INTRODUCTION: Following the introduction of work-hour restrictions, residents' workload has become an important theme in postgraduate training. The efficacy of restrictions on workload, however, remains controversial, as most research has only examined objective workload. The purpose of this study was to explore the less clearly understood component of subjective workload and, in particular, the factors that influenced residents' subjective workload.

METHOD: This study was conducted in Japan at three community teaching hospitals. We recruited a convenience sample of 31 junior residents in seven focus groups at the three sites. Audio-recorded and transcribed data were read iteratively and analyzed thematically, identifying, analyzing and reporting themes within the data and developing an interpretive synthesis of the topic.

RESULTS: Seven factors influenced residents' subjective workload: (1) interaction within the professional community, (2) feedback from patients, (3) being in control, (4) professional development, (5) private life, (6) interest and (7) protected free time.

DISCUSSION AND CONCLUSION: Our findings indicate that residents who have good interaction with colleagues and patients, are competent enough to control their work, experience personal development through working, have greater interest in their work, and have fulfilling private lives will have the least subjective workload.

Aims/hypothesis The aim of this study was to investigate the association between routine vaccinations and the risk of childhood type 1 diabetes mellitus by systematically reviewing the published literature and performing meta-analyses where possible.

Methods A comprehensive literature search was performed of MEDLINE and EMBASE to identify all studies that compared vaccination rates in children who subsequently developed type 1 diabetes mellitus and in control children. ORs and 95% CIs were obtained from published reports or derived from individual patient data and then combined using a random effects meta-analysis.

Results In total, 23 studies investigating 16 vaccinations met the inclusion criteria. Eleven of these contributed to meta-analyses which included data from between 359 and 11,828 childhood diabetes cases. Overall, there was no evidence to suggest an association between any of the childhood vaccinations investigated and type 1 diabetes mellitus. The pooled ORs ranged from 0.58 (95% CI 0.24, 1.40) for the measles, mumps and rubella (MMR) vaccination in five studies up to 1.04 (95% CI 0.94, 1.14) for the haemophilus influenza B (HiB) vaccination in 11 studies. Significant heterogeneity was present in most of the pooled analyses, but was markedly reduced when analyses were restricted to study reports with high methodology quality scores. Neither this restriction by quality nor the original authors’ adjustments for potential confounding made a substantial difference to the pooled ORs.

Conclusions/interpretation This study provides no evidence of an association between routine vaccinations and childhood type 1 diabetes.

Transpiration of two year-old olive trees of three different varieties, Arbequina, Cobrançosa and Galega (18 trees per variety), irrigated with three levels of salt (0, 80 or 200 mM NaCl) for about 90 days, was measured by a gravimetric method. To determine leaf area, each tree was photographed from the side against a white background and the total area of each projected image was determined with ImageJ software. To calibrate these area determinations, one tree of each variety was subsequently stripped of all its leaves and its total leaf area was accurately measured. A correlation was then obtained between the area on the photograph of this particular tree and the total area of the detached leaves of the same tree. Using the leaf area determined by this procedure, transpiration rates of the trees could be calculated. Knowing leaf and air temperatures and RH, it was possible to determine the difference in molar fraction of water between the leaf and the air. Using this and the values of the transpiration rate, stomatal conductance could be calculated (gs calc) and compared with the conductance measured on the same trees with a porometer (gs). Actual leaf area of a plant was 1,40 (Arbequina), 1,42 (Cobrançosa) or 1,24 (Galega) times the area measured with ImageJ on the photograph of the same plant. Leaf area of the trees, on average of all salt irrigations, was significantly higher on Arbequina (0,187 m2) then on the other two varieties (0,138 m2 or 0,148 m2, for Cobrançosa or Galega, respectively), but did not differ significantly in percentage of controls (0 salt). On average of all three varieties, leaf area was also higher on plants irrigated without salt (0,181 m2) than on plants exposed to 80 or 200 mM NaCl (0,152 m2 or 0,140 m2, respectively), which did not differ between them. The same significant difference was observed when leaf area was expressed as percentage of controls. Transpiration rate was significantly higher on Cobrançosa (1,17 mmol m-2 s-1), on average of all treatments, but there were no significant differences between Arbequina (1,08 mmol m-2 s-1) and Galega (0,82 mmol m-2 s-1). In percentage of controls, there were no significant differences between varieties. Salt reduced significantly the transpiration rate in all varieties, both the actual and percentual values, to about 50% or 30% of controls when exposed to 80 mM or 200 mM NaCl, respectively. Stomatal conductance (gs), assessed by porometry, was significantly higher in control plants, mainly in Cobrançosa (102 mmol m-2 s-1), then in Arbequina (77 mmol m-2 s-1) and the lower values were found in Galega (51 mmol m-2 s-1). Salt reduced gs, on average of the three varieties to 30% or 10% of controls on exposure to 80 mM or 200 mM NaCl, respectively. Calculated (gs calc) and measured (gs) values of stomatal conductance showed a close relation between them (0,967, R2 = 0,837) which indicates this non-destructive method to determine whole-plant leaf area to be reasonably accurate.

RESUMO - A 8 de Maio de 2008 surgiu o centro de atendimento “Linha Saúde24” (S24) no sentido de modernizar o SNS, aproximando-o do cidadão. O serviço surge baseado no modelo inglês – o NHS Direct – que pode ser encarado como um serviço de informação telefónico apoiado por enfermeiros, disponível 24h por dia, concebido para expandir os serviços púbicos de acesso à rede prestadora de cuidados com intuito de aliviar a pressão da procura na rede de urgências hospitalares e médicos de família, assim como diluir as iniquidades regionais na prestação de serviços. A S24 assenta na perspectiva de ser um ponto de contacto inicial do utente com a rede de prestação de cuidados de saúde com capacidade de orientação. O objectivo da linha está na tentativa mais eficiente no uso dos recursos disponíveis, ao mesmo tempo que delega responsabilidade no cidadão na forma como este utiliza os recursos disponíveis, com melhor racionalização financeira na área da saúde aliada a uma melhor qualidade de serviço prestada e adequada, colocando os cidadãos no mesmo patamar, diluindo as dificuldades de acesso a aqueles que necessitam na tentativa de harmonizar e racionalizar o consumo de serviços de saúde. Esta estrutura permite ao cidadão conhecer melhor o seu estado de saúde e decidir mais acertadamente quanto à decisão a tomar. Com este estudo, e com base na literatura nacional e internacional, pretende-se descrever o perfil de utilizador que acede à S24 – definir o tipo de utilizador, disposição geográfica, motivos pelo qual acede ao serviço e qual o seu destino final, fazendo comparação com o perfil do NHS Direct. Assim, e com os dados obtidos, far-se-á uma avaliação preliminar em termos do contributo da linha S24 no que concerne à sua eficiência, equidade e empowerment dado ao utilizador. --- ------------------------------ABSTRACT - Saúde 24 (S24) is a national 24-hour health line initiated in May 2008 aiming at modernizing the Portuguese NHS by bringing it closer to the citizen. Indeed, S24 be seen as an initial contact point between the patient and the healthcare network, facilitating a better a management of health care demand. The service is inspired on the UK NHS Direct – a nurse-led telephone line to provide easier and faster advice information to people about health, illness and NHS services. It is expected to provide information so that people can deal with their health problems or their families´ on their own, with the purpose of reducing demand to A&E department and out-of-hours GP services. Additionally it can contribute to a reduction in regional inequities in healthcare provision through bringing health care advice to remote areas. The purpose of S24 is to handle more efficiently the available resources by enabling responsibilities in citizens. By doing so, S24 encourages a more appropriate use of available resources, with better financial outcomes and a better quality of care. It is meant, in terms of empowerment, to help people to be in control of their health and healthcare interactions by participating in the final decision. Based on quantitative data, this study defines the S24 caller user profile in terms of type, geographical reference, reasons for calling and outcome. This analysis allows us to perform a preliminary evaluation of the S24 in terms of its contribution to efficiency, equity and empowerment. Then the S24 is compared to

Neste documento descreve-se o projeto desenvolvido na unidade curricular de Tese e Dissertação durante o 2º ano do Mestrado de Engenharia Eletrotécnica e de Computadores no ramo de Automação e Sistemas, no Departamento de Engenharia Eletrotécnica (DEE) do Instituto Superior de Engenharia do Porto (ISEP). O projeto escolhido teve como base o uso da tecnologia das redes neuronais para implementação em sistemas de controlo. Foi necessário primeiro realizar um estudo desta tecnologia, perceber como esta surgiu e como é estruturada. Por último, abordar alguns casos de estudo onde as redes neuronais foram aplicadas com sucesso. Relativamente à implementação, foram consideradas diferentes estruturas de controlo, e entre estas escolhidas a do sistema de controlo estabilizador e sistema de referência adaptativo. No entanto, como o objetivo deste trabalho é o estudo de desempenho quando aplicadas as redes neuronais, não se utilizam apenas estas como controlador. A análise exposta neste trabalho trata de perceber em que medida é que a introdução das redes neuronais melhora o controlo de um processo. Assim sendo, os sistemas de controlo utilizados devem conter pelo menos uma rede neuronal e um controlador PID. Os testes de desempenho são aplicados no controlo de um motor DC, sendo realizados através do recurso ao software MATLAB. As simulações efetuadas têm diferentes configurações de modo a tirar conclusões o mais gerais possível. Assim, os sistemas de controlo são simulados para dois tipos de entrada diferentes, e com ou sem a adição de ruído no sensor. Por fim, é efetuada uma análise das respostas de cada sistema implementado e calculados os índices de desempenho das mesmas.

The pathogenesis of Duchenne muscular dystrophy (DMD), characterised by lack of the cytoskeletal protein dystrophin, is not completely understood. An early event in the degenerative process of DMD muscle could be a rise in cytosolic calcium concentration. In order to investigate whether this leads to alterations of contractile behaviour, we studied the excitability and contractile properties of cultured myotubes from control (C57BL/10) and mdx mice, an animal model for DMD. The myotubes were stimulated electrically and their motion was recorded photometrically. No significant differences were found between control and mdx myotubes with respect to the following parameters: chronaxy and rheobase (0.33 +/- 0.03 ms and 23 +/- 4 V vs. 0.39 +/- 0.07 ms and 22 +/- 2 V for C57 and mdx myotubes, respectively), tetanisation frequency (a similar distribution pattern was found between 5 and 30 Hz), fatigue during tetanus (found in 35% of both types of myotubes) and post-tetanic contracture. In contrast, contraction and relaxation times were longer (P < 0.005) in mdx (36 +/- 2 and 142 +/- 13 ms, respectively) than in control myotubes (26 +/- 1 and 85 +/- 9 ms, respectively). Together with our earlier findings, these results suggest a decreased capacity for calcium removal in mdx cells leading, in particular, to alterations of muscle relaxation.

Coxiella burnetii and members of the genus Rickettsia are obligate intracellular bacteria. Since cultivation of these organisms requires dedicated techniques, their diagnosis usually relies on serological or molecular biology methods. Immunofluorescence is considered the gold standard to detect antibody-reactivity towards these organisms. Here, we assessed the performance of a new automated epifluorescence immunoassay (InoDiag) to detect IgM and IgG against C. burnetii, Rickettsia typhi and Rickettsia conorii. Samples were tested with the InoDiag assay. A total of 213 sera were tested, of which 63 samples from Q fever, 20 from spotted fever rickettsiosis, 6 from murine typhus and 124 controls. InoDiag results were compared to micro-immunofluorescence. For acute Q fever, the sensitivity of phase 2 IgG was only of 30% with a cutoff of 1 arbitrary unit (AU). In patients with acute Q fever with positive IF IgM, sensitivity reached 83% with the same cutoff. Sensitivity for chronic Q fever was 100% whereas sensitivity for past Q fever was 65%. Sensitivity for spotted Mediterranean fever and murine typhus were 91% and 100%, respectively. Both assays exhibited a good specificity in control groups, ranging from 79% in sera from patients with unrelated diseases or EBV positivity to 100% in sera from healthy patients. In conclusion, the InoDiag assay exhibits an excellent performance for the diagnosis of chronic Q fever but a very low IgG sensitivity for acute Q fever likely due to low reactivity of phase 2 antigens present on the glass slide. This defect is partially compensated by the detection of IgM. Because it exhibits a good negative predictive value, the InoDiag assay is valuable to rule out a chronic Q fever. For the diagnosis of rickettsial diseases, the sensitivity of the InoDiag method is similar to conventional immunofluorescence.

Imaging studies have shown reduced frontal lobe resources following total sleep deprivation (TSD). The anterior cingulate cortex (ACC) in the frontal region plays a role in performance monitoring and cognitive control; both error detection and response inhibition are impaired following sleep loss. Event-related potentials (ERPs) are an electrophysiological tool used to index the brain's response to stimuli and information processing. In the Flanker task, the error-related negativity (ERN) and error positivity (Pe) ERPs are elicited after erroneous button presses. In a Go/NoGo task, NoGo-N2 and NoGo-P3 ERPs are elicited during high conflict stimulus processing. Research investigating the impact of sleep loss on ERPs during performance monitoring is equivocal, possibly due to task differences, sample size differences and varying degrees of sleep loss. Based on the effects of sleep loss on frontal function and prior research, it was expected that the sleep deprivation group would have lower accuracy, slower reaction time and impaired remediation on performance monitoring tasks, along with attenuated and delayed stimulus- and response-locked ERPs. In the current study, 49 young adults (24 male) were screened to be healthy good sleepers and then randomly assigned to a sleep deprived (n = 24) or rested control (n = 25) group. Participants slept in the laboratory on a baseline night, followed by a second night of sleep or wake. Flanker and Go/NoGo tasks were administered in a battery at 1O:30am (i.e., 27 hours awake for the sleep deprivation group) to measure performance monitoring. On the Flanker task, the sleep deprivation group was significantly slower than controls (p's <.05), but groups did not differ on accuracy. No group differences were observed in post-error slowing, but a trend was observed for less remedial accuracy in the sleep deprived group compared to controls (p = .09), suggesting impairment in the ability to take remedial action following TSD. Delayed P300s were observed in the sleep deprived group on congruent and incongruent Flanker trials combined (p = .001). On the Go/NoGo task, the hit rate (i.e., Go accuracy) was significantly lower in the sleep deprived group compared to controls (p <.001), but no differences were found on false alarm rates (i.e., NoGo Accuracy). For the sleep deprived group, the Go-P3 was significantly smaller (p = .045) and there was a trend for a smaller NoGo-N2 compared to controls (p = .08). The ERN amplitude was reduced in the TSD group compared to controls in both the Flanker and Go/NoGo tasks. Error rate was significantly correlated with the amplitude of response-locked ERNs in control (r = -.55, p=.005) and sleep deprived groups (r = -.46, p = .021); error rate was also correlated with Pe amplitude in controls (r = .46, p=.022) and a trend was found in the sleep deprived participants (r = .39, p =. 052). An exploratory analysis showed significantly larger Pe mean amplitudes (p = .025) in the sleep deprived group compared to controls for participants who made more than 40+ errors on the Flanker task. Altered stimulus processing as indexed by delayed P3 latency during the Flanker task and smaller amplitude Go-P3s during the Go/NoGo task indicate impairment in stimulus evaluation and / or context updating during frontal lobe tasks. ERN and NoGoN2 reductions in the sleep deprived group confirm impairments in the monitoring system. These data add to a body of evidence showing that the frontal brain region is particularly vulnerable to sleep loss. Understanding the neural basis of these deficits in performance monitoring abilities is particularly important for our increasingly sleep deprived society and for safety and productivity in situations like driving and sustained operations.

Le système endothéline (ET) est activé en condition d’hypertension pulmonaire (HTP). L’efficacité des antagonistes des récepteurs à l’ET a clairement été démontrée et a menée à l’approbation clinique de tels antagonistes dans le traitement de l’hypertension artérielle pulmonaire (HTAP). Toutefois, il existe présentement un important débat opposant l’utilisation d’un antagoniste sélectif des récepteur ETA à l’utilisation d’un antagoniste double ETA/ETB dans le traitement de cette pathologie. Bien que nous sachions que le système ET est activé et contribue à l’HTAP, les modifications locales de ce système induites par la pathologie, particulièrement au niveau des artères de résistance pulmonaires, demeurent inconnues. De plus, l’impact de ces modifications sur la réponse pharmacologique aux divers antagonistes des récepteurs à l’ET (sélectifs versus double) est d’une importance capitale. Ainsi, le but de la première étude de cette thèse était d’évaluer les modifications potentielles de la pharmacologie du système ET au niveau des artères de résistance pulmonaires induites par l’HTAP. Dans cette étude, nous avons démontré qu’en condition contrôle l’antagoniste sélectif ETA et l’antagoniste double n’ont eu aucun effet sur la réponse vasoconstrictrice à l’ET-1. Toutefois, en condition d’HTAP, les antagonistes sélectif et double ont tous deux été en mesure de réduire la vasoconstriction pulmonaire induite par l’ET-1. Une diminution importante de l’expression génique du récepteur ETB pourrait être à l’origine de cette modification du profil pharmacologique des antagonistes. Une meilleure compréhension des rôles joués par les récepteurs ETA et ETB au niveau des artères de résistance pulmonaires pourrait permettre l’optimisation des traitements de l’HTAP. Ainsi, le but de la deuxième étude était d’évaluer les effets d’un traitement antisens ex vivo dirigé contre l’ARNm des récepteurs ETA et ETB dans la vasoconstriction des artères de résistance pulmonaires induite par l’ET-1. Dans cette étude, nous avons démontré dans un premier temps que les récepteurs ETA et ETB pouvaient former des dimères au niveau des artères de résistance pulmonaires. De plus, nous avons observé qu’une réduction de l’expression protéique du R-ETA entraînait une potentialisation de la vasoconstriction ETB dépendante suggérant ainsi qu’en condition contrôle, le récepteur ETA aurait un effet inhibiteur sur la vasoconstriction pulmonaire induite par la stimulation du récepteur ETB. Les effets délétères de l’ET-1 sur la circulation pulmonaire sont bien connus, toutefois seules quelques études ont porté leur attention sur l’implication de l’ET-3 dans l’HTAP. Ainsi, le but de la troisième étude était d’évaluer l’implication potentielle de l’ET-3 dans l’HTAP. Dans cette étude, nous avons démontré qu’il était nécessaire en condition contrôle de bloquer simultanément les récepteurs ETA et ETB afin de réduire la réponse vasoconstrictrice pulmonaire à l’ET-3. En condition d’HTAP, nous avons observé une augmentation non-significative des concentrations plasmatiques d’ET-3 ainsi qu’une modification du profil pharmacologique des antagonistes des récepteurs à l’ET. En effet, l’utilisation de l’antagoniste sélectif ETA ou de l’antagoniste double était dans les deux cas en mesure de réduire la vasoconstriction pulmonaire à l’ET-3. Les résultats de ces trois études suggèrent qu’il est préférable d’utiliser un antagoniste double dans le traitement de l’HTAP. En effet, (1) en condition d’HTAP, l’utilisation d’un antagoniste double est aussi efficace que l’utilisation d’un antagoniste sélectif ETA; (2) les récepteurs ETA et ETB peuvent former des dimères au niveau des artères de résistance pulmonaires et (3) le récepteur ETB joue un rôle prédominant dans la vasoconstriction pulmonaire, il semble donc essentiel de bloquer simultanément les récepteurs ETA et ETB afin d’inhiber la réponse vasoconstrictrice induite par l’ET. Nous avons étudié les relations anatomiques entre les systèmes de neurotransmission à substance P (SP) et à sérotonine (5-hydroxytryptamine, 5-HT) dans le noyau du raphé dorsal (NRD) du rongeur, afin de mieux comprendre les interactions entre ces systèmes durant la régulation de l’humeur. Le NRD reçoit une innervation SP provenant de l’habenula, et le blocage pharmacologique des récepteurs neurokinine-1 (rNK1) de la SP aurait des effets antidépresseurs. Chez le rongeur, le traitement par les antagonistes des rNK1 s’accompagne d’une désensibilisation des autorécepteurs 5-HT1A de la 5-HT et d’une hausse de l’activité des neurones 5-HT dans le NRD, suggérant des interactions locales entre ces deux systèmes. Dans un premier temps, nous avons démontré par doubles marquages immunocytochimiques en microscopies optique, confocale et électronique, la présence du rNK1 dans une sous-population de neurones 5-HT du NRD caudal. Lors de l’analyse en microscopie électronique, nous avons pu constater que les rNK1 étaient principalement cytoplasmiques dans les neurones 5-HT et membranaires sur les neurones non 5-HT du noyau. Grâce à d’autres doubles marquages, nous avons aussi pu identifier les neurones non-5-HT porteurs de rNK1 comme étant GABAergiques. Nous avons ensuite combiné l’immunomarquage de la SP avec celui du rNK1, dans le but d’examiner les relations entre les terminaisons (varicosités *) axonales SP et les neurones 5-HT (pourvus de rNK1 cytoplasmiques du NRD caudal. En simple marquage de la SP, nous avons pu estimer à 41% la fréquence avec laquelle les terminaisons SP font synapse. Dans le matériel doublement marqué pour la SP et son récepteur, les terminaisons SP ont été fréquemment retrouvées en contact direct ou à proximité des dendrites munies de rNK1 cytoplasmiques, mais toujours éloignées des dendrites à rNK1 membranaires. Pour tester l’hypothèse d’une internalisation soutenue des rNK1 par la SP dans les neurones 5-HT, nous avons ensuite examiné la localisation subcellulaire du récepteur chez le rat traité avec un antagoniste du rNK1, le RP67580. La densité du marquage des rNK1 a été mesurée dans le cytoplasme et sur la membrane des deux types de dendrites (5-HT: rNK1 cytoplasmiques; non 5-HT: rNK1 membranaires). Une heure après une injection unique de l’antagoniste, la distribution du rNK1 est apparue inchangée dans les deux types de neurones (5-HT et non 5-HT). Par contre, après un traitement quotidien de 7 ou 21 jours avec l’antagoniste, nous avons mesuré une augmentation significative des densités cytoplasmique et membranaire du rNK1 dans les neurones 5-HT, sans aucun changement dans les neurones non 5-HT. Ces traitements ont aussi augmenté l’expression du gène rNK1 dans le NRD. Enfin, nous avons mesuré une hausse de la densité membranaire du rNK1 dans les neurones 5-HT, sans hausse de densité cytoplasmique, par suite d’une lésion bilatérale de l’habenula. Ces résultats confortent l’hypothèse d’une activation et d’une internalisation soutenues des rNK1 par la SP dans les neurones 5-HT du NRD caudal. Ils suggèrent aussi que le trafic des rNK1 dans les neurones 5-HT du NRD représente un mécanisme cellulaire en contrôle de l’activation du système 5-HT par les afférences SP en provenance de l’habenula.
