PURPOSE: Repeated-sprint training in hypoxia (RSH) was recently shown to improve repeated-sprint ability (RSA) in cycling. This phenomenon is likely to reflect fiber type-dependent, compensatory vasodilation, and therefore, our hypothesis was that RSH is even more beneficial for activities involving upper body muscles, such as double poling during cross-country skiing. METHODS: In a double-blinded fashion, 17 competitive cross-country skiers performed six sessions of repeated sprints (each consisting of four sets of five 10-s sprints, with 20-s intervals of recovery) either in normoxia (RSN, 300 m; FiO2, 20.9%; n = 8) or normobaric hypoxia (RSH, 3000 m; FiO2, 13.8 %; n = 9). Before (pre) and after (post) training, performance was evaluated with an RSA test (10-s all-out sprints-20-s recovery, until peak power output declined by 30%) and a simulated team sprint (team sprint, 3 × 3-min all-out with 3-min rest) on a double-poling ergometer. Triceps brachii oxygenation was measured by near-infrared spectroscopy. RESULTS: From pretraining to posttraining, peak power output in the RSA was increased (P < 0.01) to the same extent (29% ± 13% vs 26% ± 18%, nonsignificant) in RSH and in RSN whereas the number of sprints performed was enhanced in RSH (10.9 ± 5.2 vs 17.1 ± 6.8, P < 0.01) but not in RSN (11.6 ± 5.3 vs 11.7 ± 4.3, nonsignificant). In addition, the amplitude in total hemoglobin variations during sprints throughout RSA rose more in RSH (P < 0.01). Similarly, the average power output during all team sprints improved by 11% ± 9% in RSH and 15% ± 7% in RSN. CONCLUSIONS: Our findings reveal greater improvement in the performance of repeated double-poling sprints, together with larger variations in the perfusion of upper body muscles in RSH compared with those in RSN.

Introduction. - En Suisse, la prescription de produits biologiqueschez les patients souffrant de polyarthrite rhumatoïde (PR) n'est nilimitée à des centres hospitaliers de rhumatologie, ni soumise à desdirectives strictes éditées par les autorités sanitaires sur le type oule nombre de traitements de fond préalables. La notion d'échec auxtraitements de fond n'est pas non plus précisément définie, et enparticulier l'activité de la maladie ne doit pas répondre à des critèresstricts, notamment en terme de valeurs de DAS, et ce contrairementà de nombreux autres pays où l'impact de ces restrictions aété publié récemment (1, 2).Le registre SCQM peut être considéré comme un bon reflet de lapopulation suisse avec PR, aussi bien pour la population suivie pardes centres hospitaliers que par les practiciens en cabinet privé, eton estime qu'environ 30 % des patients avec PR recevant des traitementsbiologiques en Suisse sont inclus dans ce registre.L'objectif primaire de cette étude est de comparer les caractéristiquesdes patients de notre registre à l'initiation et après un an de traitementbiologique avec celles de registres du même type dans des pays avecun accès plus restreint aux traitements biologiques. Les objectifssecondaires sont de comparer les patients traités en milieu hospitalieret ceux pris en charge en cabinet privé, et aussi d'examiner s'il existedes tendances temporelles (avant et après 2005).Patients et Méthodes. - Les données sont extraites du registre suissede PR (SCQM) qui comprend 4 500 patients inclus entre 1997 et2011. 2 715 patients bénéficient d'un traitement biologique, dont2 427 avec des données à l'introduction du traitement : DAS28/VS,DAS28/CRP, HAQ, durée de la maladie, nbre de tttt préalables, comorbidités,etc. Les principales données démographiques sont : âgemoyen 55 ans, 77 % de femmes 72 % FR+, médecins prescripteurs :62 % en cabinet, 21 % en centre hospitaliers et 16 % en centres universitaires.Nous avons calculés les moyennes (+/- écart type) pourdifférents paramètres de l'activité de la maladie.Résultats. - La moyenne du DAS28/VS à l'introduction du traitement(4,4 +/- 1,3) est nettement inférieur aux valeurs publiées pard'autres registres européens ou canadien (5,3 < > 6,6 ; 1,2). Il en ende même pour le HAQ (1 versus 1,4). Les biologiques sont introduitsaprès en moyenne 1,1 +/- 1 DMARD préalable contre > 3 en Suède,au Danemark ou au Canada.Les caractéristiques démographiques, le degré d'activité et les traitementsprodigués sont similaires entre les patients traités encabinet privé ou en milieu hospitalier, hormis pour une proportionmoindre de traitements iv en cabinet (20 % versus 40 %). Après2005, les traitements biologiques sont introduits beaucoup plusprécocemment, avec une durée médiane de maladie avant l'introductionde thérapies biologiques diminuant de 96 à 51 mois. Onnote également une répartition entre les divers produits biologiquesqui se diversifie. Même si les traitements sont introduits à undegré d'activité similaire (DAS28/VS moyen à 4,4 +/- 1,3) onobserve de meilleurs résultats à 1 an avec un DAS moyen à 1 an :3,5 +/- 1,4 avant 2005 contre 3,1 +/- 1,3 après 2005 (p = 0.0001).Conclusion. - Les données du registre suisse des PR (SCQM) suggèrentque, en l'absence de critères restrictifs d'accès aux traitements biologiques,ceux-ci sont prescrits à des scores d'activité de la maladie(DAS et HAQ) inférieurs, et plus précocemement en terme de nombrede DMARD préalables. Cette tendance se confirme dans le temps, etse retrouve aussi bien en milieu hospitalier qu'en cabinet.En terme de résultats, après 2005, plus de 50 % des patients atteignentun bas degré d'activité de la maladie en terme de DAS aprèsun an de traitement, chiffre qui semble justifier ce type de systèmepeu restrictif favorisant certainement une approche thérapeutiqueplus proche des nouveaux paragidmes de traitement avec une stratégiede type « treat to target ».Références[1] Curtis J R et al. Background Following the discovery that mutant KRAS is associated with resistance to anti-epidermal growth factor receptor (EGFR) antibodies, the tumours of patients with metastatic colorectal cancer are now profiled for seven KRAS mutations before receiving cetuximab or panitumumab. However, most patients with KRAS wild-type tumours still do not respond. We studied the effect of other downstream mutations on the efficacy of cetuximab in, to our knowledge, the largest cohort to date of patients with chemotherapy-refractory metastatic colorectal cancer treated with cetuximab plus chemotherapy in the pre-KRAS selection era. Methods 1022 tumour DNA samples (73 from fresh-frozen and 949 from formalin-fixed, paraffin-embedded tissue) from patients treated with cetuximab between 2001 and 2008 were gathered from 11 centres in seven European countries. 773 primary tumour samples had sufficient quality DNA and were included in mutation frequency analyses; mass spectrometry genotyping of tumour samples for KRAS, BRAF, NRAS, and PIK3CA was done centrally. We analysed objective response, progression-free survival (PFS), and overall survival in molecularly defined subgroups of the 649 chemotherapy-refractory patients treated with cetuximab plus chemotherapy. Findings 40.0% (299/747) of the tumours harboured a KRAS mutation, 14.5% (108/743) harboured a PIK3CA mutation (of which 68.5% [74/108] were located in exon 9 and 20.4% [22/108] in exon 20), 4.7% (36/761) harboured a BRAF mutation, and 2.6% (17/644) harboured an NRAS mutation. KRAS mutants did not derive benefit compared with wild types, with a response rate of 6.7% (17/253) versus 35.8% (126/352; odds ratio [OR] 0.13, 95% CI 0.07-0.22; p<0.0001), a median PFS of 12. weeks versus 24 weeks (hazard ratio [HR] 1 98, 1.66-2.36; p<0.0001), and a median overall survival of 32 weeks versus 50 weeks (1.75, 1.47-2.09; p<0.0001). In KRAS wild types, carriers of BRAF and NRAS mutations had a significantly lower response rate than did BRAF and NRAS wild types, with a response rate of 8.3% (2/24) in carriers of BRAF mutations versus 38.0% in BRAF wild types (124/326; OR 0.15, 95% CI 0.02-0.51; p=0.0012); and 7.7% (1/13) in carriers of NRAS mutations versus 38.1% in NRAS wild types (110/289; OR 0.14, 0.007-0.70; p=0.013). PIK3CA exon 9 mutations had no effect, whereas exon 20 mutations were associated with a worse outcome compared with wild types, with a response rate of 0.0% (0/9) versus 36.8% (121/329; OR 0.00,0.00-0.89; p=0.029), a median PFS of 11.5 weeks versus 24 weeks (HR 2.52, 1.33-4.78; p=0.013), and a median overall survival of 34 weeks versus 51 weeks (3.29, 1.60-6.74; p=0.0057). Multivariate analysis and conditional inference trees confirmed that, if KRAS is not mutated, assessing BRAF, NRAS, and PIK3CA exon 20 mutations (in that order) gives additional information about outcome. Objective response rates in our series were 24.4% in the unselected population, 36.3% in the KRAS wild-type selected population, and 41.2% in the KRAS, BRAF, NRAS, and PIK3CA exon 20 wild-type population. Interpretation While confirming the negative effect of KRAS mutations on outcome after cetuximab, we show that BRAF, NRAS, and PIK3CA,exon 20 mutations are significantly associated with a low response rate. Objective response rates could be improved by additional genotyping of BRAF, NRAS, and PIK3CA exon 20 mutations in a KRAS wild-type population.

Pulmonary arterial hypertension is a rare disease with a poor prognosis. Epidemiological data are scarce, particularly in the paediatric population. A registry was recently developed in order to collect epidemiological data on patients with pulmonary arterial hypertension (PAH) in Switzerland. This is the first description of the paediatric data. Paediatric patients aged 0-18 years with the diagnosis of PAH were enrolled in the registry from 1999 to 2005 with informed consent from their parents. Patient characteristics, PAH aetiology, functional capacity, exercise capacity, treatments and outcome were among the most important data collected. A total of 23 patients (12 male, 11 female) have been thus far included in the registry. Median age at time of diagnosis was 3 years (range 1 month-18 years) and median follow-up was 3.47 years (range 1 day-12.6 years). PAH aetiologies are diagnosed as idiopathic in 8/23 patients (34.8%) and associated with congenital heart diseases in 12/23 (52.2%) or with pulmonary diseases in 3/23 patients (13.0%). Death occurred in 1 patient before treatment was initiated. Single treatments include medications with a calcium channel blocker in 2/23 patients, with bosentan in 10/23, and with inhaled iloprost in 1/23. Combined therapies include bosentan and inhaled iloprost in 7/23 patients, bosentan and sildenafil in 2/23 patients, and bosentan, sildenafil and inhaled iloprost in 2/23 patients. Additional oral anticoagulation is given to 14/23 patients and 8/23 patients are on oxygen therapy. NYHA class at baseline visit was obtained in 22/23 patients (4 NYHA 2, 17 NYHA 3 and 1 NYHA 4). Changes in NYHA class were observed over a 2-year period in 3/22 patients who improved from NYHA 3 to NYHA 2. Initial improvement of 6-minute walk distance was observed in 6/13 patients with a sustained improvement in 4. These preliminary results provide information on the epidemiology of PAH in children in Switzerland and demonstrate that most paediatric patients show stabilisation of the disease under new treatments. This underscores the utility of registries for rare diseases in providing crucial information in the era of new therapies. It may also help to improve the future medical approach.

O estudo teve como objetivos verificar a ocorrência de cefaleia como a principal dor e caracterizar essa experiência e o prejuízo causado por ela nas atividades cotidianas de estudantes universitárias de enfermagem. Trata-se de um estudo transversal, realizado na Faculdade de Enfermagem da Universidade Federal de Goiás, Goiânia, GO, Brasil, de maio a junho de 2008, com 203 estudantes, (idade média de 21 anos; d.p.=1,8), 48,5% da classe econômica A. A cefaleia foi a principal dor para 34,5% das universitárias; de forte intensidade; descrita como latejante (74,3%), pontada (62,9%) e enjoada (55,7%); com episódios à tarde (52,9%), com duração de algumas horas do dia (51,4%). Os fatores relacionados ao início da dor foram: os estudos (17,1%) e o estresse (11,4 %) e as atividades mais prejudicadas: a capacidade de concentração (84,3%) e o humor (84,3%) (p<0,05). A cefaleia é menos frequente nesta população comparada a estudos realizados em outros países e prejudica as atividades cotidianas das universitárias.

OBJECTIVETo assess the quality of prenatal care in mothers with premature and term births and identify maternal and gestational factors associated with inadequate prenatal care.METHODCross-sectional study collecting data with the pregnant card, hospital records and interviews with mothers living in Maringa-PR. Data were collected from 576 mothers and their born alive infants who were attended in the public service from October 2013 to February 2014, using three different evaluation criteria. The association of prenatal care quality with prematurity was performed by univariate analysis and occurred only at Kessner criteria (CI=1.79;8.02).RESULTSThe indicators that contributed most to the inadequacy of prenatal care were tests of hemoglobin, urine, and fetal presentation. After logistic regression analysis, maternal and gestational variables associated to inadequate prenatal care were combined prenatal (CI=2.93;11.09), non-white skin color (CI=1.11;2.51); unplanned pregnancy (CI=1.34;3.17) and multiparity (CI=1.17;4.03).CONCLUSIONPrenatal care must follow the minimum recommended protocols, more attention is required to black and brown women, multiparous and with unplanned pregnancies to prevent preterm birth and maternal and child morbimortality.

In heart transplantation (HTx), acute antibody-mediated rejection (AMR) is infrequent but carries high mortality and increased risk of graft vasculopathy. The diagnosis requires evidence of acute graft dysfunction, capillary lesions on endomyocardial biopsy (EMB), and immunopathological criteria of antibodymediated injury. Multiple markers of antibody-mediated injuries have been proposed, but there is ample debate on their usefulness. In kidney transplantation, C4d deposition in peritubular capillaries is a reliable marker of alloantibody-dependant graft injury. In this study, we prospectively screened all EMBs for C4d and CD68 in new HTx recipients, and correlated pathological fi ndings with immunological evidence of donor-specifi c antibodies (DSA) and graft dysfunction. Methods Between Nov 05 and Aug 08, we had 22 HTx, and 17 cases were analysed. All recipients received polyclonal rabbit anti-thymocytes globulin, calcineurin inhibitors, mycophenolate mofetil, and corticosteroids (weaning in 6 -12 months). They had EMB every 1-2 weeks in the fi rst 3 months, and then monthly for 9 months. C4d and CD 68 were assessed by immunochemistry. Echocardiography and DSA assessment or crossmatch (early phase) were realised if C4d or CD68 staining was positive. Results There was 1 early and 1 late AMR. However, these markers showed high specifi city for the diagnosis of AMR in the late phase. Of course these data need to be confi rmed in larger multi-center studies.
