Résumé : Désormais, la lutte contre le blanchiment d'argent constitue une priorité pour les Etats et les gouvernements dans le but, d'une part, de préserver l'économie et l'intégrité des places financières et, d'autre part, de priver les organisations criminelles des ressources financières. Dans ce contexte, la préoccupation majeure des autorités algériennes en charge de la lutte contre ce phénomène est de mettre en place un dispositif capable de détecter les mécanismes de blanchiment, d'en évaluer la menace et sur la base de cette connaissance, de définir et de déployer les moyens de riposte les plus efficaces et efficients. Mais nous constatons que mener des enquêtes de blanchiment en conséquence à un crime sous-jacent a montré ses limites en matière d'établissement de preuves, d'élucidation d'affaires et de recouvrement des avoirs. Par ailleurs, nous pensons qu'il serait plus judicieux de mettre en place en amont un contrôle «systématique» des flux financiers et des opérations inhabituelles et/ou suspectes et de là, identifier d'éventuelles opérations de blanchiment, sans forcément connaître le crime initial, en veillant au maintien de l'équilibre entre le «tout sécuritaire» orienté vers la surveillance accrue des flux et la préservation de la vie privée et des libertés individuelles. Notre thèse apporte un regard critique sur le dispositif actuel de lutte contre le blanchiment existant en Algérie que nous évaluons et sur lequel nous relevons plusieurs lacunes. Pour répondre aux problèmes identifiés nous proposons des solutions stratégiques, organisationnelles, méthodologiques et technologiques intégrées dans un cadre opérationnel cohérent au niveau national et international.

OBJECTIVE: The aim of this study was to review the literature on clinician characteristics influencing patient-clinician communication or patient outcome in oncology. METHODS: Studies investigating the association of clinician characteristics with quality of communication and with outcome for adult cancer patients were systematically searched in MEDLINE, PSYINFO, PUBMED, EMBASE, CINHAL, Web of Science and The Cochrane Library up to November 2012. We used the preferred reporting items for systematic reviews and meta-analyses statement to guide our review. Articles were extracted independently by two of the authors using predefined criteria. RESULTS: Twenty seven articles met the inclusion criteria. Clinician characteristics included a variety of sociodemographic, relational, and personal characteristics. A positive impact on quality of communication and/or patient outcome was reported for communication skills training, an external locus of control, empathy, a socioemotional approach, shared decision-making style, higher anxiety, and defensiveness. A negative impact was reported for increased level of fatigue and burnout and expression of worry. Professional experience of clinicians was not related to communication and/or to patient outcome, and divergent results were reported for clinician gender, age, stress, posture, and confidence or self-efficacy. CONCLUSIONS: Various clinician characteristics have different effects on quality of communication and/or patient outcome. Research is needed to investigate the pathways leading to effective communication between clinicians and patients. Copyright © 2013 John Wiley & Sons, Ltd.

As is known, the Kyoto Protocol proposes to reinforce national policies for emission reduction and, furthermore, to cooperate with other contracting parties. In this context, it would be necessary to assess these emissions, both in general and specifically, by pollutants and/or among productive sectors. The object of this paper is precisely to estimate the polluting emissions of industrial origin in Catalonia in the year 2001, in a multivariate context which explicitly allows a distinction to be made between the polluter and/or the productive sector causing this emission. Six pollutants considered, four directly related to greenhouse effect. A multi-level model, with two levels, pollutants and productive sectors, was specified. Both technological progress and elasticity of capital were introduced as random effects. Hence, it has been permitted that these coefficients vary according to one or other level. The most important finding in this paper is that elasticity of capital has been estimated as very non-elastic, with a range which varies between 0.162 (the paper industry) and 0.556 (commerce). In fact, and generally speaking, the greater capital the sector has, the less elasticity of capital has been estimated. Key words: Kyoto protocol, multilevel model, technological progress

OBJECTIVES: To explore the relationship between patient's intention to change regarding future alcohol consumption following brief alcohol intervention (BAI) and changes in alcohol consumption 12-months later and the communication characteristics between patient and counselor during BAI. DESIGN, SETTING AND SUBJECTS: Data from 367 patients (experimental arm) of a pragmatic randomized controlled trial were used to assess the effectiveness of BAI among hazardous drinkers attending an Emergency Department (Lausanne University Hospital, Lausanne, Switzerland). Alcohol outcome measures at baseline and 12 months follow-up included usual number of drinks per week, monthly frequency of heavy episodic drinking (5 or more standard drinks for men; 4 or more for women), and the Alcohol Use Disorders Identification Test (AUDIT) score. In addition, the communication characteristics between patient and counselor were analyzed via tape recordings using the Motivational Interviewing Skill Code (MISC) from 97 participants. Patient readiness and importance to change on a 10-point Likert scale (readiness/importance to change ruler) was asked during BAI, and patient intention to change alcohol consumption (yes/no) was asked at the last step. Differences in alcohol outcome at follow-up between the 367 patients who did or did not have an intention to change consumption at baseline were compared, as were differences between these two groups in communication characteristics for the 97 who completed tape recordings. RESULTS: Patients with an intention to decrease alcohol consumption reduced alcohol use and related problems more often, and reported higher levels of importance and readiness to change than did their counterparts. Analyses of MISC-coded data showed a significantly higher use of MI-consistent skills among those with a moderation intention, but no group differences on the 8 other counselor communication skills measures were found. Analyses of patient speech during the intervention indicated that those with an intention to change their alcohol consumption significantly more often self-explored personal ambivalence towards alcohol, expressed more intensely their ability, commitment, desire, need and reason to change their alcohol use than did those in the no decrease group. CONCLUSIONS: The intention expressed by hazardous drinkers when concluding BAI is associated with both patient change talk during BAI and drinking outcome 12 months later, but is mainly independent of counselor communication skills. This intention may be an important clinical indicator of which hazardous drinkers are most likely to improve after BAI.

This paper addresses the impact of payment systems on the rate of technology adoption. We present a model where technological shift is driven by demand uncertainty, increased patients' benefit, financial variables, and the reimbursement system to providers. Two payment systems are studied: cost reimbursement and (two variants of) DRG. According to the system considered, adoption occurs either when patients' benefits are large enough or when the differential reimbursement across technologies offsets the cost of adoption. Cost reimbursement leads to higher adoption of the new technology if the rate of reimbursement is high relative to the margin of new vs. old technology reimbursement under DRG. Having larger patient benefits favors more adoption under the cost reimbursement payment system, provided that adoption occurs initially under both payment systems.

Cette année, six études utiles pour la pratique ont été retenues. L'indication à la mammographie entre 40 et 49 ans devrait être évaluée individuellement et en tenant compte des risques/bénéfices de cet examen. Au-delà de 65 ans, un dépistage systématique de la fibrillation auriculaire avec prise de pouls puis ECG (si pouls irrégulier) pourrait être réalisé de manière systématique. Les risques de complications postcolonoscopie existent, particulièrement suite à des biopsies/polypectomies, et ce risque devrait être discuté. Les inhibiteurs de la pompe à protons au long court sont un facteur de risque de fracture de hanche. S'il est important de prendre en charge des pressions artérielles élevées au-delà de 80 ans, il faut être prudent (orthostatisme). Une corticothérapie précoce suite à une paralysie faciale périphérique est efficace. This year we have selected six studies useful for the day to day practice. A mammography in women 40 to 49 years of age should be evaluated taking into account the patient's profile and the possible risks and benefits of this exam. In patients over 65 years of age, a systematic atrial fibrillation screening, with pulse rate measuring then ECG (if irregular beat) should be realised on a regular basis. The risks for complications following colonoscopies do exist, especially after biopsies/polypectomies and this risk should be discussed. Long term proton pump inhibitor treatment is a risk factor for hip fracture. It is important to treat high blood pressure problems in the elderly, but the orthostatic risks should be adressed. A corticoid treatment started quickly for Bell's palsy is efficient

Critical limb ischemia is a major public health problem in our western countries due to the epidemia of (diabesity). The outcome of patients suffering from critical limb ischemia reains poor with an amputation free survival rate at one year of about 50%. The treatment should be multidiciplinary and done in emergency in specialized centers to ensure the limb salvage: this management should be centered aroud 3 axis: the screening of the cardiovascular risk factors, the best medical treatment and the invasive approaches. Due to multiple endovascular technical innovations, more frail patients with com plex diseases can be treated with good results. Therefore, the endovascular treatment is essential in the management of such patients by vascular surgeons.

Ante el nuevo reto que supone la adaptación al Espacio Europeo de Educación Superior (EEES) con una acción política que propone introducir importantes modificaciones en la concepción del proceso de enseñanza-aprendizaje entre las que se encuentran el replanteamiento del papel del profesor y del alumno, y el proceder metodológico en la acción docente, todos los agentes educativos deben aunar esfuerzos para articular medidas que posibiliten la adaptación a este nuevo escenario de manera óptima. Las universidades están ofreciendo a los docentes herramientas que puedan facilitar el acceso a los procesos de innovación educativa que exige el nuevo espacio de formación. En este marco es donde nace el proyecto titulado 'Programa de Introducción a la Investigación y al Desarrollo de las Destrezas Comunicativas (hablar y escribir correctamente)' que, valiéndose de los recursos tecnológicos que nos ofrece el Servicio de Innovación Educativa de la Universidad de Málaga, busca mejorar las competencias de expresión oral y escrita de los alumnos, así como la capacidad para organizar sus trabajos de investigación de manera rigurosa y ordenada, siguiendo una secuenciación razonada y científica. En la presente comunicación se detallan los objetivos del proyecto, la descripción del mismo, el modo de proceder en su desarrollo, las partes de las que ha constado y las conclusiones a las que se ha llegado.

In recent years, the strategy for the control of schistosomiasis has placed increased emphasis on the role of health education, public information, and communication. This should, not only bring about specific changes in behavior aiming at disease prevention, but also stimulate participation of the community in health programs. Beyond this, it is desirable that both community members and researchers should seek better life conditions through a transformative social action. The present paper addresses these concerns; first, by critically reviewing some health education programs that were developed in Brazil, and, secondly, by analyzing and suggesting ways to improve this area.

Hem establert les bases metodològiques i teòriques per investigar la pregunta “Tenen les nacions sense estat el dret de controlar el seu propi espai de comunicació?”. La investigació ajusta el concepte d’espai de comunicació a la teoria política, cercant els seus límits en els drets individuals i, des de la perspectiva del liberalisme 2, aportant la justificació del seu control en quant que plataforma que incideix en la conservació i supervivència d’una cultura nacional. El primer article i fase de la tesi és l’adaptació i definició del concepte espai de comunicació. Fins ara, la recerca ha proposat diferents models d’espai de comunicació entenent si es tracta d’una visió emfatitzant la distribució i la producció de material marcat amb els símbols de la identitat nacional de la societat emissora, o bé si emfatitza la idea d’un espai de circulació de fluxos comunicatiu ajustat a un territori tradicionalment vinculat a una identitat nacional o nació sense estat. Igualment, es distingeix la dimensió d’emissió –sortir del territori al món- i la de recepció –fluxos informatius rebuts des del món al territori, concretament, al ciutadà; el paper d’intervenció de les institucions democràtiques és diferent en una dimensió o una altra i, per tant, també són diferents els drets afectats i les teories o principis que neguen o justifiquen el control de l’espai de comunicació. També s’ha indagat en les teories sobre els efectes cognitius dels mitjans de comunicació per relacionar-los amb la construcció nacional com a cohesió simbòlica i cultural. Si bé els mitjans no poden fer canviar de pensament immediatament, sí que poden conformar a llarg termini una percepció nacional general. Una comunitat és imaginada, donada la distància física dels seus components, i la comunicació social és, juntament amb l’educació, el principal factor de construcció nacional, avui en dia.

Le travail policier et l'enquête judiciaire nécessitent de prendre de nombreuses décisions : choisir quelle trace analyser, mettre sous surveillance ou en détention un suspect, sont autant de décisions qui sont prises quotidiennement par les acteurs du système judiciaire. Ces décisions font l'objet de pesées d'intérêts qui se fondent sur l'analyse de l'information accessible. C'est le rôle de l'analyse criminelle de mettre en perspective l'information colligée pour la rendre intelligible aux décideurs compétents. L'usage de représentations graphiques est notamment recommandé pour soutenir l'analyse et la communication de ces informations.Des techniques de visualisation relationnelle sont notamment exploitées dans les enquêtes judiciaires afin de faciliter le traitement d'affaires d'envergure. Les éléments pertinents de l'enquête sont représentés sous la forme de schémas décrivant les relations entre les événements et les entités d'intérêts (tel que des personnes, des objets et des traces). Les exploitations classiques de ces techniques qui s'apparentent à des graphes, sont par exemple : la représentation de réseaux criminels, de trafics de marchandises, de chronologies d'événements, ainsi que la visualisation de relations téléphoniques et financières. Dans ce contexte, la visualisation soutient un nombre importants d'objectifs, tels qu'analyser les traces et les informations collectées, évaluer a posteriori une investigation, aider à qualifier les infractions, faciliter l'appréhension d'un dossier, voire soutenir une argumentation lors du procès.La pratique intègre des outils logiciels simples qui produisent des graphiques élégants et souvent percutants. Leur utilisation semble néanmoins soulever des difficultés. Cette recherche tend à montrer qu'il existe des disparités étonnantes lors de l'exploitation de ces techniques. Des biais de raisonnement et de perception peuvent être induits, allant jusqu'à provoquer des décisions aux conséquences parfois désastreuses. Ce constat révèle la nécessité de consolider les méthodes pratiquées.Pour mettre en évidence ces difficultés, des évaluations ont été effectuées avec des praticiens et des étudiants. Elles ont permis d'établir une image empirique de l'étendue des variations de conception et d'interprétation des représentations, ainsi que de leurs impacts sur la prise de décision. La nature et la diversité des concepts à représenter, l'absence d'un consensus émergeant sur la manière de représenter les données, la diversité des solutions visuelles envisageables, les contraintes imposées par les outils exploités et l'absence d'une formalisation claire du langage, sont autant de causes supposées des difficultés.Au cours des vingt dernières années, plusieurs axes de développement ont été proposés pour traiter les difficultés observées, tels que l'amélioration des automatismes facilitant la conception d'une représentation, l'exploitation des techniques de réseaux sociaux, l'automatisation de l'identification et de l'extraction des entités dans du texte non-structuré et la définition de langages formels. Cette recherche propose une approche parallèle fondée sur une exploitation adaptée de structures de graphe et de propriétés visuelles pour optimiser la représentation en fonction des objectifs définis et de la nature des informations à représenter.Des solutions ont été recherchées selon plusieurs axes. Des recommandations générales, issues de diverses communautés de recherche liées à la visualisation, ont été recherchées afin de proposer une démarche générale de conception des schémas relationnels. Par ailleurs, le développement d'un catalogue de bonnes pratiques formalisées sous la forme de patterns de visualisation a été amorcé. Chaque pattern décrit une solution particulière pour un problème d'analyse récurrent, tel que l'analyse d'une série de cambriolages. Finalement, l'impact sur les outils de la méthodologie proposée est discuté en regard des limites qu'ils imposent. Un prototype de visualisation multidimensionnel a été élaboré.Cette recherche met en évidence les difficultés rencontrées lors de l'exploitation de représentations graphiques pour soutenir le processus de l'enquête judiciaire et propose des éléments de méthode et des innovations techniques utiles tant pour l'enseignement de la discipline que pour sa pratique.

The aim of this exploratory study was to assess the impact of clinicians' defense mechanisms-defined as self-protective psychological mechanisms triggered by the affective load of the encounter with the patient-on adherence to a communication skills training (CST). The population consisted of oncology clinicians (N = 31) who participated in a CST. An interview with simulated cancer patients was recorded prior and 6 months after CST. Defenses were measured before and after CST and correlated with a prototype of an ideally conducted interview based on the criteria of CST-teachers. Clinicians who used more adaptive defense mechanisms showed better adherence to communication skills after CST than clinicians with less adaptive defenses (F(1, 29) = 5.26, p = 0.03, d = 0.42). Improvement in communication skills after CST seems to depend on the initial levels of defenses of the clinician prior to CST. Implications for practice and training are discussed. Communication has been recognized as a central element of cancer care [1]. Ineffective communication may contribute to patients' confusion, uncertainty, and increased difficulty in asking questions, expressing feelings, and understanding information [2, 3], and may also contribute to clinicians' lack of job satisfaction and emotional burnout [4]. Therefore, communication skills trainings (CST) for oncology clinicians have been widely developed over the last decade. These trainings should increase the skills of clinicians to respond to the patient's needs, and enhance an adequate encounter with the patient with efficient exchange of information [5]. While CSTs show a great diversity with regard to their pedagogic approaches [6, 7], the main elements of CST consist of (1) role play between participants, (2) analysis of videotaped interviews with simulated patients, and (3) interactive case discussion provided by participants. As recently stated in a consensus paper [8], CSTs need to be taught in small groups (up to 10-12 participants) and have a minimal duration of at least 3 days in order to be effective. Several systematic reviews evaluated the impact of CST on clinicians' communication skills [9-11]. Effectiveness of CST can be assessed by two main approaches: participant-based and patient-based outcomes. Measures can be self-reported, but, according to Gysels et al. [10], behavioral assessment of patient-physician interviews [12] is the most objective and reliable method for measuring change after training. Based on 22 studies on participants' outcomes, Merckaert et al. [9] reported an increase of communication skills and participants' satisfaction with training and changes in attitudes and beliefs. The evaluation of CST remains a challenging task and variables mediating skills improvement remain unidentified. We recently thus conducted a study evaluating the impact of CST on clinicians' defenses by comparing the evolution of defenses of clinicians participating in CST with defenses of a control group without training [13]. Defenses are unconscious psychological processes which protect from anxiety or distress. Therefore, they contribute to the individual's adaptation to stress [14]. Perry refers to the term "defensive functioning" to indicate the degree of adaptation linked to the use of a range of specific defenses by an individual, ranging from low defensive functioning when he or she tends to use generally less adaptive defenses (such as projection, denial, or acting out) to high defensive functioning when he or she tends to use generally more adaptive defenses (such as altruism, intellectualization, or introspection) [15, 16]. Although several authors have addressed the emotional difficulties of oncology clinicians when facing patients and their need to preserve themselves [7, 17, 18], no research has yet been conducted on the defenses of clinicians. For example, repeated use of less adaptive defenses, such as denial, may allow the clinician to avoid or reduce distress, but it also diminishes his ability to respond to the patient's emotions, to identify and to respond adequately to his needs, and to foster the therapeutic alliance. Results of the above-mentioned study [13] showed two groups of clinicians: one with a higher defensive functioning and one with a lower defensive functioning prior to CST. After the training, a difference in defensive functioning between clinicians who participated in CST and clinicians of the control group was only showed for clinicians with a higher defensive functioning. Some clinicians may therefore be more responsive to CST than others. To further address this issue, the present study aimed to evaluate the relationship between the level of adherence to an "ideally conducted interview", as defined by the teachers of the CST, and the level of the clinician' defensive functioning. We hypothesized that, after CST, clinicians with a higher defensive functioning show a greater adherence to the "ideally conducted interview" than clinicians with a lower defensive functioning.
