A articulação entre a psicanálise e a música, mais especificamente a produzida a partir do paradigma de Arnold Schönberg, renovado por John Cage, se mostra emblemática para pensar a constituição do sujeito em Sigmund Freud e Jacques Lacan, bem como para refletir sobre a escuta clínica, o ato analítico enquanto poético, e a escrita pulsional do sujeito como resposta à invocação da voz. O momento de estruturação do sujeito implica a dimensão de musicalidade da linguagem que permite o ato da fala. O sujeito nasce em um ponto em que o significante (simbólico) escreve no real do corpo um possível, um começo, uma marca que invoca uma nota e uma letra, sendo estes os dois aspectos da linguagem: a musicalidade (continuidade) e a fala (descontinuidade em movimento). Este ponto escreve e cria um vazio no sujeito que está e estará sempre em pulsação. Se o real grita caoticamente, é possível que se cante e se musique a vida com a criação de notas singulares, efeito do movimento desejante e de uma escrita pelo circuito da pulsão invocante na partitura já dada pelo Outro e face aos encontros com pedaços de real. A música tem a capacidade de retirar o sujeito de uma surdez de seu próprio desejo, o convocando a recriar a linguagem por seus atos. O paradigma de Schönberg, bem como a música criada a partir deste momento, nos dá a ouvir um saber-fazer com a voz no qual a dimensão equivocante (de equivoco e de invocação) da linguagem pode ressurgir por uma via nova. A transmissão de um saber-fazer com o objeto voz por ele efetuado se apresenta como uma radicalização do efeito de verdade do real, ressoando borromeanamente sobre o simbólico e o imaginário, invocando o momento originário do sujeito, de um começo sempre a recomeçar, que se faz ouvir como uma invocação de musicar a vida de uma maneira ética, estética e poética. É através dos eixos acima expostos que nos é possível sustentar, com Lacan, uma prática clínica orientada para além da repetição em direção a um significante novo. Trata-se de uma orientação que parte dos encontros com o real aos quais o sujeito é confrontado ao acaso visando o movimento renascente pelo qual ele pode re-escutar o inaudito do real contínuo perdido, o que faz com que seu ritmo singular possa ser, uma vez mais e de modo inédito, reinventado. A psicanálise pode ser, portanto, entendida como uma prática invocante, como uma abertura para que o sujeito possa, com entusiasmo, musicar a vida.

Os aspectos quânticos de teorias de campo formuladas no espaço-tempo não comutativo têm sido amplamente estudados ao longo dos anos. Um dos principais aspectos é o que na literatura ficou conhecido como mixing IR/UV. Trata-se de uma mistura das divergências, que foi vista pela primeira vez no trabalho de Minwalla et al [28], onde num estudo do campo escalar não comutativo com interação quártica vemos já a 1 loop que o tadpole tem uma divergência UV associada a sua parte planar e, junto com ela, temos uma divergência IR associada com um gráfico não planar. Essa mistura torna a teoria não renormalizável. Dado tal problema, houve então uma busca por mecanismos que separassem essas divergências a fim de termos teorias renormalizáveis. Um mecanismo proposto foi a adição de um termo não local na ação U*(1) para que esta seja estável.Neste trabalho, estudamos através da renormalização algébrica a estabilidade deste modelo. Para tal, precisamos localizar o operador não local através de campos auxiliares e seus respectivos ghosts (metodo de Zwanziger) na intenção de retirar os graus de liberdade indesejados que surgem. Usamos o approachda quebra soft de BRST para analisar o termo que quebra BRST, que consiste em reescrevermos tal termo com o auxílio de fontes externas que num determinado limite físico voltam ao termo original.Como resultado, vimos que a teoria com a adição deste termo na ação só é renormalizável se tivermos que introduzir novos termos, sendo alguns deles quárticos. Porém, estes termos mudam a forma do propagador, que não desacopla as divergências. Um outro aspecto que podemos salientar é que, dependendo da escolha de alguns parâmetros, o propagador dá indícios de termos um fótonconfinante, seguindo o critério de Wilson e o critério da perda da positividade do propagador.

A presente dissertação de mestrado propõe uma reflexão sobre os lugares da arte no contexto de um mundo globalizado em que o deslocamento se coloca como uma questão fundamental em diversas esferas da vida, dos homens, das coisas. Nesse mundo da mobilidade física ou virtual, concreta ou imaterial, objetiva ou simbólica, as obras de arte percorrem caminhos diversos e nos interrogam sobre como suportam, em seus deslocamentos, serem carregadas de um sistema de visibilidade para outro. O artista assume diferentes papéis e se relaciona com as questões da arte a partir de perspectivas móveis. Quando se deslocam artista e obra partem de um lugar conhecido e se movem para desconhecer. Caminham por Algum Lugar para chegar a Lugar Algum. Com o objetivo de especular sobre o que chamei de Algum Lugar, Lugar Algum, construo uma sequência de ações possíveis: caminhar, parar, compartilhar / perceber, imaginar / modificar, inventar, re-significar / afastar, aproximar / entrar, sair. Construir meu trabalho é orientar-me por um conjunto de ações em aberto nem sempre na mesma ordem. Como estratégia, exercito a ação de mover-me por lugares desconhecidos e busco manter sempre aumentado o meu nível de percepção dos lugares por onde passo. Uso meu corpo como um mediador sensível e me permito ser afetada. Procuro, de forma proposital, variar entre o estado de conhecer e de desconhecer. Trabalho a partir do cotidiano para tirar dele o que se mostre saliente e para me dedicar a reinventar lugares com o meu corpo e na minha mente. Ao relacionar a geografia do que está visível com um amplo universo que ainda se encontra num estado de vir a ser, coloco-me diante de uma geografia imaginária, busco uma forma de continuar infinitamente a construir hipóteses sobre Algum Lugar. Através de relações entre o meu processo de trabalho e algumas obras de artistas contemporâneos procuro estabelecer conexões que contribuam para deixar em aberto um campo potencial de investigações sobre os lugares da arte

After 9/11, it has become increasingly obvious that strongly held religious convictions about the end of the world cannot be dismissed as the predictable consequences of deprivation, as several generations of social scientists once claimed. Instead, it has become clear that these kinds of ideas, having a life of their own, may establish discourses which may have extraordinary capacity to cross nations, cultures and even religions, encouraging passive withdrawal from the political world as well as inspiring vicious and sometimes violent attempts at its subjugation, underwriting the ‘war on terror’ as well as inspiring some of those intent on the destruction of the United States. This article describes one of Ireland’s most successful intellectual exports – a very specific system of thinking about the end of the world known as ‘dispensational premillennialism.’ And the article will move from county Wicklow in the early nineteenth century, through the troubled decades of American modernity, to arrive, perhaps unexpectedly, in the company of the soldiers of radical jihad. The article will describe the globalisation of a discourse which was developed among the most privileged classes of early nineteenth-century Ireland to explain and justify their attempt to withdraw from the world, and which has more recently been used to explain and justify sometimes violent political interventions by both prominent Western politicians and some of the most marginalised and desperate inhabitants of our broken twenty-first century.

Purpose: Some phase II studies have suggested that the combination of interferons (IFNs) with dacarbazine (DTIC) in the treatment of malignant melanoma (MM) increases the antitumor activity of DTIC alone. In an attempt to confirm this hypothesis, a randomized study was performed with the further intent of observing whether low doses of recombinant interferon alfa-2a (rIFNα2a) could be as effective as intermediate doses. Patients and Methods: Two hundred sixty-six patients were randomized onto three different treatment arms: DTIC 800 mg/m 2 intravenously (IV) days 1 and 21; DTIC plus rIFNα2a 9 mIU intramuscularly (IM) daily; and DTIC plus rIFNα2a 3 mIU IM three times per week. Major prognostic factors were well balanced among the three arms. Chemotherapy was administered for a maximum of eight cycles. After 6 months of therapy, rIFNα2a was continued until disease progression at 3 mIU three times per week in responding patients who had received the combined treatment. Results: The percentage of objective responses did not differ among the three groups (20%, 28%, and 23%, respectively), although a significant prolongation of response duration was observed when rIFNα2a was added to DTIC (2.6 v 8.4 v 5.5 months, respectively). However, this improvement in response duration did not translate into an amelioration of overall survival. The addition of rIFNα2a led to the onset of flu-like syndrome, but in no case was it necessary to withdraw the treatment program and no toxic deaths or life-threatening toxicities were reported. Conclusion: In this study, rIFNα2a significantly prolonged response duration, whereas no effects on response rate and survival were observed; rIFNα2a 3 mIU appeared to be equally effective and better tolerated than 9 mIU.

Objective: The purpose of the study was to examine the relationship of surveillance and control activities in Canadian hospitals with rates of nosocomial methicillin-resistant S. aureus (MRSA), C. difficile associated diarrhea (CDAD), and vancomycin-resistant Enterococcus (VRE). Methods: Surveys were sent to Infection Control programs in hospitals that participated in an earlier survey of infection control practices in Canadian acute care hospitals. Results: One hundred and twenty of 145 (82.8%) hospitals responded to the survey. The mean MRSA rate was 2.0 (SD 2.9) per 1,000 admissions, the mean CDAD rate was 3.8 (SD 4.3), and the mean VRE rate was 0.4 (SD 1.5). Multiple stepwise regression analysis found hospitals that reported infection rates by specific risk groups (r = - 0.27, p < 0.01) and that kept attendance records of infection control teaching activities (r = - 0.23, p < 0.01) were associated with lower MRSA rates. Multiple stepwise regression analysis found larger hospitals (r = 0.25, p < 0.01) and hospitals where infection control committees or staff had the direct authority to close a ward or unit to further admissions due to outbreaks (r = 0.22, p < 0.05) were associated with higher CDAD rates. Multiple logistic regression analysis found larger hospitals (OR = 1.6, CI 1.2 - 2.0, p = 0.003) and teaching hospitals (OR = 3.7, CI 1.2 - 11.8, p = 0.02) were associated with the presence of VRE. Hospitals were less likely to have VRE when infection control staff frequently contacted physicians and nurses for reports of new infections (OR = 0.5, CI 0.3 - 0.7, p = 0.02) and there were in-service programs for updating nursing and ancillary staff on current infection control practices (OR = 0.2, CI 0.1 - 0.7, p = 0.01). Conclusions: Surveillance and control activities were associated with MRSA and CDAD rates and the presence of VRE. Surveillance and control activities might be especially beneficial in large and teaching hospitals.

The phenomenon of paediatric social admission describes the hospitalisation of children for medicallynon- urgent and/or social reasons. Much of the research in this field has been in relation to avoidable admissions which have been identified, studied and condoned based on strict medical criteria. Such research has tended to mask the significance of social factors and the commonplace practice of Paediatric Social Admission. This paper examines decision making from the perspective of the health professionals at the clinical coal face. It explores how health professionals are influenced by social, organisational and subjective factors which determine their decision to admit or not to admit a child to hospital. The paper reports findings from interviews with 27 health professionals involved in PSA and a document audit of 21 specific cases. In doing so it begins to address the inherent paradox of responding to a child and families social needs in a medical context.

The paradox of paediatric social admission involves the hospitalisation of children for medically non- urgent and/or social reasons. Much of the research in this field has been in relation to avoidable admissions which have been identified, studied and condoned based on strict medical criteria. Such research has tended to mask the significance of social influences and the reasons why health professionals make such decisions. This paper explores social, organisational and subjective influences on medical decision making and is based on study involving interviews with 27 health professionals directly involved in paediatric social admissions (PSA). The findings highlight inherent paradoxes as a consequence of responding to social concerns in a medical context.

Title. A concept analysis of renal supportive care: the changing world of nephrology

Aim. This paper is a report of a concept analysis of renal supportive care.

Background. Approximately 1.5 million people worldwide are kept alive by renal dialysis. As services are required to support patients who decide not to start or to withdraw from dialysis, the term renal supportive care is emerging. Being similar to the terms palliative care, end-of-life care, terminal care and conservative management, there is a need for conceptual clarity.

Method. Rodgers' evolutionary method was used as the organizing framework for this concept analysis. Data were collected from a review of CINAHL, Medline, PsycINFO, British Nursing Index, International Bibliography of the Social Sciences and ASSIA (1806-2006) using, 'renal' and 'supportive care' as keywords. All articles with an abstract were considered. The World Wide Web was also searched in English utilizing the phrase 'renal supportive care'.

Results. Five attributes of renal supportive care were identified: available from diagnosis to death with an emphasis on honesty regarding prognosis and impact of disease; interdisciplinary approach to care; restorative care; family and carer support and effective, lucid communication to ensure informed choice and clear lines of decision-making.

Conclusion. Renal supportive care is a dynamic and emerging concept relevant, but not limited to, the end phase of life. It suggests a central philosophy underpinning renal service development that allows patients, carers and the multidisciplinary team time to work together to realize complex goals. It has relevance for the renal community and is likely to be integrated increasingly into everyday nephrology practice.

Autonomic management can be used to improve the QoS provided by parallel/distributed applications. We discuss behavioural skeletons introduced in earlier work: rather than relying on programmer ability to design “from scratch” efficient autonomic policies, we encapsulate general autonomic controller features into algorithmic skeletons. Then we leave to the programmer the duty of specifying the parameters needed to specialise the skeletons to the needs of the particular application at hand. This results in the programmer having the ability to fast prototype and tune distributed/parallel applications with non-trivial autonomic management capabilities. We discuss how behavioural skeletons have been implemented in the framework of GCM(the Grid ComponentModel developed within the CoreGRID NoE and currently being implemented within the GridCOMP STREP project). We present results evaluating the overhead introduced by autonomic management activities as well as the overall behaviour of the skeletons. We also present results achieved with a long running application subject to autonomic management and dynamically adapting to changing features of the target architecture.
Overall the results demonstrate both the feasibility of implementing autonomic control via behavioural skeletons and the effectiveness of our sample behavioural skeletons in managing the “functional replication” pattern(s).

Aim: This paper is a report of a study to examine the role of personality and self-efficacy in predicting academic performance and attrition in nursing students.

Background: Despite a considerable amount of research investigating attrition in nursing students and new nurses, concerns remain. This particular issue highlights the need for a more effective selection process whereby those selected are more likely to complete their preregistration programme successfully, and remain employed as Registered Nurses.

Method: A longitudinal design was adopted. A questionnaire, which included measures of personality and occupational and academic self-efficacy, was administered to 384 students early in the first year of the study. At the end of the programme, final marks and attrition rates were obtained from university records for a total of 350 students. The data were collected from 1999 to 2002.

Findings: Individuals who scored higher on a psychoticism scale were more likely to withdraw from the programme. Occupational self-efficacy was revealed to be a statistically significant predictor of final mark obtained, in that those with higher self-efficacy beliefs were more likely to achieve better final marks. Extraversion was also shown to negatively predict academic performance in that those with higher extraversion scores were more likely to achieve lower marks.

Conclusion: More research is needed to explore the attributes of successful nursing students and the potential contribution of psychological profiling to a more effective selection process.

Throughout Africa, charismatic Christianity has been caricatured as an inhibitor of democratization. Its adherents are said either to withdraw from the rough and tumble of politics ('pietism') or to preach a prosperity gospel that encourages believers to pour their resources into their churches in the hope that God will 'bless' them. Both courses of action are said to encourage such people to be politically quietist, with no interest in democratization or other forms of political activity. This is said to thwart democratization. This article utilizes an ethnographic case study of a 'progressive' charismatic congregation in Harare, Zimbabwe, in 2007, to provide evidence that 'pietism' and 'prosperity' are not the only options for charismatic Christianity. Drawing on the concept of 'spiritual capital', it argues that some varieties of charismatic Christianity have the resources to contribute to democratization. For example, this congregation's self-styled 'de-institutionalization' process is opening up new avenues for people to learn democratic skills and develop a worldview that is relationship-centred, participatory, and anti-authoritarian. The article concludes that spiritual capital can be a useful tool for analysing the role of religions in democratizations. It notes, however, that analysts should take care to identify and understand what variety of spiritual capital is generated in particular situations, focusing on the worldviews it produces and the consequences of those worldviews for democratization. © 2009 Taylor & Francis.

Community asset transfer enables local groups to own or manage a government owned facility and/or related services. For critics, it is merely an extension of roll-back neoliberalism, permitting the state to withdraw from welfare and transfer risk from local government to ill-defined communities. The paper uses quantitative and case study data from Northern Ireland to demonstrate its transformative potential by challenging the notion of private property rights, enabling communities to accumulate and endanger forms of cooperative consumption. It concludes by highlighting the implications for more progressive forms of social economics in relation to public and private markets and government sponsorship of its own development.
