Partner behavioral responses to pain can have a significant impact on patient pain and depression, but little is known about why partners respond in specific ways. Using a cognitive-behavioral model, the present study examined whether partner cognitions were associated with partner behavioral responses, which prior work has found to predict patient pain and depressive symptoms. Participants were 354 women with provoked vestibulodynia and their partners. Partner pain-related cognitions were assessed using the partner versions of the Pain Catastrophizing Scale and Extended Attributional Style Questionnaire, whereas their behavioral responses to pain were assessed with the Multidimensional Pain Inventory. Patient pain was measured using a numeric rating scale, and depressive symptoms were assessed using the Beck Depression Inventory–II. Path analysis was used to examine the proposed model. Partner catastrophizing and negative attributions were associated with negative partner responses, which were associated with higher patient pain. It was also found that partner pain catastrophizing was associated with solicitous partner responses, which in turn were associated with higher patient pain and depressive symptoms. The effect of partner cognitions on patient outcomes was partially mediated by partner behavioral responses. Findings highlight the importance of assessing partner cognitions, both in research and as a target for intervention. Perspective The present study presents a cognitive-behavioral model to partially explain how significant others' thoughts about pain have an effect on patient pain and depressive symptoms. Findings may inform cognitive-behavioral therapy for couples coping with PVD.

Hypoxia is one of the major causes of damage to the fetal and neonatal brain and cardiac functions. in earlier studies we have reported the brain damage caused by hypoxia and resusciation with oxygen and epinephrine and have found that glucose treatment to hypoxic rats and hypoxic rats treated with oxygen shows a reversal of brain damage. during this study the findings may have clinical significance in the proper management of heart and brain functions.

Hypoxia in neonates can lead to biochemical and molecular alterations mediated through changes in neurotransmitters resulting in permanent damage to brain. In this study, we evaluated the changes in the receptor status of GABAA in the cerebral cortex and brainstem of hypoxic neonatal rats and hypoxic rats supplemented with glucose and oxygen using binding assays and gene expression of GABAAa1 and GABAAc5. In the cerebral cortex and brainstem of hypoxic neonatal rats, a significant decrease in GABAA receptors was observed, which accounts for the respiratory inhibition. Hypoxic rats sup- plemented with glucose alone and with glucose and oxygen showed, respectively, a reversal of the GABAA receptors, andGABAAa1 and GABAAc5 gene expression to control. Glucose acts as an immediate energy source thereby reducing the ATP-depletion-induced increase in GABA and oxygenation, which helps in encountering anoxia. Resuscitation with oxygen alone was less effective in reversing the receptor alterations. Thus, the results of this study suggest that reduction in the GABAA receptors functional regulation during hypoxia plays an important role in mediating the brain damage. Glucose alone and glucose and oxygen supplementation to hypoxic neonatal rats helps in protecting the brain from severe hypoxic damage.

This research was undertaken with an objective of studying software development project risk, risk management, project outcomes and their inter-relationship in the Indian context. Validated instruments were used to measure risk, risk management and project outcome in software development projects undertaken in India. A second order factor model was developed for risk with five first order factors. Risk management was also identified as a second order construct with four first order factors. These structures were validated using confirmatory factor analysis. Variation in risk across categories of select organization / project characteristics was studied through a series of one way ANOVA tests. Regression model was developed for each of the risk factors by linking it to risk management factors and project /organization characteristics. Similarly regression models were developed for the project outcome measures linking them to risk factors. Integrated models linking risk factors, risk management factors and project outcome measures were tested through structural equation modeling. Quality of the software developed was seen to have a positive relationship with risk management and negative relationship with risk. The other outcome variables, namely time overrun and cost over run, had strong positive relationship with risk. Risk management did not have direct effect on overrun variables. Risk was seen to be acting as an intervening variable between risk management and overrun variables.

The present study was designed to investigate the protective effect of glucose, oxygen and epinephrine resuscitation on impairment in the functional role of GABAergic, serotonergic, muscarinic receptors, PLC, BAX, SOD, CAT and GPx expression in the brain regions of hypoxia induced neonatal rats. Also, the role of hormones - Triiodothyronine (T3) and insulin, second messengers – cAMP, cGMP and IP3 and transcription factors – HIF and CREB in the regulation of neonatal hypoxia and its resuscitation methods were studied. Behavioural studies were conducted to evaluate the motor function and cognitive deficit in one month old control and experimental rats. The efficient and timely supplementation of glucose plays a crucial role in correcting the molecular changes due to hypoxia, oxygen and epinephrine. The sequence of glucose, epinephrine and oxygen administration at the molecular level is an important aspect of the study. The additive neuronal damage effect due to oxygen and epinephrine treatment is another important observation. The corrective measures by initial supply of glucose to hypoxic neonatal rats showed from the molecular study when brought to practice will lead to healthy intellectual capacity during the later developmental stages, which has immense clinical significance in neonatal care.

In the present work, the role of oxygen, epinephrine and glucose supplementation in regulating neurotransmitter contents, adrenergic and glutamate receptor binding parameters in the cerebral cortex of experimental groups of neonatal rats were investigated. The study of neurotransmitters and their receptors in the cerebral cortex and the EEG pattern in the brain regions of neonatal rats were taken as index for brain damage due to hypoxia, oxygen and epinephrine. Real-Time PCR work was done to confirm the binding parameters. Second messenger, cyclic Adenosine Monophosphate (cAMP) was assayed to find the functional correlation of the receptors. Behavioural studies were carried out to confirm the biochemical and molecular studies. The efficient and timely supplementation of glucose plays a crucial role in correcting the molecular changes due to hypoxia, oxygen and epinephrine. The addictive neuronal damage effect due to oxygen and epinephrine treatment is another important observation. The corrective measures from the molecular study brought to practice will lead to maintain healthy intellectual capacity during the later developmental stages, which has immense clinical significance in neonatal care.

In this study the relationship between Innovative HR practices and selected HR outcomes is investigated.The current study represents a unique attempt to study the effects of innovative HR practices,with job satisfaction,organisational commitment and organisational citizenship bahaviour considered as the consequent variables.Results have affirmed the role of intervening variables such as job satisfaction and organisational commitment in establishing the link between IHRP and OCB obliterating any direct relation between IHRP and organisational citizenship behaviour.This finding may enable researchers in the human resource management to develop more robust understandings of the positive effects of innnovative HR practices on HR outcomes.Thus the present study provides the obvious contribution of weaving up yet another linkage between the two complimentary disciplines of Human Resource Management and Organisational Behaviour.The present study also contributes to the understanding of OCB by exploring its antecedents and extending the intervening role of job satisfaction and organisational commitment.The findings indicate that a higher level of introduction/initiation and satisfaction of innovative HR practices produces high job satisfaction and organisational commitment which lead to OCB.The researcher drew upon the perception-attitude-behaviour model to further realise the expected relationship among innovative HR practices,job satisfaction,organisational commitment and organisational citizenship behaviour.Consequently,this study makes a contribution to the broader organisational citizenship behaviour literature by manifesting the extended relationship path from innovative HR practices to organisational citizenship behaviour,and demonstrating that innovative Hr practices at the organisational level has an effect on employee attitudes and behaviours as well.

In the present work we studied the potential of Bacopa monnieri and Bacoside A treatment to enhance the antioxidant system and support the neuronal survival in the hypoglycemic neonatal brain. For achieving the aim, DAD1 and DAD2 receptors functional regulation, gene expression of growth factors, neuronal survival and apoptotic factors during insulin induced hypoglycemic neonatal brain in rats were studied.

El incremento en la prevalencia de infecciones intrahospitalarias causadas por especies de Klebsiella spp. productora de ß-lactamasa de espectro extendido (BLEE) se ha incrementado en la población hospitalizada constituyendo un problema de salud pública, aunque datos locales en infantes y neonatos es limitada. El objetivo de este estudio fue definir los factores de riesgo asociados a la presencia de bacteremia por Klebsiella pneumoniae BLEE en neonatos que ingresan a la unidad de cuidado intensivo neonatal. Se realizó un estudio de casos y controles en una UCIN de Bogotá (Colombia) de Enero de 2004 a Diciembre de 2005. Fueron definidos como caso los neonatos con diagnóstico de bacteremia por Klebsiella pneumoniae productora de BLEE nosocomial, y como controles a los neonatos admitidos en el mismo periodo de tiempo sin bacteremia por éste microorganismo. Un total de 72 neonatos fueron analizados (24 casos y 48 controles) encontrando en el análisis multivariado una asociación significativa con la exposición previa a antibióticos (OR: 12.85; IC 95%: 1.08–91.6; p<0.001) y con el uso de ventilación mecánica (OR: 40.2; IC 95%: 5.67–285.94; p<0.001). Resultados que presentan concordancia con otros estudios publicados. En conclusión la ventilación mecánica y el uso previo de de antibióticos se relacionan con la presencia de bacteremia nosocomial por Klebsiella pneumoniae productora de BLEE en neonatos. Esta relación debe ser confirmada con estudios de mayor nivel de evidencia.

Introducción : las infecciones intrahospitalarias llevan alta morbilidad y mortalidad en pacientes críticamente enfermos en unidades neonatales. Materiales y métodos : después de un período de observación de 5 meses, se realizó una intervención educativa en una unidad neonatal de referencia en Bogotá sobre adecuadas técnicas de aislamiento y precauciones de contacto, basados en la toma de cultivos de piel para evaluar la colonización microbiológica. Se evaluó la tasa de infecciones intrahospitalarias (IIH) de los dos períodos y se realizó un ajuste teniendo en cuenta variables confusoras que afectan el riesgo de infección intrahospitalaria a través de un modelo de regresión logística incondicional. Resultados : se evaluaron 450 neonatos. 78 pacientes tuvieron IIH, siendo mayor en el primer grupo (45 de 247; 18,2%), que en el de intervención educativa: (33 de 203; 16,2%). La IIH se presentó en 17 de 25 pacientes colonizados (66,7%), 15 de 44 pacientes no colonizados (34%) y uno de los 134 pacientes a los que no se les tomó muestra (0,75%; p<0,01). El modelo mostró que el riesgo de IIH fue mayor en el grupo no intervenido, (O.R. 1,99, I.C. 95%: 1,39-12,30); otros factores de riesgo identificados fueron el uso de nutrición parenteral, la presencia de cardiopatía congénita y una mayor estancia hospitalaria. Discusión : una intervención educativa sobre adecuadas técnicas de aislamiento y precauciones de contacto en neonatos, basados en la toma de cultivos de piel para evaluar la colonización microbiológica, se asoció con una menor tasa de IIH en el grupo intervenido en una UCIN.

la colonización materna por estreptococo del grupo B (SGB) en países en desarrollo es de 4-20%, 50% de sus hijos nacen colonizados y el 1-2% desarrollan enfermedad invasiva con alto riesgo de morbimortalidad y secuelas. La incidencia de infección es diez veces más alta en menores de 1500gramos. Objetivo: determinar los factores de riesgo maternofetales asociados a enfermedad severa y mortalidad neonatal por Streptococcus agalactidae en una unidad de recién nacidos. Materiales y Métodos: se realizó un estudio observacional analítico de cohorte histórica durante un periodo de 2 años. Se tomaron 11 (once) recién nacidos con cuadro clínico de enfermedad invasiva por SGB, con confirmación en ocho (8) casos con hemocultivos, un (1) caso con cultivo de líquido cefalorraquídeo y dos (2) con ambos. Quince (15) controles que correspondían a las madres colonizadas con recién nacidos asintomáticos y con cultivos negativos. Las características demográficas de los dos (2) grupos no mostraron diferencias significativas. Se calcularon frecuencias absolutas y relativas y se buscaron asociaciones mediante el cálculo del estadístico Chi2, se aceptaron valores de p < 0.05, bajo el programa SPSS 15.0 para Windows. Resultados: los factores maternos predictivos para enfermedad por SGB incluyeron, fiebre periparto mayor a 37.5 grados centígrados (p <0.05), corioamnionitis y ruptura de membranas mayor a 18 horas (p<0.05). Los factores de riesgo neonatal incluyeron prematurez (<37 semanas) y bajo peso al nacer(<2500 gramos) (p<0.05). La severidad de la enfermedad se valoró por la presencia de neumonía, meningitis o hemorragia pulmonar. Se encontró una mortalidad de 5(45%). Conclusiones: se encontró relación estadísticamente significativa entre la corioaminionitis materna, la ruptura de membranas mayor a 18 horas, la prematurez y el peso bajo al nacer con la severidad de la enfermedad y la mortalidad. La incidencia estimada de infección neonatal en la unidad de recién nacidos fue de 1,8 casos/1000 nacidos vivos y la de colonización materna fue de 4,3 casos/ 1000 maternas. Se deben realizar más estudios en el país para establecer la incidencia real de enfermedad neonatal por SGB y hacer investigación sobre la costoefectividad de las medidas de prevención.

Introducción: El manejo de los neonatos pretermino en la Unidad de Cuidados Intensivos (UCIN) de la Fundación Santafé sigue protocolos internacionales, sin embargo no se conoce si las estadísticas internacionales se correlacionan con las locales. El estudio pretende determinar la morbimortalidad en esta población de pretérminos, buscar si existen factores de riesgo asociados a esta y plantear propuestas para mejorar en los cuidados de los pacientes. Materiales y Métodos: Estudio descriptivo de corte Transversal, que incluyó neonatos preterminos menores de 1500 gr que hubieran estado hospitalizados en la UCIN en un periodo de 5 años desde enero del 2005 a diciembre de 2009. Se analizo la información estadísticamente y se buscaron diferencias entre los grupos que presentaron mortalidad y los que sobrevivieron. Resultados: Fueron recolectadas 106 historias clínicas, murieron (12,3%) y sobrevivieron (87,7%). La diferencia entre el peso medio fue de 387 gramos (IC95% 245.5 a 528,5; p<0,05), mayor en los que sobrevivieron que en los que murieron; al igual que la diferencia entre las semanas de gestación fue de 2 semanas (IC95% 0.7 a 3,3; p<0,05) también mayor en los sobrevivientes. La principal indicación de parto fue Ruptura Prematura de Membranas (23,1% y 25,8% respectivamente) y el 87,7% fue diagnosticado con Enfermedad de Membrana Hialina. Conclusión: La Fundación Santafé presenta estadísticas de mortalidad de preterminos muy por debajo del nivel mundial, pero debido a la falta de seguimiento de los pacientes que egresan no es posible la comparación con los datos de los estudios encontrados en la literatura.
