L’endométriose est une maladie gynécologique, touchant les femmes en âge de procréer. Cette pathologie est caractérisée par la présence de tissu endométrial ectopique, c’est-à-dire en dehors de la cavité utérine. Des dysfonctions du système immunitaire sont de plus en plus souvent suspectées comme étant un des éléments responsables de la pathogenèse de cette maladie. L’objectif général de ce projet a donc été d’étudier les mécanismes cellulaires de molécules pro-inflammatoires aux propriétés variées et à l’expression anormalement élevée dans cette pathologie, que sont MIF et les prostaglandines PGE2 et PGF2α, dans les anomalies inflammatoires et invasives en cause dans cette pathologie. La première partie de nos travaux a porté sur l’étude d’un modèle murin de l’endométriose déficient du gène MIF. Le nombre et le volume des lésions collectées à partir des souris déficientes pour le gène MIF sont significativement inférieurs à ceux mesurés dans des souris sauvages utilisées comme contrôle. L’analyse par PCR des cellules isolées des lésions de souris déficientes du gène MIF a révélé une expression réprimée des protéines d’adhésion, d’inflammation et d’angiogenèse. Ces données démontrent pour la première fois que le MIF agit directement sur la croissance et la progression de lésions d’endométriose in vivo. Une partie de nos travaux a porté sur les molécules nécessaires au métabolisme de PGE2 et PGF2α dans l’endomètre eutopique des femmes normales et l’endomètre eutopique et ectopique des femmes atteintes d’endométriose. Selon nos données, l’expression de certains de ces facteurs est perturbée durant cette maladie, ce qui peut avoir des effets délétères sur la physiologie de la procréation. La stimulation des cellules ectopiques par PGF2α entraîne une libération accrue de VEGF et CXCL-8, ceci via l’induction de COX-2 et des deux variants d’épissage du récepteur FP. De plus, la PKC joue un rôle dans ce phénomène, dépendamment et indépendamment de la PLC. Par son effet inducteur sur la libération de VEGF et CXCL-8, PGF2α pourrait favoriser l’aspect inflammatoire et le développement ectopique des lésions d’endométriose, notamment par des phénomènes d’angiogenèse et de prolifération cellulaire accrus. L’effet de PGF2α sur la libération de VEGF et CXCL-8 par les cellules endométriales ectopiques pourrait également expliquer les quantités élevées de ces cytokines dans le liquide péritonéal des femmes atteintes d’endométriose, un phénomène suspecté dans l’infertilité et les douleurs associées à cette maladie. Nos derniers résultats obtenus à partir du liquide péritonéal montrent un profil cytokinique en faveur de l’angiogenèse et la prolifération des lésions d’endométriose, avec une forte augmentation des facteurs suivants : EGF, FGF-2, IL-1α, MIP-1β, TGFα, PDGF-AA, PDGF-BB, MCP-3, sCD40L, Gro Pan, IL-17α, MDC et Rantes, confortant nos observations préalables redéfinissant la maladie comme étant d’origine angio-inflammatoire. L’endométriose et ses symptômes sont des phénomènes complexes ayant probablement plus qu’une seule origine. Parmi les nombreux facteurs à l’expression altérée dans l’endométriose, notre étude montre que MIF, PGE2 et PGF2α, ainsi qu’une pléthore de facteurs pro-angiogéniques pourraient être de ceux jouant un rôle dans l’infertilité et les douleurs reliées à cette maladie.

Der ischämische Schlaganfall ist nicht nur die zweithäufigste Todesursache weltweit, sondern auch eine der Hauptursachen für körperliche Beeinträchtigungen im Erwachsenenalter. Das Ausmaß der durch den Schlaganfall hervorgerufenen Gewebeschädigung ist stark durch das Immunsystem geprägt. Die im Zentralnervensystem (ZNS) ansässigen Mikroglia und die aus dem Blutsystem infiltrierenden Makrophagen sind die Schlüsselzellen der lokalen und systemischen Entzündungsantwort nach dem ischämischen Schlaganfall. Sowohl Mikroglia als auch Makrophagen spielen in der Entwicklung der Gewebeschädigung eine duale Rolle. Zum einen phagozytieren sie Zelltrümmer und unterstützen neuroregenerative Prozesse, zum anderen sind diese Zellen in der Lage den Zustand der Gewebsschädigung zu verschlimmern und einer Regeneration des ZNS entgegenzuwirken. Die Polarisierung der Mikroglia/Makrophagen hin zu verschiedenen Phänotypen ist abhängig von der jeweiligen Phase der Gewebeschädigung. Der destruktive, proinflammatorische Phänotyp (M1) steht dabei dem protektiven, antiinflammatorischen Phänotyp (M2) gegenüber. Die Notwendigkeit einer zielgerichteten Regulierung der polarisierten Mikroglia/Makrophagen zum protektiven M2-Phänotyp wurde bereits mehrfach im Zusammenhang mit der Behandlung von neurodegenerativen Erkrankungen erwähnt. In der vorliegenden Dissertation soll die immunregulierende und neuroprotektive Wirkung der microRibonukleinsäure-124 (miRNA-124) in Bezug auf die Polarisierung von Mikroglia/Makrophagen zu verschiedenen Zeitpunkten nach Verschluss der Arteria cerebri media (ACM) im Gehirn von Mäusen gezeigt werden. Zu diesem Zweck wurde die liposomierte miRNA-124 zu einem frühen Zeitpunkt (Tag 2) und zu einem späten Zeitpunkt (Tag 10) nach Verschluss der ACM verabreicht. Die Behandlung mit der miRNA-124 zu einem frühen Zeitpunkt resultierte dabei in einem signifikanten Anstieg in der Anzahl der M2-positiven Mikroglia/Makrophagen im Vergleich zur Kontrollgruppe. Gleichzeitig nahm die Anzahl der M1-positiven Mikroglia/Makrophagen signifikant ab. Im Wesentlichen resultierte die Behandlung mit der miRNA-124 zu beiden Zeitpunkten in einem geringeren Verhältnis von proinflammatorischen (M1) zu antiinflammatorischen (M2) Mikroglia/Makrophagen. Zu den weiteren Erkenntnissen einer frühzeitigen Behandlung im Rahmen dieser Dissertation gehören: (i) eine signifikante Zunahme des neuronalen Überlebens, das zudem positiv mit der Anzahl der M2-positiven Mikroglia/Makrophagen korreliert, (ii) eine verbesserte funktionelle Erholung, welche in Verbindung mit den veränderten neuroinflammatorischen Ereignissen steht und (iii) ein signifikant verkleinertes Läsionsareal, welches durch reaktive Astrozyten zum gesunden Gewebe hin abgegrenzt wird. Die Ergebnisse dieser Dissertation zeigen, dass die Verabreichung von miRNA-124 eine neue Möglichkeit zur Regulierung der Immunantwort und der Neuroprotektion im Rahmen der Behandlung des ischämischen Schlaganfalls darstellt.

Extra Ovarian Primary Peritoneal Carcinoma (EOPPC) is a rare type of adenocarcinoma of the pelvic and abdominal peritoneum. The objective examination and the histological aspect of the neoplasia virtually overlaps with that of ovarian carcinoma. The reported case is that of a 72 year-old patient who had undergone a total hysterectomy with bilateral annessiectomy surgery 20 years earlier subsequently to a diagnosis for uterine leiomyomatosis. The patient came to our attention presenting recurring abdominal pain, constipation, weight loss, severe asthenia and fever. Her blood test results showed hypochromic microcytic anemia and a remarkable increase CA125 marker levels. Instrumental diagnostics with Ultrasound (US) and CT scans indicated the presence of a single peritoneal mass (10-12 cm diameter) close to the great epiploon. The patient was operated through a midline abdominal incision and the mass was removed with the great omentum. No primary tumor was found anywhere else in the abdomen and in the pelvis. The operation lasted approximately 50 minutes. The post-operative course was normal and the patient was discharged four days later. The histological exam of the neoplasia, supported by immunohistochemical analysis, showed a significant positivity for CA 125, vimentin and cytocheratin, presence of psammoma bodies, and cytoarchitectural pattern resembling that of a serous ovarian carcinoma even in absence of primitiveness, leading to a final diagnosis of EOPPC. The patient later underwent six cycles of chemotherapy with paclitaxel (135 mg/m2/24 hr) in association with cisplatin (75mg/m2). At the fourth year follow-up no sign of relapse was observed. .

Analizamos en base a nuestra experiencia apoyados por bibliografía reciente, la importancia, validez, precisión y actualidad del Lavado Peritoneal como método diagnóstico de lesión intraabdominal en pacientes con trauma contuso y penetrante por arma blanca hemodinámicamente estables que cumplían los criterios de inclusión predeterminados para el caso. Durante un período de 10 meses desde Octubre de 1998 a Julio de 1999, tomando como Universo a los pacientes politraumatizados de la sala de emergencia del Hospital Vicente Corral Moscoso de Cuenca - Ecuador, se obtuvo una muestra de 50 casos con sospecha de lesión intraabdominal por trauma abdominal contuso o herida penetrante por objeto corto punzante, en quienes se efectuó un estudio cuasi experimental descriptivo al ser sometido a lavado peritoneal diangóstico para determinar casos positivos, negativos, falsos positivos y falsos negativos. Se utilizó la técnica abierta de Fisher por considerarla la más segura y con menos complicaciones. Se decidió laparotomía de urgencia en los casos positivos registrándose los hallazgos transoperatorios. Se obtuvieron 27 lavados positivos, 20 negativos, un falso positivo que al ser laparotomizado se comprobó que la sangre en cavidad provenía de la pared del abdomen y dos falsos negativos que se desestabilizaron hemodinámicamente, presentaron reacción peritoneal y también fueron intervenidos quirúrgicamente. Por lo tanto, se realizaron 30 laparotomías, justificándose 27, dos resultaron ser los casos falsos negativos y una sola laparotomía fue innecesaria. Las complicaciones por el procedimiento estuvieron ausentes. En este estudio, el Lavado Peritoneal Diagnosticó demostró una sensibilidad del 93.1 por ciento especificidad del 95.2 por ciento, un valor predictivo positivo del 96.4 por ciento, un valor predictivo negativo del 90.9 por ciento; datos que se equiparan con otros estudios. De acuerdo a la experiencia realizada concluimos que el Lavado Peritoneal Diagnóstico constituye una prueba efectiva, precisa, rápida, económica para la manejor evaluación clínica en centros de baja complejidad o faltos de otros métodos complementarios diagnósticos como ultrasonido o T.A.C. y que ante su existencia, el Lavado Peritoneal Diagnóstico es coadyuvante, por tanto, debe formar parte de los protocolos de valoración de traumatismo abdominal en nuestros hospitales

Kaposi's sarcoma (KS) is one of the most frequent neoplastic diseases in patients infected with human immunodeficiency virus (HIV). The authors report the case of a 40-year-old male with ascites, peripheral edema and peritoneal carcinomatosis secondary to a gastric KS related to human herpes virus type 8 (HHV-8). The patient had severe immunodeficiency, with a TCD4+ count of 86 cells/µl and newly diagnosed acquired immunodeficiency syndrome. His clinical condition rapidly deteriorated, with multiorgan failure, and he died without the possibility of initiating antiretroviral therapy or chemotherapy. To the authors’ knowledge, carcinomatosis is a rare feature in KS.

We present the case of a 55-year-old man, with a self-limited febrile condition associated to polyserositis with inconclusive investigation. Bilateral pleural and pericardial effusions resolved. The peritoneal fluid loculated and was compatible with an exudate. The patient remained clinically asymptomatic. Two years later, examination revealed a palpable and painless abdominal mass, which imaging study suggested a cystic lesion. Surgical resection was performed and the histological examination revealed a mesenteric pseudocyst. Mesenteric pseudocysts are rare intra-abdominal cystic masses, mostly benign, without causing specific symptoms. Although imaging tests are useful for their differential diagnosis, the histology is mandatory.

A patient undergoing peritoneal dialysis was admitted with acute peritoneal dialysis-related peritonitis. On the 10th day of intraperitoneal antibacterial therapy, the patient’s dialysate developed a green color. Gallbladder perforation was revealed. The patient underwent a conventional cholecystectomy with a preserved peritoneal catheter. There are few cases in the medical literature about green dialysate. In all of them, peritoneal dialysis catheters were removed after cholecystectomy. The present case report shows a patient with an intact peritoneal catheter and with no further complications.

Background: Rheumatoid arthritis (RA) is a chronic inflammatory arthritis that causes significant morbidity and mortality and has no cure. Although early treatment strategies and biologic therapies such as TNFα blocking antibodies have revolutionised treatment, there still remains considerable unmet need. JAK kinase inhibitors, which target multiple inflammatory cytokines, have shown efficacy in treating RA although their exact mechanism of action remains to be determined. Stratified medicine promises to deliver the right drug to the right patient at the right time by using predictive ‘omic biomarkers discovered using bioinformatic and “Big Data” techniques. Therefore, knowledge across the realms of clinical rheumatology, applied immunology, bioinformatics and data science is required to realise this goal. Aim: To use bioinformatic tools to analyse the transcriptome of CD14 macrophages derived from patients with inflammatory arthritis and define a JAK/STAT signature. Thereafter to investigate the role of JAK inhibition on inflammatory cytokine production in a macrophage cell contact activation assay. Finally, to investigate JAK inhibition, following RA synovial fluid stimulation of monocytes. Methods and Results: Using bioinformatic software such as limma from the Bioconductor repository, I determined that there was a JAK/STAT signature in synovial CD14 macrophages from patients with RA and this differed from psoriatic arthritis samples. JAK inhibition using a JAK1/3 inhibitor tofacitinib reduced TNFα production when macrophages were cell contact activated by cytokine stimulated CD4 T-cells. Other pro-inflammatory cytokines such as IL-6 and chemokines such as IP-10 were also reduced. RA synovial fluid failed to stimulate monocytes to phosphorylate STAT1, 3 or 6 but CD4 T-cells activated STAT3 with this stimulus. RNA sequencing of synovial fluid stimulated CD4 T-cells showed an upregulation of SOCS3, BCL6 and SBNO2, a gene associated with RA but with unknown function and tofacitinib reversed this. Conclusion: These studies demonstrate that tofacitinib is effective at reducing inflammatory mediator production in a macrophage cell contact assay and also affects soluble factor mediated stimulation of CD4 T-cells. This suggests that the effectiveness of JAK inhibition is due to inhibition of multiple cytokine pathways such as IL-6, IL-15 and interferon. RNA sequencing is a useful tool to identify non-coding RNA transcripts that are associated with synovial fluid stimulation and JAK inhibition but these require further validation. SBNO2, a gene that is associated with RA, may be biomarker of tofacitinib treatment but requires further investigation and validation in wider disease cohorts.

A 140,0 kDa lectin was purified and characterized from the mushroom Clavaria cristata. The purification procedures from the crude extract of the mushroom comprised gel filtration chromatography on Sephacryl s200 and ion exchange on Resource Q column. The purified lectin agglutinated all types of human erythrocytes with preference for trypsinized type O erythrocytes. The haemagglutinating activity is dependent of Ca 2+ ions and was strongly inhibited by the glycoprotein bovine submaxillary mucin (BSM) up to the concentration of 0, 125 mg/mL. The C. cristata lectin (CcL) was stable in the pH range of 2,5-11,5 and termostable up to 80 °C. CcL molecular mass determined by gel filtration on a Superose 6 10 300 column was approximately 140,3 kDa. SDS polyacrilamide gel electrophoresis revealed a single band with a molecular mass of approximately 14,5 kDa, when the lectin was heated at 100 ⁰C in the presence or absence of β-mercaptoethanol. CcL induced activation of murine peritoneal macrophages in vitro resulting in the release of nitric oxide (NO), reaching the maximum production at 24 h. In experimental paw oedema model in mice, CcL showed proinflammatory activity being able to induce oedema formation. Cell viability of HepG2, MDA 435 e 3T3 cell lines was examined after 72 h of incubation with CcL in different concentrations (0,5-50 μg/mL). CcL inhibited HepG2 cells growth with an IC50 value of 50 μg/mL. In the present work, the observed immunomodulatory and antiproliferative effects indicate CcL as a possible immunomodulator compound, interfering in the macrophages immune response, taking possible anti-parasitic, anti-tumoral effects or diagnostic and/or therapeutic

La falla renal crónica definitiva ocasiona la acumulación de productos metabólicos de desecho, alteraciones en el volúmen y en la concentración de electrólitos, así como diversos trastornos endocrinos. La diálisis peritoneal es una forma efectiva de terapia sustitutiva de la función renal. El Hospital Nacional Rosales, es la institución que más demanda tiene de servicios, y como en todo centro de salud es necesario conocer los patrones de susceptibilidad y las posibles terapias empíricas para las diferentes infeccines nosocomiales asociadas a la diálisis peritoneal. El objetivo del presente estudio es el de determinar cuáles son los microorganismos más comunes y su sensibilidad antimicrobiana en pacientes con peritonitis asociada a diálisis peritoneal en el programa de nefrología, de mayo a octubre de 2013. Materiales y método: observacional descriptivo y transversal. Periodo de investigación: mayo a octubre de 2013. Universo: cultivos de líquido peritoneal de pacientes de nefrología con diálisis peritoneal con catéter rígido o blando en el periodo mayo a octubre de 2013. Muestra: cultivos de líquido peritoneal positivos, de pacientes de nefrología con diálisis peritoneal con catéter rígido o blando en el periodo mayo a octubre de 2013. Tipo de muestreo: muestreo por cuotas. Resultados: el estudio mostró al igual que lo descrito en la literatura, que los microorganismos Gram positivos son los más frecuentemente identificados en las infecciones relacionadas a diálisis peritoneal, en catéter blando, y aunque a nivel internacional no está descrito el uso también en catéter rígido. Y que cada hospital, dependiendo de sus estudios de susceptibilidad, debe implementar su terapia empírica. Nosotros recomendamos iniciar el tratamiento con Vancomicina y Cefalosporina de cuarta generación o Amikacina, mientras se obtiene el reporte del cultivo del Líquido Ascítico.

Type 1diabetes (T1D) is an autoimmune disease, which is influenced by a variety of environmental factors including diet and microbes. These factors affect the homeostasis and the immune system of the gut. This thesis explored the altered regulation of the immune system and the development of diabetes in non-obese diabetic (NOD) mice. Inflammation in the entire intestine of diabetes-prone NOD mice was studied using a novel ex-vivo imaging system of reactive oxygen and nitrogen species (RONS), in relation to two feeding regimens. In parallel, gut barrier integrity and intestinal T-cell activation were assessed. Extra-intestinal manifestations of inflammation and decreased barrier integrity were sought for by studying peritoneal leukocytes. In addition, the role of pectin and xylan as dietary factors involved in diabetes development in NOD mice was explored. NOD mice showed expression of RONS especially in the distal small intestine, which coincided with T-cell activation and increased permeability to macromolecules. The introduction of a casein hydrolysate (hydrolysed milk protein) diet reduced these phenomena, altered the gut microbiota and reduced the incidence of T1D. Extra-intestinally, macrophages appeared in large numbers in the peritoneum of NOD mice after weaning. Peritoneal macrophages (PM) expressed high levels of interleukin-1 receptor associated kinase M (IRAK-M), which was indicative of exposure to ligands of toll-like receptor 4 (TLR-4) such as bacterial lipopolysaccharide (LPS). Intraperitoneal LPS injections activated T cells in the pancreatic lymph nodes (PaLN) and thus, therefore potentially could activate islet-specific T cells. Addition of pectin and xylan to an otherwise diabetes-retarding semisynthetic diet affected microbial colonization of newly-weaned NOD mice, disturbed gut homeostasis and promoted diabetes development. These results help us to understand how diet and microbiota impact the regulation of the gut immune system in a way that might promote T1D in NOD mice.

Heparin, a sulfated polysaccharide, was the first compound used as an anticoagulant and antithrombotic agent. Due to their structural characteristics, also has great potential anti-inflammatory, though such use is limited in inflammation because of their marked effects on coagulation. The occurrence of heparin-like compounds that exhibit anticoagulant activity decreased in aquatic invertebrates, such as crab Goniopsis cruentata, sparked interest for the study of such compounds as anti-inflammatory drugs. Therefore, the objective of this study was to evaluate the potential modulator of heparin-like compound extracted from Goniopsis cruentata in inflammatory events, coagulation, and to evaluate some aspects of its structure. The heparin-type compound had a high degree of N-sulphation in its structure, being able to reduce leukocyte migration into the peritoneal cavity at lower doses compared to heparin and diclofenac sodium (anti-inflammatory commercial). Furthermore, it was also able to inhibit the production of nitric oxide and tumor necrosis factor alpha by activated macrophages, inhibited the activation of the enzyme neutrophil elastase in low concentrations and showed a lower anticoagulant effect in high doses as compared to porcine mucosal heparin. Because of these observations, the compound extracted from crab Goniopsis cruentata can be used as a structural model for future anti-inflammatory agents

The Present work deals with the study of exudate of Dacryodes edulis with regards to various seasonal physicochemical properties of the purified solids exudate, acid hydrolysis product and saponification product like charring temperature (oC) (195.73 ± 4.75, 190 ± 7.9, 190 ± 3.4, ); flash point (oC) (105 ± 5.0, 100 ± 7.9, 100 ± 3.4); moisture content % (1.32 – 1.35, 1.05 – 1.08, 1.00 - 1.10); ash content (%) (1.77 – 1.85, 1.21 – 1.31, 1.26 -1.37); lignin content (%) (7.77 – 7.85, 6.21 – 6.31, 5.26 – 5.37); electrical conductivity (μS/cm) (28.40 -29.20, 32.10 – 33.00, 30.30 – 30.90); density (g/cm3) (0.76 -0.77, 0.94 – 0.98, 0.88 – 0.98); melting point (oC) (61.08 – 73.35, 58.69 – 73.35, 55.35 – 65.67). The physicochemical characteristic of Dacryodes edulis is found to be fluctuated with seasonal variations during the present investigation. The correlation coefficient showed positive relationship among the properties. The study reveals that exudate of Daryodes edulis can be tapped in large quantity in the dry season and is rich in lignin content but the seasons does not affect the properties.
