Objective: To develop a 'quality use of medicines' coding system for the assessment of pharmacists' medication reviews and to apply it to an appropriate cohort. Method: A 'quality use of medicines' coding system was developed based on findings in the literature. These codes were then applied to 216 (111 intervention, 105 control) veterans' medication profiles by an independent clinical pharmacist who was supported by a clinical pharmacologist with the aim to assess the appropriateness of pharmacy interventions. The profiles were provided for veterans participating in a randomised, controlled trial in private hospitals evaluating the effect of medication review and discharge counselling. The reliability of the coding was tested by two independent clinical pharmacists in a random sample of 23 veterans from the study population. Main outcome measure: Interrater reliability was assessed by applying Cohen's kappa score on aggregated codes. Results: The coding system based on the literature consisted of 19 codes. The results from the three clinical pharmacists suggested that the original coding system had two major problems: (a) a lack of discrimination for certain recommendations e. g. adverse drug reactions, toxicity and mortality may be seen as variations in degree of a single effect and (b) certain codes e. g. essential therapy were in low prevalence. The interrater reliability for an aggregation of all codes into positive, negative and clinically non-significant codes ranged from 0.49-0.58 (good to fair). The interrater reliability increased to 0.72-0.79 (excellent) when all negative codes were excluded. Analysis of the sample of 216 profiles showed that the most prevalent recommendations from the clinical pharmacists were a positive impact in reducing adverse responses (31.9%), an improvement in good clinical pharmacy practice (25.5%) and a positive impact in reducing drug toxicity (11.1%). Most medications were assigned the clinically non-significant code (96.6%). In fact, the interventions led to a statistically significant difference in pharmacist recommendations in the categories; adverse response, toxicity and good clinical pharmacy practice measured by the quality use of medicine coding system. Conclusion: It was possible to use the quality use of medicine coding system to rate the quality and potential health impact of pharmacists' medication reviews, and the system did pick up differences between intervention and control patients. The interrater reliability for the summarised coding system was fair, but a larger sample of medication regimens is needed to assess the non-summarised quality use of medicines coding system.

An 8 1/2-year-old neutered male Beagle was diagnosed with acquired myasthenia gravis associated with a non-invasive thymic carcinoma. The thymic mass was surgically excised and the dog was treated with pyridostigmine, prednisolone and azathioprine. Serial acetylcholine receptor antibody titres were increased initially but slowly declined to normal values over a period of 24 weeks. Improved exercise tolerance was seen following therapy, however, oesophageal dysfunction persisted. The dog was euthanased 26 weeks after initial presentation due to a complicating illness. A necropsy showed no regrowth or metastasis of the thymic carcinoma.

The role of sport-specific practice in the development of decision-making expertise in the sports of field hockey, netball, and basketball was examined. Fifteen expert decision-makers and 13 experienced non-expert athletes provided detailed information about the quantity and type of sport-specific and other related practice activities they had undertaken throughout their careers. Experts accumulated more hours of sport-specific practice from age 12 years onwards than did non-experts, spending on average some 13 years and 4,000 hours on concentrated sport-specific practice before reaching international standard. A significant negative correlation existed between the number of additional activities undertaken and the hours of sportspecific training required before attaining expertise, suggesting a functional role for activities other than sport-specific training in the development of expert decision-making.

Based on the refined non-conforming element method for geometric nonlinear analysis, a refined nonlinear non-conforming triangular plate element is constructed using the Total Lagrangian (T.L.) and the Updated Lagrangian (U.L.) approach. The refined nonlinear non-conforming triangular plate element is based on the Allman's triangular plane element with drilling degrees of freedom [1] and the refined non-conforming triangular plate element RT9 [2]. The element is used to analyze the geometric nonlinear behavior of plates and the numerical examples show that the refined non-conforming triangular plate element by the T.L. and U.L. approach can give satisfactory results. The computed results obtained from the T.L. and U.L. approach for the same numerical examples are somewhat different and the reasons for the difference of the computed results are given in detail in this paper. © 2003 Elsevier Science Ltd. Background: The surgical cure rate for primary hyperparathyroidism is greater than 95%. For those who have recurrent or persistent disease, preoperative localization improves reoperation success rates. Selective parathyroid venous sampling (SPVS) for intact parathyroid hormone is particularly useful when non-invasive localization techniques are negative or inconclusive. Methods: We present all known cases (n = 13) between 1994 and 2002 who had venous sampling for localization at our institution prior to reoperation for recurrent or persistent primary hyperparathyroidism. Comparison was made with non-invasive localization procedures. Results of invasive and non-invasive localization were correlated with surgical findings. Results: Of the nine reoperated cases, eight had positive correlations between SPVS and operative findings and histopathology. SPVS did not reveal the parathyroid hormone source in one case with negative non-invasive localization procedures. Comparisons between SPVS, computerized tomography (CT), and parathyroid scintigraphy (MIBI) as expressed in terms of true positive (TP), false positive (FP) and false negative (FN) were: SPVS - TP 88.8%, FP 0%, FN 11.1%; CT - TP 22.2%, FP 22.2%, FN 55.5%; and MIBI - TP 33.3%, FP 0%, FN 66.6%. At least seven of the nine operated cases have been cured; another remained normocalcaemic 2 weeks after subtotal parathyroidectomy. Conclusion: In our institution SPVS has proven to be a valuable tool in cases with recurrent or persistent primary hyperparathyroidism and negative non-invasive localization procedures.

The design of Internet Relay Chat (IRC) affords for, and itself produces, non-response situations that are not possible in FTF or telephone interaction. These system-occasioned nonresponses produce almost isomorphic stimuli to participant non-responses. Situations thus arise in which non-responses are interpersonally accountable despite agentive ambiguity. This study explores four intersections of participant-action and system-occasioned non-responses. An extension to Pomerantz's (1984b) 'pursuing a response' problems/solutions is proposed. The impact of IRC's design on its popularity is discussed in contrast to more recent chat systems. Suggestions are made for active and passive presence and non-response accounting features in future chat systems.

In this study, a combination of recA-based PCR assays and 16S rDNA restriction fragment length polymorphism (RFLP) analysis was used to determine the genomovar diversity of clinical Burkholderia cepacia complex isolates. Twenty-eight isolates were prospectively collected from patients attending a large Australian adult cystic fibrosis (CF) unit, 22 isolates were referred from other Australian CF units and a further eight isolates originated from patients without CF. The 28 prospectively collected isolates were distributed amongst the following genomovars: Burkholderia cepacia genomovar I (28.6%), Burkholderia multivorans (21.4%), Burkholderia cepacia genomovar III (39.3%), Burkholderia vietnamiensis (3.6%) and Burkholderia ambifaria (7.1%). The results of this study highlight the usefulness of 16S rDNA RFLP typing for the identification of other Burkholderia spp. and non-fermenting gram-negative bacteria.

This paper is concerned with methods for refinement of specifications written using a combination of Object-Z and CSP. Such a combination has proved to be a suitable vehicle for specifying complex systems which involve state and behaviour, and several proposals exist for integrating these two languages. The basis of the integration in this paper is a semantics of Object-Z classes identical to CSP processes. This allows classes specified in Object-Z to be combined using CSP operators. It has been shown that this semantic model allows state-based refinement relations to be used on the Object-Z components in an integrated Object-Z/CSP specification. However, the current refinement methodology does not allow the structure of a specification to be changed in a refinement, whereas a full methodology would, for example, allow concurrency to be introduced during the development life-cycle. In this paper, we tackle these concerns and discuss refinements of specifications written using Object-Z and CSP where we change the structure of the specification when performing the refinement. In particular, we develop a set of structural simulation rules which allow single components to be refined to more complex specifications involving CSP operators. The soundness of these rules is verified against the common semantic model and they are illustrated via a number of examples.

In this paper, we revisit the surface mass excess in adsorption studies and investigate the role of the volume of the adsorbed phase and its density in the analysis of supercritical gas adsorption in non-porous as well as microporous solids. For many supercritical fluids tested (krypton, argon, nitrogen, methane) on many different carbonaceous solids, it is found that the volume of the adsorbed phase is confined mostly to a geometrical volume having a thickness of up to a few molecular diameters. At high pressure the adsorbed phase density is also found to be very close to but never equal or greater than the liquid phase density. (C) 2003 Elsevier Science Ltd. All rights reserved.

In recent years there has been a dramatic increase in reports of glycosylation of proteins in various Gram-negative systems including Neisseria meningitidis, Neisseria gonorrhoeae, Campylobacter jejuni, Pseudomonas aeruginosa, Escherichia coli, Caulobacter crescentus, Aeromonas caviae and Helicobacter pylori. Although this growing list contains many important pathogens (reviewed by Benz and Schmidt [Mol. Microbiol. 45 (2002) 267-276]) and the glycosylations are found on proteins important in pathogenesis such as pili, adhesins and flagella the precise role(s) of the glycosylation of these proteins remains to be determined. Furthermore, the details of the glycosylation biosynthetic process have not been determined in any of these systems. The definition of the precise role of glycosylation and the mechanism of biosynthesis will be facilitated by a detailed understanding of the genes involved. Until recently, glycosylation of proteins in prokaryotes was regarded as uncommon and thought to be limited to special cases such as S-layer proteins and some archeal outer membrane proteins. Now, there are an increasing number of reports of bacterial proteins that are glycosylated. Pilin of pathogenic Neisseria is one of the best characterised post-translation ally modified bacterial proteins, with four different types of modifications reported, including a novel glycosylation. Pilin monomers assemble to form pilus fibres, which are long protein filaments that protrude from the surface of bacterial cells and are key virulence factors. To aid in the investigation of these modifications, pure pilin is required. A number of pilin purification methods have been published, but none are appropriate for the routine purification of pilin from many different isolates. This study describes a novel, rapid, and simple method of pilin purification from Neisseria meningitidis C311#3, which facilitates the production of consistent quantities of pure, native pilin. A 6 x histidine tag was fused to the C-terminus of the pilin subunit structural gene, pilE, via homologous recombination placing the 6 x histidine-tagged allele in the chromosome of N. meningitidis C311#3. Pilin was purified under non-denaturing conditions via a two-step process using immobilised metal affinity chromatography (IMAC), followed by dye affinity chromatography. Analysis of the purified pilin confirmed that it retained both of the post-translational modifications examined. This novel approach may prove to be a generally applicable method for purification and analysis of post-translationally modified proteins in bacteria. (C) 2003 Elsevier Science (USA). A Reserva Biológica de Duas Bocas (2.190 ha) é um dos maiores remanescentes de Mata Atlântica do Estado do Espírito Santo, Sudeste do Brasil. Nós amostramos tetrápodes não voadores nessa área entre maio de 2007 e abril de 2008, utilizando armadilhas de queda, armadilhas de isca, armadilhas fotográficas e buscas oportunísticas diurnas e noturnas. Além disso, nós compilamos registros de vertebrados não voadores ocorrentes nesta área disponíveis na literatura e através de espécimes em museus. Nós documentamos 52 espécies de anfíbios, 24 espécies de répteis não voadores e 39 espécies de mamíferos não voadores. Do total de 115 espécies, 47 configuram novos registros para a área e seis outras espécies tiveram sua distribuição geográfica ampliada com os resultados do presente estudo. Além disso, apresentamos o registro de predação da perereca Hypsiboas faber pela serpente Chironius bicarinatus. Cinco das espécies registradas são listadas como ameaçadas no Estado do Espírito Santo e muitas outras possuem estado de conservação incerto. A Reserva Biológica de Duas Bocas é um importante refúgio de vida selvagem, principalmente quando consideramos a expansão de áreas urbanas no seu entorno.

A irradiação de alimentos é um método de conservação não térmico utilizado em diferentes alimentos e em diferentes doses. Sabe-se que, dependendo da dose aplicada, esse método pode ocasionar alterações nas características físico-químicas e sensoriais, que afetam a qualidade do produto. Portanto, um dos objetivos deste estudo foi analisar o efeito da irradiação nas características físico-químicas e sensoriais de morangos irradiados. A irradiação das amostras de morango ocorreu um dia após a colheita e foi realizada no Laboratório de Radiação Gama do Centro de Desenvolvimento da Tecnologia Nuclear (CDTN), em Belo Horizonte-MG. As doses utilizadas foram 1 kGy, 2 kGy, 3 kGy e 4 kGy e a amostra controle (0 kGy). Foram avaliados pH, acidez total titulável, sólidos solúveis, ratio (relação entre teor de sólidos solúveis e acidez total titulável), açúcares redutores, totais e não redutores, pectina, ácido ascórbico, firmeza, cor, antocianinas, compostos fenólicos e capacidade antioxidante. Para a caracterização sensorial das amostras, foi utilizado o método Perfil Descritivo Otimizado (PDO), sendo avaliados os atributos sensoriais cor, sabor adocicado, gosto ácido, firmeza e suculência, por 15 julgadores previamente selecionados. Além da caracterização sensorial, foi realizada a análise de aceitação sensorial por 60 consumidores de morango em duas sessões. A irradiação não alterou significativamente a maioria das características físico-químicas analisadas, ao comparar todas as doses empregadas, incluindo o morango controle (não irradiado); apenas a firmeza apresentou valores diferentes estatisticamente: com o aumento da dose, houve diminuição da firmeza do fruto. No PDO, as amostras diferiram quanto à cor, sabor adocicado, firmeza e suculência, sendo que a amostra controle e a amostra irradiada com 1 kGy tiveram maiores valores de firmeza e menores de suculência e sabor adocicado. Em relação à aceitação, a amostra controle e a amostra irradiada com 1 kGy não diferiram estatisticamente pelo teste Tukey a 5%, mas diferiram das amostras irradiadas com doses de 3 kGy e de 4 kGy; no entanto, a amostra irradiada a 2 kGy não diferiu de nenhuma outra dose. Foram realizados estudos de correlações entre os dados físico-químicos de pH, acidez total titulável (ATT), sólidos solúveis (SS), açúcares totais e açúcares não redutores, antocianina (método pH diferencial), pectina, firmeza instrumental, relação entre SS/ATT (ratio) e alguns parâmetros de cor (a*, b*, c* e L), e os dados do teste sensorial descritivo, ou seja, os atributos de firmeza, suculência, sabor adocicado e gosto ácido. Os resultados obtidos foram correlações significativamente positivas entre pH e ratio, sólidos solúveis e pectina, antocianinas e parâmetros de cor (a* e c*), açúcares e parâmetros de cor (b* e c*), firmeza sensorial e firmeza instrumental, suculência e sabor xvi adocicado e firmeza sensorial e gosto ácido. Já as correlações negativas foram entre acidez e ratio, sólidos solúveis e firmeza, antocianina e L*, firmeza sensorial e sabor adocicado e suculência com: firmeza instrumental, firmeza sensorial e gosto ácido. Neste trabalho, também foi realizada a caracterização físico-química e sensorial de morangos irradiados nas doses determinadas por Lima Filho et al. (2014) como sendo correspondentes ao limiar de detecção (LD) sensorial (0,405 kGy) e ao limiar de rejeição (LR) sensorial (3,6 kGy) para morangos irradiados. Além disso, foi verificada a influência da embalagem na aceitação sensorial de morangos irradiados na dose 3,6 kGy. Todas as medidas físico-químicas apresentaram pequena variação entre as amostras controle (0 kGy) e o LD e o LR. Não houve diferença significativa em relação à aceitação da amostra irradiada na ausência e na presença da embalagem, provavelmente devido ao grau de conhecimento do painel de consumidores em relação ao processo de irradiação de alimentos. Conclui-se que a irradiação é um método de conservação que pode ser utilizado em morangos, uma vez que exerce pouca influência, no período de tempo estudado, nas suas características físico-químicas e sensoriais.
