Bone morphogenetic proteins (BMPs) are multifunctional growth factors belonging to the transforming growth factor β (TGFβ) superfamily with a central role in bone formation and mineralization. BMP2, a founding member of this family, has demonstrated remarkable osteogenic properties and is clinically used to promote bone repair and fracture healing. Lack of basic data on factors regulating BMP2 expression and activity have hampered a better understanding of its role in bone formation and bone-related diseases. The objective of this work was to collect new functional data and determine spatiotemporal expression patterns in a fish system aiming towards a better understanding of BMP2 function and regulation. Transcriptional and post-transcriptional regulation of gilthead seabream BMP2 gene was inferred from luciferase reporter systems. Several bone- and cartilage-related transcription factors (e.g. RUNX3, MEF2c, SOX9 and ETS1) were found to regulate BMP2 transcription, while microRNA 20a was shown to affect stability of the BMP2 transcript and thus the mineralogenic capacity of fish bone-derived host cells. The regulation of BMP2 activity through an interaction with the matrix Gla protein (MGP) was investigated in vitro using BMP responsive elements (BRE) coupled to luciferase reporter gene. Although we demonstrated the functionality of the experimental system in a fish cell line and the activation of BMP signaling pathway by seabream BMP2, no conclusive evidence could be collected on a possible interaction beween MGP and BMP2. The evolutionary relationship among the members of BMP2/4/16 subfamily was inferred from taxonomic and phylogenetic analyses. BMP16 diverged prior to BMP2 and BMP4 and should be the result of an ancient genome duplication that occurred early in vertebrate evolution. Structural and functional data suggested that all three proteins are effectors of the BMP signaling pathway, but expression data revealed different spatiotemporal patterns in teleost fish suggesting distinct mechanisms of regulation. In this work, through the collection of novel data, we provide additional insight into the regulation, the structure and the phylogenetic relationship of BMP2 and its closely related family members.

Introduction: Prevention, early detection and management of diabetes are fundamental for stopping this present epidemic. Medication review may contribute to the early detection of negative clinical outcomes risk factors associated to the use of medicines. Aims: To use the medication review to identify the situations that may lead to clinical negative outcomes, described as findings, in the process of medicines use in type 2 diabetic patients. To verify if the inclusion criteria list of the Pharmaceutical Society of Australia for medication review allows identifying patients of higher risks of obtaining clinical negative outcomes.

The chapter addresses the professional, legal and ethical issues associated with medicines management and the role of the nurse. To ensure safe practice it is imperative safeguards are recognised and applied; therefore pertinent legislative frameworks, policy and professional regulation are discussed. Safeguarding patients when administering prescribed medicines means the nurse must have up-to-date knowledge and skill and a key aspect of this is to ensure consent to treatment from the service user is secured; for this reason drawing on relevant legislation, the consensual process is reviewed. Not infrequently medicine management provokes ethical and legal challenges for the health care professional; these demand reflection and careful consideration; consequently in this chapter legal and ethical parameters and professional boundaries are appraised.

Waterways are one of the oldest systems for the transportation of cargo and continue to play a vital role in the economies of some countries. Due to societal change, climate change and the ageing of assets, the conditions influencing the effective functioning of these systems seem to be changing. These changing conditions require measures to renew, adapt or renovate these waterway systems. However, measures with the sole aim of improving navigation conditions have encountered resistance, as the general public, and stakeholders in particular, value these waters in many more ways than navigation alone. Therefore, a more inclusive, integrated approach is required, rather than a sectoral one. Addressing these contemporary challenges requires a shift in the traditional waterway authorities' regimes. The aim of this study is to identify elements in the institutional setting where obstacles and opportunities for a more inclusive approach can be found. Two major waterway systems, the American and the Dutch, have been analyzed using the Institutional Analysis and Development framework to reveal those obstacles and opportunities. The results show that horizontal coordination and a low pay-off for an inclusive approach is particularly problematic. The American case also reveals a promising aspect – mandatory local co-funding for federal navigation projects acts as a stimulus for broad stakeholder involvement. Improving horizontal coordination and seizing opportunities for multifunctional development can open pathways to optimize the value of waterway systems for society.

Angiotensin-converting enzyme (ACE) plays a critical role in rennin-angiotensin system. Recently, natural products isolated from herbal medicines revealed inhibitory effects against ACE which suggested their potential activities in regulating blood pressure. In this study, ACE inhibition (ACEI) of 21 phenylethanoid glycosides and related phenolic compounds were investigated by measuring the production of HA a rapid, sensitive, accurate and specific ultra-performance liquid chromatography-tandem quadrupole mass spectrometry (UPLC-MS/MS) method. The test compounds showed different inhibitory potencies on ACE ranging from 5.29 to 95.01% at 50 mM, and the compounds with ACEI higher than 50% were selected for further IC50 determination. The IC50 values were from 0.53 ± 0.04 to 15.035 ± 0.036 mM. The structure-inhibition relationship were then explored and the result showed that cinnamoyl groups played an essential role in ACEI of phenylethanoid glycosides. Furthermore, the sub-structures of increasing ACEI for phenylethanoid glycosides is more hydroxyls and less steric hindrance to chelate the active site Zn2+ of ACE. In summary, our results suggested that phenylethanoid glycosides are a widely available source of anti-hypertensive natural products and the information provided from structure-inhibition relationship study could aid the design of structurally modified phenylethanoid glycosides as anti-hypertensive drugs.

Objective To explore people's experiences of starting antidepressant treatment. Design Qualitative interpretive approach combining thematic analysis with constant comparison. Relevant coding reports from the original studies (generated using NVivo) relating to initial experiences of antidepressants were explored in further detail, focusing on the ways in which participants discussed their experiences of taking or being prescribed an antidepressant for the first time. Participants 108 men and women aged 22–84 who had taken antidepressants for depression. Setting Respondents recruited throughout the UK during 2003–2004 and 2008 and 2012–2013 and in Australia during 2010–2011. Results People expressed a wide range of feelings about initiating antidepressant use. People's attitudes towards starting antidepressant use were shaped by stereotypes and stigmas related to perceived drug dependency and potentially extreme side effects. Anxieties were expressed about starting use, and about how long the antidepressant might begin to take effect, how much it might help or hinder them, and about what to expect in the initial weeks. People worried about the possibility of experiencing adverse effects and implications for their senses of self. Where people felt they had not been given sufficient time during their consultation information or support to take the medicines, the uncertainty could be particularly unsettling and impact on their ongoing views on and use of antidepressants as a viable treatment option. Conclusions Our paper is the first to explore in-depth patient existential concerns about start of antidepressant use using multicountry data. People need additional support when they make decisions about starting antidepressants. Health professionals can use our findings to better understand and explore with patients’ their concerns before their patients start antidepressants. These insights are key to supporting patients, many of whom feel intimidated by the prospect of taking antidepressants, especially during the uncertain first few weeks of treatment.

Historians of Chinese medicine acknowledge the plurality of Chinese medicine along both synchronic and diachronic dimensions. Yet, there remains a tendency to think of tradition as being defined by some unchanging features. The Chinese medical body is a case in point. This is assumed to have been formalised by the late Han dynasty around a system of internal organs, conduits, collaterals, and associated body structures. Although criticism was voiced from time to time, this body and the micro/ macrocosmic cosmological resonances that underpin it are seen to persist until the present day. I challenge this view by attending to attempts by physicians in China and Japan in the period from the mid 16th to the late 18th century to reimagine this body. Working within the domain of cold damage therapeutics and combining philological scholarship, empirical observations, and new hermeneutic strategies these physicians worked their way towards a new territorial understanding of the body and of medicine as warfare that required an intimate familiarity with the body’s topography. In late imperial China this new view of the body and medicine was gradually re-absorbed into the mainstream. In Japan, however, it led to a break with this orthodoxy that in the Republican era became influential in China once more. I argue that attending further to the innovations of this period—commonly portrayed as one of decline—from a transnational perspective may help to go beyond the modern insistence to frame East Asian medicines as traditional.

Fluoxetine is a selective serotonin reuptake inhibitor (SSRI) widely used in the treatment of major depression. It has been detected in surface and wastewaters, being able to negatively affect aquatic organisms. Most of the ecotoxicity studies focused only in pharmaceuticals, though excipients can also pose a risk to non-target organisms. In this work the ecotoxicity of five medicines (three generic formulations and two brand labels) containing the same active substance (fluoxetine hydrochloride) was tested on the alga Chlorella vulgaris, in order to evaluate if excipients can influence their ecotoxicity. Effective concentrations that cause 50% of inhibition (EC50) ranging from 0.25 to 15 mg L−1 were obtained in the growth inhibition test performed for the different medicines. The corresponding values for fluoxetine concentration are 10 times lower. Higher EC50 values had been published for the same alga considering only the toxicity of fluoxetine. Therefore, this increase in toxicity may be attributed to the presence of excipients. Thus more studies on ecotoxicological effects of excipients are required in order to assess the environmental risk they may pose to aquatic organisms.

A fitoterapia é umas das Medicinas Alternativas e/ou Complementares mais utilizadas pela população no quotidiano. Esta terapêutica é constituída por misturas de compostos químicos, que são responsáveis pelas suas ações no organismo. Estes compostos não atuam de forma independente, mas sim por efeito aditivo, antagónico ou sinérgico, resultando numa interação dos vários constituintes e dos diversos locais de ação. Nos últimos anos ressurgiu o interesse por esta terapia nos países desenvolvidos, principalmente devido aos efeitos secundários que os medicamentos convencionais podem provocar, e também pelo uso descontrolado e abusivo de certos fármacos. Deste modo, o número de estudos científicos com plantas e seus compostos tem vindo a aumentar ao longo dos anos, fornecendo evidências científicas quanto à sua segurança, aceitabilidade, eficácia, e mostrando menos efeitos secundários que os medicamentos convencionais. Com este projeto pretende-se caraterizar o conhecimento e o consumo de fitoterápicos pela população do distrito de Viana do Castelo. Aplicou-se um estudo observacional, descritivo do tipo transversal e analítico. A população alvo do estudo é a população em geral, residente no distrito de Viana do Castelo, com mais de 18 anos de idade. Para a recolha de informação foi realizado um questionário anónimo, confidencial e voluntário, a 914 individuos. A amostra é constituída maioritariamente por indivíduos do género feminino (58,3%). A faixa etária mais frequente é dos 18 aos 25 anos (19,7%) distribuída por todos os concelhos do distrito. Verificou-se que mais de 40% da população tem doença crónica, e mais de 60% recorreu a produtos fitoterápicos no último ano, havendo uma relação entre quem recorre a esta terapia e a existência de uma patologia crónica associada. Observou-se ainda que mais da 80% da população que utiliza estes produtos se encontra satisfeita com os resultados, sendo que mais de 85% dos utilizadores de fitoterápicos pretende voltar a utilizá-los.

Medicamentos órfãos destinam-se ao diagnóstico, prevenção ou tratamento de doenças muito graves ou que causem risco de vida, que são raras e não afetam mais do que 5 em cada 10.000 pessoas na União Europeia. Estes medicamentos são apelidados de órfãos, já que em condições normais de mercado, não existe interesse para as empresas farmacêuticas desenvolver tais medicamentos, devido ao baixo retorno financeiro causado pela raridade. O objetivo deste trabalho consiste em avaliar a utilização de medicamentos órfãos em Portugal e analisar o impacto económico destes medicamentos na região da Beira Interior. Os dados recolhidos permitiram avaliar retrospetivamente o consumo de medicamentos no período de janeiro a dezembro de 2013 em Portugal e em particular nos hospitais: Centro Hospitalar Cova da Beira, Unidade Local de Saúde da Guarda e Unidade Local de Saúde de Castelo Branco. A informação dos dados refere-se ao consumo dos Hospitais e Instituições do Serviço Nacional de Saúde, reportada mensalmente ao INFARMED (Autoridade Nacional do Medicamento e Produtos de Saúde, I.P.). Foram identificados 48 medicamentos órfãos com Autorização de Introdução no Mercado na Europa, 47 são medicamentos com Autorização de Introdução no Mercado português, sendo a Mercaptopurina o único medicamento órfão que necessita de Autorização de Utilização Especial. O Tafamidis foi o medicamento que envolveu maior custo económico a nível nacional no tratamento destas doenças. O valor total de consumo no Centro Hospitalar Cova da Beira é de 11% em relação ao consumo total de medicamentos nesta unidade. As doenças Lisossomais são o grupo mais representativo, do qual faz parte a doença de Pompe. No entanto, a doença com uma prevalência mais elevada é a Hipertensão Arterial Pulmonar. Esta e as doenças oncológicas são as doenças comuns nas três unidades hospitalares. Na Unidade Local de Saúde da Guarda a doença com maior consumo de medicamentos órfãos é a Hipertensão Arterial Pulmonar e na Unidade Local de Saúde de Castelo Branco são as doenças oncológicas. O valor acumulado de medicamentos órfãos na região da Beira Interior no ano de 2013 representa1% do valor global de consumo destes medicamentos em Portugal. A representatividade de consumo destes medicamentos em Portugal em relação ao consumo total de medicamentos em meio hospitalar é de cerca de 7,7%.Em conclusão, o consumo de medicamentos órfãos é bastante elevado em relação aos restantes medicamentos, tanto na Beira Interior como no resto do país, apesar disso é bastante importante que continuem a existir medidas no sentido de incentivar os fabricantes e melhorar o acesso a estes medicamentos.
