RESUMO Introdução: a bronquiolite aguda afecta 30 a 60% das crianças saudáveis até ao terceiro aniversário. Destas, 50% evoluem para sibilância recorrente, com consequências pessoais, familiares e sociais relevantes. Objectivos: identificação de parâmetros clínicos e laboratoriais associados à sibilância recorrente em crianças saudáveis. A detecção da actividade local de quatro citocinas intervenientes nas vias imunológicas Th1 ou Th2 permitiu, gualmente, o estudo do respectivo comportamento em função dos referidos parâmetros. Material e Métodos: estudo prospectivo de coorte, efectuado no serviço de urgência pediátrica do HSFX no Inverno 2004/2005. Recrutada amostra de conveniência de 188 lactentes de idade inferior a 24 meses e diagnóstico de bronquiolite aguda. Excluídos portadores de doença crónica, ex-prematuros e os doentes sujeitos a corticoterapia nas duas semanas anteriores. A todos foi feito questionário padronizado, exame clínico e estratificação da gravidade através de índice validado. Na mucosa nasal, foram pesquisados Ag víricos e estudada a expressão génica de quatro citocinas - representativas da via Th1 (IFN-g e IL-12) ou Th2 (IL-4 e IL-13). Esta determinação foi efectuada por RT-PCR, no Laboratório de Biologia Molecular do Centro de Histocompatibilidade do Sul. Durante o seguimento, em consulta de pneumologia pediátrica até à idade de 36 meses, foram excluídos os seguintes casos: patologia anatómica ou funcional que justificasse sibilância recorrente, valores totais de IgE elevados, Phadiatop® positivo ou défice de IgG, IgA ou IgM. Os resultados foram estudados por estatística descritiva, análise univariada e multivariada. Foi considerado com significado estatístico, em todas as análises, o valor de p < 0,05. Resultados: das 188 crianças recrutadas, foram excluídas 36 (principalmente por identificação de outras causas de sibilância recorrente). Da amostra final de 152 doentes, 76 (50%) foram considerados sibilantes e 76 não sibilantes. Os seguintes parâmetros à entrada relacioram-se com a evolução para sibilância recorrente: idade inferior a 6 meses (OR: 3,8; IC95% 1,8-9,1); exposição ao fumo de tabaco na vida intrauterina (OR: 2,5; IC95% 1,2-5,2) ou após o nascimento (OR: 2,4; IC95% 1,0-5,8); atopia materna (OR: 4,0; IC95% 1,5-10,3); maior gravidade (OR: 1,2; IC95% 1,1-1,4); apirexia (OR: 4,7; IC95% 1,8-11,8); identificação de vírus (OR: 3,5; IC95% 1,4-8,7); predomínio imunitário de tipo Th2 em lactentes com idade superior a seis meses (OR:1,3; IC95% 1,0-1,7). Verificou-se predomínio Th1 na mucosa nasal das crianças com irmãos ou que frequentavam o infantário (OR: 3,1; IC95% 1,4-6,5); nas amamentadas (OR: 1,2; IC95% 1,1-1,4) e naquelas em que se identificaram Ag víricos (OR: 4,0; IC95% 1,3-11,9). O contacto com fumo de tabaco no domicílio foi o determinante mais importante de predomínio Th2 (OR: 2,3; IC95% 1,1-4,7). Discussão e Conclusões: Foram considerados factores de risco para sibilância recorrente em lactentes saudáveis: idade inferior no momento do primeiro episódio de bronquiolite; exposição passiva (antenatal ou pós-natal) ao fumo de tabaco;antecedentes maternos de atopia; primeiro episódio com maior gravidade, sem febre e com dentificação de Ag víricos nas secreções nasais. Os resultados do presente estudo reforçam as recomendações usuais de aleitamento materno, limitação de contactos sociais em lactentes e evicção tabágica durante a gravidez e após o nascimento.

The hamster check pouch is an invagination of oral mucosa, characterized histologically as skin-like. In this paper we describe anatomical, histological and embriological features of the pouch and coment on the pouch as an immunologically privileged site since it lacks lymphatic drainage and has few Langerhans cells. We present the review from literature and our observations after inoculation in the pouch of mycobacteriae (BCG, Mycobacterium tuberculosis and Mycobacterium leprae) and a fungus (Paracoccidioides brasiliensis). Lesions in the pouch were granulomatous but smaller and long lasting; even granulomatous, the reaction was inefficient to control the proliferation of agents compared with inoculation in other sites, except for BCG. Appearance of immunity was also delayed or absent and, when it was detected, a sharp decrease in number of agents in pouch lesions was observed. These observations make the pouch an interesting site for the study of the role of immune system in infeccious diseases and in granuloma formation.

Systematic examination of the upper respiratory and digestive tracts (URDT) was performed in a group of 80 paracoccidioidomycosis (PCM) patients submitted to post-treatment follow-up ranging from 8 months to 17 years. Mucosae of the URDT had been involved prior to specific treatment in 74 patients, distributed as follows: oropharynx, 50 (41 alone, 7 in association with the larynx, and 2 with the nasal mucosa); larynx, 30 (23 alone and 7 in association); and nasal mucosa, 3(1 alone and 2 in association). Inactive lesions were observed in all the 50 patients with lesions of the oropharynx, 3 of whom with deforming scars (1 with retraction of the tongue and 2 with narrowing of the oral orifice). One case presented a destructive lesion, with perfuration of the palate. Of the other 46 cases, examination showed nacreous white striated scars which were nearly imperceptible in some cases and in others displayed partial retraction of anatomical structures without any alteration of their features. Patients presented a high rate of missing teeth. In 3 patients with involvement of the nasal mucosa, none of whom presented active PCM lesions, 2 still had nasal voices. In 30 patients with lesions of the larynx, 1 suffered a relapse of PCM and 2 developed epidermoid carcinoma. Of the other 27 cases, none of whom had active PCM lesions, 15 presented dysphonia, 3 were tracheotomized, and 9 were asymptomatic.

Colonization of the colon and rectum by intestinal spirochetes is detected for the first time in Brazil in 4 of 282 (1.41%) patients who had undergone sigmoidoscopy and/or colonoscopy with a histopathological diagnosis of chronic non specific-colitis. This frequency is probably understimated, since surgically obtained specimens were not considered in the present study. Histopathological diagnosis was performed using routine stains like hematoxylin-eosin which showed the typical, of 3-µm thick hematoxyphilic fringe on the brush border of the surface epithelium, and by silver stains like the Warthin-Starry stain. Immunohistochemical procedures using two, polyclonal, primary antibodies, one against Treponema pallidum and the other against Leptospira interrogans serovar copenhageni serogroup Icterohaemorrhagiae cross-reacted with spirochetal antigen/s producing a marked contrast of the fringe over the colonic epithelium, preserving the spiral-shaped morphology of the parasite. In one case with marked diarrhea, immunohistochemistry detected spirochetal antigen/s within a cell in an intestinal crypt, thus demonstrating that the infection can be more widely disseminated than suspected using routine stains. Immunohistochemical procedures, thus, greatly facilitate the histological diagnosis of intestinal spirochetosis and may contribute to a better understanding of the pathogenesis of the disease. Transmission and scanning electron microscopy performed in one case showed that the spirochete closely resembled the species designated as Brachyspira aalborgi.

The basidiomycosis, fungal infections provoked by basidiomycetes or agaric fungi have been recorded at growing frequencies in the medical literature, especially after the advent of AIDS in 1991. The basidiospores of these fungi, scattered in the atmosphere and transported by winds or air currents, reach the maxillary sinuses through the nasal route, most of the times causing signs and symptoms of chronic sinusitis. Basidiomycetes have also been isolated from sputum, especially Schizophyllum commune. Lesions of the buccal mucosa, brain abscesses, onychomycosis and endocarditis have been described, with a growing interest in this type of deep mycosis on the part of mycologists and infectologists. The present paper reports descriptions of mycetism as well as infectious processes caused by basidiomycetes, such as Schizophyllum commune, Ustilago maydis (= Ustilago zeae) and Coprinus cinereus

Three cases of the juvenile form of paracoccidioidomycosis are reported. Emphasis has been given to the oral manifestations, particularly the periodontal involvement. The main periodontal findings were: generalized and progressive alveolar bone destruction leading to gingival recession with exposure of the tooth roots, and spontaneous tooth losses. The gingival mucosa was predominantly smooth, erithematous and slightly swollen. These aspects, although rare, may be the earliest signs of the disease and sometimes its only manifestation.

ABSTRACT – Background: According to the Report on Carcinogens, formaldehyde ranks 25th in the overall U.S. chemical production, with more than 5 million tons produced each year. Given its economic importance and widespread use, many people are exposed to formaldehyde environmentally and/or occupationally. Presently, the International Agency for Research on Cancer classifies formaldehyde as carcinogenic to humans (Group 1), based on sufficient evidence in humans and in experimental animals. Manyfold in vitro studies clearly indicated that formaldehyde can induce genotoxic effects in proliferating cultured mammalian cells. Furthermore, some in vivo studies have found changes in epithelial cells and in peripheral blood lymphocytes related to formaldehyde exposure. Methods: A study was carried out in Portugal, using 80 workers occupationally exposed to formaldehyde vapours: 30 workers from formaldehyde and formaldehyde-based resins production factory and 50 from 10 pathology and anatomy laboratories. A control group of 85 non-exposed subjects was considered. Exposure assessment was performed by applying simultaneously two techniques of air monitoring: NIOSH Method 2541 and Photo Ionization Detection equipment with simultaneously video recording. Evaluation of genotoxic effects was performed by application of micronucleus test in exfoliated epithelial cells from buccal mucosa and peripheral blood lymphocytes. Results: Time-weighted average concentrations not exceeded the reference value (0.75 ppm) in the two occupational settings studied. Ceiling concentrations, on the other hand, were higher than reference value (0.3 ppm) in both. The frequency of micronucleus in peripheral blood lymphocytes and in epithelial cells was significantly higher in both exposed groups than in the control group (p < 0.001). Moreover, the frequency of micronucleus in peripheral blood lymphocytes was significantly higher in the laboratories group than in the factory workers (p < 0.05). A moderate positive correlation was found between duration of occupational exposure to formaldehyde (years of exposure) and micronucleus frequency in peripheral blood lymphocytes (r = 0.401; p < 0.001) and in epithelial cells (r = 0.209; p < 0.01). Conclusions: The population studied is exposed to high peak concentrations of formaldehyde with a long-term exposure. These two aspects, cumulatively, can be the cause of the observed genotoxic endpoint effects. The association of these cytogenetic effects with formaldehyde exposure gives important information to risk assessment process and may also be used to assess health risks for exposed worker

Anticorpos anti-Trypanosoma cruzi (isotipo IgG) foram detectados no transudato da mucosa oral (TMO) através de um ensaio imunoenzimático. Foram estudados 21 indivíduos com doença de Chagas crônica comprovada através de diagnóstico clínico, eletrocardiográfico, epidemiológico e sorológico: 10 com forma cardíaca, 2 com forma digestiva, 6 com forma mista e 3 com forma assintomática. Sete indivíduos de área endêmica, com sorologia negativa, constituiram o grupo controle. O soro destes grupos foi armazenado a -20 oC. A coleta de TMO de ambos os grupos foi realizada com o dispositivo OraSureâ seguindo orientação do fabricante (OraSureâ , Epitope Inc., Beaverton, OR, USA). As amostras de TMO foram diluídas (1:2) e testadas em duplicata através de um ensaio imunoenzimático da Abbott Laboratories para detectar anticorpos IgG contra doença de Chagas. Vinte dos vinte e um pacientes chagásicos apresentaram densidade óptica acima do limiar de reatividade e foram considerados positivos para doença de Chagas. Nenhuma das amostras provenientes de indivíduos soronegativos foi positiva. A sensibilidade e especificidade foram de 95% e 100%, respectivamente. Estes resultados indicam que TMO poderá ser utilizado como um fluido biológico alternativo para o diagnóstico da doença de Chagas. Nós estamos aumentando o número de indivíduos para validar estes resultados incluindo a análise comparativa entre amostras de TMO e soro.

Whipple's disease (WD) is a rare systemic disease of infectious etiology which involves the small intestine but can virtually affect any organ. We present here five cases (four males and one female) ranging in age from 20 to 59 years. All patients had intestinal involvement associated or not with clinical manifestations linked to this organ. Vegetation in the tricuspid valve was observed in one patient, suggesting endocarditis caused by Tropheryma whippelii, with disappearance of the echocardiographic alterations after treatment. In one of the male patients the initial clinical manifestation was serologically negative spondylitis, with no diarrhea occurring at any time during follow-up. Ocular involvement associated with intestinal malabsorption and significant weight loss were observed in one case. In the other two cases, diarrhea was the major clinical manifestation. All patients were diagnosed by histological examination of the jejunal mucosa and, when indicated, of extraintestinal tissues by light and electron microscopy. After antibiotic treatment, full remission of symptoms occurred in all cases. A control examination of the intestinal mucosa performed after twelve months of treatment with sulfamethoxazole-trimethoprim revealed the disappearance of T. whippelii in four patients. The remaining patient was lost to follow-up.

This review discusses experimental evidences that indicate the IgE participation on the effector mechanisms that leads to gastrointestinal nematode elimination. Data discussed here showed that, for most experimental models, the immune response involved in nematode elimination is regulated by Th-2 type cytokines (especially IL-4). However, the mechanism(s) that result in worm elimination is not clear and might be distinct in different nematode species. Parasite specific IgE production, especially the IgE produced by the intestinal mucosae or associated lymphoid organs could participate in the intestinal elimination of Trichinella spiralis from infected rats. Intestinal IgE may also be important to the protective mechanism developed against other gastrointestinal nematodes that penetrate the murine duodenum mucosa tissue, such as Strongyloides venezuelensis and Heligmosomoides polygyrus. At least in Trichinella spiralis infected rats, the results indicated that intestinal IgE might work independently from mast cell degranulation for worm elimination.

A esofagite eosinofílica constitui uma doença infl amatória do esófago, mundialmente emergente, caracterizada pela infiltração significativa e isolada da mucosa esofágica por eosinófilos. A sintomatologia de apresentação revela-se bastante variável, mas frequentemente manifesta -se como uma doença de refluxo gastroesofágico que não responde à terapêutica anti-refluxo. O impacto alimentar esofágico e o desenvolvimento de estenoses esofágicas são complicações graves que podem ocorrer, com necessidade de remoção urgente do alimento e dilatação esofágica via endoscópica,respectivamente. A potencial gravidade destes sintomas justifica a importância do reconhecimento e do tratamento da doença, sobretudo tendo em conta que a sua prevalência tem vindo a aumentar na última década. A presente revisão sumariza o actual conhecimento sobre epidemiologia, apresentação clínica, possíveis mecanismos fisiopatológicos, história natural, abordagem diagnóstica e terapêutica desta condição.

Bartonellosis or Carrion's disease is endemic in some regions of Peru, classically found in the inter-Andean valleys located between 500 and 3200 meters above sea level. We report the case of a 43 year-old male patient, farmer, who was born in the Pichanaki district (Chanchamayo, Junin), located in the High Forest of Peru. He presented with disseminated, raised, erythematous cutaneous lesions, some of which bled. The distribution of these lesions included the nasal mucosa and penile region. Additionally subcutaneous nodules were distributed over the trunk and extremities. Hematologic exams showed a moderate anemia. Serologic studies for HIV and Treponema pallidum were negative. The histopathologic results of two biopsies were compatible with Peruvian wart. Oral treatment with ciprofloxacin (500 mg BID) was begun. Over 10 days, the patient showed clinical improvement. This is the first report of a confirmed case of bartonellosis in the eruptive phase originating from the Peruvian High Forest, showing the geographical expansion of the Carrion's disease.

O diagnóstico de metástases cardíacas pré-mortem é raro. Apresenta-se um caso de metástases cardíacas de adenocarcinoma do cólon - massa intracavitária na aurícula direita e massa vegetante na válvula tricúspide - as quais foram diagnosticadas por ecocardiografia transtorácica bidimensional. As autoras fazem uma breve referência aos aspectos clínicos e imagiológicos dos tumores cardíacos secundários, com especial ênfase para a ecocardiografia.

A Granulomatose de Wegener é uma doença sistémica de etiologia desconhecida, caracterizada pela tríade clínica: envolvimento da mucosa nasal e dos seios perinasais, infiltração e cavitação pulmonar e doença renal com hematúria. A Granulomatose de Wegener afecta normalmente adultos jovens e de meia idade e embora seja incomum nas crianças, pode surgir em qualquer estrato etário. Apresentamos o caso clínico de uma adolescente de 14 anos, com sintomas de epistaxis, cuja investigação posterior mostrou insuficiência renal e múltiplos nódulos pulmonares. O teste positivo para o anticorpo antineutrofílico citoplasmático suportou o diagnóstico de Granulomatose de Wegener, necessitando de terapêutica com hemodiálise e imunossupressão.
