Introdução: A Reabilitação cardíaca (Rc) é um elemento fundamental na prevenção secundária após síndrome coronária aguda (ScA). A American Association of Cardiovascular and Pulmonary Rehabilitation (AAcPR)definiu critérios de estratificação de risco para os doentes elegíveis para programas de Rc. Contudo, tem sido questionada a integração de doentes de baixo risco em programas estruturados de treino de exercício. Objectivo: comparar o impacto da Rc nos doentes de baixo risco cardiovascular versus risco moderado/alto, segundo os critérios a AAcPR, de forma a avaliar o real benefício da Rc na capacidade de exercício nos doentes de baixo risco, quantificada por prova de esforço cardiorrespiratória (PecR). Métodos: Análise retrospetiva dos doentes submetidos a Rc após ScA e submetidos a PecR antes e após a integração num programa de treino de exercício supervisionado constituído por 36 sessões. Foram incluídos doentes desde janeiro de 2004 a dezembro de 2013, num centro Hospitalar Terciário. Os doentes foram divididos em dois grupos: grupo de risco cardiovascular baixo (GRB) e grupo de doentes com risco moderado a alto (GRMA) de acordo com os critérios da AAcPR. Os parâmetros avaliados na PecR foram: pico de consumo de oxigénio (pVO2), pVO2 em relação ao valor previsto para idade e género (%pVO2), declive da rampa do equivalente ventilatório de dióxido de carbono (Ve/VcO2slope), (Ve/VcO2slope)/pVO2 e potência circulatória de pico (PcP). Resultados: Foram incluídos em programa de Rc, pós ScA, 129 doentes, 86,0% do género masculino, com idade média de 56,3±9,8 anos. Setenta e nove doentes (61,3%) foram incluídos no grupo de baixo risco e 50 doentes (38,7%) no grupo de risco moderado a alto.Comparando os resultados da PecR basal verifica-se uma melhor capacidade funcional nos indivíduos de baixo risco (pVO2 26,7±7,0 versus 23,9±5,7 ml/kg/min; p=0,019). Esta diferença significativa desapareceu após a conclusão do programa de Rc, apresentando o GRB pVO2 final de 28,5±7,3ml/kg/min e o GRMA 27,0±7,0ml/kg/min (p=0,232). Ao confrontar os parâmetros da prova de esforço cardiorrespiratória prévios e após reabilitação cardíaca, verifica-se, em ambos os grupos, um aumento significativo da capacidade funcional expresso pelo aumento do pVO2, (Ve/VcO2slope)/pVO2, PcP e duração da prova. No entanto, apenas no grupo de baixo risco se evidencia uma diminuição signi-ficativa do Ve/VcO2 slope (26,7±6,2 versus 25,7±5,3; p=0,029). A amplitude da melhoria de pVO2 foi menos marcada no GRB (1,8±6,5 ml/kg/min versus 3,1±5,0 ml/kg/min; p=0.133). Um incremento no pVO2 superior a 10% em relação ao valor inicial foi atingido em 41,8% dos doentes no GRB e 58,0% dos doentes no GRMA (p=0,072). Conclusão: independentemente do grau de risco cardiovascular inicial, existe benefício na capacidade funcional de exercício após programa de Rc com 36 sessões de treino de exercício, objetivamente quantificado pelos parâmetros da PecR. No entanto, esta melhoria é mais acentuada nos indivíduos de risco moderado a alto comparativamente aos indivíduos de baixo risco. Atendendo à limitação de recursos, deverá ser privilegiada a inclusão de indivíduos de moderado e alto risco, não subvalorizando, no entanto, o benefício também alcançado pelos indivíduos de baixo risco cardiovascular.

Foram estudados 259 casos de donovanose registrados no Serviço de Dermatologia do Departamento de Medicina Tropical/UFPA entre 1954-1990. Observa-se que no período 1954-1974 existiam nos arquivos apenas 56 prontuários da doença, enquanto que no último quinquénio estudado (1986-1990) foram identificados 133 casos. Para os autores esse crescimento está ligado a excessiva liberalidade sexual, ao homossexualismo e as precárias condições sócio-econômicas, acentuada nos últimos tempos.

RESUMO - A Terapêutica de Ressincronização Cardíaca (TRC) apresenta benefícios significativos na classe funcional, função ventricular, hospitalização e mortalidade. É uma técnica com custos elevados e, com os actuais métodos de selecção de doentes, a taxa de não-respondedores ronda os 30%. Objectivo: Compreender se a inclusão da dessincronia mecânica (DM) na selecção de doentes para TRC contribui para a sua relação custo-efectividade, na perspectiva do Serviço Nacional de Saúde português. Metodologia: Estudo prospectivo baseado em coortes histórias de 12 meses de dois grupos submetidos a TRC com desfibrilhador, o grupo de intervenção com doentes seleccionados com inclusão da DM (n=133) e o de controlo com selecção baseada exclusivamente nas recomendações internacionais (n=71). Reuniram-se dados clínicos e de custos nos 12 meses subsequentes à implantação, para cálculo do rácio custo-efectividade incremental (RCEI). Resultados: O grupo de intervenção apresentou uma sobrevivência de 91% e o de controlo de 93%, aos 12 meses (p=0,335). O grupo de intervenção apresentou 60 re-internamentos e o de controlo 46 re-internamentos por qualquer causa aos 12 meses (p=0,032), com RCEI=6.886,09€/re-internamento evitado. O grupo de intervenção apresentou 19 re-internamentos e o de controlo apresentou 31 re-internamentos por Insuficiência Cardíaca (IC) descompensada aos 12 meses (p<0,001), com RCEI=2.686,26€/re-internamento por IC descompensada evitado. Relativamente à melhoria da classe funcional e da fracção de ejecção não foi possível estabelecer associações com custos (p>0,05). Conclusão: É seguro afirmar que recomendar a selecção com inclusão da DM, nos hospitais com capacidade instalada, é um passo positivo na redução de custos com re-internamentos de doentes com TRC.

Endemic pemphigus foliaceus, and long-term corticotherapy may affect serum lipid levels. The aim of this study was to compare serum lipids of pemphigus foliaceus patients on glucocorticoid therapy to a healthy control group. Fifteen patients receiving prednisone (0.33 ± 0.22mg/kg) for at least 12 months and 15 controls were submitted to 48-h food intake records, anthropometry, and biochemical measurements. Data were compared by chi2, Mann-Whitney and Student "t" tests. The groups were matched for gender, age, weight, body mass index, arm circumference and triceps skin fold. No differences were observed in relation to energy, fat, protein and carbohydrate daily intakes, total cholesterol, HDL, LDL, uric acid, and serum creatinine levels. Pemphigus foliaceus patients had higher triglyceride [159 (64-371) vs. 100 (45-133) mg/dl], VLDL [32 (13-74) vs. 20 (9-114) mg/dl] and ESR [44 (9-87) vs. 7 (1-30) mm/h] levels than controls, probably due to metabolic effects of inflammatory disease and corticotherapy.

This report describes the prevalence of anti-HAV antibodies in children from elementary school in the Municipality of Vila Velha, ES, Brazil. Anti-HAV antibodies were investigated by ELISA method in the serum of 606 children (four to fourteen years old) from three elementary schools, located in neighborhoods with varying household monthly income levels: São José School, 200 chidren, household income higher than US$700; São Torquato School, 273 children, US$200 to 300; and Cobi School, 133 children, less than US$200. From each children data on age, gender, skin color, sanitary conditions, frequency of contact with sea or river water and family history of hepatitis were recorded. Anti-HAV antibodies were present in 38.6% of all children, 9% in São José School, 49.1% in São Torquato School and 61.7% in Cobi School. Logistic regression analysis demonstrated a positive correlation of positive anti-HAV test with age, non white color of the skin, absence of sewage treatment and domestic water filter, and a past history of hepatitis. The prevalence of anti-HAV antibodies in school children in Vila Velha, ES, was lower than that observed in the same age group in North and Northeast Brazil and was significantly higher in children from families with low socioeconomic status. In addition the results indicate a changing epidemiologic pattern of hepatitis A in our country, with an increasing number of children and adolescents with high risk for HAV infection, mainly in high socioeconomic class. A consideration must be given to the feasibility of vaccination programs for children and adolescents in our country.

The efficacy of treatment with nifurtimox and/or benznidazole among adults with chronic Chagas disease with no previous electrocardiographic disturbances was evaluated over a mean follow-up of 21 years, by means of conventional serology, xenodiagnosis, clinical examination, electrocardiograms and chest X-ray. One hundred and eleven patients, between 17 and 46 years old, were studied: 54 underwent treatment (nifurtimox 27, benznidazole 27) and 57 remained untreated (control group). Xenodiagnosis was performed on 65% of them: 36/38 of the treated and 9/34 of the untreated patients had previous positive xenodiagnosis. Post-treatment, 133 xenodiagnoses were performed on 41 patients, all resulting negative. In the control group, 29 xenodiagnoses were performed on 14 patients; 2 resulted positive. Sera stored during the follow-up were simultaneously analyzed through conventional serology tests (IHA; DA-2ME; IIF). The serological evolution in the treated group was: a) 37% underwent negative seroconversion (nifurtimox 11, benznidazole 9); b) 27.8% decreased titers (nifurtimox 9, benznidazole 6), 9 showed inconclusive final serology (nifurtimox 7, benznidazole 2); c) 35.2% remained positive with constant titers (nifurtimox 7; benznidazole 12). The control group conserved the initial antibody levels during the follow-up. In the clinical evolution, 2/54 (3.7%) of the treated and 9/57 (15.8%) of the untreated patients showed electrocardiographic disturbances attributable to Chagas myocardiopathy, with a statistically relevant difference (p<0.05). Treatment caused deparasitation in at least 37% of the chronically infected adults and a protective effect on their clinical evolution.

Neste estudo, foi estimada a prevalência da infecção pelo HTLV 1/2 em gestantes no Estado de Mato Grosso do Sul, por meio das técnicas ELISA, Western Blot e PCR, em amostras de sangue obtidas por punção venosa periférica. Foram examinadas 116.689 gestantes, sendo diagnosticadas 153 infectadas pelo HTLV 1/2, com prevalência de 0,13%. Deste total, 133 (86,9%) eram do tipo 1 e 20 (11,1%) do tipo 2. Das 153 gestantes, 73,2% eram negras, pardas ou índias, cerca de 90% tinham atividades domésticas e 75,8% (116/153) tinham 7 anos ou menos de escolaridade. As 153 gestantes tiveram 172 gestações, durante o período do estudo, sendo que 164 tiveram acompanhamento. Das gestações acompanhadas, 6,7% (11/164) evoluíram para aborto, 26,8% (41/153) gestantes relataram abortos anteriores, sendo que 31,7% (13/41) apresentaram mais de dois abortos. Co-morbidades foram detectadas em 17% (26/153) sendo 3,3% (5/153) com HIV (p<0,000002). Os autores enfatizam a importância da identificação das gestantes infectadas pelo HTLV1/2 na estratégia de controle e prevenção da doença.

INTRODUCTION: Bats are one of the most important reservoirs and vectors of the rabies virus in the world. METHODS: From 1988 to 2003, the Zoonosis Control Center in São Paulo City performed rabies diagnosis on 5,670 bats by direct immunofluorescent test and mouse inoculation test. Blood samples were collected from 1,618 bats and the sera were analyzed using the rapid fluorescent focus inhibition test to confirm rabies antibodies. RESULTS: Forty-four (0.8%) bats were positive for rabies. The prevalence of rabies antibodies was 5.9% using 0.5IU/ml as a cutoff. Insectivorous bats (69.8%) and bats of the species Molossus molossus (51.8%) constituted the majority of the sample; however, the highest prevalence of antibodies were observed in Glossophaga soricina (14/133), Histiotus velatus (16/60), Desmodus rotundus (8/66), Artibeus lituratus (5/54), Nyctinomops macrotis (3/23), Tadarida brasiliensis (3/48), Carollia perspicillata (3/9), Eumops auripendulus (2/30), Nyctinomops laticaudatus (2/16), Sturnira lilium (2/17) and Eumops perotis (1/13). The prevalence of rabies antibodies was analyzed by species, food preference and sex. CONCLUSIONS: The expressive levels of antibodies associated with the low virus positivity verified in these bats indicate that rabies virus circulates actively among them.

INTRODUCTION: Children are an important high-risk group for helminth and protozoa infections. Daycare centers are environments where children have proven to be more susceptible to acquiring intestinal parasites. Thus, the purpose of this study was to verify the prevalence of intestinal parasites in children who attended the two daycare centers maintained by the local government of Uberlândia, State of Minas Gerais, Brazil. METHODS: Fecal samples were collected from 133 children (73 children at the Public Preschool for Early Childhood Education, PPECE A, and 60 at the PPECE B) following identification according to sex and age and agreement to participate by parents or guardians who signed the free, informed consent form. The samples were examined by the Lutz method. RESULTS: Coproparasitological tests performed on 133 children showed that 29.3% of them were parasitized for enteroparasites or commensals, 6.7% of the children presented polyparasitism. Among the protozoa, Giardia lamblia were the most prevalent and Hymenolepis nana were the most frequent among the helminths. CONCLUSIONS: Thus, analysis of the results showed that intestinal parasites still represent a public health problem, especially among children and in areas where the socioeconomic and educational conditions are less favorable.

Introduction: This study aimed to inventory the phlebotomine sandfly fauna present in the urban area of Juiz de Fora, with an emphasis on the genus Lutzomyia. Methods: Capture was performed from March to September 2012, using HP light traps placed at peridomestic sites, in a municipal kennel and a forest biome. Results: A total of 133 specimens were captured, representing eight species of the genus Lutzomyia. Lutzomyia pascalei was the most prevalent species. Conclusions: This research provides an inventory and description of the spatial locations of the phlebotomine sandfly fauna of Juiz de Fora.

Introduction Saint Louis encephalitis virus (SLEV) primarily occurs in the Americas and produces disease predominantly in humans. This study investigated the serological presence of SLEV in nonhuman primates and horses from southern Brazil. Methods From June 2004 to December 2005, sera from 133 monkeys (Alouatta caraya, n=43; Sapajus nigritus, n=64; Sapajus cay, n=26) trap-captured at the Paraná River basin region and 23 blood samples from farm horses were obtained and used for the serological detection of a panel of 19 arboviruses. All samples were analyzed in a hemagglutination inhibition (HI) assay; positive monkey samples were confirmed in a mouse neutralization test (MNT). Additionally, all blood samples were inoculated into C6/36 cell culture for viral isolation. Results Positive seroreactivity was only observed for SLEV. A prevalence of SLEV antibodies in sera was detected in Alouatta caraya (11.6%; 5/43), Sapajus nigritus (12.5%; 8/64), and S. cay (30.8%; 8/26) monkeys with the HI assay. Of the monkeys, 2.3% (1/42) of A. caraya, 6.3% 94/64) of S. nigritus, and 15.4% (4/26) of S. cay were positive for SLEV in the MNT. Additionally, SLEV antibodies were detected by HI in 39.1% (9/23) of the horses evaluated in this study. Arboviruses were not isolated from any blood sample. Conclusions These results confirmed the presence of SLEV in nonhuman primates and horses from southern Brazil. These findings most likely represent the first detection of this virus in nonhuman primates beyond the Amazon region. The detection of SLEV in animals within a geographical region distant from the Amazon basin suggests that there may be widespread and undiagnosed dissemination of this disease in Brazil.

INTRODUCTION: This study aimed to describe the epidemiology of congenital and maternal syphilis in the Brazilian Federal District in 2010. METHODS: A retrospective descriptive study was conducted on the basis of the cases recorded in the System of Notifiable Disease Information. RESULTS: The study population comprised 133 cases of congenital syphilis; of these, 116 (52.6%) mothers received prenatal care, and 70 (60.4%) were diagnosed with syphilis during pregnancy. Only 1 mother was adequately treated, and 100 (75.2%) of the pregnant women's partners did not undergo treatment for syphilis. CONCLUSIONS: Although mothers attended prenatal care, not all were diagnosed during pregnancy or received adequate treatment for syphilis, as their partners did not undergo treatment for syphilis.

INTRODUCTION: Strongyloides stercoralis is a soil-transmitted helminth that produces an infection that can persist for decades. The relationships between certain clinical conditions and strongyloidiasis remains controversial. This study aims to identify the clinical conditions associated with intestinal strongyloidiasis at a reference center for infectious diseases in Rio de Janeiro, Brazil. METHODS: The clinical conditions that were assessed included HIV/AIDS, HTLV infection, cardiovascular diseases, diabetes, obstructive respiratory diseases, viral hepatitis, tuberculosis, cancer, chronic renal disease, nutritional/metabolic disorders, psychiatric conditions, rheumatic diseases and dermatologic diseases. We compared 167 S. stercoralis-positive and 133 S. stercoralis-negative patients. RESULTS: After controlling for sex (male/female OR = 2.29; 95% (CI): (1.42 - 3.70), rheumatic diseases remained significantly associated with intestinal strongyloidiasis (OR: 4.96; 95% CI: 1.34-18.37) in a multiple logistic regression model. With respect to leukocyte counts, patients with strongyloidiasis presented with significantly higher relative eosinophil (10.32% ± 7.2 vs. 4.23% ± 2.92) and monocyte (8.49% ± 7.25 vs. 5.39% ± 4.31) counts and lower segmented neutrophil (52.85% ± 15.31 vs. 61.32% ± 11.4) and lymphocyte counts (28.11% ± 9.72 vs. 30.90% ± 9.51) than S. stercoralis-negative patients. CONCLUSIONS: Strongyloidiasis should be routinely investigated in hospitalized patients with complex conditions facilitate the treatment of patients who will undergo immunosuppressive therapy. Diagnoses should be determined through the use of appropriate parasitological methods, such as the Baermann-Moraes technique.

Consumer relations, established between the Consumer and the Creditor, which carry a consequent inequality of contractual positioning between the parties, have been pushing the legislator to adopt more rigid regulations with regard to lending for the purchase of goods or services of consum issues. In this sense, the Decree-Law 359/91 was approved, meanwhile repealed by the Decree-Law 133/2009, which regulates the consumer credit agreement’s regime in the portuguese legal system. Through this contract, the financier makes available to the consumer a certain amount of money, which the consumer must repay, plus the respective remuneration (interest) and other charges, according to a refund plan agreed by the parties. The consumer will be in delay if he breaches this stipulation. In case of default, the creditor, notwithstanding, can choose to wait for the performance by the debtor, promote the loss of benefit of the term or the termination of the contract. From the outset it would seem that, in one way or another, the financier, by imposing a forced shortening of the contract duration initially agreed, will lose the right to remuneration for the provision of capital agreed, but not verified. Nevertheless, unlike presently, the previous regime allowed the parties to rule otherwise, being permitted to agree to the payment of interest of outstanding installments. On the other hand, in the consumer credit contract the principle of freedom of contractual provision of the parties is strongly mitigated by the special legislation, which prevents the waiver of rights by the consumer, and by the regime of general contractual terms, which restricts the freedom of the financier to stipulate the contractual content freely and the freedom of the consumer to negotiate. For all these reasons, associated with the growing need of credit resource to satisfy their needs of consumption, it is confirm the relevance of legislative intervention on consumers protection in the context of hiring credit.
