Introduzione delle catene di rinnovo di Markov, del nucleo di semi-Markov e delle catene di semi-Markov, risultati ad essi relativi. Analisi delle equazioni di rinnovo di Markov, caratterizzazione degli stati di una catena di semi-Markov, teorema di esistenza e unicità dell'equazione di rinnovo di Markov. Un esempio concreto.

Neste objeto, Cadeia de Sobrevivência, é abordado como o profissional deve realizar cada passo da “corrente de sobrevivência” otimizadamente uma vez que as chances de sobrevivência das vítimas de RCP crescer muito se observarmos os quatro elos da corrente: acesso rápido ao serviço de emergência, manobras de RCP imadiatas, rápida desvibrilação e cuidados avançados sem demora. Na sequencia trata da utilização do Suporte Básico de Vida e destaca que durante o processo de reanimação mecânico utilizado para manter a circulação sanguínea e as trocas gasosas, evitando sequelas e a morte da vítima de uma parada cardiorrespiratória (PCR), até que medidas sejam tomadas para que o coração retome a automaticidade. Para fechar salienta que após as fases do atendimento inicial o profissional deve dar continuidade seguindo as manobras de ressuscitação cardiopulmonar e procurar praticar todas as nove etapas durante um atendimento de urgência dessa natureza. Unidade 2 do módulo 15 que compõe o Curso de Especialização em Saúde da Família.

Resumo Perez Belkis Gonzalez. Melhoria da Atenção aos Usuários com Hipertensão e/ou Diabetes da USF Cadeia Velha I, Rio Branco/AC. 2015.87f. Trabalho de Conclusão de Curso de Especialização em Saúde da Família, Departamento de Medicina Social, Faculdade de Medicina, Universidade Federal de Pelotas, Pelotas, 2015. Este trabalho teve como objetivo melhorar a qualidade da atenção em saúde aos usuários hipertensos e/ou diabéticos, pertencentes à área de abrangência da USF Cadeia Velha I, Rio Branco/AC.Foi escolhida para esta intervenção, a saúde dos usuários hipertensos e/ou diabéticos pertencentes à área de abrangência de nossa USF por ser grupos priorizados e mais sensíveis, também os resultados podem ser visualizados em menor prazo.O estudo é de caráter interventivo, descritivo e de corte quantitativo e qualitativo onde se propõe um conjunto de objetivos específicos que abarcam a cobertura do programa, qualidade de atendimento, adesão e promoção de saúde dos hipertensos e diabéticos,para os quais se elaboraram metas com as suas respectivas ações implementadas, desenvolvidas, e executadas nos quatro eixos: organização e gestão do serviço, monitoramento e avaliação, engajamento público e qualificação da prática clínica, com seus indicadores que permitiram o monitoramento das metas.Os dados mais relevantes foram relacionados àampliação da cobertura da atenção à saúde dos usuários hipertensos ediabéticos da nossa área de abrangência que foram de 100% para hipertensos e 100% para diabéticos. Esta intervenção trouxe benefícios para a comunidade não somente para aqueles hipertensos e diabéticos vinculadas ao projeto, mas também para a população em geral devido a que ganharam em consciência sobre a importância do acompanhamento dos hipertensos e diabéticos e conhecimento sobre saúde, também trouxe um grande impacto sobre o serviço, ganhando-se em organização dos processos de trabalho que se visualizou numa melhoria na qualidade e integralidade da atenção à saúde deste doente e maior credibilidade por parte da comunidade. Foi muito importante para a equipe de saúde de nossa unidade por que se aprendeu a trabalhar unido, devido aos resultados obtidos, se mantêm esta estrutura incorporada à rotina de atendimento da unidade. Often in biomedical research, we deal with continuous (clustered) proportion responses ranging between zero and one quantifying the disease status of the cluster units. Interestingly, the study population might also consist of relatively disease-free as well as highly diseased subjects, contributing to proportion values in the interval [0, 1]. Regression on a variety of parametric densities with support lying in (0, 1), such as beta regression, can assess important covariate effects. However, they are deemed inappropriate due to the presence of zeros and/or ones. To evade this, we introduce a class of general proportion density, and further augment the probabilities of zero and one to this general proportion density, controlling for the clustering. Our approach is Bayesian and presents a computationally convenient framework amenable to available freeware. Bayesian case-deletion influence diagnostics based on q-divergence measures are automatic from the Markov chain Monte Carlo output. The methodology is illustrated using both simulation studies and application to a real dataset from a clinical periodontology study.

A missense G209A mutation of the alpha-synuclein gene was recently described in a large Contursi kindred with Parkinson's disease (PD). The objective of this study is to determine if the mutation G209A of the alpha-synuclein gene was present in 10 Brazilian families with PD. PD patients were recruited from movement disorders clinics of Brazil. A family history with two or more affected in relatives was the inclusion criterion for this study. The alpha-synuclein G209A mutation assay was made using polymerase chain reaction and the restriction enzyme Tsp45I. Ten patients from 10 unrelated families were studied. The mean age of PD onset was 42.7 years old. We did not find the G209A mutation in our 10 families with PD. Our results suggest that alpha-synuclein mutation G209A is uncommon in Brazilian PD families.

Machado-Joseph disease (MJD) is the most common autosomal dominant spinocerebellar ataxia and presents great phenotypic variability. MJD presenting with spastic paraparesis was recently described in Japanese patients. We report the case of 41-year-old woman with the phenotype of complicated hereditary spastic paraplegia. Her father died at the age of 56 years due to an undiagnosed progressive neurological disease that presented parkinsonism. She had an expanded allele with 66 CAG repeats and a normal allele with 22 repeats in the gene of MJD. MJD should be considered in the differential diagnosis of autosomal dominant complicated HSP. A patient with the phenotype of complicated HSP and relatives with other clinical features of a neurodegenerative disease should raise the suspicion of MJD.

The purpose of this work was to analyze the logistical distribution of Brazilian soybean by applying a quadratic programming to a spatial equilibrium model. The soybean transportation system is an important part of the soybean complex in Brazil, since the major part of the costs of this commodity derives from the transportation costs. Therefore, the optimization of this part of the process is essential to a better competitiveness of the Brazilian soybean in the international market. The Brazilian soybean complex have been increasing its agricultural share in the total of the exportation value in the last ten years, but due to other countries' investments the Brazilian exportations cannot be only focused on increasing its production but it still have to be more efficient. This way, a model was reached which can project new frames by switching the transportation costs and conduce the policy makers to new investments in the sector.

Losses of horticulture product in Brazil are significant and among the main causes are the use of inappropriate boxes and the absence of a cold chain. A project for boxes is proposed, based on computer simulations, optimization and experimental validation, trying to minimize the amount of wood associated with structural and ergonomic aspects and the effective area of the openings. Three box prototypes were designed and built using straight laths with different configurations and areas of openings (54% and 36%). The cooling efficiency of Tommy Atkins mango (Mangifera Indica L.) was evaluated by determining the cooling time for fruit packed in the wood models and packed in the commercially used cardboard boxes, submitted to cooling in a forced-air system, at a temperature of 6ºC and average relative humidity of 85.4±2.1%. The Finite Element Method was applied, for the dimensioning and structural optimization of the model with the best behavior in relation to cooling. All wooden boxes with fruit underwent vibration testing for two hours (20 Hz). There was no significant difference in average cooling time in the wooden boxes (36.08±1.44 min); however, the difference was significant in comparison to the cardboard boxes (82.63±29.64 min). In the model chosen for structural optimization (36% effective area of openings and two side laths), the reduction in total volume of material was 60% and 83% in the cross section of the columns. There was no indication of mechanical damage in the fruit after undergoing the vibration test. Computer simulations and structural study may be used as a support tool for developing projects for boxes, with geometric, ergonomic and thermal criteria.

The durability and postharvest quality of cut flowers are fundamental attributes in value along the production chain and in consumer satisfaction. The objective of this study was to evaluate the effect of chemical inhibitors of ethylene action on maintaining the postharvest quality of chrysanthemum stems (Chrysanthemum morifolium Ramat cv. Dragon). The experiment tested maintenance solutions with silver thiosulfate (STS) under five levels (distilled water, a 0.2 mM STS, the STS 0.2 mM + sucrose at 50 g L-1, STS at 0.4 mM; STS at 0.4 mM + sucrose at 50 g L-1), and date of sampling, for three levels (0, 3, 6 days). Three replications with two flower stems in each treatment were used in the experiment. Physical assessments were made: color, fresh mass and relative water content; chemical evaluations: reducing sugars and pigments, and qualitative assessments: turgidity, flower color, and number of buds, open flowers and partially open flowers. Treatment with 0.2 mM STS resulted in better maintenance of fresh mass of stems. The concentration of pigments and reducing sugar was higher in those treatments in which sucrose was associated. The color and relative water content were favored in treatments STS 0.2 mM and 0.4 mM. The concentration of 0.2 mM STS obtained the best results, prolonging the vase life the stems. The quality of these stems was higher, with the best assessments of water content, color and turgidity.

X-linked adrenoleukodystrophy (X-ALD) is an inherited disease with clinical heterogeneity varying from presymptomatic individuals to rapidly progressive cerebral ALD forms. This disease is characterized by increased concentration of very long chain fatty acids (VLCFAs) in plasma and in adrenal, testicular and nervous tissues. Affected individuals can be classified in different clinical settings, according to phenotypic expression and age at onset of initial symptoms. Molecular defects in X-ALD individuals usually result from ABCD1 gene mutations. In the present report we describe clinical data and the ABCD1 gene study in two boys affected with the childhood cerebral form that presented with different symptomatic manifestations at diagnosis. In addition, their maternal grandfather had been diagnosed with Addison's disease indicating phenotypic variation for X-ALD within this family. The mutation p.Trp132Ter was identified in both male patients; additionally, three females, out of eleven family members, were found to be heterozygous after screening for this mutation. In the present report, the molecular analysis was especially important since one of the heterozygous females was in first stages of pregnancy. Therefore, depending on the fetus outcome, if male and p.Trp132Ter carrier, storage of the umbilical cord blood should be recommended as hematopoietic stem cell transplantation could be considered as an option for treatment in the future.

Several characteristics are important in a traceability system of animal products, such as age at slaughter, breed composition, besides information of the productive chain. In general, the certification agent records information about the animals and the system which it came from, although cannot guarantee that the slaughtering, meat processing and distribution are error proof. Besides, there is a differential price, at least at the international market, based on sex and breed composition of the animals. Genetic markers allow identification of characteristics controlled in the beef cattle traceability program, as sex and breed composition, in order to correctly identify and appraise the final product for the consumer. The hypothesis of this study was that the majority beef samples retailed in the local market originate from female with a great participation of zebu breeds. Therefore, the objective of this work was to characterize retail beef samples with DNA markers that identify cattle sex and breed composition. Within 10 beef shops localized in Pirassununga, SP, Brazil, 61 samples were collected, all were genotyped as harboring Bos taurus mitochondrial DNA and 18 were positive for the Y chromosome amplification (male). For the marker sat1711b-Msp I the frequency of the allele A was 0.278 and for the marker Lhr-Hha I the frequency of the allele T was 0.417. The results of sat1711b-Msp I and Lhr-Hha I allelic frequencies are suggestive that the proportion of indicus genome compared with the taurine genome in the market meat is smaller than the observed in the Nellore breed. The procedure described in this study identified sex and subspecies characteristics of beef meat samples, with potential application in meat products certification in special as an auxiliary tool in beef cattle traceability programs.

Technical evaluation of analytical data is of extreme relevance considering it can be used for comparisons with environmental quality standards and decision-making as related to the management of disposal of dredged sediments and the evaluation of salt and brackish water quality in accordance with CONAMA 357/05 Resolution. It is, therefore, essential that the project manager discusses the environmental agency's technical requirements with the laboratory contracted for the follow-up of the analysis underway and even with a view to possible re-analysis when anomalous data are identified. The main technical requirements are: (1) method quantitation limits (QLs) should fall below environmental standards; (2) analyses should be carried out in laboratories whose analytical scope is accredited by the National Institute of Metrology (INMETRO) or qualified or accepted by a licensing agency; (3) chain of custody should be provided in order to ensure sample traceability; (4) control charts should be provided to prove method performance; (5) certified reference material analysis or, if that is not available, matrix spike analysis, should be undertaken and (6) chromatograms should be included in the analytical report. Within this context and with a view to helping environmental managers in analytical report evaluation, this work has as objectives the discussion of the limitations of the application of SW 846 US EPA methods to marine samples, the consequences of having data based on method detection limits (MDL) and not sample quantitation limits (SQL), and present possible modifications of the principal method applied by laboratories in order to comply with environmental quality standards.

Uma análise da distribuição geográfica de Schefflera no Brasil extra-amazônico foi realizada com base em mapas atualizados plotando as ocorrências conhecidas das 26 espécies do gênero encontradas nessa grande área: S. angustissima (Marchal) Frodin, S. aurata Fiaschi, S. botumirimensis Fiaschi & Pirani, S. burchellii (Seem.) Frodin & Fiaschi, S. calva (Cham.) Frodin & Fiaschi, S. capixaba Fiaschi, S. cephalantha (Harms) Frodin, S. cordata (Taub.) Frodin & Fiaschi, S. distractiflora (Harms) Frodin, S. fruticosa Fiaschi & Pirani, S. gardneri (Seem.) Frodin & Fiaschi, S. glaziovii (Taub.) Frodin & Fiaschi, S. grandigemma Fiaschi, S. kollmannii Fiaschi, S. longipetiolata (Pohl ex DC.) Frodin & Fiaschi, S. lucumoides (Decne. & Planch. ex Marchal) Frodin & Fiaschi, S. macrocarpa (Cham. & Schltdl.) Frodin, S. malmei (Harms) Frodin, S. morototoni (Aubl.) Maguire, Steyermark & Frodin, S. racemifera Fiaschi & Frodin, S. ruschiana Fiaschi & Pirani, S. selloi (Marchal) Frodin & Fiaschi, S. succinea Frodin & Fiaschi, S. villosissima Fiaschi & Pirani, S. vinosa (Cham. & Schltdl.) Frodin & Fiaschi e S. aff. varisiana Frodin. Dois centros de endemismo associados com áreas de altitude elevada foram reconhecidos: Cadeia do Espinhaço em Minas Gerais e florestas montanas do Estado do Espírito Santo. Os padrões de distribuição geográfica ilustrados são discutidos com base em dados obtidos para outros grupos de angiospermas e em estudos fitogeográficos das principais fitocórias do Brasil extra-amazônico. São apresentadas também hipóteses acerca de prováveis relações filogenéticas entre alguns táxons, visando à busca de possíveis correlações entre estas e a biogeografia do grupo.

Troglobitic (exclusively subterranean) organisms usually present, among their apomorphies related to the subterranean life (troglomorphisms), the regression of eyes and melanic pigmentation. The degree of regression varies among species, from a slight reduction to the complete loss of eyes and dark pigmentation, without a taxonomic correlation. While mechanisms of eye reduction have been intensively investigated in some troglobites such as the Mexican blind tetra characins, genus Astyanax, and the European salamander, Proteus anguinus, few studies have focused on pigmentation. The Brazilian subterranean ichthyofauna distinguishes not only by the species richness (23 troglobitic fishes so far known) but also by the variation in the degree of reduction of eyes and pigmentation. This study focused on Brazilian fishes completely devoid of melanic pigmentation: the characiform Stygichthys typhlops (Characidae) and the siluriforms Ancistrus formoso (Loricariidae), Rhamdiopsis sp.1 (Heptapteridae; from caves in the Chapada Diamantina, Bahia) and Rhamdiopsis sp. 2 (cave in Campo Formoso, Bahia). In order to investigate if such depigmentation is the result of blockage in some step in the melanogenesis, in vitro tests of administration of L-DOPA were done, using caudal-fin fragments extracted from living fish. Except for Rhamdiopsis sp. 2, all the studied species were DOPA(+), i.e., melanin was synthesized after L-DOPA administration. This indicates these fish do have melanophores but they are unable to convert L-tyrosine to L-DOPA. On the other hand, Rhamdiopsis sp. 2, like the albino specimens of Trichomycterus itacarambiensis previously studied (which correspond to one third of the population), are DOPA(-), either because the block of melanin synthesis occurs downstream in melanogenesis, which is probably the case with T. itacarambiensis (monogenic system in view of the phenotypic discontinuity), or because the so-called albinos do no possess melanophores. The physiological loss in the ability to synthesize melanin, apparently caused by different genetic processes in DOPA(+) and in DOPA(-) fishes, may co-exist in subterranean populations with a decrease in the density of melanophores, as observed in the pigmented two thirds of T. itacarambiensis population, a morphological reduction apparently controlled by polygenic systems producing a continuous phenotypic variation.

In a preliminary study in Juruti, a mining municipality in western Pará State, Brazil, 12 out of 21 patients suspected of presenting cutaneous leishmaniasis showed positive PCR (SSUrDNA and G6PD): Leishmania (Viannia) braziliensis (9/12; 75%) and L. (V.) sp. (3/12; 25%). Entomological studies in the same location revealed the presence of 12 different phlebotomine species (n =105). One of the most common species was Lutzomyia (Psychodopygus) complexa (17%) which is both highly anthropophilic and a known vector of L. (V.) braziliensis in other regions of Pará. These preliminary findings should serve to guide future epidemiological surveillance in Juruti.
