It consists of 5 subprograms and combines interventions on neurocognition, social cognition, and social competence. As a further development of the cognitive part of IPT we developed the Integrated Neurocognitive Therapy (INT), which focuses on all social and neurocognitive domains defined by MATRICS. Methods: The aim was to investigate whether the application of the complete IPT is superior in comparison to the use of single IPT subprograms. Data were based on 37 independent IPT studies including a total sample of 1692 schizophrenia patients. Additionally, the proximal outcome in cognitive domains as well as in more distal outcome areas was investigated in an international RCT on INT including 169 schizophrenia outpatients. Results: All IPT subprogram variations obtained significant effects in proximal outcome. Each subprogram domain reached the largest effects in the targeted area. With regard to distal outcomes, combinations of subprograms showed a significant reduction of negative symptoms and an improvement in not targeted areas of functioning. This strongly supports vertical generalization effects to other functional domains. Regarding INT, results support efficacy compared to TAU in various cognitive domains, in psychosocial functioning and symptoms after therapy and at 1-year-follow-up. Conclusion: Results support evidence for the efficacy of longer lasting integrated therapy. The success of these treatment concepts is strongly based on successful therapy of social cognitive functions.

Since 9/11, 2001, a new form of religious extremism has arguably emerged, one which paradoxically portrays itself as a counter to another perceived extremism regarded as a real and imminent threat. Within North America and Western Europe, as elsewhere, there is an upsurge of various forms of reactionary rhetoric and opposition expressed towards Islam and Muslims. An increase in extremist behaviour, even violence, is appearing from quarters opposed to, or varyingly fearful of, Islamic extremism if not Islam or Muslims. Islamophobia, as a manifestation of fear of an exclusionary Islam, manifests as exclusionary or negatively reactive behaviours with Muslims and Islam as the target. This article explores the idea that Islamophobia can be regarded as a manifestation of religious extremism and, further, that such extremism is construable as “reactive co-radicalization.” It focuses on two European cases – the 2009 Swiss ban on the building of minarets and the 2011 Norwegian massacre carried out by Anders Breivik – as examples of this “reactive co-radicalization.” This term, I suggest, is an apt denominator for the exclusionary reaction to the rising presence of Islam within otherwise secular, albeit nominally Christian, Western European and North American societies, among others.
