Background Patients with diabetic foot disease require frequent screening to prevent complications and may be helped through telemedical home monitoring. Within this context, the goal was to determine the validity and reliability of assessing diabetic foot infection using photographic foot imaging and infrared thermography. Subjects and Methods For 38 patients with diabetes who presented with a foot infection or were admitted to the hospital with a foot-related complication, photographs of the plantar foot surface using a photographic imaging device and temperature data from six plantar regions using an infrared thermometer were obtained. A temperature difference between feet of > 2.2 °C defined a ''hotspot.'' Two independent observers assessed each foot for presence of foot infection, both live (using the Perfusion-Extent-Depth- Infection-Sensation classification) and from photographs 2 and 4 weeks later (for presence of erythema and ulcers). Agreement in diagnosis between live assessment and (the combination of ) photographic assessment and temperature recordings was calculated. Results Diagnosis of infection from photographs was specific (> 85%) but not very sensitive (< 60%). Diagnosis based on hotspots present was sensitive (> 90%) but not very specific (<25%). Diagnosis based on the combination of photographic and temperature assessments was both sensitive (> 60%) and specific (> 79%). Intra-observer agreement between photographic assessments was good (Cohen's j = 0.77 and 0.52 for both observers). Conclusions Diagnosis of foot infection in patients with diabetes seems valid and reliable using photographic imaging in combination with infrared thermography. This supports the intended use of these modalities for the home monitoring of high-risk patients with diabetes to facilitate early diagnosis of signs of foot infection.

- Introduction Heat-based training (HT) is becoming increasingly popular as a means of inducing acclimation before athletic competition in hot conditions and/or to augment the training impulse beyond that achieved in thermo-neutral conditions. Importantly, current understanding of the effects of HT on regenerative processes such as sleep and the interactions with common recovery interventions remain unknown. This study aimed to examine sleep characteristics during five consecutive days of training in the heat with the inclusion of cold-water immersion (CWI) compared to baseline sleep patterns. - Methods Thirty recreationally-trained males completed HT in 32 ± 1 °C and 60% rh for five consecutive days. Conditions included: 1) 90 min cycling at 40 % power at VO2max (Pmax) (90CONT; n = 10); 90 min cycling at 40 % Pmax with a 20 min CWI (14 ± 1 °C; 90CWI; n = 10); and 30 min cycling alternating between 40 and 70 % Pmax every 3 min, with no recovery intervention (30HIT; n = 10). Sleep quality and quantity was assessed during HT and four nights of 'baseline' sleep (BASE). Actigraphy provided measures of time in and out of bed, sleep latency, efficiency, total time in bed and total time asleep, wake after sleep onset, number of awakenings, and wakening duration. Subjective ratings of sleep were also recorded using a 1-5 Likert scale. Repeated measures analysis of variance (ANOVA) was completed to determine effect of time and condition on sleep quality and quantity. Cohen's d effect sizes were also applied to determine magnitude and trends in the data. - Results Sleep latency, efficiency, total time in bed and number of awakenings were not significantly different between BASE and HT (P > 0.05). However, total time asleep was significantly reduced (P = 0.01; d = 1.46) and the duration periods of wakefulness after sleep onset was significantly greater during HT compared with BASE (P = 0.001; d = 1.14). Comparison between training groups showed latency was significantly higher for the 30HIT group compared to 90CONT (P = 0.02; d = 1.33). Nevertheless, there were no differences between training groups for sleep efficiency, total time in bed or asleep, wake after sleep onset, number of awakenings or awake duration (P > 0.05). Further, cold-water immersion recovery had no significant effect on sleep characteristics (P > 0.05). - Discussion Sleep plays an important role in athletic recovery and has previously been demonstrated to be influenced by both exercise training and thermal strain. Present data highlight the effect of HT on reduced sleep quality, specifically reducing total time asleep due to longer duration awake during awakenings after sleep onset. Importantly, although cold water recovery accelerates the removal of thermal load, this intervention did not blunt the negative effects of HT on sleep characteristics. - Conclusion Training in hot conditions may reduce both sleep quantity and quality and should be taken into consideration when administering this training intervention in the field.

Exposure to ambient air pollution is a major risk factor for global disease. Assessment of the impacts of air pollution on population health and the evaluation of trends relative to other major risk factors requires regularly updated, accurate, spatially resolved exposure estimates. We combined satellite-based estimates, chemical transport model (CTM) simulations and ground measurements from 79 different countries to produce new global estimates of annual average fine particle (PM2.5) and ozone concentrations at 0.1° × 0.1° spatial resolution for five-year intervals from 1990-2010 and the year 2013. These estimates were then applied to assess population-weighted mean concentrations for 1990 – 2013 for each of 188 countries. In 2013, 87% of the world’s population lived in areas exceeding the World Health Organization (WHO) Air Quality Guideline of 10 μg/m3 PM2.5 (annual average). Between 1990 and 2013, decreases in population-weighted mean concentrations of PM2.5 were evident in most high income countries, in contrast to increases estimated in South Asia, throughout much of Southeast Asia, and in China. Population-weighted mean concentrations of ozone increased in most countries from 1990 - 2013, with modest decreases in North America, parts of Europe, and several countries in Southeast Asia.

Proximity of molecules is a crucial factor in many solid- state photochemical processes.'S2 The biomolecular photodimerization reactions in the solid state depend on the relative geometry of reactant molecules in the crystal lattice with center-to-center distance of nearest neighbor double bonds of the order of ca. 4 A. This fact emanates from the incisive studies of Schmidt and Cohen.2 One of the two approaches to achieve this distance requirement is the so-called "Crystal-Engineering" of structures, which essentially involves the introduction of certain functional groups that display in-plane interstacking interactions (Cl...Cl, C-He-0, etc.) in the crystal The chloro group is by far the most successful in promoting the /3- packing m ~ d e ,th~o,u~gh recent studies have shown its limitations? Another approach involves the use of constrained media in which the reactants could hopefully be aligned.

BACKGROUND Polygenic risk scores comprising established susceptibility variants have shown to be informative classifiers for several complex diseases including prostate cancer. For prostate cancer it is unknown if inclusion of genetic markers that have so far not been associated with prostate cancer risk at a genome-wide significant level will improve disease prediction. METHODS We built polygenic risk scores in a large training set comprising over 25,000 individuals. Initially 65 established prostate cancer susceptibility variants were selected. After LD pruning additional variants were prioritized based on their association with prostate cancer. Six-fold cross validation was performed to assess genetic risk scores and optimize the number of additional variants to be included. The final model was evaluated in an independent study population including 1,370 cases and 1,239 controls. RESULTS The polygenic risk score with 65 established susceptibility variants provided an area under the curve (AUC) of 0.67. Adding an additional 68 novel variants significantly increased the AUC to 0.68 (P = 0.0012) and the net reclassification index with 0.21 (P = 8.5E-08). All novel variants were located in genomic regions established as associated with prostate cancer risk. CONCLUSIONS Inclusion of additional genetic variants from established prostate cancer susceptibility regions improves disease prediction. Prostate 75:1467–1474, 2015. © 2015 Wiley Periodicals, Inc.

Background Epidemiological studies suggest a potential role for obesity and determinants of adult stature in prostate cancer risk and mortality, but the relationships described in the literature are complex. To address uncertainty over the causal nature of previous observational findings, we investigated associations of height- and adiposity-related genetic variants with prostate cancer risk and mortality. Methods We conducted a case–control study based on 20,848 prostate cancers and 20,214 controls of European ancestry from 22 studies in the PRACTICAL consortium. We constructed genetic risk scores that summed each man’s number of height and BMI increasing alleles across multiple single nucleotide polymorphisms robustly associated with each phenotype from published genome-wide association studies. Results The genetic risk scores explained 6.31 and 1.46 % of the variability in height and BMI, respectively. There was only weak evidence that genetic variants previously associated with increased BMI were associated with a lower prostate cancer risk (odds ratio per standard deviation increase in BMI genetic score 0.98; 95 % CI 0.96, 1.00; p = 0.07). Genetic variants associated with increased height were not associated with prostate cancer incidence (OR 0.99; 95 % CI 0.97, 1.01; p = 0.23), but were associated with an increase (OR 1.13; 95 % CI 1.08, 1.20) in prostate cancer mortality among low-grade disease (p heterogeneity, low vs. high grade <0.001). Genetic variants associated with increased BMI were associated with an increase (OR 1.08; 95 % CI 1.03, 1.14) in all-cause mortality among men with low-grade disease (p heterogeneity = 0.03). Conclusions We found little evidence of a substantial effect of genetically elevated height or BMI on prostate cancer risk, suggesting that previously reported observational associations may reflect common environmental determinants of height or BMI and prostate cancer risk. Genetically elevated height and BMI were associated with increased mortality (prostate cancer-specific and all-cause, respectively) in men with low-grade disease, a potentially informative but novel finding that requires replication.

Dance is a potential asset for peacebuilding, creating opportunities for nonverbal, embodied learning, exploring identity, and relationships. Peace scholars consider identity and relationships to the ‘other’ as key components in transforming conflict. Focusing on a case study in Mindanao, the Philippines, this paper explores the potential of dance in a peacebuilding context through embodied identity and relationships. In Mindanao, deep-seated cultural prejudices contribute to ongoing conflict entwined with identity. The permeable membrane (Cohen, Gutiérrez & Walker, 2011) is the organising framework describing the constant interaction between artists, facilitators, participants, and communities. It expands peace scholar John Paul Lederach’s concept of the moral imagination, requiring the capacity to envisage one’s self within a web of relationships. In this paper multiple methods of qualitative research including personal interviews are used to further the discussion regarding dance’s potential to diversify the nonverbal tools available for peacebuilding.

I denna pro gradu avhandling undersöker jag Jokela-fallets diskursiva efterdyningar utgående från ett moralpanikperspektiv. Syftet är att se huruvida reaktionerna på och förklaringarna till skolmassakern i Jokela kan anses präglas av moralpanik, samt vilka andra förklaringsmodeller som präglade den omfattande Jokela-debatten. Avhandling grundar sig på en kvalitativ diskursanalys av förklaringsmodellerna så som de presenterades i opinionstexter och insändare i Helsingin Sanomat hösten 2007. Jag analyserar hur den medierade Jokela-debatten utvecklades och formade perceptionerna om skolskjutningar, ungdomar och det finländska samhällstillståndet i stort. Jokela-fallet väckte starka reaktioner och rädsla. Man försökte förstå och förklara orsakerna till skottdramat. Flera olika förklaringsmodeller presenterades i medierna; en oro över ungdomens mentala hälsa och mobbning, webbens och populärkulturens farliga inflytande, det hårdnade samhällsklimatet och en försvagad välfärd, också den bristfälliga vapenlagstiftningen kritiserades. Den gemensamma nämnaren var att Jokela-fallet uppfattades som en indikator på hela samhällets tillstånd där en gemensam värdegrund och moral håller på att erodera. Mitt teoretiska perspektiv grundar sig främst på moralpanikteorier av Stanley Cohen och Chas Critcher som jag kopplar ihop med Ulrich Becks teori om risksamhället och Emile Durkheims anomiteori. Det här speglar jag mot bland annat Hille Koskelas forskning om rädslans kultur och trygghetssamhället, samt jämför med amerikansk forskning om skolskjutningar. Jokela-debatten formades främst av två dominerande diskurser: a) en rationaliseringsdiskurs som kännetecknas av att man försöker förklara Jokela-fallet genom förnuftsmässiga och logiska resonemang genom att peka på olika samhällsaspekter, b) en risk- och kontrolldiskurs som kännetecknas av moralpaniktendenser främst i form av oro över ungdomar, internet och försvagade samhällsnormer, samt krav på ökad social kontroll och reglering för att säkra trygghet och ordning i samhället. Jokela-fallet var inte ett klassiskt moralpanikfall, men fyllde många av kriterierna för moralpanik. Jokela-debatten uttryckte en rädslans kultur som genomsyrar dagens risksamhälle och som legitimerar ökad riskhantering och social kontroll, som försvar mot de hot och problem som samhället kollektivt upplever. Det ställdes krav på mera social kontroll och reglering på grund av upplevda hot mot samhällets normer och en försvagad moral, vilket är ett uttryck för moralpanik. Jokela-fallets diskursiva efterdyningar visar att tryggheten har blivit ett moraliskt imperativ. Trygghet har blivit något vi förutsätts eftersträva och som vi är färdiga att betala ett allt högre pris för, både mätt i pengar och i frihet.

Työni tutkimuskohteena ovat erilaiset distribuutiomallit ja redistribuution oikeutus. Lähtökohtanani työssäni toimi John Rawlsin aloittama keskustelu distributiivisesta oikeudenmukaisuudesta. Rawlsin teos A Theory of Justice (1971) toimi modernin tulonjaon oikeudenmukaisuus-keskustelun lähtölaukauksena. Rawlsin näkemyksien pääkommentaattoreina työssäni toimivat G.A Cohen, Robert Nozick, Michael Walzer, Thomas Nagel, Liam Murphy ja Thomas Pogge. Tutkimukseni tarkoituksena on tutkia yhteiskunnallisen tulonjaon oikeudenmukaisuutta. Käsittelen kysymyksiä siitä kuinka yhteiskunnalliset hyvät tulisi jakaa ja millaisia tulisi olla niiden rakenteellisten ratkaisujen, jotka mahdollistavat oikeudenmukai- sen yhteiskunnan. Toinen tärkeä kysymys työssäni on, kuinka voimme oikeuttaa jo kertaalleen jaetun uudelleenjakamisen eli redistribuution, ilman että kajoamme yksilöiden perusoikeuksiin. Tutkin työssäni distribuutioon ja redistribuutioon liittyvien näkemysten suhdetta niihin arvoihin, joita länsimaiset liberalismiin ja demokratiaan sitoutuneet yhteiskunnat korostavat, kuten ihmisten perusoikeudet, riittävä toimeentulo ja vapaus. Millaisilla tulonjakoon liittyvillä keinolla voimme päästä lähemmäksi kyseisten arvoja toteutumista yhteiskunnissamme? Tällöin kuvaan astuu myös kysymys lähtökohtien ja lopputuloksen oikeudenmukaisuuden merkityksestä. Voimmeko muutella toteutettua distribuutiota redistribuution avulla, jos emme saa aikaan niitä tuloksia, joita olimme toivoneet? Työni lopussa kysyn: jos toteamme, että meidän tulisi pyrkiä kohti oikeudenmukaisempaa tulonjakoa kansallisvaltioiden sisällä, niin miten tämän tulisi vaikuttaa näkemyksiimme kansainvälisestä tulonjaosta? Tutkimuksessani selvisi, että on vaikea löytää moraalisia perusteita sille, miksi yhteiskunnissamme ei tulisi pyrkiä kohti tasa- arvoisempaa tulonjakoa. Markkinoiden vapautta ja absoluuttisia omistusoikeuksia korostavilla näkemyksillä ei ole moraalisesti kestäviä perusteita, minkä vuoksi niihin vetoaminen ei ole riittävä syy olla puuttumatta tulonjakoon oikeudenmukaisuuden näkökulmasta. Länsimaisissa yhteiskunnissa arvopohjanamme ovat ihmis- ja kansalaisoikeudet. Samoin on linjattu, että yhteis- kuntaan kuuluu alueita, jotka eivät toimi markkinapohjaisesti. Yhteiskunnan puuttuminen tulonjakoon ei itsessään loukkaa kenenkään perusoikeuksia Redistribuutio on osa samaa prosessia kuin distribuutio. Mitään distribuutiota on vaikea pitää abso- luuttisena, eikä mikään distribuutio pysy muuttumattomana. This thesis is composed of an introductory chapter and four applications each of them constituting an own chapter. The common element underlying each of the chapters is the econometric methodology. The applications rely mostly on the leading econometric techniques related to estimation of causal effects. The first chapter introduces the econometric techniques that are employed in the remaining chapters. Chapter 2 studies the effects of shocking news on student performance. It exploits the fact that the school shooting in Kauhajoki in 2008 coincided with the matriculation examination period of that fall. It shows that the performance of men declined due to the news of the school shooting. For women the similar pattern remains unobserved. Chapter 3 studies the effects of minimum wage on employment by employing the original Card and Krueger (1994; CK) and Neumark and Wascher (2000; NW) data together with the changes-in-changes (CIC) estimator. As the main result it shows that the employment effect of an increase in the minimum wage is positive for small fast-food restaurants and negative for big fast-food restaurants. Therefore, it shows that the controversial positive employment effect reported by CK is overturned for big fast-food restaurants and that the NW data are shown, in contrast to their original results, to provide support for the positive employment effect. Chapter 4 employs the state-specific U.S. data (collected by Cohen and Einav [2003; CE]) on traffic fatalities to re-evaluate the effects of seat belt laws on the traffic fatalities by using the CIC estimator. It confirms the CE results that on the average an implementation of a mandatory seat belt law results in an increase in the seat belt usage rate and a decrease in the total fatality rate. In contrast to CE, it also finds evidence on compensating-behavior theory, which is observed especially in the states by the border of the U.S. Chapter 5 studies the life cycle consumption in Finland, with the special interest laid on the baby boomers and the older households. It shows that the baby boomers smooth their consumption over the life cycle more than other generations. It also shows that the old households smoothed their life cycle consumption more as a result of the recession in the 1990s, compared to young households.

Pro gradu -tutkielmani käsittelee tulonjaon oikeudenmukaisuuteen liittyviä kysymyksiä viime vuosikymmenien poliittisessa filosofiassa. Työni tutkimuskohteena ovat erilaiset distribuutiomallit ja redistribuution oikeutus. Lähtökohtanani työssäni toimi John Rawlsin aloittama keskustelu distributiivisesta oikeudenmukaisuudesta. Using path integrals, we derive an exact expression-valid at all times t-for the distribution P(Q,t) of the heat fluctuations Q of a Brownian particle trapped in a stationary harmonic well. We find that P(Q, t) can be expressed in terms of a modified Bessel function of zeroth order that in the limit t > infinity exactly recovers the heat distribution function obtained recently by Imparato et al. Phys. Rev. E 76, 050101(R) (2007)] from the approximate solution to a Fokker-Planck equation. This long-time result is in very good agreement with experimental measurements carried out by the same group on the heat effects produced by single micron-sized polystyrene beads in a stationary optical trap. An earlier exact calculation of the heat distribution function of a trapped particle moving at a constant speed v was carried out by van Zon and Cohen Phys. Rev. E 69, 056121 (2004)]; however, this calculation does not provide an expression for P(Q, t) itself, but only its Fourier transform (which cannot be analytically inverted), nor can it be used to obtain P(Q, t) for the case v=0.
