A infecção sexualmente transmissível (IST), causada por Chlamydia trachomatis, é considerada mundialmente como um importante problema de saúde pública, porque pode causar infertilidade e morbilidade a longo termo, existindo a possibilidade de transmissão ao recém-nascido de mãe infectada. Os indivíduos entre os 15 e os 24 anos de idade são os que se encontram em maior risco. Para além das suas consequências, a presença desta infecção sem tratamento pode potenciar até dez vezes a transmissão do vírus da imunodeficiência humana adquirida. Contribuindo a consciencialização sobre os vários aspectos da saúde sexual e o conhecimento sobre este agente e suas sequelas para que os jovens em risco se protejam e admitindo o Plano Nacional de Saúde lacunas no conhecimento dos profissionais da área sobre este tema, o presente trabalho teve como objectivos: 1) estudar o conhecimento sobre a infecção por C. trachomatis, sinais, sintomas, sequelas e vias de transmissão das IST; 2) conhecer as atitudes e práticas associadas à transmissão de IST e 3) avaliar a prevalência da infecção por C. trachomatis e Neisseria gonorrhoeae numa população de estudantes universitários das áreas da Saúde. Para tal, utilizou-se um questionário, cuja avaliação sugeriu que estes jovens possuem um baixo conhecimento sobre os sinais, sintomas e sequelas deste agente e das outras IST, sendo preocupante verificar que existem estudantes que não reconhecem os diferentes tipos de relação sexual como vias de transmissão destas infecções. A pesquisa dos microrganismos citados, efectuou-se através da aplicação de uma técnica de biologia molecular em amostras de urina e a presença de anticorpos anti-C. trachomatis efectuou-se através uma técnica de imunofluorescência indirecta no soro. Não foram identificados casos de infecção activa, mas registou-se uma taxa de 14,4% de títulos reactivos de anticorpos anti-C. trachomatis, o que indica uma percentagem considerável desta população já foi infectada por este agente. Estudos futuros semelhantes a este, em diferentes populações, são necessários para avaliar a prevalência global de infecção por C. trachomatis em Portugal, assim como para relacionar as atitudes e práticas com a transmissão das IST e com o conhecimento entre a população portuguesa. Medidas básicas de prevenção como a informação à população parecem ser urgentemente necessárias.

Mundialmente, estima-se que 40 milhões de pessoas estejam infectadas com VIH, 5 milhões das quais cronicamente infectadas com VHC. A co-infecção com VIH aumenta a taxa de persistência do VHC, acelera a velocidade de progressão da doença hepática e reduz significativamente a resposta à terapia do VHC. O VHC possui extensa diversidade genética, sendo classificado em seis genótipos e cerca de 90 subtipos com padrões epidemiológicos e resposta à terapia distintos. Tendo isto presente e devido a serem limitados os dados relativos aos genótipos e subtipos do VHC circulantes em Portugal e ainda ao facto de os utilizadores de drogas injectáveis (UDIs) serem um grupo de risco importante para a co-infecção por VIH e VHC, realizámos um estudo retrospectivo para determinar a prevalência da infecção por VHC e a distribuição de subtipos deste vírus num grupo de UDIs infectados com VIH. Amostras de plasma de 66 indivíduos (1998-2001) foram testadas para anticorpos anti-VHC (ensaio imunoenzimático) e RNA do VHC (amplificação da 5’UTR por RT-PCR). Para identificar os subtipos de VHC e detectar recombinantes, as amostras com RNA viral detectável foram sujeitas a amplificação, sequenciação e análise filogenética de sequências nucleotídicas parciais para C/E1 e NS5B. Encontrámos que 86,4% dos indivíduos possuíam anticorpos anti-VHC, 93,0% dos quais com infecção activa. Todas as amostras, exceptuando duas, com RNA viral detectável originaram amplicões para C/E1 e NS5B. A análise filogenética permitiu incluir as estirpes de VHC nos subtipos 1a (43,8%), 1b (3,6%), 2a (1,8%), 3a (21,1%), 4a (8,8%) e 4d (15,8%), revelando um padrão epidemiológico semelhante ao dos UDIs de outros países do Sul da Europa. Apenas uma amostra apresentou discordância entre os subtipos das duas regiões (4d para C/E1 e 4a para NS5B) sugerindo um potencial recombinante intragenótipo. No total, os subtipos 1a, 4a e 4d do VHC são responsáveis por 68,4% das infecções nos UDIs infectados com VIH analisados. Considerando que apenas 20-30% dos doentes positivos para VIH e co-infectados com os genótipos 1 e 4 respondem à terapia do VHC e que ocorre transmissão do VHC dos UDIs para a população em geral, são prioritários estudos alargados de vigilância epidemiológica e implementação de estratégias de prevenção para controlar ambos os vírus neste grupo de risco.

Com o objetivo de determinar a soroprevalência de tripanossomíase americana, sífilis, toxoplasmose, rubéola, hepatite B, hepatite C e infecção pelo vírus da imunodeficiência humana em gestantes atendidas no Hospital Universitário Regional Norte do Paraná, da Universidade Estadual de Londrina, Paraná, foi realizado estudo retrospectivo dos resultados dos testes sorológicos efetuados no período de junho de 1996 a junho de 1998. As taxas de positividade encontradas foram: 0,9% para tripanossomíase americana, 1,6% para sífilis, 67% (IgG) e 1,8% (IgM) para toxoplasmose, 89% (IgG) e 1,2% (IgM) para rubéola, 0,8% para hepatite B (AgHBs), 0,8% para hepatite C e 0,6% para infecção pelo vírus da imunodeficiência humana. Observou-se associação entre o aumento da soroprevalência de tripanossomíase americana com a idade das gestantes (p = 0,006). Os resultados reafirmam a importância da realização destes testes sorológicos no atendimento pré-natal, com a finalidade de realizar o diagnóstico e, eventualmente, adotar medidas para prevenir a transmissão congênita ou perinatal dessas doenças.

A dengue fever case is described in a 58-year-old male patient with febrile illness and thrombocytopenia complicated by neurological involvement characterized by transverse myelitis followed by weakness of both legs and flaccid paralysis. Muscle strength was much diminished and bilateral areflexia was observed. Dengue 2 (DEN-2) virus was isolated and the patient sero-converted by hemagglutination-inhibition and IgM-ELISA tests. The RT-PCR test was positive to DEN-2 in acute phase serum and culture supernatant, but negative in the cerebrospinal fluid. After three weeks of hospitalization the patient was discharged. No other infectious agent was detected in the blood and cerebrospinal fluid samples. The patient had full recovery from paralysis six months after the onset of DEN-2 infection.

Forty voluntary blood donors from two different blood banks in Havana, Cuba, who were repeatedly reactive on the routine screening of antibodies to hepatitis C virus, by Umelisa HCV test, were analyzed for the presence of HCV RNA using a nested PCR assay of the HCV 5' untranslated region, Umelosa HCV qualitative. Sera from 45 patients of a specialized gastroenterology consultation, positive to Umelisa HCV, were also assayed with the Umelosa HCV qualitative, to establish their condition related to the presence of HCV RNA previously to the indication of a treatment or after three, six or twelve months of antiviral therapy. Serum HCV-RNA was detected in 21/40 (52.5%) donors who had repeatedly positive ELISA results, confirming the HCV infection for them. In specialized consultation HCV-RNA was detected by PCR analysis in 30/45 (66%) analyzed sera.

Com o objetivo de estudar a soroprevalência de vírus linfotrópico de células T humanas I/II (HTLV-I/II), vírus da imunodeficiência humana, sífilis e toxoplasmose, em gestantes atendidas em unidade básicas de saúde do município de Botucatu - São Paulo - Brasil, bem como os fatores de risco para a infecção pelo HTLV -I/II, foram realizados inquérito sorológico e avaliação dos resultados de exames solicitados na rotina do prénatal. Em 913 gestantes, a soroprevalência de HTLV- I e de HTLV- II foi de 0,1%. Sífilis, toxoplasmose e infecção pelo HIV foram encontradas. Nenhum dos fatores de risco pesquisados mostrou-se seguro para identificar gestantes com infecção pelo HTLV- I/II. A comparação da proporção de gestantes infectadas e de doadores de sangue da região sudeste do Brasil com testes reagentes para HTLV- I/II não mostrou diferença estatística.

The prevalence and age distribution of Epstein-Barr virus infection varies in different populations and there is little information about the epidemiology of this infection in Brazil. We studied the prevalence of EBV antibodies in a sample of 283 children and adolescents between 1 and 21 years old. The sample was taken from two neighborhoods in Vitória (capital city of Espirito Santo, Brazil). The São Pedro (SP) neighborhood represented an area with lower socioeconomic status and the Praias (P) neighborhood represented an area with higher SES. Anti-VCA (Virus Capsid Antigen) antibodies were detected by ELISA and anti-EBNA (Epstein-Barr Nuclear Antigen) antibodies were detected by an anti-complement immunofluorescence method, both using commercial kits. The results showed an overall prevalence rates of anti-VCA and anti-EBNA of 71% and 54% respectively. The prevalence for both anti-EBV antibodies was higher and probably the infection occurred earlier in the SP neighborhood. Among the various socioeconomic factors studied only low family income and maternal education level were significantly correlated with a higher frequency of positive serology for anti-VCA. These results demonstrate that there is a high prevalence of EBV antibodies in children and adolescents living in Vitória, that occurs more frequently at a younger age in children from families with low socioeconomic status. In addition, the results demonstrate an intermediate age distribution pattern between those reported in developed and underdeveloped countries.

In order to investigate the pathogenicity of the virus strain GOI 4191 that was isolated from a fatal adverse event after yellow fever virus (YFV) vaccination, an experimental assay using hamsters (Mesocricetus auratus) as animal model and YFV 17DD vaccine strain as virus reference was accomplished. The two virus strains were inoculated by intracerebral, intrahepatic and subcutaneous routes. The levels of viremia, antibody response, and aminotransferases were determined in sera; while virus, antigen and histopathological changes were determined in the viscera. No viremia was detected for either strain following infection; the immune response was demonstrated to be more effective to strain GOI 4191; and no significant aminotransferase levels alterations were detected. Strain GOI 4191 was recovered only from the brain of animals inoculated by the IC route. Viral antigens were detected in liver and brain by immunohistochemical assay. Histothological changes in the viscera were characterized by inflammatory infiltrate, hepatocellular necrosis, and viral encephalitis. Histological alterations and detection of viral antigen were observed in the liver of animals inoculated by the intrahepatic route. These findings were similar for both strains used in the experiment; however, significant differences were observed from those results previously reported for wild type YFV strains.

Were analyzed 648 serum samples from laboratory staff in Goiânia, Goiás aiming detection of three serological markers of HBV: HBsAg, anti-HBsAg and anti-HBcAg. The HBsAg and anti-HBcAg positive samples were also analyzed for HBeAg, anti-HBeAg and anti-HBcAgIgM markers. HBV infection rate of 24.1% was observed and, from them, 0.7% were positive for HBsAg. Viral DNA was detected by PCR in two HBsAg positive samples. A vaccination index of 74.5% and a global index of 89.9% of serological response to vaccination were observed. The direct work with biological fluids as well as cleaning workers represented significant risks for acquisition of HBV infection. The data from the present study showed an increase of the vaccination index among laboratory staff but the rates of HBV infection did not change through the years in the region.

The association of hepatitis C virus infection and the hepatosplenic form of schistosomiasis mansoni has been claimed to result in the concomitant evolution of the two pathologies, with a poor prognosis due to aggravated liver disease. Recently, however, some authors have begun to reject the hypothesis of a higher susceptibility of hepatosplenic schistosomal patients to HCV. The aim of the present transverse study carried out between July and August 1990 was to determine the possible association between SM and HCV markers in residents of Catolândia, Bahia State. Anti-HCV markers were assayed by ELISA-II and RIBA-II in serum samples obtained from 1,228 residents (85.8%). The anti-HCV antibody (ELISA-II) was positive in six (0.5%) individuals, eight (0.6%) cases were inconclusive and 1,214 (98.9%) were negative. However, only in one ELISA-positive serum sample (0.08%) were antibodies confirmed by RIBA-II, while two other samples assayed by RIBA-II were indeterminate. These three patients presented the hepatointestinal form of SM during the follow-up period (1976 to 1996). In conclusion, no association was observed between HCV and SM in the endemic area studied, especially among patients with the hepatosplenic form of the disease.

The aim of this investigation was to determine nitric oxide metabolite levels in saliva samples from hepatitis C virus-positive patients in an attempt to test the hypothesis if increased levels of nitric oxide metabolites correlates with the presence of HCV-RNA in saliva. Saliva of 39 HCV-positive patients and 13 HCV-negative patients, without clinical or laboratorial evidence of liver disease were tested for nitric oxide metabolites. HCV-RNA was detected in serum and saliva by a RT-PCR method and nitric oxide level was determined by evaluation of its stable degradation products, nitrate and nitrite. No differences were found between the concentration of nitrite in saliva from HCV patients and controls, in despite of the presence or not of HCV RNA in saliva. Patients with HCV and cirrhosis had higher concentrations of nitrite but not significantly different from the control group or the groups of anti-HCV patients without cirrhosis. Increased levels of nitrite were not detected in anti-HCV positive patients, an indirect indication that chronic sialoadenitis are infrequent in these patients or occurs with low intensity not sufficient to increase nitric oxide metabolite levels in saliva.

This study assessed the number of CD4 T lymphocytes, the parasitemia and serum levels of interferon gamma (IFN-gamma), tumor necrosis factor alpha (TNF-alpha), interleukin-1 (IL-1), IL-4 and IL-10 of patients infected by human immunodeficiency virus (HIV) and human immunodeficiency virus/Chagas' disease coinfection. CD4 T lymphocytes were low in the two groups of patients, although significantly lower in patients without Chagas' disease. Serum levels of IFN-gamma, IL-4 and TNF-alpha were significantly higher in patients with HIV/Chagas' disease. IL-4/IFN-gamma ratios were higher in patients with HIV/Chagas' disease, which showed a clear balance in favor of Th2-like cytokines in this group of patients. This Th2 balance was higher in patients with detectable parasitemia. We conclude that, although immunosuppression was observed, with CD4 T lymphocytes bellow 200/µm³, these patients did not display reactivation of T. cruzi infection and that a balance favorable to Th2 was associated with the presence of parasitemia.
