Sandflies attracted by human bait were caught in an endemic focus of localized cutaneous leishmaniasis in the state of Campeche, Mexico. Catches were carried out monthly from February 1994 to January 1995 between 18:00 and 22:00 h. Lutzomyia cruciata was the only species caught. The highest population peak of Lu. cruciata was found in March with lesser peaks in February, December 1994, and January 1995. Maximum biting rate of Lu. cruciata was found between 18:00 and 19:00 h. The host-seeking females of Lu. cruciata were directly related to levels of humidity between 88 and 100%. Low and high temperature had a negative effect upon Lu. cruciata activity. The possible role of Lu. cruciata as vector of leishmaniasis in the state of Campeche, Mexico is discussed.

To evaluate the effect of concurrent infection by HIV on HBV infection or immunity, we have studied a group of 66 HIV1+ symptomatic Caucasian patients and another of 38 African HIV2+ asymptomatic individuals, concerning their HBV status: serological markers of infection and presence of HBV-DNA in serum, the last taken as sign of hepatitis B virus active replication, were monitored. HIV+ groups were compared with seronegative controls, adequately matched for age, sex and ethnological background. HBV DNA was found in 7.6% of HIV1+ Caucasian patients and 3.2% of seronegative controls; in African HIV2+ individuals 2.6% were also HBV DNA+, a percentage close to that found in HIV2 seronegative controls (2.9%). No correlation was found between HIV infection and HBV active replication. Immunodepression that follows HIV infection over time may be compatible with a degree of T cell function capable of avoiding reinfection with or reactivation of HBV, even in symptomatic stages of acquired immunodeficiency syndrome. Our findings are relevant to the choice of preventive strategies in populations at risk for HIV and HBV infection.

Among the determinant factors in the resistance and susceptibility of Biomphalaria to Schistosoma mansoni, hemocytes play an important role. Aiming at studying S. mansoni/Biomphalaria interactions related to hemocytes, the first step is certainly connected with the standardization of this cell population in uninfected Biomphalaria. In this way, quantification of this cell population in hemolymph, as well as its phagocitary capacity, have been determined for the first time. Furthermore, using susceptible and resistant strains of B. glabrata and B. tenagophila, the hemocytegram and phagocytary capacity of hemocytes after infection with S. mansoni were determined too. Resistant and susceptible strains of B.glabrata (BA and BH, respectively), as well as resistant and susceptible strains of B. tenagophila (Taim and CF, respectively) were infected with 10 miracidia of the LE and SJ strains of S. mansoni, respectively. These infected snails and respective uninfected controls were assessed in relation to the number of circulating hemocytes and alteration in the phagocytary capacity, by using Zymozan and MTT. Reading was taken by means of a spectrophotometer at 5 hours and 1,2,5,10,20 and 30 days after infection. The results showed a decrease in population of the circulating phagocytary cells, 5 hours after infection. One day post-infection, the circulating cells of the susceptible snails showed an increased metabolic activity, but the same event could not be observed in the resistant strains. In the subsequent observation periods, significant differences among the strains studied could not be observed until the end of the experiment

A survey for canine tegumentary leishmaniasis (CTL) has been carried out between 1986 and 1993 in seven endemic localities for American cutaneous leishmaniasis in the State of Rio de Janeiro. 270 dogs have been examined for their clinical aspects, the development of delayed hypersensitivity (DHS) with Immunoleish antigen and with immunofluorescent antibody research of IgG (IF). 28.2% of them had ulcer lesions and 3.3% had scars. The lesions consisted of single (39.5%) and mucocutaneous lesions (31.6%), multiple cutaneous (25.0%) and mucocutaneous lesions associated with cutaneous ulcers (4.0%). Twelve (15.8%) isolates from biopsies were analyzed by zimodeme and schizodeme and identified as L. (V.) braziliensis. The overall prevalence of canine infection that was evaluated with the skin test was of 40.5% and with IF it was of 25.5%. Both tests showed a high positive rate with relation to the animals with mucosal lesions, as in the case of human mucocutaneous leishmaniasis. The comparison of the two tests showed the skin test to have a better performance although there was no statistical difference (p>0.05) between them. The proportional sensitivity and specificity was of 84.0% and 74.0%, respectively. The Immunoleish skin test and IF are useful tools to be employed in CTL field epidemiological surveys.

The aim is to examine the temporal trends of hip fracture incidence in Portugal by sex and age groups, and explore the relation with anti-osteoporotic medication. From the National Hospital Discharge Database, we selected from 1st January 2000 to 31st December 2008, 77,083 hospital admissions (77.4% women) caused by osteoporotic hip fractures (low energy, patients over 49 years-age), with diagnosis codes 820.x of ICD 9-CM. The 2001 Portuguese population was used as standard to calculate direct age-standardized incidence rates (ASIR) (100,000 inhabitants). Generalized additive and linear models were used to evaluate and quantify temporal trends of age specific rates (AR), by sex. We identified 2003 as a turning point in the trend of ASIR of hip fractures in women. After 2003, the ASIR in women decreased on average by 10.3 cases/100,000 inhabitants, 95% CI (− 15.7 to − 4.8), per 100,000 anti-osteoporotic medication packages sold. For women aged 65–69 and 75–79 we identified the same turning point. However, for women aged over 80, the year 2004 marked a change in the trend, from an increase to a decrease. Among the population aged 70–74 a linear decrease of incidence rate (95% CI) was observed in both sexes, higher for women: − 28.0% (− 36.2 to − 19.5) change vs − 18.8%, (− 32.6 to − 2.3). The abrupt turning point in the trend of ASIR of hip fractures in women is compatible with an intervention, such as a medication. The trends were different according to gender and age group, but compatible with the pattern of bisphosphonates sales.

Hepatitis B has proved to be a major health hazard in hemodialysis patients. In order to investigate the hepatitis B virus (HBV) infection profile in the hemodialysis population of Goiânia city - Central Brazil, all dialysis patients (N=282) were studied. The prevalence of any HBV marker (HBsAg, anti-HBs, and anti-HBc) was 56.7% (95% CI: 51.1-62.7), ranging from 33.3% to 77.7% depending on dialysis unit. HBV-DNA was detected in 67.6% and 88.2% of the HBsAg-positive serum samples, in 91.3% and 100% of the HBsAg/HBeAg-positive samples, and in 18.2% and 63.6% of the HBsAg/anti-HBe-reactive sera by hybridization and PCR, respectively. The length of time on hemodialysis was significantly associated with HBV seropositivity. Only 10% of the patients reported received hepatitis B vaccination. The findings of a high HBV infection prevalence in this population and the increased risk for HBV infection on long-term hemodialysis suggest the environmental transmission, emphasizing the urgent need to evaluate strategies of control and prevention followed in these units.

O instável mas tendencialmente crescente preço dos combustíveis associado a preocupações ambientais cada vez mais enraizadas nas sociedades, têm vindo a despoletar uma maior atenção à procura de combustíveis alternativos. Por outro lado, várias projecções indicam um aumento muito acentuado do consumo energético global no curto prazo, fruto do aumento da população e do nível de industrialização das sociedades. Neste contexto, o biodiesel (ésteres de ácidos gordos) obtido através da transesterificação de triglicerídeos de origem vegetal ou animal, surge como a alternativa “verde” mais viável para utilização em equipamentos de combustão. A reacção de transesterificação é catalisada, por norma com recurso a catalisadores homogéneos alcalinos (NaOH ou KOH). Este tipo de processo, o único actualmente com expressão a nível industrial, apresenta algumas desvantagens que, para além de aumentarem o custo do produto final, contribuem para reduzir a benignidade do mesmo: a impossibilidade de reutilização do catalisador, o aumento do número e complexidade das etapas de separação e a produção de efluentes resultantes das referidas etapas. Com o intuito de minimizar ou eliminar estes problemas, vários catalisadores heterogéneos têm vindo a ser estudados para esta reacção. Apesar de muitos apresentarem resultados promissores, a grande maioria não tem viabilidade para aplicação industrial seja devido ao seu próprio custo, seja devido aos pré-tratamentos necessários à sua utilização. Entre estes catalisadores, o óxido de cálcio é talvez o que apresenta resultados mais promissores. O crescente número de estudos envolvendo este catalisador em detrimento de outros, é por si mesmo prova do potencial do CaO. A realização deste trabalho pretendia atingir os seguintes objectivos principais: • Avaliar a elegibilidade do óxido de cálcio enquanto catalisador da reacção de transesterificação de óleos alimentares usados com metanol; • Avaliar qual a sua influência nas características dos produtos finais; • Avaliar as diferenças de performance entre o óxido de cálcio activado em atmosfera inerte (N2) e em ar, enquanto catalisadores da reacção de transesterificação de óleos alimentares usados com metanol; • Optimizar as condições da reacção com recurso às ferramentas matemáticas disponibilizadas pelo planeamento factorial, através da variação de quatro factores chave de influência: temperatura, tempo, relação metanol / óleo e massa de catalisador utilizado. O CaO utlizado foi obtido a partir de carbonato de cálcio calcinado numa mufla a 750 °C durante 3 h. Foi posteriormente activado a 900 °C durante 2h, em atmosferas diferentes: azoto (CaO-N2) e ar (CaO-Ar). Avaliaram-se algumas propriedades dos catalisadores assim preparados, força básica, concentração de centros activos e áreas específicas, tendo-se obtido uma força básica situada entre 12 e 14 para ambos os catalisadores, uma concentração de centros activos de 0,0698 mmol/g e 0,0629 mmol/g e áreas específicas de 10 m2/g e 11 m2/g respectivamente para o CaO-N2 e CaO-Ar. Efectuou-se a transesterificação, com catálise homogénea, da mistura de óleos usados utilizada neste trabalho com o objectivo de determinar os limites para o teor de FAME’s (abreviatura do Inglês de Fatty Acid Methyl Esters’) que se poderiam obter. Foi este o parâmetro avaliado em cada uma das amostras obtidas por catálise heterogénea. Os planos factoriais realizados tiveram como objectivo maximizar a sua quantidade recorrendo à relação ideal entre tempo de reacção, temperatura, massa de catalisador e quantidade de metanol. Verificou-se que o valor máximo de FAME’s obtidos a partir deste óleo estava situado ligeiramente acima dos 95 % (m/m). Realizaram-se três planos factoriais com cada um dos catalisadores de CaO até à obtenção das condições óptimas para a reacção. Não se verificou influência significativa da relação entre a quantidade de metanol e a massa de óleo na gama de valores estudada, pelo que se fixou o valor deste factor em 35 ml de metanol / 85g de óleo (relação molar aproximada de 8:1). Verificou-se a elegibilidade do CaO enquanto catalisador para a reacção estudada, não se tendo observado diferenças significativas entre a performance do CaO-N2 e do CaO-Ar. Identificaram-se as condições óptimas para a reacção como sendo os valores de 59 °C para a temperatura, 3h para o tempo e 1,4 % de massa de catalisador relativamente à massa de óleo. Nas referidas condições, obtiveram-se produtos com um teor de FAME’s de 95,7 % na catálise com CaO-N2 e 95,3 % na catálise com CaO-Ar. Alguns autores de estudos consultados no desenvolvimento do presente trabalho, referiam como principal problema da utilização do CaO, a lixiviação de cálcio para os produtos obtidos. Este facto foi confirmado no presente trabalho e na tentativa de o contornar, tentou-se promover a carbonatação do cálcio com a passagem de ar comprimido através dos produtos e subsequente filtração. Após a realização deste tratamento, não mais se observaram alterações nas suas propriedades (aparecimento de turvação ou precipitados), no entanto, nos produtos obtidos nas condições óptimas, a concentração de cálcio determinada foi de 527 mg/kg no produto da reacção catalisada com CaO-N2 e 475 mg/kg com CaO-A. O óxido de cálcio apresentou-se como um excelente catalisador na transesterificação da mistura de óleos alimentares usados utilizada no presente trabalho, apresentando uma performance ao nível da obtida por catálise homogénea básica. Não se observaram diferenças significativas de performance entre o CaO-N2 e o CaO-Ar, sendo possível obter nas mesmas condições reaccionais produtos com teores de FAME’s superiores a 95 % utilizando qualquer um deles como catalisador. O elevado teor de cálcio lixiviado observado nos produtos, apresenta-se como o principal obstáculo à aplicação a nível industrial do óxido de cálcio como catalisador para a transesterificação de óleos.

RESUMO: O novo cenário demográfico origina mudanças na estrutura etária, nas relações familiares e de trabalho, nas exigências de cuidados de saúde e de protecção social. Estas constituem fortes desafios a enfrentar pelas políticas públicas, exigindo a preparação dos organismos responsáveis. O “envelhecimento activo” começa a ter lugar como paradigma de intervenção, representando já um marco político mundial, na medida em que pode contrariar ou inverter as tendências nefastas das novas transformações. A investigação incidiu numa reflexão em torno das políticas que procuram responder à intensificação e diversidade dos problemas inerentes ao envelhecimento populacional. Envolvemo-nos numa tarefa de análise detalhada de programas municipais em três concelhos da Região Interior de Portugal, segundo a perspectiva e os determinantes do “envelhecimento activo”. Embora não exista uma solução única e definitiva, se nos detivermos no conceito como estratégia global, conclui-se o dever de considerar diversos determinantes numa perspectiva integrada e individualizada, que percorra todo o curso de vida. Os programas devem procurar enfatizar estilos de vida saudáveis com vista num alcance da maior qualidade de vida possível e ter em conta as situações sócio-económicas, mas sem se afastarem do dever de reforçar a componente reparadora e inclusiva da velhice.-----------ABSTRACT: The new demographic scenario creates changes in the age structure, in the family and work relations, in healthcare and social protection demands. All these changes create a great challenge to public policies, demanding the preparation of the responsible organizations. The “active ageing” starts to be seen as the intervention model, which represents a milestone in global policies as it can counteract or reverse the negative trends of these demographic changes. This research focused on the policies that seek to respond to the intensity and diversity of the population aging problems in Portugal. It was produced a detailed analysis of three local programs following the mainstreaming of “active ageing” in interior country. Although there is no unique and definitive solution, if we take the concept as a global strategy, then we can conclude the obligation to consider several determinants in an integrated and individualized perspective, which covers the entire course of life. These programs should emphasize healthy lifestyles aiming the highest quality of life possible taking into account the socioeconomic situations, without disregarding the duty of reinforce the repairing and inclusive component of ageing.

This study was carried out in order to obtain base-line data concerning the epidemiology of American Visceral Leishmaniasis and Chagas Disease in an indigenous population with whom the government is starting a dwelling improvement programme. Information was collected from 242 dwellings (1,440 people), by means of house to house interviews about socio-economic and environmental factors associated with Leishmania chagasi and Trypanosoma cruzi transmission risk. A leishmanin skin test was applied to 385 people and 454 blood samples were collected on filter paper in order to detect L. chagasi antibodies by ELISA and IFAT and T. cruzi antibodies by ELISA. T. cruzi seroprevalence was 8.7% by ELISA, L. chagasi was 4.6% and 5.1% by IFAT and ELISA, respectively. ELISA sensitivity and specificity for L. chagasi antibodies were 57% and 97.5% respectively, as compared to the IFAT. Leishmanin skin test positivity was 19%. L. chagasi infection prevalence, being defined as a positive result in the three-immunodiagnostic tests, was 17.1%. Additionally, 2.7% of the population studied was positive to both L. chagasi and T. cruzi, showing a possible cross-reaction. L. chagasi and T. cruzi seropositivity increased with age, while no association with gender was observed. Age (p<0.007), number of inhabitants (p<0.05), floor material (p<0.03) and recognition of vector (p<0.01) were associated with T. cruzi infection, whilst age ( p<0.007) and dwelling improvement (p<0.02) were associated with L. chagasi infection. It is necessary to evaluate the long-term impact of the dwelling improvement programme on these parasitic infections in this community.

Between 1992 and 1997, 790 blood donors with anti-HCV EIA-2 strongly reagent (relationship between the sample optical density/cut-off > 3) detected at the blood bank serological screening, were evaluated in ambulatory environment. They were all negative for Chagas disease, syphilis, hepatitis B (HBsAg) and AIDS. Blood samples were collected at the first ambulatorial evaluation, for hemogram, biochemical tests and new serological tests for HCV (anti-HCV EIA-2). In blood samples of 226 repeatedly reagent anti-HCV EIA-2 blood donors, supplementary "immunoblot" test for HCV (RIBA-2) was used. In 209 donors, the presence of HCV-RNA was investigated by the PCR test. The abdominal ultrasonography was realized in 366 donors. In 269 patients liver biopsy was performed for the histopathological study. The follow-up of blood donors showed that 95.6% were repeatedly EIA-2 reagent, 94% were symptomless and denied any hepatitis history, with only 2% mentioning previous jaundice. In 47% of this population at least one risk factor has been detected for the HCV transmission, the use of intravenous drugs being the main one (27.8%). Blood transfusion was the second factor for HCV transmission (27.2%). Hepatomegaly was detected in 54% of the cases. Splenomegaly and signs of portal hypertension have seldom been found in the physical examination, indicating a low degree of hepatic compromising in HCV. Abdominal ultrasound showed alterations in 65% of the subjects, being the steatosis the most frequent (50%). In 83.5% of the donors submitted to the liver biopsy, the histopathological exam showed the presence of chronic hepatitis, usually classified as active (89%) with mild or moderate grade in most of the cases (99.5%). The histopathological exam of the liver was normal in 1.5% of blood donors. The RIBA-2 test and the HCV-RNA investigation by PCR were positive in respectively 91.6 and 75% of the anti-HCV EIA-2 reagent donors. The HCV-RNA research was positive in 82% of the RIBA-2 positive subjects, in 37.5% of the indeterminate RIBA-2 donors and in 9% of the negative RIBA-2 donors. Chronic hepatitis has also been observed in 50% of the histopathological exams of the anti-HCV EIA-2 reagent donors which were indeterminate RIBA-2. Among 18 blood donors with minimal changes histopathological exam 11 (61%) were HCV-RNA positive. Our blood donors anti-HCV reagent generally had clinical, laboratorial and histopathological features observed in patients with chronic HCV hepatitis and a high proportion could be identified in interviews and medical evaluation realized in blood blanks. Generally, these HCV infected donors are identified and discharged only by the serological tests results.

Advanced glycation end-products are Maillard reaction products that are found in thermal processed food. This compounds are often referred as unhealthy for human diet, namely because of their capacity to form amino-acid dimers. There is a broad range of answers to get about how these products are formed, how they interact with the organism and how these reactions can be inhibited to prevent the referred effects. Some compounds from garlic are thought to be able to inhibit these reactions. This study using spectrophotometric, High Performance Liquid Chromatography-Tandem Mass Spectrometry (HPLC-MS/MS) and Fourier transformed infrared spectroscopy (FTIR) analysis, helps to understand better not only not only the effect of some compounds obtained from garlic, diallyl sulfide (DAS), diallyl disulfide (DADS) and diallyl trisulfide (DATS), on these AGEs production reaction, but also helped to understand better the reaction itself.

RESUMO: Num quadro de profundas transformações demográficas, o envelhecimento populacional coloca muitas questões à sociedade. A promoção da saúde e de comportamentos saudáveis desempenha um papel importante numa perspectiva de Envelhecimento Activo. Hoje em dia existem inúmeras evidências científicas de que a Actividade Física regular tem importantes benefícios, tanto a nível físico como psicológico, com um impacto significativo no bem-estar e na qualidade de vida das pessoas em todas as idades. Para fomentar a prática de Actividade Física há um envolvimento cada vez maior dos profissionais de saúde, visto serem figuras de referência para os pacientes. Desta forma, foi objectivo do nosso estudo verificar a existência de uma associação positiva entre a prática de Actividade Física e a Consultoria em Actividade Física, baseado nas seguintes variáveis: análise sócio-demográfica dos indivíduos; percepções e práticas de Actividade Física. Para tal estudámos uma amostra de conveniência, seleccionada a partir dos utentes de ambulatório das Unidades de Saúde Familiar de Benfica e Carnide, constituída por 5 pessoas, todas do sexo feminino, com idades compreendidas entre os 50 e os 65 anos, que se encontravam clinicamente estáveis. Os instrumentos de avaliação utilizados foram os seguintes: Entrevista Semi- Estruturada Inicial e Final, Questionário Internacional de Actividade Física – Versão Longa, e Questionário de Satisfação. Com os resultados obtidos podemos concluir que a Consultoria em Actividade Física contribuiu para o aumento da consciência das pessoas para a necessidade de movimentos e de Actividade Física para melhorar a saúde a nível físico, psicológico e social.-------------- ABSTRACT: In a context of profound demographic changes, population aging poses many questions to society. Health promotion and healthy behaviors play an important role in an Active Ageing perspective. Nowadays there are plenty of scientific evidence that regular physical activity has important benefits, both physically and psychologically, with significant impact on the wellbeing and quality of life for people of all ages. To encourage the practice of physical activity there is an increasing involvement of health professionals, because they are reference figures for the patients. Thus, the aim of our study was to verify the existence of a positive association between the practice of Physical Activity and Physical Activity Consulting, based on the following variables: social demographic features of individuals, perceptions and practices of Physical Activity. To this end we studied a convenience sample, selected from outpatients of Family Health Units in Benfica and Carnide, consisting of 5 persons, all female, aged between 50 and 65, who were clinically stable. The assessment instruments used were the following: Initial and Final Semi-Structured Interview, International Physical Activity Questionnaire - Long Version, and Satisfaction Questionnaire. With these results we can conclude that Physical Activity Consulting contributed to increase people's awareness of the need for movement and physical activity, in order to improve health at the physical, psychological and social level.

Considering the impact of cysticercosis on public health, especially the neurologic form of the disease, neurocysticercosis (NC), we studied the frequency of positivity of anti-Taenia solium cysticercus antibodies in serum samples from 1,863 inhabitants of Cássia dos Coqueiros, SP, a municipal district located 80 km from Ribeirão Preto, an area considered endemic for cysticercosis. The 1,863 samples were tested by enzyme linked immunosorbent assay (ELISA) using an antigenic extract from Taenia crassiceps vesicular fluid (Tcra). The reactive and inconclusive ELISA samples were tested by immunoblotting. Of the 459 samples submitted to immunoblotting, 40 were strongly immunoreactive to the immunodominant 18 and 14 kD peptides. Considering the use of immunoblotting as confirmatory due to its high specificity, the anti-cysticercus serum prevalence in this population was 2.1%.
