On 30 March, Turkey’s ruling Justice and Development Party (AKP) scooped a significant victory in local elections, taking almost 44 percent of the vote despite accusations of corruption, undermining the rule of law, fundamental rights and freedoms. While there have been claims of election fraud and the main opposition party, the Republican People’s Party (CHP), has demanded recounts in several cities including Istanbul and Ankara, it is clear that even allowing for some level of fraud the win was substantial and more than most people expected. Prime Minister Recep Tayyip Erdoğan has reached a juncture. He has two choices: return to the path of democracy after a period of democratic back-sliding which included passing several controversial reforms such as a new internet law which led to the recent banning of Twitter and Youtube; or alternatively he can forge ahead with his much talked of revenge campaign against those he has accused of creating a “parallel state” and conspiring to remove him from power. Given that Erdoğan viewed this election as a referendum on his popularity and leadership there is a serious risk that he will do the latter; using the significant mandate given to him to do whatever he wants, including further cracking down on democracy.

Often described as complex, opaque and unfair, the EU budget financing system is an ‘unfinished journey’. One of the most critical issues is that EU revenue, drawn from the cashbox of national taxation, remains intangible to the general public. The nature of the EU as a union of states and their nationals makes the visibility of EU revenue unavoidable. The political sustainability of a move that would put the legitimacy of EU revenue at the forefront of public discussion will depend on the EU institutions and member states’ ability to demonstrate that EU funds can achieve results that are truly beyond member states’ reach. In this, his third, CEPS book on the EU budget, Gabriele Cipriani assesses the current system of financing the EU budget against the criteria of simplicity, transparency, equity and democratic accountability and offers two possible options for reforming the EU revenue system. He finds that the value-added tax (VAT) is a natural choice for funding the EU budget, through a dedicated EU VAT rate as part of the national VAT and designed as such in fiscal receipts, whose use as a means for raising EU citizens’ awareness could be encouraged already in the current arrangements.

Enquadramento: O luto é algo complexo e abarca múltiplas dimensões que podem resultar em luto patológico. Objetivos: Determinar a prevalência de luto complicado em familiares de pessoas em situação paliativa; identificar que variáveis sociodemográficas e de contexto do luto interferem no processo de luto complicado; averiguar se há relação entre a funcionalidade familiar, satisfação quanto aos cuidados prestados ao ente querido, a sobrecarga do cuidador e o luto em familiares de pessoas em situação paliativa. Métodos: Estudo quantitativo, transversal, descritivo e correlacional, realizado numa amostra de 75 familiares de doentes em situação paliativa. Utilizou-se um Questionário Sociodemográfico; Escala de Apgar Familiar (Agostinho & Rebelo, 1988), Escala do Contexto do Luto (Cunha, 2014), Escala de Avaliação da Satisfação dos Familiares dos Doentes em Cuidados Paliativos - Escala FAMCARE (Almeida, 2012), Escala de Sobrecarga do Cuidador (Sequeira, 2010) e o Inventory of Complicated Grief (ICG) (Frade & Rocha, 2008). Resultados: Dos 75 participantes, apenas 24 (35.2%) que se encontravam pelo menos há 6 meses enlutados obtiveram uma pontuação superior a 30 no ICG, tradutora de vivência de luto complicado. Destes, 58.3% eram mulheres e 41.7% homens; na faixa etária =<49 anos (37.5%); com companheiro(a) (54.2%); 70.8% revelam funcionalidade familiar; 62.5% dos participantes perdeu um familiar direto, cujo falecimento ocorreu em 71.4% dos casos no domicílio; 83.3% dos familiares revelam sobrecarga com os cuidados; 29.2% revelam-se insatisfeitos com os cuidados prestados ao seu familiar. Conclusão: Os resultados revelam a necessidade de identificação dos fatores de risco no luto, de modo a mobilizarem-se os recursos para a promoção da vivência saudável do luto. BACKGROUND Although it has been well established that methadone use can result in prolonged QTc/torsades de pointes (TdP) and has been labeled as one of the main drugs that cause TdP, it is still prescribed indiscriminately, and several cases of methadone-associated TdP have been seen in our community. METHODS Our objective was to determine the associated factors for prolonged QTc and the development of torsades de pointes (TdP) in our underserved patient population. We found 12,550 ECGs with prolonged QTc between 2002 and 2013. Medical records were reviewed in order to identify precipitating factors for prolonged QTc and to detect incidence of TdP. RESULTS We identified 2735 patients with prolonged QTc who met the inclusion criteria. Of these, 89 (3%) experienced TdP. There was a greater prevalence of HIV infection in the TdP group (11.2 vs. 3.7%, p < 0.001). Furosemide, hydrochlorothiazide, selective serotonin reuptake inhibitors (SSRIs), amiodarone, ciprofloxacin, methadone, haloperidol, and azithromycin were the drugs most often associated with prolonged QTc (31, 8.2, 7.6, 7.1, 3.9, 3.4 and 3.3%, respectively). However, the agents most commonly associated with TdP were furosemide (39.3%), methadone (27%), SSRIs (19.1%), amiodarone (18%), and dofetilide (9%). The medications with statistical significance in the multivariate analysis for TdP development in descending order were as follows: ranolazine (odds ratios [OR] = 53.61, 95% confidence interval [CI] 5.4-524, p < 0.001), dofetilide (OR = 25, CI 6.47-103.16, p < 0.001), voriconazole (OR = 21.40, CI 3.24-124.25, p < 0.001), verapamil (OR = 10.98, CI 2.62-44.96, p < 0.001), sotalol (OR = 12.72, 1.95-82.81, p = 0.008), methadone (OR = 9.89, CI 4.05-24.15, p < 0.001), and SSRI (OR = 2.26, CI 1.10-5.96, p < 0.001). This multivariate analysis revealed that amiodarone and HIV infection were not implicated in TdP. CONCLUSION Methadone was by far the leading medication implicated in the development of TdP and an independent predictor in both univariate and multivariate analyses despite the fact that it was not the most common QT-prolonging medication in our population.
