OBJETIVO: Avaliar o volume orbitário de coelhos recém-nascidos, enucleados ou eviscerados, sem ou com reposição do volume orbitário por esferas de polietileno poroso de 10 mm, usando a tomografia computadorizada, o teste de deslocamento de água e a medida da abertura orbitária por Image-J. MÉTODOS: Estudo experimental, usando 48 coelhos albinos, com idade de 42 dias, submetidos à evisceração ou enucleação, com implante de esfera de polietileno poroso de 10 mm de diâmetro ou sem implante. O lado esquerdo não foi submetido à cirurgia e foi usado como controle. Após 1, 3 e 6 meses, foram sacrificados quatro animais de cada grupo. Os crânios foram preparados para estudo do osso seco por maceração da peça, e analisados em relação ao volume orbitário por tomografia computadorizada, teste de deslocamento de água e avaliação da abertura orbitária usando o software Image-J. RESULTADOS: O volume das órbitas enucleadas ou evisceradas, com ou sem o uso do implante não apresentaram diferenças estatisticamente significativas (p<0,05) entre si, quando avaliadas por tomografia computadorizada, pelo teste de deslocamento de água e pelo Image-J. CONCLUSÃO: O desenvolvimento orbitário de coelhos com idade superior a 42 dias de vida não se altera quando se opta pela enucleação ou pela evisceração, ou quando se utiliza ou não implantes orbitários. O teste de deslocamento de água apresentou resultados semelhantes aos fornecidos pela tomografia, sugerindo-se sua utilização em experimentação científica.

OBJETIVOS: Avaliar o posicionamento palpebral em portadores de cavidade anoftálmica com e sem prótese ocular externa, utilizando o processamento de imagem digital. MÉTODOS: Dezoito pacientes foram avaliados qualitativa e quantitativamente na Faculdade de Medicina de Botucatu - Universidade Estadual Paulista - UNESP, com e sem a prótese externa. Usando imagens obtidas por filmadora e processadas usando o programa Scion Image, mediu-se a altura do sulco palpebral superior, a altura da fenda palpebral e os ângulos palpebrais dos cantos interno e externo. RESULTADOS: Pseudo-estrabismo e sulco palpebral superior profundo foram as alterações mais freqüentes ao exame externo. Houve diferença significativa em todas as variáveis estudadas, com diminuição da altura do sulco palpebral superior, aumento da área da fenda palpebral e aumento dos ângulos palpebrais interno e externo quando o paciente estava usando a prótese externa. CONCLUSÃO: Todos os pacientes avaliados apresentaram algum tipo de anormalidade órbito-palpebral, o que reflete a dificuldade em se proporcionar ao portador de cavidade anoftálmica um aspecto idêntico ao que existe na órbita normal. O processamento de imagens digitais permitiu avaliação objetiva das dimensões óculo-palpebrais, o que poderá contribuir nas avaliações seqüenciais dos portadores de cavidade anoftálmica.

Keratocystic odontogenic tumor (KCOT) is a benign intraosseous neoplasm of odontogenic origin with high recurrence rate. To date, various conservative or aggressive management strategies have been suggested as a method of treatment. Decompression is a conservative method that has been used in the treatment of large odontogenic cysts. The present paper reports a case of KCOT located in the mandible and discusses the importance of its management using conservative methods. The authors present a case of a 38-year-old patient with a KCOT located in the right mandibular angle and ascending ramus, which was treated by decompression followed by enucleation and curettage. The lesion did not recur during a follow-up period of 3 years after surgery. Preserving important structures of the bone and soft tissue decompression is a method with low morbidity. In addition, according to the literature, decompression has a success rate at least as high as the one of most aggressive treatments.

Odontogenic myxomas (OMs) are nonencapsulated rare benign tumors that can occur in gnathic bones. They are locally invasive and have a high recurrence rate. Radiologically, OMs show a multilocular (in the majority of cases) or unilocular radiolucency, with either distinct or poorly defined margins. Histopathologically, OMs are characterized by spindle-, wedge-, or stellate-shaped cells loosely arranged in an abundant mucoid background. Myxomas are mainly asymptomatic. Radical surgery, excision, and enucleation followed by curettage of the surrounding bony tissue have all been advocated as treatment options. This study presents a successful case of conservative treatment of OMs with a 5-year follow-up.

The objectives of this study were, through a literature review, to point the differences between orbital implants and their advantages and disadvantages, to evaluate prosthesis motility after orbital implants are inserted, and to point the implant wrapping current risks. Sixty-seven articles were reviewed. Enucleation implants can be autoplastics or alloplastics and porous (including natural and synthetic hydroxyapatite [HA]) or nonporous (silicone). Hydroxyapatite is the most related in the literature, but it has disadvantages, too, that is, all orbital implants must be wrapped. Exposure of the porous orbital implant can be repaired using different materials, which include homologous tissue, as well as autogenous graft, xenograft, and synthetic material mesh. The most used materials are HA and porous polyethylene orbital implant. The HA implant is expensive and possibly subject corals to damage, different from porous polyethylene orbital implants. Porous implants show the best prosthesis motility and a minimum rate of implants extrusion. Implant wraps can facilitate smoother entry of the implant into the orbit and allow reattachment of extraocular muscles. They also serve as a barrier between the overlying soft tissue and the rough surface of the implant, protecting implants from exposure or erosion.

Keratocystic odontogenic tumor is characterized by high recurrence rates. Conservative or aggressive management has been suggested as a method of treatment. Decompression is a conservative treatment that has been used in the treatment of large odontogenic cysts. The authors report a case of a 14-year-old patient with a keratocystic odontogenic tumor located in the right maxilla, which was treated by decompression followed by enucleation with curettage. The lesion did not recur on follow-up for 3 years after the enucleation surgery.

Estudaram-se as alterações tonométricas, paquimétricas e de comprimento axial em cães com glaucoma submetidos à ablação uveal intravítrea. Foram avaliados 13 olhos irreversivelmente cegos de cães que apresentavam glaucoma crônico unilateral, nos quais realizou-se a ablação uveal intra-vítrea, por meio de injeção na câmara vítrea de 0,5ml de sulfato de gentamicina (40mg/ml) associado a 0,3ml de fosfato de dexametasona (4mg/ml). As mensurações da pressão intra-ocular (Po), espessura corneana e eixo axial com tonometria de aplanação, paquimetria ultra-sônica e ultra-sonografias modos A e B foram realizadas no dia da ablação (M0) e após uma (M1), quatro (M4), oito (M8), 12 (M12), 24 (M24) e 48 semanas (M48). A Po diminuiu significativamente em todos os momentos em relação ao M0, com aumento da espessura corneana ao longo do experimento. Nas avaliações ultra-sonográficas notou-se diminuição significativa do bulbo ocular a partir de M4, com medidas ultra-sonográficas modo A significativamente maiores que as do modo B. O procedimento foi efetivo na redução da Po e na diminuição do eixo axial, demonstrando ser viável no controle do glaucoma crônico em olhos irreversivelmente cegos, e ser uma alternativa à enucleação e inserção de prótese ocular.

Dezoito coelhos, Norfolk, fêmeas, com 45 dias de idade foram divididos em três grupos de seis animais e submetidos a enucleação transpalpebral. Os animais do grupo I receberam na cavidade orbitaria acrílico auto-polimerizável, os do grupo II pericárdio eqüino conservado em glicerina e os do grupo III foram mantidos como controle. Para avaliação macroscópica e histopatológica das cavidades orbitarias, três animais de cada grupo foram sacrificados com 30 e 60 dias após a implantação. Apesar da resina ter sido aplicada na fase pastosa, na qual a alta temperatura que ocorre durante a polimerizaçâo pode ser lesiva aos tecidos, foi o produto que apresentou os melhores resultados.

Purpose: To evaluate the macroscopy, microstructure, and tissue reaction of a double-setting a-tricalcium phosphate bone cement used as an intraocular implant in rabbits.Methods: the internal and external surface of the double-setting a-tricalcium phosphate implant was analyzed macroscopically and by scanning electron microscopy. Twelve New Zealand rabbits received 12-mm implants made of double-setting alpha-tricalcium phosphate cement after unilateral evisceration. Clinical evaluation was performed daily for the first 15 days after surgery and at 15-day intervals until the end of the study period. For histopathologic analysis, 3 animals per experimental period were submitted to enucleation at 15, 45, 90, and 180 days.Results: on gross inspection, the external surface of the implant was solid, smooth, and compact. The microarchitecture was characterized by the formation of columns of hexagonal crystals with interconnecting channels forming micropores. No wound dehiscence, signs of infection, or implant extrusion were observed in any animal throughout the study period. Histologic examination revealed the formation of fibrovascular tissue surrounding the implants, and there were signs of minimal integration of the surface limiting the fibrocellular cap with the space previously occupied by the implant.Conclusions: the double-setting alpha-tricalcium phosphate implant behaved as an inert and nonintegratable material. The lack of incorporation of this material by fibrovascular tissue is related to its characteristics of compactness and high resistance.

Described is a case of squamous-cell carcinoma arising in a residual cyst in a 60-year-old edentulous woman. The clinical and radiographic diagnosis was residual cyst, and the treatment employed was conservative surgical enucleation. The cause of these changes is unknown. Theoretically, the inflammatory reaction may have been a predisposing factor.

PURPOSE: Gelatinous polyethylene (GP) has been used as an orbital implant to evaluate volume maintenance and tissue reaction in the anophthalmic socket in a rabbit model. METHODS: Twenty-eight Norfolk white rabbits underwent unilateral evisceration with placement of 2ml GP (GP group - 16 animals) or 2ml 0.9% sodium chloride solution (Control group - 12 animals). Animal behavior, postoperative tissue response and socket conditions were evaluated. The animals were sacrificed at 7, 15, 30 and 60 days after the surgery and the bulbs were enucleated; volume maintenance and histopathological evaluation were done in the enucleated bulbs. The results were submitted to statistical analysis. RESULTS: All animals showed normal behavior. The orbital volume maintenance was better in the GP group. Histopathology in the GP animals showed mild tissue inflammation throughout the evaluation period and a thin pseudocapsule formation surrounding the orbital implant material. CONCLUSION: The clinical findings, good orbital volume maintenance and mild inflammatory tissue response make GP a good choice to replace the lack of volume in anophthalmic socket reconstruction.

Dentigerous cysts are benign odontogenic cysts associated with the crowns of permanent teeth. They are usually single in occurrence and located in the mandible. The purpose of this case report was to describe the management of 2 dentigerous cysts in children. The treatments instituted were the extraction of the deciduous tooth involved followed by marsupialization in the first case and enucleation in the second one. Both treatments allowed rapid healing of the lesion and eruption of the permanent teeth without the need for orthodontic treatment.

Purpose: To evaluate the characteristics of patients with anophthalmic cavity who developed sphere extrusion. Methods: A retrospective observational study was done evaluating 37 patients with anophthalmic cavity and sphere extrusion at the Faculdade de Medicina de Botucatu-UNESP. Results: Extrusion was observed in enucleated and eviscerated cavities. The majority of the patients had the eye removed because of phthisis bulbi or trauma and the extrusion happened 1 or 2 years after the surgery. Extrusion was preceded by conjunctival dehiscence and exposure of the sphere and occurred with all used implants. Conclusion: Complications after orbital implant placement are a possibility. Dehiscence and sphere extrusion may happen and another surgery would be necessary. The patient and the ophthalmologist have to be prepared for this.

The odontogenic keratocyst, also known as the keratocystic odontogenic tumor, is an aggressive, intraosseous lesion of odontogenic origin that presents a high rate of recurrence. Treatment modalities include aggressive surgical procedures and more conservative approaches that significantly influence the lesion's recurrence potential. The purpose of this case report was to demonstrate a conservative approach in the treatment of an extensive keratocystic odontogenic tumor, located in the mandible's posterior region, using decompression and enucleation.
