Type 2 diabetes mellitus results from the complex association of insulin resistance and pancreatic beta-cell failure. Obesity is the main risk factor for type 2 diabetes mellitus, and recent studies have shown that, in diet-induced obesity, the hypothalamus becomes inflamed and dysfunctional, resulting in the loss of the perfect coupling between caloric intake and energy expenditure. Because pancreatic beta-cell function is, in part, under the control of the autonomic nervous system, we evaluated the role of hypothalamic inflammation in pancreatic islet function. In diet-induced obesity, the earliest markers of hypothalamic inflammation are present at 8 weeks after the beginning of the high fat diet; similarly, the loss of the first phase of insulin secretion is detected at the same time point and is restored following sympathectomy. Intracerebroventricular injection of a low dose of tumor necrosis factor a leads to a dysfunctional increase in insulin secretion and activates the expression of a number of markers of apoptosis in pancreatic islets. In addition, the injection of stearic acid intracerebroventricularly, which leads to hypothalamic inflammation through the activation of tau-like receptor-4 and endoplasmic reticulum stress, produces an impairment of insulin secretion, accompanied by increased expression of markers of apoptosis. The defective insulin secretion, in this case, is partially dependent on sympathetic signal-induced peroxisome proliferator receptor-gamma coactivator Delta a and uncoupling protein-2 expression and is restored after sympathectomy or following PGC1 alpha expression inhibition by an antisense oligonucleotide. Thus, the autonomic signals generated in concert with hypothalamic inflammation can impair pancreatic islet function, a phenomenon that may explain the early link between obesity and defective insulin secretion.

The metastable phase diagram of the BCC-based ordering equilibria in the Fe-Al-Mo system has been calculated via a truncated cluster expansion, through the combination of Full-Potential-Linear augmented Plane Wave (FP-LAPW) electronic structure calculations and of Cluster Variation Method (CVM) thermodynamic calculations in the irregular tetrahedron approximation. Four isothermal sections at 1750 K, 2000 K, 2250 K and 2500 K are calculated and correlated with recently published experimental data on the system. The results confirm that the critical temperature for the order-disorder equilibrium between Fe(3)Al-D0(3) and FeAl-B2 is increased by Mo additions, while the critical temperature for the FeAl-B2/A2 equilibrium is kept approximately invariant with increasing Mo contents. The stabilization of the Al-rich A2 phase in equilibrium with overstoichiometric B2-(Fe,Mo)Al is also consistent with the attribution of the A2 structure to the tau(2) phase, stable at high temperatures in overstoichiometric B2-FeAl. Habitually, capuchin monkeys access encased hard foods by using their canines and premolars and/or by pounding the food on hard surfaces. Instead, the wild bearded capuchins (Cebus libidinosus) of Boa Vista (Brazil) routinely crack palm fruits with tools. We measured size, weight, structure, and peak-force-at-failure of the four palm fruit species most frequently processed with tools by wild capuchin monkeys living in Boa Vista. Moreover, for each nut species we identify whether peak-force-at-failure was consistently associated with greater weight/volume, endocarp, thickness, and structural complexity. The goals of this study were (a) to investigate whether these palm fruits are difficult, or impossible, to access other than with tools and (b) to collect data on the physical properties of palm fruits that are comparable to those available for the nuts cracked open with tools by wild chimpanzees. Results showed that the four nut species differ in terms of peak-force-at-failure and that peak-force-at-failure is positively associated with greater weight (and consequently volume) and apparently with structural complexity (i.e. more kernels and thus more partitions); finally for three out of four nut species shell thickness is also positively associated with greater volume. The finding that the nuts exploited by capuchins with tools have very high resistance values support the idea that tool use is indeed mandatory to crack them open. Finally, the peak-force-at-failure of the piassava nuts is similar to that reported for the very tough panda nuts cracked open by wild chimpanzees; this highlights the ecological importance of tool use for exploiting high resistance foods in this capuchin species.

A correlation between the physicochemical properties of mono- [Li(I), K(I), Na(I)] and divalent [Cd(II), Cu(II), Mn(II), Ni(II), Co(II), Zn(II), Mg(II), Ca(II)] metal cations and their toxicity (evaluated by the free ion median effective concentration. EC50(F)) to the naturally bioluminescent fungus Gerronema viridilucens has been studied using the quantitative ion character activity relationship (QICAR) approach. Among the 11 ionic parameters used in the current study, a univariate model based on the covalent index (X(m)(2)r) proved to be the most adequate for prediction of fungal metal toxicity evaluated by the logarithm of free ion median effective concentration (log EC50(F)): log EC50(F) = 4.243 (+/-0.243) -1.268 (+/-0.125).X(m)(2)r (adj-R(2) = 0.9113, Alkaike information criterion [AIC] = 60.42). Additional two- and three-variable models were also tested and proved less suitable to fit the experimental data. These results indicate that covalent bonding is a good indicator of metal inherent toxicity to bioluminescent fungi. Furthermore, the toxicity of additional metal ions [Ag(I), Cs(I), Sr(II), Ba(II), Fe(II), Hg(II), and Pb(II)] to G. viridilucens was predicted, and Pb was found to be the most toxic metal to this bioluminescent fungus (EC50(F)): Pb(II) > Ag(I) > Hg(I) > Cd(II) > Cu(II) > Co(II) Ni(II) > Mn(II) > Fe(II) approximate to Zn(II) > Mg(II) approximate to Ba(II) approximate to Cs(I) > Li(I) > K(I) approximate to Na(I) approximate to Sr(II)> Ca(II). Celebrated as a theorist of science, and a source of stimulating ideas for theologians and philosophers of religion, Michael Polanyi explicitly denied cognitive relativism. Yet cognitive relativism, this paper suggests, is implied by Polanyi's account of conceptual frameworks and intellectual controversies.

In 'The Stability of Beliefs' (1952) Polanyi understands conceptual frameworks (science, psychoanalysis, Azande witchcraft, Marxism) as embedded in, and as expressed in the use of, their own languages. The language-with-theory limits the range of discussable subjects, interprets relevant facts in its own terms, permits only certain questions to be asked, with answers to these questions serving to confirm the framework.

In Polanyi's masterwork, Personal Knowledge (1958), these ideas inform his discussion of controversies over scientific frameworks and frameworks vying to become part of science. In each controversy, frameworks are logically disconnected, Polanyi foreshadowing the incommensurability thesis

I argue that Polanyi's ideas satisfy recognised criteria of cognitive relativism. Perception is undetermined by objects and conditioned by language. Empirical propositions, in Polanyi's view, are accepted as true only within a conceptual framework. Polanyi regards supporters of logically disconnected frameworks as thinking differently, living in different worlds, speaking different languages and as experiencing communication failure. There is no framework-independent argument or evidence to distinguish any framework as the best available approximation to the truth. Frameworks are logically disconnected and incommensurable.

The activation of slip and twinning deformation modes in Mg-3Al-1Zn alloy was investigated by means of both in-situ and ex-situ methods at ambient temperature using electron back scattering diffraction (EBSD). The results confirm the importance of non-basal slip and c-axis compression double twinning. During tensile deformation of rolled sheet, 63% of the observed slip traces were ascribed to prismatic slip, 33% to basal slip and 4% to <c+a> slip. Prismatic slip was frequently observed in grain interiors. The density of twinning was quantified in samples tested along transverse, extrusion and rolling directions at failure. The values in the range of 0.02-0.18 twins per square micron were found depending on sample orientation. The results show the effect of twinning on failure.

Objective: To investigate the relative importance of methicillin resistant Staphylococcus aureus (MRSA) in the community in Melbourne by describing circulating S. aureus strains and infection characteristics.

Methods: Patients with any community-onset S. aureus infection were identified via clinical specimens submitted to a community-based pathology service in 2006. The referring doctors confirmed community onset and defined site and severity of each infection. Patient isolates were characterised by antibiotic resistance subtype and presence of the Panton-Valentine leukocidin gene (pvl).

Results: Between April and September 2006, 2,094 S. aureus isolates were processed. Of these, 133 (6.4%) were multiresistant MRSA (mMRSA) and 110 (5.3%) were resistant to less than 3 non-betalactam antibiotics (non-multiresistant MRSA or nmMRSA). We followed-up all nmMRSA (34) and mMRSA (15) confirmed community-onset infections, and a random subset of eligible patients with MSSA infections (57), for whom clinical data were available from referring doctors (82% response).

The majority of isolates were from skin infections (99/106), but drainage was performed in less than one third of cases (29/99). Antibiotics were prescribed for 89% (95%CI: 82, 94) of infections. The isolates were resistant to the prescribed antibiotic 100% of the time for mMRSA infections and 80% for nmMRSA. Those with infections caused by MRSA had on average one additional visit to their doctor compared with MSSA infections.

Ten nmMRSA clones were identified, including one new pvl positive nmMRSA. Of the 29 nmMRSA isolates, 14 were pvl positive (48%; 95%CIs: 30%, 66%) compared with 16% of MSSA and 0% mMRSA.

Patients with an infection caused by pvl positive strains (23) were younger ((mean age 23 years (95%CI: 16, 30) compared with the 55 years (95%CI: 50, 61)). Infection site also varied with presence of pvl; more pvl positive infections were found in the axilla (17.9% compared with 0%) and head and neck (35.7% compared with 8.2%), and less for the leg or foot (21.4% compared with 55.7%).

Conclusions: We estimate that 3.5% of community-onset S. aureus infections in Melbourne in 2006 were caused by MRSA, and 70 to 90% of patients with MRSA infections were treated initially with antibiotics to which their isolate was resistant. pvl positive isolates of S. aureus were associated with younger age and axillary or head and neck infections.

Colonel Muammar Qaddafi has been the leader of Great Socialist People's Libyan Arab Jamahiriya since September 1969. After 40 years in control of his nation, Qaddafi is actually the world’s longest serving non‐monarchial head of state. This year’s anniversary makes it timely to review his four decades of rule. From being the leader of a successful military coup to being America’s bête noire of the 1980s and then the head of a much‐vilified rogue state under twenty‐seven years of UN sanctions, the mercurial Qaddafi has lately steered his nation to something of a rapprochement with the West, been elected to the chairmanship of the African Union and simultaneously had a very public falling out with some of his Arab leaders. This paper examines the highs and lows of Qaddafi’s leadership and where his small but prominent North African state may be heading.

The selection of an inappropriate regime by policy makers can thwart the effective implementation of public policy and lead to implementation failure. Competing values in the implementation process have a significant impact on the results of implementation, and the regime selected for implementation implies the choice of one value over another. Stoker has argued that on the one hand central leadership is valued for consistency, benefits of scale, co-ordination and cost sharing (instrumental model); and on the other, diffuse authority is valued as knowledge is particular and situational, and the task of analysis is to understand the problems, perspectives and interactions of implementation participants at the contact point between public programs and their clients (accommodation model). This study examines the implementation regime chosen by the Victorian Government for the introduction of a modern internal auditing function into the Victorian Public Service, using the Bureau of Internal Audit within its Department of Management and Budget. The selection of the Department of Management and Budget for the introduction of Internal Audit as a managerial accountability mechanism indicates that the instrumental model was held by the Victorian Government to be the most important in implementation. It is argued that use of a top-down central agency leadership approach resulted in implementation failure. Three propositions suggesting the mechanisms by which this result is brought about are examined: that lack of co-ordinated planning at the outset will result in inadequate definition of client needs as part of policy formulation; that intraorganisational conflict during the implementation success, as individual participants are likely to exercise their veto; and the increasing the number of participants contributes to complexity, so that they should only be involved in implementation when their presence is absolutely required. It is argued that the essential task of implementation is to create an environment where participants are likely to co-operate to achieve predetermined public policy goals; and that the introduction of a modern internal auditing approach into the Victorian Public Service required a more participatory implementation regime in order to facilitate policy outcomes and prevent implementation failure. It is also argued that the dominance of economic reform over accountability in Victoria restricted the ability of the Bureau of Internal Audit to implement change to internal audit practices within Victorian Government departments. The selection of an instrumental model of implementation by the Department of Management and Budget is examined in the context of the environment that existed in Victoria between 1982-1987; and while some of the values which Stoker associates with the top-down approach to policy making were observed, an alternative view to the development of internal auditing in the Victorian Public Service can be sustained.

The machined chips of 5083 Al alloy were recycled by hot extrusion at 723 K with an extrusion ratio of 44:1 in air. Corrosion and mechanical properties of the recycled specimens have been compared with those of a virgin extrusion which was processed from the ingot block. As a result of salt immersion tests, mass loss of the recycled specimen was not less than twice of that of the virgin extrusion. The deterioration in corrosion properties for the recycled specimen was attributed to the excessive contamination of Fe which promoted galvanic corrosion. As a result of tensile tests, the recycled specimen exhibited a good combination of high strength and high elongation to failure at room temperature. The excellent mechanical properties for the recycled specimen were attributed to the refined microstructure. However, the elongation to failure of the recycled specimen at elevated temperatures more than 573 K was lower than that of the virgin extrusion. The contamination of oxide particles is likely to be responsible for the lower elongation in the solid recycled specimen.

Participants in the experimental group reported significant improvements in their constipation symptoms.

This paper discusses the use of action research with teachers in remote primary schools in PNG to provide sustainable professional learning to help improve the quality of schooling. It arises from an ADRA research project being undertaken by the authors. The paper describes the project’s research design and its implementation to investigate the introduction, implementation and feasibility of teachers using action research to solve their own problems related to providing basic education in remote communities. If successful, action research may prove to be an effective approach to sustaining professional learning communities in locations where traditional approaches and means of professional development are difficult or impossible to sustain.

The paper describes the research team’s approach to identifying and engaging schools in remote districts of Western and East Sepik provinces, surveying teachers in those districts about their professional learning needs and circumstances, identifying schools to trial action research, and to undertake the fieldwork to implement action research and to study its implementation. The teachers’ experiences with using action research are presented in the context of their particular research topics chosen for their school. To date, the findings suggest that teachers can use action research to help them improve the quality of the education they provide for children. However, the initiation and sustainability of such an action research approach is influenced by the capacities and commitment of head teachers and standards officers, in particular, valuing and understanding reflective practice and action research for professional learning in school communities.

The aim of this work is to devise an effective method for static summarization of home video sequences. Based on the premise that the user watching a summary is interested in people related (how many, who, emotional state) or activity related aspects, we formulate a novel approach to video summarization that works to specifically expose relevant video frames that make the content spotting tasks possible. Unlike existing approaches, which work on low-level features which often produce the summary not appealing to the viewer due to the semantic gap between low-level features and high-level concepts, our approach is driven by various utility functions (identity count, identity recognition, emotion recognition, activity recognition, sense of space) that use the results of face detection, face clustering, shot clustering and within cluster frame alignment. The summarization problem is then treated as the problem of extracting the set of key frames that have the maximum combined utility.

Summary Despite targeted attempts to reduce post-fracture care gaps, we hypothesized that a larger care gap would be experienced by First Nations compared to non-First Nations people. First Nations peoples were eight times less likely to receive post-fracture care compared to non-First Nations peoples, representing a clinically significant ethnic difference in post-fracture care.

Introduction First Nations peoples are the largest group of aboriginal (indigenous or native) peoples in Canada. Canadian First Nations peoples have a greater risk of fracture compared to non-First Nations peoples. We hypothesized that ethnicity might be associated with a larger gap in post-fracture care.

Methods Non-traumatic major osteoporotic fractures for First Nations and non-First Nations peoples aged ≥50 years were identified from a population-based data repository for Manitoba, Canada between April 1996 and March 2002. Logistic regression analysis was used to examine the probability of receiving a BMD test, a diagnosis of osteoporosis, or beginning an osteoporosis-related drug in the 6 months post-fracture.

Results A total of 11,234 major osteoporotic fractures were identified; 502 occurred in First Nations peoples. After adjustment for confounding covariates, First Nations peoples were less likely to receive a BMD test [odds ratio (OR) 0.1, 95% confidence interval (CI), 0.0–0.5], osteoporosis-related drug treatment (OR, 0.5; 95% CI, 0.3–0.7), or a diagnosis of osteoporosis (OR, 0.5; 95% CI, 0.3–0.7) following a fracture compared to non-First Nations peoples. Females were more likely to have a BMD test (OR, 5.0; 95% CI, 2.6–9.3), to be diagnosed with osteoporosis (OR, 1.7; 95% CI, 1.5–2.0), and to begin drug treatment (OR, 4.1; 95% CI, 2.7–6.4) compared to males.

Conclusions An ethnicity difference in post-fracture care was observed. Further work is needed to elucidate underlying mechanisms for this difference and to determine whether failure to initiate treatment originates with the medical practitioner, the patient, or a combination of both. It is imperative that all residents of Manitoba receive efficacious and equal care post-fracture, regardless of ethnicity.

The possibility of winning the tender is subject to the cost estimation proposed by the contractors compared to the cost estimation prepared by the client’s representative. Studies indicated that one of the construction company failures in the tendering process is due to the uncertain, incorrect, and unrealistic cost estimation. Failure to provide a competitive cost estimation on time is the main reason the contractor is not successful in winning the tender, and will not be considered for the tender evaluation stage. The aim of this paper is to validate the key factors that affect the cost estimation activities in the tender preparation process. This paper describes the results of a pilot study, from a questionnaire survey on the factors that affect the cost estimation activities in the tender preparation process. The study shows the importance of all the factors in the cost estimation activities in the tender preparation process in terms of human factors, working culture, and information technology.
