Com esta investigação pretende-se compreender o perfil do empreendedor social em Portugal, tomando como referência o caso da Bolsa de Valores Sociais. Para estudar as características psicográficas e demográficas que os mentores destes projetos apresentam, a investigação adota um método quantitativo, através de um inquérito por questionário on-line. Para o tratamento e a análise dos dados foram aplicadas técnicas de análise descritiva e de redução de dados (análise fatorial por componentes principais). A investigação sugere que a par de uma componente inata, os fatores demográficos, contingenciais ao indivíduo, influenciam a criação de projetos sociais. Os dados revelam que os empreendedores sociais partilham de traços de personalidade comuns, apresentando um perfil marcado por um elevado nível de extroversão, de abertura à experiência e de conscenciosidade. A análise de dados revela uma forte presença de empreendedores do género feminino e com idades compreendidas entre os 18 e 55 anos. Verifica-se que os empreendedores sociais possuem um elevado nível de formação e em diferentes quadrantes científicos. São indivíduos que antes de se envolverem no lançamento do projeto social na maioria dos casos se encontravam empregados (em particular no setor sem fins lucrativos e no setor empresarial) e satisfeitos com a sua situação ocupacional. A maioria dos indivíduos não possui uma experiência empreendedora anterior (não tendo nem o indivíduo, nem os seus pais, criado previamente qualquer tipo de organização), embora maioritariamente possuam experiência na gestão de organizações. O desenvolvimento do projeto não é precedido por grandes mudanças na vida pessoal do empreendedor, surgindo o contacto com a questão social frequentemente anos antes da decisão de criação da iniciativa e sendo comum que o indivíduo tenha tido um envolvimento anterior em outros projetos sociais.

Cryptococcosis is a worldwide disease caused by the etiological agent Cryptococcus neoformans. It affects mainly immunocompromised humans. It is relatively rare in animals only affecting those that have received prolonged antibiotic therapy. The propolis is a resin that can present several biological properties, including antibacterial, antifungal and antiviral activities. The standard strain C. neoformans ATTC 90112 was used to the antifungal evaluation. The tests were realized with propolis ethanol extract (PEE) G12 in concentrations from 0.1 to 1.6 mg mL-1. The evaluation of MIC and MFC were done according to DUARTE (2002)5. The inhibitory effect of PEE G12 on the fungal growing was seen at the concentration of 0.2 mg mL-1 and 1.6 mg mL-1 was considered a fungicidal one.

PURPOSE: To describe the anatomy and imaging findings of the prostatic arteries (PAs) on multirow-detector pelvic computed tomographic (CT) angiography and digital subtraction angiography (DSA) before embolization for symptomatic benign prostatic hyperplasia (BPH). MATERIALS AND METHODS: In a retrospective study from May 2010 to June 2011, 75 men (150 pelvic sides) underwent pelvic CT angiography and selective pelvic DSA before PA embolization for BPH. Each pelvic side was evaluated regarding the number of independent PAs and their origin, trajectory, termination, and anastomoses with adjacent arteries. RESULTS: A total of 57% of pelvic sides (n = 86) had only one PA, and 43% (n = 64) had two independent PAs identified (mean PA diameter, 1.6 mm ± 0.3). PAs originated from the internal pudendal artery in 34.1% of pelvic sides (n = 73), from a common trunk with the superior vesical artery in 20.1% (n = 43), from the anterior common gluteal-pudendal trunk in 17.8% (n = 38), from the obturator artery in 12.6% (n = 27), and from a common trunk with rectal branches in 8.4% (n = 18). In 57% of pelvic sides (n = 86), anastomoses to adjacent arteries were documented. There were 30 pelvic sides (20%) with accessory pudendal arteries in close relationship with the PAs. No correlations were found between PA diameter and patient age, prostate volume, or prostate-specific antigen values on multivariate analysis with logistic regression. CONCLUSIONS: PAs have highly variable origins between the left and right sides and between patients, and most frequently arise from the internal pudendal artery.

CONTEXTO: A drepanocitose é uma anemia hemolítica hereditária com características pró-adesivas, pró-inflamatórias e pró-coagulantes, incluindo alterações na hemostase e activação da cascata da coagulação. PLANO DO ESTUDO: Neste estudo analisaram-se, em 140 drepanocíticos africanos e 126 indivíduos sem hemoglobina S também de origem africana, variantes genéticas polimórficas em quatro loci envolvidos na coagulação (F2 20210G>A e F5 R506Q), na fibrinólise (PAI-1 5G>4G), ou no metabolismo da homocisteína (MTHFR 677C>T). Estratificaram-se os pacientes em dois grupos de acordo com a ocorrência ou não de, pelo menos, uma complicação vaso-oclusiva (CVO) grave até à data da sua participação no estudo. RESULTADOS: Não se observou uma associação estatisticamente significativa entre a ocorrência de uma CVO grave e a herança do alelo predisponente à trombose, 4G no locus PAI-1 ou 677T no locus MTHFR. Nenhum drepanocítico apresentava os alelos F2 20210A ou F5 506Q (factor V Leiden). Visando excluir a possibilidade de que genuínas diferenças inter-grupos fossem mascaradas pela presença de indivíduos mais jovens no grupo sem-CVO, dividiu-se este num sub-grupo de pacientes mais novos e num sub-grupo de pacientes cuja idade não diferia significativamente do grupo com-CVO grave. Mesmo assim, não foi encontrada associação significativa. No entanto, pode observar-se, no grupo de doentes com o alelo PAI-1 4G (cuja expressão resulta numa diminuição da actividade fibrinolítica), uma tendência (OR=1,6) para um risco acrescido de CVO. Esta tendência era ligeiramente maior (OR=2,1) se se considerasse apenas a CVO síndrome torácica aguda. CONCLUSÕES: O alelo 4G no promotor do PAI-1 poderá ser um factor de risco para CVO na drepanocitose, uma hipótese a testar numa série maior de doentes, idealmente oriunda de uma população homogénea e com alta prevalência de drepanocitose.

Tungiasis is a common parasitic skin disease in Brazil, caused by the flea Tunga penetrans. We conducted a cross-sectional study to evaluate the occurrence of infestation with T. penetrans in primary schoolchildren in Criciúma, Santa Catarina State (South Brazil). In total, 917 children (6-10 years) of 68 schools were included in the study (11% of target population). Physical examination was followed by residential visits of those infested, to assess housing conditions and to interview the children's guardians. Of the 917, 15 (1.6%) children were infested with the parasite. The prevalence was twice as high in females (10 children - 2.2%) as compared to males (five children - 1.1%), but due to the low number of infested individuals, the difference was statistically not significant (p = 0.18). The mean number of lesions found was 1.4 per child. One child presented bacterial superinfection. Of the 15 children infested, 10 (66.7%) studied in schools built on pyritiferous soil. The families of 12 children (79.9%) had been living in the area in which the study was conducted for less than eight years and were considered underprivileged families with the head of the household having a low education status. We conclude that in Criciúma tungiasis occurs in primary school children, but prevalence and parasite load are low.

OBJECTIVE: Long-term follow-up after endovascular aneurysm repair (EVAR) is very scarce, and doubt remains regarding the durability of these procedures. We designed a retrospective cohort study to assess long-term clinical outcome and morphologic changes in patients with abdominal aortic aneurysms (AAAs) treated by EVAR using the Excluder endoprosthesis (W. L. Gore and Associates, Flagstaff, Ariz). METHODS: From 2000 to 2007, 179 patients underwent EVAR in a tertiary institution. Clinical data were retrieved from a prospective database. All patients treated with the Excluder endoprosthesis were included. Computed tomography angiography (CTA) scans were retrospectively analyzed preoperatively, at 30 days, and at the last follow-up using dedicated tridimensional reconstruction software. For patients with complications, all remaining CTAs were also analyzed. The primary end point was clinical success. Secondary end points were freedom from reintervention, sac growth, types I and III endoleak, migration, conversion to open repair, and AAA-related death or rupture. Neck dilatation, renal function, and overall survival were also analyzed. RESULTS: Included were 144 patients (88.2% men; mean age, 71.6 years). Aneurysms were ruptured in 4.9%. American Society of Anesthesiologists classification was III/IV in 61.8%. No patients were lost during a median follow-up of 5.0 years (interquartile range, 3.1-6.4; maximum, 11.2 years). Two patients died of medical complications ≤ 30 days after EVAR. The estimated primary clinical success rates at 5 and 10 years were 63.5% and 41.1%, and secondary clinical success rates were 78.3% and 58.3%, respectively. Sac growth was observed in 37 of 142 patients (26.1%). Cox regression showed type I endoleak during follow-up (hazard ratio, 3.74; P = .008), original design model (hazard ratio, 3.85; P = .001), and preoperative neck diameter (1.27 per mm increase, P = .006) were determinants of sac growth. Secondary interventions were required in 32 patients (22.5%). The estimated 10-year rate of AAA-related death or rupture was 2.1%. Overall life expectancy after AAA repair was 6.8 years. CONCLUSIONS: EVAR using the Excluder endoprosthesis provides a safe and lasting treatment for AAA, despite the need for maintained surveillance and secondary interventions. At up to 11 years, the risk of AAA-related death or postimplantation rupture is remarkably low. The incidences of postimplantation sac growth and secondary intervention were greatly reduced after the introduction of the low-permeability design in 2004.

Extended-spectrum β-lactamases (ESBL) in enterobacteria are recognized worldwide as a great hospital problem. In this study, 127 ESBL-producing Enterobacteriaceae isolated in one year from inpatients and outpatients at a public teaching hospital at São Paulo, Brazil, were submitted to analysis by PCR with specific primers for blaSHV, blaTEM and blaCTX-M genes. From the 127 isolates, 96 (75.6%) Klebsiella pneumoniae, 12 (9.3%) Escherichia coli, 8 (6.2%) Morganella morganii, 3 (2.3%) Proteus mirabilis, 2 (1.6%) Klebsiella oxytoca, 2 (1.6%) Providencia rettgeri, 2 (1.6%) Providencia stuartti, 1 (0.8%) Enterobacter aerogenes and 1 (0.8%) Enterobacter cloacae were identified as ESBL producers. BlaSHV, blaTEM and blaCTX-M were detected in 63%, 17.3% and 33.9% strains, respectively. Pulsed field gel eletrophoresis genotyping of K. pneumoniae revealed four main molecular patterns and 29 unrelated profiles. PCR results showed a high variety of ESBL groups among strains, in nine different species. The results suggest the spread of resistance genes among genetically different strains of ESBL-producing K. pneumoniae in some hospital wards, and also that some strongly related strains were identified in different hospital wards, suggesting clonal spread in the institutional environment.

Background: Rickettsia conorii is the most frequent species of RickettsiaI causing disease in Portugal. In general the disease manifests itself by fever, exanthema, headaches and the presence of an eschar. However atypical forms can be present and physicians should be aware. Aims: Analyse the atypical presentation of rickettsiosis. Material and Methods: Children admitted at the CHLC Hospital from 2000 to 2010 with atypical presentation of rickettsiosis. Clinical diagnosis was confirmed by serology and molecular techniques (PCR). Results: Five cases of children with a median age of 2 years, 1 of which female, were admitted between June and August. The diagnoses were: myositis (1), synovitis (1), cholecystitis (1), orchiepididymitis (1) and meningitis (1). Myositis developped with functional disability, CPK 9600 U/L, lower limbs’ edema, hypoalbuminemia (1,6 g/dL) and arterial hypertension. Synovitis developped with functional disability, synovial fluid increase and CRP 16,2 mg/dL. The child with cholecystitis had abdominal pain, intraabdominal fluid increase, leukopenia (1900/μL), thrombocytopenia (75000/μL) and CRP 15,3 mg/dL. Orchiepididymitis developped with testicle’s inflammatory signs, leukopenia (2900/μL), thrombocytopenia (90000/μL) and CRP 14,45 mg/dL. The patient with meningitis, who had pleocytosis (320 cells/μL), hyperproteinorrachia (284 mg/dL), hypoglicorrachia (36 mg/dL), presented only with fever and headaches. The tache noire and the classical triad were present in 3/5 cases. The clinical course was favourable in all cases. Antibodies against Rickettsia of spotted fever group were detected in 3/5 cases. In one patient Rickettsia conorii Malish strain was identified by PCR and sequencing. Conclusions: Rickettsial infection may present itself unusually. In a country of high prevalence, especially during summer months and in the presence of an inoculation eschar, it is of the uttermost importance to study the atypical presentations for a possible rickettsial infection.

Objective To examine the combined effects of physical activity and weight status on blood pressure (BP) in preschool-aged children. Study design The sample included 733 preschool-aged children (49% female). Physical activity was objectively assessed on 7 consecutive days by accelerometry. Children were categorized as sufficiently active if they met the recommendation of at least 60 minutes daily of moderate-to-vigorous physical activity (MVPA). Body mass index was used to categorize children as nonoverweight or overweight/obese, according to the International Obesity Task Force benchmarks. BP was measured using an automated BP monitor and categorized as elevated or normal using BP percentile-based cut-points for age, sex, and height. Results The prevalence of elevated systolic BP (SBP) and diastolic BP was 7.7% and 3.0%, respectively. The prevalence of overweight/obese was 32%, and about 15% of children did not accomplish the recommended 60 minutes of daily MVPA. After controlling for age and sex, overweight/obese children who did not meet the daily MVPA recommendation were 3 times more likely (OR 3.8; CI 1.6-8.6) to have elevated SBP than nonoverweight children who met the daily MVPA recommendation. Conclusions Overweight or obese preschool-aged children with insufficient levels of MVPA are at significantly greater risk for elevated SBP than their non overweight and sufficiently active counterparts. O principal objectivo deste trabalho consistiu na produção de biodiesel a partir de resíduos fornecidos pela Empresa SOCIPOLE, S.A. Esta, forneceu, pastas de gordura (depósito formado nos óleos usados), só aproveitadas pela empresa para a produção de sabões, e resíduo de glicerol proveniente da produção de biodiesel. Com as pastas de gordura, foi feito um estudo onde se variaram alguns parâmetros, tais como: o tipo de reactor, catalisador, temperatura, agitação, tempo de reacção, razão molar gordura:metanol e condições de secagem. O estudo mais pormenorizado foi efectuado com o NaOH, como catalisador. Foram feitos vários ensaios para os tempos de reacção de 30minutos, 1hora e 2horas, a 65ºC e razão molar gordura:metanol 1:6, tendo-se obtido respectivamente 86,7; 88,1 e 87,5% para o teor de ésteres. No caso da resina catiónica forte como catalisador, aparentemente não houve reacção ao fim de 12h, enquanto no ensaio que utilizou o ácido sulfúrico, o teor de ésteres foi de 82,6%, ao fim de 12h com razão molar gordura:metanol igual a 1:11. Quanto à produção de biodiesel a partir do resíduo de ácidos gordos resultante da purificação do resíduo de glicerol, foi feito um estudo preliminar usando diferentes catalisadores, uma resina catiónica forte, ácido sulfúrico e zircónia sulfatada. Este resíduo continha cerca de 60% de ácidos gordos e aproximadamente 39% de ésteres metílicos. Dos três catalisadores usados, o que permitiu obter maior teor de ésteres foi a zircónia sulfatada, tendo-se obtido 82,9% trabalhando a 60ºC durante 18horas com agitação de 230rpm e razão molar ácidos gordos:metanol de 1:12 . O ensaio em que se obteve menor percentagem de ésteres (56,4%) foi com resina catiónica. Com as duas matérias-primas obteve-se sempre um teor de ésteres, no produto final, abaixo dos 96,5% estabelecidos pela Norma Europeia EN 14103. Estes resultados reflectem provavelmente a composição complexa das matérias-primas. As pastas de gordura podem conter compostos não esterificáveis, solúveis no biodiesel, ou mais dificilmente esterificáveis nas condições operacionais usadas. Já o resíduo de ácidos gordos apresenta uma cor escura o que indica a existência de compostos corados, talvez polímeros. Portanto, para se tirar conclusões fundamentadas é necessário fazer análises mais completas das pastas de gordura e do resíduo de ácidos gordos, assim como ao biodiesel produzido de forma a identificar as possíveis contaminações existentes nos produtos com esta proveniência.

Between April 2003 and May 2009 phlebotomine sandflies were collected in Parque do Sabiá complex, Uberlândia municipality, Minas Gerais State, Brazil, using CDC and Shannon traps. The objective was to associate the sandfly species captured to the risk of the transmission of leishmaniasis in the municipality. The 126 captured specimens belonging to six species of phlebotomine, among which Lutzomyia (Psychodopygus) davisi (Root, 1934) predominated with 113 specimens (89.7%). The remaining captured species were Lutzomyia (Pintomyia) mamedei Oliveira, Afonso, Dias & Brazil, 1994 - five specimens (3.9%); Lutzomyia (Nyssomyia) flaviscutellata (Mangabeira, 1942) - four specimens (3.2%); Lutzomyia lenti (Mangabeira, 1938) - two specimens (1.6%); Brumptomyia avellari (Costa Lima, 1932) - one specimen (0.8%); and Lutzomyia (Nyssomyia) whitmani (Antunes & Coutinho, 1939) - one specimen (0.8%). The collection of species that may be involved in the transmission of Leishmania reveals the need for continuous entomological surveillance.

A terapêutica anticoagulante oral está em mudança. A varfarina e o acenocumarol constituem ainda o padrão anticoagulante oral em muitos contextos clínicos. Mas, face às limitações dos antivitamínicos K, resultantes do lento início de ação farmacológica, da janela terapêutica estreita, do metabolismo variável dependente do citocromo P450, das múltiplas interações com fármacos e com alimentos e do risco potencial de complicações hemorrágicas, nos últimos anos tem-se procurado afirmar, na terapêutica anticoagulante oral, novos grupos farmacológicos capazes de superar estes problemas. Atualmente, a investigação farmacológica centra-se no desenvolvimento de novas moléculas, não peptídicas, inibidoras de instantes fulcrais do sistema de coagulação (trombina e fator Xa), com um padrão farmacodinâmico e farmacocinético previsível e consistente, administradas por via oral. Destes compostos, três(dabigatrano, rivaroxabano e apixabano) têm já indicações terapêuticas definidas (ou a definir), assentes em largos estudos de fase III de intervenção, enquanto muitos outros compostos estão em fases menos avançadas do seu desenvolvimento. Nesta revisão, sumariamos e discutimos a farmacologia da varfarina/acenocumarol e dos novos inibidores diretos da trombina e do fator Xa, objetivando semelhanças e assinalando diferenças que ajudam a fundamentar as nossas opções terapêuticas.

A duração da diabetes mellitus é um factor de risco no aparecimento de complicações oculares. A prevalência da retinopatia diabética é praticamente nula antes dos 10 anos de idade, independentemente da duração da doença, atingindo 95% após 20-30 anos de evolução. Na Consulta de Oftalmologia Pediátrica do Hospital D. Estefânia avaliámos retrospectivamente 62 crianças com diagnóstico de diabetes meliitus tipo I, no intervalo d etempo compreendido entre 1 de Janeiro de 1999 e 31 de Junho de 2000. As idades oscilavam entre os 3 e os 17 anos(média 11,8 anos), tendo sido 29 casos do sexo feminino e 33 do sexo masculino. O período de evolução da doença variava entre 6 meses e 16 anos (média 6,2 anos). Na população estudada constatámos uma incidência de 6,4% de lesões oculares. Vericámos 1 caso (1,6%) de retinopatia de fundo, numa adolescente de 17 anos de idade e 11 anos de evoluão da doença, e 3 casos (4,8%) de catarata bilateral. Na diabetes juvenil a probabilidade de ocorrência de lesões oculares precoces é baixa. Exceptuando a catarata que pode ocorrer com alguma precocidade, a retinopatia clínica é raramente demosntrável antes do inicio da puberdade.

Toxoplasmosis is considered one of the opportunistic infections for individuals with the Acquired Immunodeficiency Syndrome (AIDS), and is also a major cause of morbidity and mortality. The aim of this study was to evaluate the prevalence of neurotoxoplasmosis, ocular toxoplasmosis and antibodies for Toxoplasma gondii in HIV-positive patients attending the SAE (Specialized Assistance Service for HIV/AIDS), as well as to associate their serological profile with epidemiological and clinical data. A total of 250 patients participated in the study from December, 2009 to November, 2010. Serological analysis was performed using the indirect immunofluorescent technique; epidemiological data were gathered by a questionnaire, and clinical history was based on the analysis of medical charts. Prevalence of seropositivity was 80%, with history of neurotoxoplasmosis in 4.8% and of ocular toxoplasmosis in 1.6% of the patients. The Highly Active Antiretroviral Treatment (HAART) was not used by 32% of the patients, 18.4% of the patients had CD4+ T- lymphocyte count less than 200 cells/mm³ and 96.8% of them were not aware of the modes of disease transmission. These findings led us to conclude that the study population is at high risk of clinical toxoplasmosis, because of both reactivation of infection in the seropositive patients who do not make a regular use of HAART, and primo-infection in seronegative patients worsened by an unawareness of the modes of infection reported in this study.
