An efficient high-resolution (HR) three-dimensional (3D) seismic reflection system for small-scale targets in lacustrine settings was developed. In Lake Geneva, near the city of Lausanne, Switzerland, the offshore extension of a complex fault zone well mapped on land was chosen for testing our system. A preliminary two-dimensional seismic survey indicated structures that include a thin (<40 m) layer of subhorizontal Quaternary sediments that unconformably overlie south-east-dipping Tertiary Molasse beds and a major fault zone (Paudeze Fault Zone) that separates Plateau and Subalpine Molasse (SM) units. A 3D survey was conducted over this test site using a newly developed three-streamer system. It provided high-quality data with a penetration to depths of 300 m below the water bottom of non-aliased signal for dips up to 30degrees and with a maximum vertical resolution of 1.1 m. The data were subjected to a conventional 3D processing sequence that included post-stack time migration. Tests with 3D pre-stack depth migration showed that such techniques can be applied to HR seismic surveys. Delineation of several horizons and fault surfaces reveals the potential for small-scale geologic and tectonic interpretation in three dimensions. Five major seismic facies and their detailed 3D geometries can be distinguished. Three fault surfaces and the top of a molasse surface were mapped in 3D. Analysis of the geometry of these surfaces and their relative orientation suggests that pre-existing structures within the Plateau Molasse (PM) unit influenced later faulting between the Plateau and SM. In particular, a change in strike of the PM bed dip may indicate a fold formed by a regional stress regime, the orientation of which was different from the one responsible for the creation of the Paudeze Fault Zone. This structure might have later influenced the local stress regime and caused the curved shape of the Paudeze Fault in our surveyed area.

Severe head injury induces major hormonal, humoral and metabolic changes, characterized by increases in stress hormone secretion, lymphokines production, associated with high lipid and protein catabolism as well as changes in energy expenditure (EE). Numerous factors influence EE in head-injured patients, particularly anthropometric data, body temperature, nutritional support, level of consciousness, muscular tone and activity. Resting EE is usually increased following brain trauma; however, normal or decreased metabolic rates can be observed in curarized patients on mechanical ventilation or in patients receiving high doses of barbiturates.

Optimal robust M-estimates of a multidimensional parameter are described using Hampel's infinitesimal approach. The optimal estimates are derived by minimizing a measure of efficiency under the model, subject to a bounded measure of infinitesimal robustness. To this purpose we define measures of efficiency and infinitesimal sensitivity based on the Hellinger distance.We show that these two measures coincide with similar ones defined by Yohai using the Kullback-Leibler divergence, and therefore the corresponding optimal estimates coincide too.We also give an example where we fit a negative binomial distribution to a real dataset of "days of stay in hospital" using the optimal robust estimates.

To perform a climatic analysis of the annual UV index (UVI) variations in Catalonia, Spain (northeast of the Iberian Peninsula), a new simple parameterization scheme is presented based on a multilayer radiative transfer model. The parameterization performs fast UVI calculations for a wide range of cloudless and snow-free situations and can be applied anywhere. The following parameters are considered: solar zenith angle, total ozone column, altitude, aerosol optical depth, and single-scattering albedo. A sensitivity analysis is presented to justify this choice with special attention to aerosol information. Comparisons with the base model show good agreement, most of all for the most common cases, giving an absolute error within 0.2 in the UVI for a wide range of cases considered. Two tests are done to show the performance of the parameterization against UVI measurements. One uses data from a high-quality spectroradiometer from Lauder, New Zealand [45.04°S, 169.684°E, 370 m above mean sea level (MSL)], where there is a low presence of aerosols. The other uses data from a Robertson–Berger-type meter from Girona, Spain (41.97°N, 2.82°E, 100 m MSL), where there is more aerosol load and where it has been possible to study the effect of aerosol information on the model versus measurement comparison. The parameterization is applied to a climatic analysis of the annual UVI variation in Catalonia, showing the contributions of solar zenith angle, ozone, and aerosols. When living in sympatry with Apodemus sylvaticus and A. flavicollis, A. alpicola dominates numerically at higher altitudes. A more efficient winter thermal isolation or a higher winter thermogenic capacity procuring a physiological advantage could explain at least part of this domination. We therefore measured body temperature (Tb), oxygen consumption (VO2), wet minimal thermal conductance (C) and non shivering thermogenesis (NST) at different ambient temperatures (Ta) on winter acclimated mice of the three species, and this for the first time in A. alpicola. NST was high and C low in the three species. No significant difference could be noticed either in Tb between 5 and -10 degrees C, in VO2 measurements at a Ta of -10 degrees C or in C. The NST measurements represent, respectively, 135.2% for A. sylvaticus, 142.8% for A. flavicollis and 140.5% for A. alpicola of the expected values, the values for A. sylvaticus being significantly lower than for the other two species. The basal metabolic rates (BMR) represent 169.4% for A. sylvaticus, 161.6% for A. flavicollis and 138.3% for A. alpicola of the expected values. Having removed the effect of body weight, the BMR value was significantly lower in A. alpicola than in A. flavicollis, but no difference could be noticed between A. sylvaticus and the other two species. In conclusion, the three species of mice have very similar acclimated thermoregulatory characteristics, well adapted to cold ambient conditions. One discriminating and advantageous factor could be the lower basal metabolic rate measured in A. alpicola compared to the other two species.

The two well-described osteolysis syndromes associated with matrix metalloproteinase-2 deficiency and mutations in the metalloproteinase-2 gene are Torg-Winchester syndrome and nodulosis-arthropathy-osteolysis variant. They are characterized by carpal-tarsal destruction, subcutaneous nodules, and generalized osteoporosis and show autosomal recessive inheritance. Herein, we report two siblings affected with a novel mutation in matrix metalloproteinase 2 gene and discuss their clinical and radiographic findings.

This study examined the validity and reliability of the French version of two observer-rated measures developed to assess cognitive errors (cognitive errors rating system [CERS]) [6] and coping action patterns (coping action patterns rating system [CAPRS]) [22,24]. The CE measures 14 cognitive errors, broken down according to their valence positive or negative (see the definitions by A.T. Beck), and the CAP measures 12 coping categories, based on an comprehensive review literature, each broken down into three levels of action (affective, behavioural, cognitive). Thirty (N = 30) subjects recruited in a community sample participated in the study. They were interviewed according to a standardized clinical protocol: these interviews were transcribed and analysed with both observer-rated systems. Results showed that the inter-rater reliability of the two measures is good and that their internal validity is satisfactory, due to a non-significant canonical correlation between CAP and CE. With regard to discriminant validity, we found a non-significant canonical correlation between CAPRS and CISS, one of most widely used self-report questionnaire measuring coping. The same can be said for the correlation with a self-report questionnaire measuring symptoms (SCL-90-R). These results confirm the absence of confounds in the assessment of cognitive errors and of coping as assessed by these observer-rated scales and add an argument in favour of the French validation of the CE-CAP rating scales. Objetivos La anemia preoperatoria es frecuente en cirugía ortopédica mayor y es un factor predictivo de transfusión y de peor evolución postoperatoria. El objetivo de este estudio es evaluar la utilidad y seguridad del hierro endovenoso como tratamiento de la anemia preoperatoria y ferropenia en cirugía ortopédica mayor, y valorar la respuesta según el hierro utilizado y/o el tipo de anemia de acuerdo con el algoritmo de actuación de nuestro programa de ahorro de sangre. Material y métodos Análisis retrospectivo de una base de datos prospectiva de los pacientes programados para prótesis total de cadera (PTC) y de rodilla (PTR), en los últimos 5 años. Utilizamos la base de datos online PAS (Programa de Ahorro de Sangre) en www.awge.org que incluye: edad, sexo, peso, ASA, metabolismo del hierro, evolución de la hemoglobina (Hb), efectos adversos y transfusión. Resultados Se incluyeron en el PAS 3.488 pacientes de los cuales 612 (17,5%) presentaban anemia preoperatoria y 169 (27,6%) fueron tratados con hierro endovenoso (HE) De los pacientes tratados con HE (169), 35 recibieron hierro sacarosa (HS) y 134 hierro carboximaltosa (HCM). De acuerdo con el algoritmo de tratamiento, 101 lo recibieron por anemia por déficit de hierro (ADH), 26 por ferropenia sin anemia (DH), 21 por anemia de proceso crónico (APC) y 21 en otros tipos de anemia. El incremento de la Hb fue de 1 ± 09 g/dl en el DH, de 1.7± 1 g/dl en ADH y de 2.1 ± 1 g/dl en APC y otras anemias (P<0.001). El grupo del HCM la adherencia al tratamiento fue superior y necesitaron menos visitas (p<0.001). Los efectos adversos fueron leves y similares en ambos grupos. Conclusiones El hierro endovenoso (HS o HCM) es un tratamiento eficaz y seguro de la anemia preoperatoria en cirugia ortopédica mayor, aunque el HCM tiene la ventaja de conseguir el mismo resultado con sólo una administración.

BACKGROUND: New ways of improving the efficacy of nicotine therapy need to be explored. We tested whether starting nicotine polacrilex gum treatment 4 weeks before the quit date improved smoking abstinence rates compared with starting treatment on the quit date. METHODS: An open randomized trial of 314 daily smokers (mean, 23.7 cigarettes/d) enrolled through the Internet and by physicians in Switzerland from November 2005 to January 2007. In the precessation treatment group, participants received nicotine polacrilex gum (4 mg, unflavored) by mail for 4 weeks before and 8 weeks after their target quit date, and they were instructed to decrease their cigarette consumption by half before quitting. In the usual care group, participants received the same nicotine gum for 8 weeks after their quit date and were instructed to quit abruptly. Instructions were limited to a booklet sent by mail and access to a smoking cessation Web site. Results are expressed as self-reported abstinence rates at the end of treatment and as biochemically verified smoking abstinence (cotinine plus carbon monoxide) after 12 months. RESULTS: Eight weeks after the target quit date, self-reported 4-week abstinence rates were 41.6% in the precessation treatment group and 44.4% in the usual care group (P = .61). One year after the target quit date, biochemically verified 4-week smoking abstinence rates were 20.8% in the precessation treatment group and 19.4% in the usual care group (P = .76). CONCLUSION: Starting nicotine gum treatment 4 weeks before the target quit date was no more effective than starting treatment on the quit date.
