SETTING: Itaborai Municipality in Rio de Janeiro, Brazil. OBJECTIVE: To evaluate access to tuberculosis (TB) diagnosis for users of the Family Health Program (FHP) and Reference Ambulatory Units (RAUs). DESIGN : A cross-sectional study was conducted in Itaborai City, Rio de Janeiro, Brazil. Between July and October 2007, a sample of 100 TB patients registered consecutively with the TB Control Program was interviewed using the primary care assessment tool. The two highest scores, describing `almost always` and `always`, or `good` and `very good`, were used as a cut-off point to define high quality access to diagnosis. RESULTS: FHP patients were older and had less education than RAU interviewees. Sex and overcrowding did not differ in the two groups. Patient groups did not differ with regard to the number of times care was sought at a unit, transport problems, cost of attending units and availability of consultation within 24 h. Adequate access to diagnosis was identified by 62% of the FHP patients and 53% of the RAU patients (P = 0.01). CONCLUSION: In Itaborai, Rio de Janeiro, TB patients believe that the FHP units provide greater access to TB diagnosis than RAUs. These findings will be used by the Department of Health to improve access to diagnosis in Itaborai.

Con la Constitución Política de 1991, se inicia una nueva estructura funcional en el país a partir de los preceptos consagrados en la carta magna como un estado social de derecho, situación que no fue ajena al sector salud en el cual se estableció la seguridad social como un servicio público basado en los principios fundamentales de universalidad, solidaridad y eficiencia, enunciados que fueron plasmados entre otros en la Ley 100 de 1993, la cual incorporó estos elementos que resultaban innovadores en el contexto. Hoy después de 18 años de promulgada la Ley, se ha generado un modelo estructural que ha permitido mejorar los resultados en esta área, que si bien posee falencias en algunos de sus componentes (Acceso, Flujo de Recursos, Salud Pública), las mismas pueden ser superadas, con una reforma estructural al sistema que elimine los intereses particulares de los diferentes actores al momento de su elaboración y promulgación. Uno de los avances significativos en la reforma al sistema es la promulgación de la salud como derecho fundamental, inherente al ser humano, otorgando el carácter de inalienable, imprescriptible y sagrado, dejando en segundo plano su condición prestacional establecida. El derecho a la salud tendrá como eje principal la salud pública armonizada con la promoción y prevención, diagnóstico y tratamiento integral de la enfermedad, las cuales deben ser articuladas con las políticas sociales, ambientales, culturales y económicas que permitan incidir en los determinantes sociales de la salud para mejorar la calidad de vida de la población. La reforma a la salud en el eje de la financiación como se encuentra propuesta, no generará cambios sustanciales toda vez que se avanza hacia un sistema en que el estado será el principal actor con las funciones afiliación, recaudo, pago, giro o transferencia de los recursos, responsable de la información. Sin embargo la administración del riesgo en salud seguirá delegada, función que es determinante para la administración de los recursos del sistema, en resumen se demuestra el interés del estado en retomar la gobernanza y su empoderamiento como administrador de lo público, sin que esto responda a las necesidades de cambio en el sistema de salud.

Metodología para la estimación de cápitas de planes de salud. Se propone un método de cuantificación de insumos a emplear, la estimación de costos indirectos asociados a las intervenciones y un método de anualización para los costos fijos que generalmente trascienden en su vida útil al período de duración de una cápita. Finalmente, a través de un ejercicio práctico de estimación de la cápita del Componente de Salud del Programa Oportunidades de México se ilustra la aplicación del desarrollo metodológico expuesto en la parte inicial.

A presente tese teve como objetivo explicar a dinâmica político-institucional que produziu um quadro de relações intergovernamentais polarizado na política de saúde no âmbito do SUS ao final da década de 1990. Tal polarização ocorreu em virtude da presença simultânea de expressivo grau de municipalização e elevada capacidade indutiva e regulatória do Ministério da Saúde. As abordagens anteriores presentes na literatura sobre a descentralização do SUS produziam explicações parciais em virtude de apontarem como fatores explicativos da polarização um conjunto de razões específicas, em especial o escopo expressivamente descentralizador da Constituição de 1988, as preferências municipalistas do Movimento da Reforma Sanitária, o conteúdo das normas operacionais, o legado centralizador da trajetória da política de saúde no Brasil, a agenda centralizadora das reformas econômicas realizadas a partir da implementação do Plano Real, entre outros. Com base no arcabouço teórico do NeoInstitucionalismo Histórico, essa tese propõe uma abordagem que integra os diversos fatores condicionantes do jogo federativo setorial em torno de uma explicação sequencial das decisões que marcaram a trajetória da descentralização do SUS. Nessa abordagem, a trajetória das relações intergovernamentais é o resultado cumulativo de uma longa cadeia de decisões tomadas em contextos singulares que marcaram os governos Collor, Itamar e FHC, onde a escolha de um governo afetou o leque de opções disponíveis ao governo seguinte, deixando-lhe menos margem de mudança. Nessa lógica, a polarização federativa é vista como o produto não intencional de uma sequência de decisões que, acumuladas ao longo da década, concentraram poder, atribuições e recursos na União e nos municípios.

Health policies in Brazil, the decentralization of SUS management responsibilities for the three spheres of government has driven the creation and regulation of the audits of health services in the National Audit Office, this is a trend of neoliberal policies imposed by international bodies like the World Bank and IMF to peripheral countries characterized by productive restructuring and reforming the state focuses on the presence of two competing projects in the area of health: Health Sector Reform Project which is based on the democratic rule of law with the assumption of health as social right and duty of the State in defending the extension of the conquest of rights and democratization of access to health care guaranteed through the public financing strategies and the effective decentralization of decisions pervaded by social control and privatized Health Project which is based on the state minimum, with a reduction in social spending or in partnerships and privatization, stronger nonprofit sector, subject to capitalist interests, is made effective through strategies targeting health policy and refilantropização actions. In this context, the present study is an analysis on the work of social audits of public health in infants from a qualitative and quantitative approach, embodied by the critical method of dialectical Marxist social theory that enabled us to unveil the characterization, the demands, challenges and outline the profile of Social Work in teams inserted audits of SUS in RN, but also provided evidence to demonstrate the prospects and possibilities of this area of activity of social workers. It was also found that through the audit work that the state fulfill its role as bureaucratic and regulator of health services with efficiency, effectiveness and economy. Yet, paradoxically, the audits of SUS may provide a vehicle for enforcing rights and ensuring the fundamental principles contained in the project of health reform, because it can be configured in a space of political struggle as representing a new field of knowledge production that needs to be appropriate for a theoretical critic able to redirect the social interests in favor of the user. From this perspective, it is concluded that the work of social audits of public health in infants despite the social relevance that prints, as they constitute an activity study of reality and its transformation proposition requires a transformative political action guided the discussion Marxist theory holds that the ethical project professional politician of Social Work

This paper will seek to explicate the changes in the New Zealand health sector informed by the concepts of problematization, inscription and the construction of networks (Callon, 1986; Latour, 1987, 1993). This will involve applying a framework of interpretation based on the concepts of Latour's sociology of translation. Material on problematization and inscription will be incorporated into the paper in order to provide an explanatory frame of reference which will enable us to make sense of the processes of change in the New Zealand health sector. The sociology of translation will be used to explain the processes which underlie the changes and will be used to capture effects, such as changes in policy and structure, producing new networks within which 'allies' could be enrolled in support of the health reforms.

We analyzed Brazil's efforts in reducing child mortality, improving maternal and child health, and reducing socioeconomic and regional inequalities from 1990 through 2007. We compiled and reanalyzed data from several sources, including vital statistics and population-based surveys. We also explored the roles of broad socioeconomic and demographic changes and the introduction of health sector and other reform measures in explaining the improvements observed. Our findings provide compelling evidence that proactive measures to reduce health disparities accompanied by socioeconomic progress can result in measurable improvements in the health of children and mothers in a relatively short interval. Our analysis of Brazil's successes and remaining challenges to reach and surpass Millennium Development Goals 4 and 5 can provide important lessons for other low- and middle-income countries

OBJECTIVE: The results of an evaluative longitudinal study, which identified the effects of health care decentralization on health financing in Mexico, Nicaragua and Peru are presented in this article. METHODS: The methodology had two main phases. In the first, secondary sources of data and documents were analyzed with the following variables: type of decentralization implemented, source of financing, funds for financing, providers, final use of resources, mechanisms for resource allocation. In the second phase, primary data were collected by a survey of key personnel in the health sector. RESULTS: Results of the comparative analysis are presented, showing the changes implemented in the three countries, as well as the strengths and weaknesses of each country in matters of financing and decentralization. CONCLUSIONS: The main financing changes implemented and quantitative trends with respect to the five financing indicators are presented as a methodological tool to implement corrections and adjustments in health financing.

The economic crisis that has been affecting Europe in the 21st century has modified social protection systems in the countries that adopted, in the 20th century, universal health care system models, such as Spain. This communication presents some recent transformations, which were caused by changes in Spanish law. Those changes relate to the access to health care services, mainly in regards to the provision of care to foreigners, to financial contribution from users for health care services, and to pharmaceutical assistance. In crisis situations, reforms are observed to follow a trend which restricts rights and deepens social inequalities.

The Government Decision1 on the Structural, Organisational, Financial Management and Systems Reform of the Health Sector of June, 2003 acknowledged that in order to increase the effectiveness of the health service generally, and its capacity to deliver the reform agenda, it was important that the service was fully concentrated on addressing its core health objectives. The Minister for Health and Children and Minister for Finance felt that there could be scope to transfer certain functions out of the health service and locate them more appropriately within other functional areas of Government. As part of the overall decision, it was agreed that a working group would be established, to include the Departments of Health and Children, Finance and An Taoiseach, to examine the scope for transfer of certain activities to other, more appropriate, Departments and agencies and that on completion of this review, the Minister for Health and Children would bring proposals to Government. Future Health – A Srategic Framework for Reform of the Health Service 2012 – 2015 details the actions that we will take to deliver on this promise. The need for change in the health service is unquestionable. In the midst of health care reform, Colombia has succeeded in increasing health insurance coverage and the quality of health care. In spite of this, efficiency continues to be a matter of concern, and small-area variations in health care are one of the plausible causes of such inefficiencies. In order to understand this issue, we use individual data of all births from a Contributory-Regimen insurer in Colombia. We perform two different specifications of a multilevel logistic regression model. Our results reveal that hospitals account for 20% of variation on the probability of performing cesarean sections. Geographic area only explains 1/3 of the variance attributable to the hospital. Furthermore, some variables from both demand and supply sides are found to be also relevant on the probability of undergoing cesarean sections. This paper contributes to previous research by using a hierarchical model and by defining hospitals as cluster. Moreover, we also include clinical and supply induced demand variables.

El artículo busca encontrar evidencia empírica de los determinantes de la salud, como una medición de capital salud en un país en desarrollo después de una profunda reforma en el sector salud. Siguiendo el modelo de Grossman (1972) y tomando factores institucionales, además de las variables individuales y socioeconómicas. Se usaron las encuestas de 1997 y 2000 donde se responde subjetivamente sobre el estado de salud y tipo de afiliación al sistema de salud. El proceso de estimación usado es un probit ordenado. Los resultados muestran una importante conexión entre las variables individuales, institucionales y socioeconómicas con el estado de salud. El efecto de tipo de acceso al sistema de salud presiona las inequidades en salud.
