Objective: To describe the barriers and facilitator factors to follow the attention flow of professionals injured by biological material in the worker perspective. Method: Qualitative descriptive study with data collected through individual interviews with 18 injured workers, assisted in reference public units in the city of Goiânia. The content analysis was carried out with assistance of the ATLAS.ti 6.2 software, under the work organization and subjective perspectives. Results: From the interviews regarding the barriers and facilitator factors emerged the categories: organizational structure, Support from close people, and Knowledge influence. Conclusion: The organized services have enabled more qualified consultations and the workers follow-up, which caused a satisfaction feeling in relation to the working environment.

Objective To analyze innovative contents on Early Child Development Promotion. Method This action-research involves nine faculties from four Higher Education Institutions at inner-state of São Paulo, Brazil.Data were collected by syllabi analyses (2009-2011), interviews and focus group. We have adopted an ECDP underpinning from international consensus, thus evaluating KT Results We have found relevant incorporation between teaching and extension in Nursing (87,5%) and Psychology (75%) undergraduate courses, while Pedagogy was restricted to teaching. Conclusion This KT evaluation has evinced innovative potential of extension, regardless teaching and research, for a better Early Childhood.

Abstract OBJECTIVE Understanding the meanings attributed by family caregivers of children in hospital environments about their interactions with nursing professionals. METHODS This qualitative study used Symbolic Interactionism as a theoretical reference and Grounded Theory as the methodological framework. It was carried out in a Pediatrics Center in southern Brazil, in the first half of 2013. Participants were 15 family caregivers of hospitalized children. Data were collected through interviews and submitted to open and axial analysis. RESULTS Interactions with the nursing team enable family to trust or distrust in the provided child care and to positively evaluate the care received. CONCLUSION Interactions between family members and the nursing team contribute to the significance attributed by the family to the nursing care received by the child. Nurses should be aware of the attitudes of the nursing team regarding the child and their family, prioritizing humanized care.

Actualmente, um desafio permanente se coloca ao sistema educativo em todo o mundo devido aos impulsos decorrentes das incomensuráveis possibilidades que as TIC oferecem ao processo de ensino e aprendizagem e na mudança que elas acarretam nas atitudes dos agentes educativos e na própria estrutura escolar. Em Cabo Verde as TIC têm despertado interesse dos responsáveis da educação no concernente ao seu uso em contexto pedagógico e a existência de projectos voltados para sua utilização no ensino secundário é uma realidade. Com efeito, iniciou-se no ano lectivo 2009/2010 a experiência de integração das TIC em duas escolas secundárias do país. Porém, volvidos cinco anos, importa saber como estão sendo vivenciadas estas experiências, particularmente no que diz respeito à disponibilidade e acessibilidade das TIC por parte dos principais agentes educativos, a forma como se organiza a escola para colocá-las à disposição destes, o grau de satisfação dos professores com relação à sua formação em TIC, as atitudes dos professores e alunos face as TIC e o nível de sua utilização em contexto de sala de aula. De modo que, o problema em estudo centra-se em saber como está a decorrer o processo de integração das TIC no ensino secundário, no âmbito das atuais políticas de incentivo ao uso das tecnologias na educação. Do ponto de vista metodológico optamos por um estudo de caso de natureza exploratória (Yin, 1993; Gil, 1994; Morgado, 2012) e para recolher os dados aplicamos dois inquéritos por questionário, em formato papel, um dirigido a alunos, (uma turma por ano) e outro a todos os professores da escola em estudo. Foram também realizadas duas entrevistas a dois dirigentes da escola. A escolha de fontes quantitativas (inquéritos) e qualitativas (entrevistas) tem base de sustentação nos argumentos apresentados por Yin (1994) e Coutinho (2005). As conclusões do estudo apontam para o reconhecimento de que há ainda muito por fazer no tocante à integração pedagógica das TIC, principalmente no que tange à disponibilidade das TIC (a maioria dos alunos não tem acesso aos computadores e não existem softwares), na organização da escola (falta de técnicos qualificados, e inexistência de projecto pedagógico para uso das TIC), na formação dos professores (necessidade de formação continua) e na ausência das TIC nas práticas pedagógicas, conforme nos ensina a literatura especializada sobre a integração das TIC (Pelgrum, 2001; Paiva, 2002; Silva & Miranda, 2005; Amante, 2007; OIE, 2008, Sunkel, 2009; Barbosa & Loureiro, 2011).

Cette recherche investigue l'impact de la transition à la parentalité sur l'identité conjugale. Afin de mettre en évidence les bouleversements induits par l'arrivée du premier enfant sur le système-couple, deux groupes de sujets ont constitué notre échantillon : des couples parents d'un premier enfant âgé de 9 à 12 mois et des couples sans enfant mais avec une durée de vie commune équivalente au premier groupe. Chaque couple a été rencontré dans le cadre d'un unique entretien. Leur première tâche a été de décrire leurs vies de couple passée et actuelle au travers de valeurs et devises. Un questionnaire créé pour cette recherche a ensuite permis d'évaluer la représentation des conjoints quant à l'évolution de leur couple, et ce sur la base de cinq dimensions à même de caractériser la manière d'être ensemble des conjoints. Finalement, les jeunes parents ont participé à un entretien semi-directif afin de témoigner des changements personnels et de couple vécus dans le cadre de la transition à la parentalité. Des analyses qualitatives et quantitatives basées sur les données récoltées au travers de nos trois outils révèlent plusieurs résultats. Les conjoints sans enfant décrivent avant tout leur couple comme un cocon au sein duquel deux individus autonomes trouvent refuge et réconfort. Les jeunes parents se démarquent quant à eux par une diminution de leur sentiment d'indépendance, reflet de la nécessaire collaboration propre au co-parentage. Une analyse des entretiens semi-structurés croisée avec l'évaluation du degré de satisfaction conjugale permet le constat suivant : la diminution avérée de la satisfaction conjugale lors de la transition à la parentalité n'est pas strictement associée aux bouleversements conjugaux. Ce déclin lors de l'arrivée et de l'accueil du premier enfant semble en effet être également en lien avec une difficile articulation, chez chaque partenaire, de leurs identités personnelle, conjugale, parentale et socio-professionnelle. - This research investigates the impact of the transition to parenthood on marital identity. To highlight the changes brought about by the arrival of the first child on the couple, two groups of subjects constituted our sample: couples with a first child of 9 to 12 months and childless couples but with a period of cohabitation equivalent to that of the first group. Each couple was interviewed once only. Their first task was to describe their lives as a couple past and present through values and principles. A questionnaire devised for this research was then used to evaluate partners' responses regarding the evolution of their relationship, this based on five criteria to characterise the couples' way of being together. Finally, young parents participated in a semi- structured interview to describe personal changes and those as a couple experienced in the transition to parenthood. Qualitative and quantitative analyses based on data collected through our three tools reveal several results. Spouses without children describe their relationship primarily as a cocoon in which two autonomous individuals find refuge and comfort. Young parents differ in reducing their feelings of independence, reflecting the collaborative needs specific to co-parenting. An analysis of the semi- structured interviews crossed with the assessment of marital satisfaction gives rise to the following observation: the pronounced reduction in marital satisfaction during the transition to parenthood is not strictly associated with marital disruption. This decline upon the arrival of the first child seems to be in line with a difficult balance for each partner between their personal, marital, parental and socio- professional identities.

On 1 January 2012 Swiss Diagnosis Related Groups (DRG), a new uniform payment system for in-patients was introduced in Switzerland with the intention to replace a "cost-based" with a "case-based" reimbursement system to increase efficiency. With the introduction of the new payment system we aim to answer questions raised regarding length of stay as well as patients' outcome and satisfaction. This is a prospective, two-centre observational cohort study with data from University Hospital Basel and the Cantonal Hospital Aarau, Switzerland, from January to June 2011 and 2012, respectively. Consecutive in-patients with the main diagnosis of either community-acquired pneumonia, exacerbation of COPD, acute heart failure or hip fracture were included. A questionnaire survey was sent out after discharge investigating changes before and after SwissDRG implementation. Our primary endpoint was LOS. Of 1,983 eligible patients 841 returned the questionnaire and were included into the analysis (429 in 2011, 412 in 2012). The median age was 76.7 years (50.8% male). Patients in the two years were well balanced in regard to main diagnoses and co-morbidities. Mean LOS in the overall patient population was 10.0 days and comparable between the 2011 cohort and the 2012 cohort (9.7 vs 10.3; p = 0.43). Overall satisfaction with care changed only slightly after introduction of SwissDRG and remained high (89.0% vs 87.8%; p = 0.429). Investigating the influence of the implementation of SwissDRG in 2012 regarding LOS patients' outcome and satisfaction, we found no significant changes. However, we observed some noteworthy trends, which should be monitored closely.

Ambulatory pediatric and family medicine takes care of adolescent patients, most of whom regularly consult a physician. Consultations with young people involve issues specifically related to their age. Regarding health care systems and physicians, adolescents' expectations vary from those of adults, not so much in terms of the issues discussed but in terms of the priorities that they give to them. Confidential interviews are not always proposed but are highly appreciated, as are certain personal qualities on the part of the caregivers such as honesty, respect, and friendliness. Finally, easy access to care together with the continuity of care are essential. Prevention of risk behaviors by screening and health education is clearly insufficient. This issue could be approached during the consultation through a psychosocial history. This is a good opportunity to discuss sensitive issues that adolescents seldom bring up themselves. More systematic prevention would probably decrease youth morbidity and mortality, which are both closely related to risk behaviors. To meet these expectations and special health care needs, the World Health Organization has developed the concept of youth-friendly health services. This concept can be applied in both a specialized adolescence center and a pediatric or family practice. Youth-friendly services are still rarely evaluated but seem to bring a clear benefit in terms of patient satisfaction and access to care.

The study's sample was composed of individuals 75 years or older cared for by a home health care service in the primary health care unit in Vilafranca del Penedès, Spain (n=26). The variables included: a) socio-demographic data; b) concept of QofL; c) perception of QofL; d) reasons for their perception; d) satisfaction with life and related aspects; and f) feeling of happiness. Face to face interviews were conducted. Background: Pharmacogenetic studies are essential in understanding the interindividual variability of drug responses. DNA sample collection for genotyping is a critical step in genetic studies. A method using dried blood samples from finger-puncture, collected on DNA-cards, has been described as an alternative to the usual venepuncture technique. The purpose of this study is to evaluate the implementation of the DNA cards method in a multicentre clinical trial, and to assess the degree of investigators' satisfaction and the acceptance of the patients perceived by the investigators.Methods: Blood samples were collected on DNA-cards. The quality and quantity of DNA recovered were analyzed. Investigators were questioned regarding their general interest, previous experience, safety issues, preferences and perceived patient satisfaction. Results: 151 patients' blood samples were collected. Genotyping of GST polymorphisms was achieved in all samples (100%). 28 investigators completed the survey. Investigators perceived patient satisfaction as very good (60.7%) or good (39.3%), without reluctance to finger puncture. Investigators preferred this method, which was considered safer and better than the usual methods. All investigators would recommend using it in future genetic studies. Conclusion: Within the clinical trial setting, the DNA-cards method was very well accepted by investigators and patients (in perception of investigators), and was preferred to conventional methods due to its ease of use and safety.

A questionnaire assessing the satisfaction of children with their hospital stay has been developed and tested with 136 children (aged 6-12 years) at 2 Swiss hospital sites. Three out of 4 children were satisfied overall with their hospital stay. Their relationships with the professional medical staff, explanations they received, games they played, and environment, all received positive evaluations. The most critical points were pain, fear, and the absence of relatives. Ninety percent of the children appreciated that their opinions were sought. These results reinforce the importance of having questionnaires available for the children to consider their opinions to enhance the quality of care.

CONTEXT: Communication guidelines often advise physicians to disclose to their patients medical uncertainty regarding the diagnosis, origin of the problem, and treatment. However, the effect of the expression of such uncertainty on patient outcomes (e.g. satisfaction) has produced conflicting results in the literature that indicate either no effect or a negative effect. The differences in the results of past studies may be explained by the fact that potential gender effects on the link between physician-expressed uncertainty and patient outcomes have not been investigated systematically. OBJECTIVES: On the basis of previous research documenting indications that patients may judge female physicians by more severe criteria than they do male physicians, and that men are more prejudiced than women towards women, we predicted that physician-expressed uncertainty would have more of a negative impact on patient satisfaction when the physician in question was female rather than male, and especially when the patient was a man. METHODS: We conducted two studies with complementary designs. Study 1 was a randomised controlled trial conducted in a simulated setting (120 analogue patients Analogue patients are healthy participants asked to put themselves in the shoes of real medical patients by imagining being the patients of physicians shown on videos); Study 2 was a field study conducted in real medical interviews (36 physicians, 69 patients). In Study 1, participants were presented with vignettes that varied in terms of the physician's gender and physician-expressed uncertainty (high versus low). In Study 2, physicians were filmed during real medical consultations and the level of uncertainty they expressed was coded by an independent rater according to the videos. In both studies, patient satisfaction was assessed using a questionnaire. RESULTS: The results confirmed that expressed uncertainty was negatively related to patient satisfaction only when the physician was a woman (Studies 1 and 2) and when the patient was a man (Study 2). CONCLUSIONS: We believe that patients have the right to be fully informed of any medical uncertainties. If our results are confirmed in further research, the question of import will refer not to whether female physicians should communicate uncertainty, but to how they should communicate it. For instance, if it proves true that uncertainty negatively impacts on (male) patients' satisfaction, female physicians might want to counterbalance this impact by emphasizing other communication skills.

Gene turnover rates and the evolution of gene family sizes are important aspects of genome evolution. Here, we use curated sequence data of the major chemosensory gene families from Drosophila-the gustatory receptor, odorant receptor, ionotropic receptor, and odorant-binding protein families-to conduct a comparative analysis among families, exploring different methods to estimate gene birth and death rates, including an ad hoc simulation study. Remarkably, we found that the state-of-the-art methods may produce very different rate estimates, which may lead to disparate conclusions regarding the evolution of chemosensory gene family sizes in Drosophila. Among biological factors, we found that a peculiarity of D. sechellia's gene turnover rates was a major source of bias in global estimates, whereas gene conversion had negligible effects for the families analyzed herein. Turnover rates vary considerably among families, subfamilies, and ortholog groups although all analyzed families were quite dynamic in terms of gene turnover. Computer simulations showed that the methods that use ortholog group information appear to be the most accurate for the Drosophila chemosensory families. Most importantly, these results reveal the potential of rate heterogeneity among lineages to severely bias some turnover rate estimation methods and the need of further evaluating the performance of these methods in a more diverse sampling of gene families and phylogenetic contexts. Using branch-specific codon substitution models, we find further evidence of positive selection in recently duplicated genes, which attests to a nonneutral aspect of the gene birth-and-death process.
