Inorganic sulfate is one of the most abundant anions in mammalian plasma and is essential for proper cell growth and development, as well as detoxification and activation of many biological compounds. To date, little is understood how physiological levels of sulfate are maintained in the body. Our studies, and of others, have identified the NAS(i)-1 protein to be a functional sulfate transporter in the kidney and intestine, and due to this localization, constitutes a strong candidate gene for maintaining body sulfate homeostasis. Several factors, including hormones and metabolic conditions, have been shown to alter NAS(i)-1 mRNA and protein levels in vivo. In this study, we describe the transcriptional regulation of NaSi-1, with a focus on the mouse NaSi-1 gene (Nas1) that was recently cloned in our laboratory. Vitamin D (1,25-(OH)(2)D-3) and thyroid hormone (T-3) led to an increase in Nas1 promoter activity in OK cells. Mutational analysis of the Nas1 promoter resulted in identification of a direct repeat 6-type vitamin-D-responsive element (DR6 VDRE) at -525 to -508 and an imperfect inverted repeat 0-type T-3 responsive element (IRO T3RE) at -426 to -425 which conferred 1,25-(OH)(2)D-3 and T-3 responsiveness respectively. These findings suggest for vitamin D and thyroid hormone regulation of NaSi-1, may provide important clues to the physiological control of sulfate homeostasis.

Although the majority of dental abscesses in children originate from dental caries or trauma, a few are associated with unusual conditions which challenge diagnosis and management. Recent research findings have shed light on these unusual entities and greatly improved understanding of their clinical implications. These conditions include developmental abnormalities such as dens invaginatus in which there is an invagination of dental tissues into the pulp chamber and dens evaginatus in which a tubercle containing pulp is found on the external surface of a tooth crown. In addition, inherited conditions which show abnormal dentine such as dentine dysplasia, dentinogenesis imperfecta, and osteogenesis imperfecta predispose the dentition to abscess formation. Furthermore, 'spontaneous' dental abscesses are frequently encountered in familial hypophosphataemia, also known as vitamin D-resistant rickets, in which there is hypomineralization of dentine and enlargement of the pulp. In addition to developmental conditions, there are also acquired conditions which may cause unusual dental abscesses,. These include pre-eruptive intracoronal resorption which was previously known as 'pre-eruptive caries' or the 'fluoride bomb'. In addition, some undiagnosed infections associated with developing teeth are now thought to be the mandibular infected buccal cysts which originate from infection of the developing dental follicles. In the present paper, these relatively unknown entities Which cause unusual abscesses in children are reviewed with the aim of updating the general practitioner in their diagnosis and management.

Mutations in the E1alpha subunit of the pyruvate dehydrogenase multienzyme complex may result in congenital lactic acidosis, but little is known about the consequences of these mutations at the enzymatic level. Here we characterize two mutants (F205L and T231A) of human pyruvate dehydrogenase in vitro, using the enzyme expressed in Escherichia coli. Wild-type and mutant proteins were purified successfully and their kinetic parameters were measured. F205L shows impaired binding of the thiamin diphosphate cofactor, which may explain why patients carrying this mutation respond to high-dose vitamin B-1 therapy. T231A has very low activity and a greatly elevated K-m for pyruvate, and this combination of effects would be expected to result in severe lactic acidosis. The results lead to a better understanding of the consequences of these mutations on the functional and structural properties of the enzyme, which may lead to improved therapies for patients carrying these mutations.

We evaluated the effects of various food groups and micronutrients in the diet on survival among women who originally participated in a population-based case-control study of ovarian cancer conducted across 3 Australian states between 1990 and 1993. This analysis included 609 women with invasive epithelial ovarian cancer, primarily because there was negligible mortality in women with borderline tumors. The women's usual diet was assessed using a validated food frequency questionnaire. Deaths in the cohort were identified using state-based cancer registries and the Australian National Death Index (NDI). Crude 5-year survival probabilities were estimated using the Kaplan-Meier technique, and adjusted hazard ratios (HRs) and 95% confidence intervals (CIs) were obtained from Cox regression models. After adjusting for important confounding factors, a survival advantage was observed for those who reported higher intake of vegetables in general (HR = 0.75, 95% CI = 0.57-0.99, p-value trend 0.01 for the highest third, compared to the lowest third), and cruciferous vegetables in particular (HR = 0.75, 95% CI = 0.57-0.98, p-value trend 0.03), and among women in the upper third of intake of vitamin E (HR = 0.76, 95% CI = 0.58-1.01, p-value trend 0.04). Inverse associations were also seen with protein (p-value trend 0.09), red meat (p-value trend 0.06) and white meat (p-value trend 0.07), and modest positive trends (maximum 30% excess) with lactose (p-value trend 0.04), calcium and dairy products. Although much remains to be learned about the influence of nutritional factors after a diagnosis of ovarian cancer, our study suggests the possibility that a diet high in vegetable intake may help improve survival. O consumo de suco de frutas vem aumentando no Brasil. Entre 2002 e 2009 o consumo de sucos, sejam eles concentrados, em pó, sucos ou néctares, aumentou em 21%. Devido ao seu sabor agradável e doce, e ao seu valor nutricional, o suco de laranja é o suco mais comum fabricado pela indústria de processamento de bebidas. Diversos fatores podem afetar a qualidade do suco de laranja. A microbiota típica presente no suco de laranja pode ser proveniente de várias etapas de sua produção. Em relação às enzimas, a pectinametilesterase (PME) é a principal causadora de alterações em suco laranja. A pasteurização e a esterilização comercial são os métodos de conservação mais comuns utilizados para inativar enzimas e micro-organismos, porém podem causar efeitos adversos em relação às características sensoriais (cor, sabor, aroma, e outros) dos produtos. A tecnologia de ultrassom vem sendo estudada recentemente como uma forma de conservar os alimentos sem causar efeitos indesejáveis como os provocados pelos tratamentos térmicos. O objetivo deste trabalho foi avaliar a utilização da tecnologia de ultrassom e de ultrassom aliado a temperaturas brandas, como forma de conservar suco de laranja. Para isto, foram analisadas a contagem de mesófilos totais e bolores e leveduras, a atividade da pectinametilesterase, o teor de vitamina C, a cor, o pH, o teor de sólidos solúveis e a estabilidade em relação à turbidez. Ainda, avaliou-se a aceitação sensorial de suco de laranja submetido à termossonicação. Os resultados foram comparados com os obtidos para o suco natural e o suco pasteurizado. Utilizou-se um ultrassom de 40 kHz, associado às temperaturas de 25 ºC, 30 ºC, 40 ºC, 50 ºC e 60 ºC durante 10 minutos. Os tratamentos utilizando ultrassom a 50 ºC e 60 ºC foram capazes de reduzir a contagem de bolores e leveduras e de mesófilos totais, apresentando uma redução de 3 ciclos logarítmicos. Resultado similar foi encontrado quando realizado o tratamento térmico a 90 ºC por 30 segundos. Observou-se que a aplicação da termossonicação permitiu uma redução significativa na atividade de PME e uma menor perda de vitamina C. O tratamento que apresentou melhor redução na atividade de PME foi utilizando ultrassom 40 kHz com temperatura de 60 ºC. Em relação ao ácido ascórbico, quanto menor a temperatura utilizada em conjunto com a sonicação, menor foi a perda deste composto. O teor de sólidos solúveis, o pH e a cor do suco não foram alterados ao longo do processamento. Avaliando a aceitabilidade do suco, verificou-se que a cor não foi influenciada por nenhum tratamento. Em relação ao aroma, sabor e aceitação global o suco submetido a termossonicação obteve aceitação sensorial superior à encontrada para o suco pasteurizado. Concluiu-se então que a utilização da termossonicação como uma forma de conservação para suco de laranja é viável.

Lychee (Litchi chinensis Sonn.) has a high commercial value; however, it has a short shelf-life because of its rapid pericarp browning. The objective of this study was to evaluate the shelf-life of 'Bengal' lychee fruits stored after treatment with hydrochloric acid and citric acid, associated with cassava starch and plastic packaging. Uniformly red pericarp fruits were submitted to treatments: 1-(immersion in citric acid 100 mM for 5 minutes + cassava starch 30 g L-1 for 5 minutes), 2-(immersion in hydrochloric acid 1 M for 2 minutes + starch cassava 30 g L-1 for 5 minutes), 3-(immersion in citric acid 100 mM for 5 minutes + polyvinyl chloride film (PVC, 14 µm thick)) and 4-(immersion in hydrochloric acid 1 M for 2 minutes + PVC film). During 20 days, the fruits were evaluated for mass loss, pericarp color, pH, soluble solids and titratable acidity, vitamin C of the pulp and pericarp and activities of polyphenol oxidase and peroxidase of the pericarp. The treatment with hydrochloric acid associated with PVC was the most effective in maintaining the red color of the pericarp for a period of 20 days and best preservation of the fruit. The cassava starch associated with citric acid, and hydrochloric acid did not reduce the mass loss and did not prevent the browning of lychee fruit pericarp.

Nutritional surveys (food consumption, clinical and biochemichal) were conducted in a small institution for homeless children. Results showed that only 30% of the children presented adequate calorie intake. Most of the children presented adequate protein intake, but almost half consumed less than 2/3 of the calcium RDA considered necessary. Food handling, processing, and distribution also proved inadequate and wastage, high. Skinfold measurement showed up one case of obesity. Furthermore, most of the children presented clinical signs of vitamin A deficiency, mostly skin lesions; while about half presented clinical signs of riboflavin deficiency. Biochemical data showed that 63.6% had deficient plasma levels of vitamin A, none showed abnormal results for riboflavin excretion, four showed packed blood cell volume below normal, and all had normal hemoglobin levels. Stool examinations revealed a high rate of pathogenic protozoa (Hymenolepis nana), in fact, one of the highest in Brazilian literature.

INTRODUÇÃO: A "cesta básica Dieese e Procon", originada de uma Pesquisa de Padrão de Vida e Emprego no Município de São Paulo, tem sido empregada como parâmetro para o acompanhamento de preços. Seria desejável que um instrumento econômico, utilizado com esta finalidade, correspondesse também a uma nutrição efetivamente saudável. Assim, foram analisados os níveis de adequação dos itens alimentares da cesta básica em relação às necessidades nutricionais de uma família-referência paulistana, e propostas técnicas de complementação dietética para sanar possíveis deficiências ou desbalanceamento. METODOLOGIA: Utilizou-se o Censo Demográfico do IBGE, de 1991, para se determinar a família-referência; adotaram-se dois terços das "Recommended Dietary Allowances" como parâmetro de necessidades nutricionais; e utilizou-se das tabelas de composição centesimal para verificação do aporte de nutrientes da cesta básica. Efetuou-se a complementação alimentar por meio de três diferentes métodos: Ad Hoc (proposta direta e fixa), Programação Linear (proposta via computador e sazonal, com ênfase na minimização de custos), Híbrido (uma combinação dos dois anteriores). RESULTADOS: Foram encontrados valores insuficientes para as vitaminas A, C, B2 e B6 e para os minerais Ca, Mg, Fe, Zn, I e Se; o percentual de lipídios no total calórico mostrou-se elevado. Entre as técnicas de complementação, o método Híbrido pareceu assimilar, mais eficientemente, os baixos custos e os hábitos dietéticos. DISCUSSÃO: Inferiu-se uma eventual correspondência entre os problemas nutricionais detectados na relação de itens da cesta básica e a chamada transição alimentar que se processa nos países em desenvolvimento. Não parece aconselhável que os riscos epidemiológicos associados a essa alteração nos padrões dietéticos sejam incorporados num instrumento econômico que tenha por finalidade mensurar os preços de uma alimentação equilibrada.

A transtirretina (TTR) é uma proteína plasmática constituída por quatro subunidades idênticas de aproximadamente 14KDa e de massa molecular de 55 KDa (Blake et al., 1978). A TTR é responsável pelo transporte de tiroxina (T4) (Andrea et al., 1980) e retinol (vitamina A), neste último tipo de transporte através da ligação à proteina de ligação ao retinol (RBP) (Kanai et al., 1968). É sintetizada principalmente pelo fígado e secretada para o sangue (Murakami et al., 1987) e também sintetizada pelas células epiteliais do plexo coróide e secretada para o líquido cefaloraquidiano (LCR) (Aleshire et al., 1983). Existem outros locais que expressam TTR mas em menor quantidade, nomeadamente: a retina do olho (Martone et al., 1988), o pâncreas (Kato et al., 1985), o saco vitelino visceral (Soprano et al., 1986) o intestino (Loughna et al., 1995); o estômago, coração, músculo e baço (Soprano et al., 1985). A TTR é uma proteína, do ponto de vista filogenético, extremamente conservada o que já de si é um indicador da sua importância biológica (Richardson, 2009) O objectivo deste trabalho foi avaliar a expressão de transtirretina ao longo do sistema gastrointestinal do murganho, nos seguintes órgãos esófago, estômago, duodeno, cólon e também bexiga, com cerca de 3 meses de idade. O segundo objectivo foi identificar as células responsáveis por essa expressão, nos órgãos em estudo. Foi possível verificar que apenas o estômago apresenta valores de expressão normalizada de TTR diferente de zero, expressão essa muito inferior à do fígado, tal como se esperava. Por imunohistoquímica/imunofluorescência foi possível determinar que as células que expressam TTR são pouco abundantes e estão presentes na região glandular do estômago do murganho e também do humano. Para além disto, verificou-se que a TTR co-localiza com somatostatina e que as células que sintetizam TTR correspondem às células D, responsáveis pela secreção de somatostatina

O ferro é encontrado em praticamente todos os seres vivos, sendo um cofator para proteínas que desempenham funções essenciais à vida. Nos mamíferos, a maioria do ferro está incorporada na hemoglobina ou armazenado no fígado, ligado à ferritina. É absorvido pelos enterócitos, sendo a principal forma de controlo dos seus níveis. A sobrecarga de ferro pode levar a hemocromatose, podendo ser tóxica para vários órgãos. O fator de transcrição Nrf2 é importante na ativação de genes citoprotetores em situações de stress oxidativo/eletrofílico, colocando-se a hipótese de que poderá estar envolvido na resposta à progressão de doença devido à sobrecarga de ferro. Com o objetivo de determinar se a via do Nrf2 representa uma proteção contra a toxicidade do ferro a nível hepático, foram realizadas duas experiências nas quais murganhos C57BL/6 (B6) e Nrf2-/- machos foram alimentados com dieta standard ou com dieta enriquecida em ferro carbonilo (FeC) (0,5% ou 2,0%). Os resultados demonstram sobrecarga de ferro nos animais que receberam dieta enriquecida, sendo que os que receberam FeC 2,0% apresentaram níveis mais elevados de ferro hepático e sérico, bem como da saturação da transferrina. Os murganhos Nrf2-/- são mais suscetíveis a esta acumulação, mostrando evidências patológicas mais graves, nomeadamente necrose hepatocítica e infiltração de células inflamatórias. A deleção do Nrf2 associado a uma dieta suplementada com FeC 2,0% parece não ser suficiente para o desenvolvimento de fibrose hepática. O estudo da expressão de genes e proteínas do metabolismo do ferro mostrou que os animais B6 e Nrf2-/- são igualmente capazes de responder à sobrecarga de ferro, sugerindo que a sua diferente suscetibilidade à toxicidade do ferro não se deverá a uma regulação ineficiente da homeostasia do Fe. A dieta com FeC 2,0% aumentou a expressão de dois genes alvo do Nrf2, Nqo1 e Gsta1, o que não se verificou com os genes e proteínas GCLC e GCLM. A expressão de genes pró-inflamatórios não mostrou evidências de inflamação nestes animais. Foi demonstrado que os animais Nrf2-/- são mais suscetíveis à toxicidade do ferro, concluindo-se que a via do Nrf2 é ativada em resposta a uma dieta contendo quantidades excessivas de FeC e que confere proteção contra a acumulação de ferro em murganhos B6.

Gene expression of three antioxidant enzymes, Mn superoxide dismutase (MnSOD), Cu,Zn superoxide dismutase (Cu,ZnSOD), and glutathione reductase (GR) was investigated in stationary phase Saccharomyces cerevisiae during menadione-induced oxidative stress. Both GR and Cu,ZnSOD mRNA steady state levels increased, reaching a plateau at about 90 min exposure to menadione. GR mRNA induction was higher than that of Cu,ZnSOD (about 14-fold and 9-fold after 90 min, respectively). A different pattern of response was obtained for MnSOD mRNA, with a peak at about 15 min (about 8-fold higher) followed by a decrease to a plateau approximately 4-fold higher than the control value. However, these increased mRNA levels did not result in increased protein levels and activities of these enzymes. Furthermore, exposure to menadione decreased MnSOD activity to half its value, indicating that the enzyme is partially inactivated due to oxidative damage. Cu,ZnSOD protein levels were increased 2-fold, but MnSOD protein levels were unchanged after exposure to menadione in the presence of the proteolysis inhibitor phenylmethylsulfonyl fluoride. These results indicate that the rates of Cu,ZnSOD synthesis and proteolysis are increased, while the rates of MnSOD synthesis and proteolysis are unchanged by exposure to menadione. Also, the translational efficiency for both enzymes is probably decreased, since increases in protein levels when proteolysis is inhibited do not reflect the increases in mRNA levels. Our results indicate that oxidative stress modifies MnSOD, Cu,ZnSOD, and GR gene expression in a complex way, not only at the transcription level but also at the post-transcriptional, translational, and post-translational levels.

Backgroung - Bariatric surgery is indicated as the most effective treatment for morbid obesity; the Roux-en-Y gastric bypass (RYGB) is considered the procedure of choice. However, nutritional deficiency may occur in the postoperative period as a result of reduced gastric capacity and change in nutrients absorption in the gastrointestinal tract. The prescription of vitamin and mineral supplementation is a common practice after RYGB; however, it may not be sufficient to prevent micronutrient deficiencies. The aim of this study was to quantify the micronutrient intake in patients undergoing RYGB and verify if the intake of supplementation would be enough to prevent nutritional deficiencies. Methods - The study was conducted on 60 patients submitted to RYGB. Anthropometric, analytical, and nutritional intake data were assessed preoperatively and 1 and 2 years postoperatively. The dietary intake was assessed using 24-h food recall; the values of micronutrients evaluated (vitamin B12, folic acid, iron, and calcium) were compared to the dietary reference intakes (DRI). Results - There were significant differences (p < 0.05) between excess weight loss at the first and second year (69.9 ± 15.3 vs 9.6 ± 62.9 %). In the first and second year after surgery, 93.3 and 94.1 % of the patients, respectively, took the supplements as prescribed. Micronutrient deficiencies were detected in the three evaluation periods. At the first year, there was a significant reduction (p < 0.05) of B12, folic acid, and iron intake. Conclusions - Despite taking vitamin and mineral supplementation, micronutrient deficiencies are common after RYGB. In the second year after surgery, micronutrient intake remains below the DRI.

Background: The aim was to evaluate the presence of metabolic bone disease (MBD) in patients with Crohn’s disease (CD) and to identify potential etiologic factors. Methods: The case–control study included 99 patients with CD and 56 controls with a similar age and gender distribution. Both groups had dual-energy x-ray absorptionmetry and a nutritional evaluation. Single nucleotide polymorphisms at the IL1, TNF-a, LTa, and IL-6 genes were analyzed in patients only. Statistical analysis was performed using SPSS software. Results: The prevalence of MBD was significantly higher in patients (P ¼ 0.006). CD patients with osteoporosis were older (P < 0.005), small bowel involvement and surgical resections were more frequent (P < 0.005), they more often exhibited a penetrating or stricturing phenotype (P < 0.05), duration of disease over 15 years (P < 0.005), and body mass index (BMI) under 18.5 kg/m2 (P < 0.01) were more often found. No association was found with steroid use. Patients with a Z-score < 2.0 more frequently had chronic active disease (P < 0.05). With regard to diet, low vitamin K intake was more frequent (P ¼ 0.03) and intake of total, monounsaturated, and polyunsaturated fat was higher in patients with Z-score < 2.0 (P < 0.05). With respect to genetics, carriage of the polymorphic allele for LTa252 A/G was associated with a higher risk of osteoporosis (P ¼ 0.02). Regression analysis showed that age over 40 years, chronic active disease, and previous colonic resections were independently associated with the risk of developing MBD. Conclusions: The prevalence of MBD was significantly higher in CD patients. Besides the usual risk factors, we observed that factors related to chronic active and long-lasting disease increased the risk of MBD.

Objectives - Evaluate the nutritional status of patients with inactive or mildly active Crohn's disease (CD), and identify possible causes for potential deficiencies. Methods - A total of 78 CD patients and 80 healthy controls were evaluated in respect of nutritional status, dietary intake, and life styles factors. Results - These 73/78 CD patients were on immunomodulating therapies. Mean body mass index (BMI) was lower in patients as compared to controls (P= 0.006) but 32% of CD patients and 33.8% of controls had a BMI > 25, whereas 8% and 23.8% in each group, respectively, were obese (BMI > 30Kg/m(2)). Fat free mass was significantly decreased in both genders (P < 0.05) whereas fat mass was decreased only in males (P= 0.01). Energy intake was significantly lower in CD patients (P < 0.0001) and we observed significantly lower adjusted mean daily intakes of carbohydrates, monounsaturated fat, fiber, calcium, and vitamins C, D, E, and K (P < 0.05). 29% of patients had excluded grains from their usual diet, 28% milk, 18% vegetables, and 11% fruits. Milk exclusion resulted in a significantly lower consumption of calcium and vitamin K (P < 0.001) and the exclusion of vegetables was associated to a lower consumption of vitamins C and E (P < 0.05). Physical activity was significantly lower in CD patients (P= 0.01) and this lack of physical activity was inversely correlated with increased fat mass percentage (r=-0.315, P= 0.001). Conclusions - Results showed that the most prevalent form of malnutrition in CD patients was an excess of body weight, which was concomitant with an inadequate dietary intake, namely micronutrients, clearly related to dietary exclusion of certain foods.

Background - Both genetic and environmental factors affect the risk of colorectal cancer (CRC). Objective - We aimed to examine the interaction between the D1822V polymorphism of the APC gene and dietary intake in persons with CRC. Design - Persons with CRC (n = 196) and 200 healthy volunteers, matched for age and sex in a case-control study, were evaluated with respect to nutritional status and lifestyle factors and for the D1822V polymorphism. Results - No significant differences were observed in energy and macronutrient intakes. Cases had significantly (P < 0.05) lower intakes of carotenes, vitamins C and E, folate, and calcium than did controls. Fiber intake was significantly (P = 0.004) lower in cases than in controls, whereas alcohol consumption was associated with a 2-fold risk of CRC. In addition, cases were significantly (P = 0.001) more likely than were controls to be sedentary. The homozygous variant for the APC gene (VV) was found in 4.6% of cases and in 3.5% of controls. Examination of the potential interactions between diet and genotype found that a high cholesterol intake was associated with a greater risk of colorectal cancer only in noncarriers (DD) of the D1822V APC allele (odds ratio: 1.66; 95% CI: 1.00, 2.76). In contrast, high fiber and calcium intakes were more markedly associated with a lower risk of CRC in patients carrying the polymorphic allele (DV/VV) (odds ratio: 0.50; 95% CI: 0.27, 0.94 for fiber; odds ratio: 0.51; 95% CI: 0.28, 0.93 for calcium) than in those without that allele. Conclusion - These results suggest a significant interaction between the D1822V polymorphism and the dietary intakes of cholesterol, calcium, and fiber for CRC risk.
