This research investigated auditory hallucinations (AH) in a sample with chronic posttraumatic stress disorder (PTSD) and examined dissociation and thought suppression as potential associated mechanisms. In all, 40 individuals with PTSD were assessed on the hallucinations subscale of the Positive and Negative Syndrome Scale and on measures of dissociation and thought suppression. Half of the sample reported AH (n = 20, 50%). Those reporting AH had higher general and pathological dissociation scores but did not differ on thought suppression or PTSD symptom severity. Results suggest that (a) AH in chronic PTSD is not a rare phenomenon, (b) dissociation is significantly related to AH, and (c) dissociation may be a potential mediating mechanism for AH in PTSD.

We employ the time-dependent R-matrix (TDRM) method to calculate anisotropy parameters for positive and negative sidebands of selected harmonics generated by two-color two-photon above-threshold ionization of argon. We consider odd harmonics of an 800-nm field ranging from the 13th to 19th harmonic, overlapped by a fundamental 800-nm IR field. The anisotropy parameters obtained using the TDRM method are compared with those obtained using a second-order perturbation theory with a model potential approach and a soft photon approximation approach. Where available, a comparison is also made to published experimental results. All three theoretical approaches provide similar values for anisotropy parameters. The TDRM approach obtains values that are closest to published experimental values. At high photon energies, the differences between each of the theoretical methods become less significant.

Recently, a hybrid distribution function was proposed to describe a plasma species with an enhanced superthermal component. This combines a Cairns-type "nonthermal" form with the Tsallis theory for nonextensive thermodynamics. Using this alternative model, the propagation of arbitrary amplitude ion acoustic solitary waves in a two-component plasma is investigated. From a careful study of the distribution function it is found that the model itself is valid only for a very restricted range in the q-nonextensive parameter and the nonthermality parameter, a. Solitary waves, the amplitude and nature of which depend sensitively on both q and a, can exist within a narrow range of allowable Mach numbers. Both positive and negative potential structures are found, and coexistence may occur. © 2013 American Physical Society.

INTRODUCTION: Intravenous sedation is the most commonly used method of sedation for the provision of adult dental care. However, disparity exists in pre-operative fasting times in use for patients throughout the United Kingdom.

AIMS: The aims of the study were to obtain information on the effects of existing extended pre-operative fasting regimens, to canvas patient opinions on the fasting process, and to record their positive and negative experiences associated with it.

METHODS: A prospective cross-sectional descriptive study using survey methodology was conducted of adult patients attending a dental hospital for operative treatment under intravenous sedation. Sixty-four questionnaires were distributed over a four-month period, beginning 2nd October 2007.

RESULTS: The surveyed patient pool consisted of 38 females and 14 males with a mean age of 32.4 years. The response rate achieved was 81.2%. Seventy-one per cent of patients indicated that normally they consumed something for breakfast, the most common items being tea and toast. Fifty-one per cent of patients indicated that they would wish to eat the same as normal prior to their appointment and 59% wished to drink as normal. Only 19% of respondents reported that they did not wish to eat anything, with 8% preferring not to drink anything at all. Seventy-nine per cent of the patients reported that they had experienced at least one adverse symptom after fasting and 42% had experienced two or more such symptoms. In general, those patients with more experience of sedation found fasting less unpleasant than those attending for the first time (P<0.05). In addition, one-quarter of all patients indicated that the fasting process had made them feel more nervous about their sedation appointment.

CONCLUSIONS: The extended fasting regimen prior to intravenous sedation appeared to affect patients' wellbeing, as the majority reported adverse symptoms.

Doubly periodic arrays of strip conductors printed on a composite ferrite-dielectric substrate have been investigated at oblique incidence of linear polarized plane waves. The simulation results revealed strong non-reciprocity of wave reflectance and transmittance at positive and negative angles of incidence. It is also shown that the non-reciprocity is further enhanced by the strip conductor pattern.

Socioeconomic status (SES) differences in attitudes towards cancer have been implicated in the differential screening uptake and the timeliness of symptomatic presentation. However, the predominant emphasis of this work has been on cancer fatalism, and many studies focus on specific community subgroups. This study aimed to assess SES differences in positive and negative attitudes towards cancer in UK adults. A population-based sample of UK adults (n=6965, age≥50 years) completed the Awareness and Beliefs about Cancer scale, including six belief items: three positively framed (e.g. 'Cancer can often be cured') and three negatively framed (e.g. 'A cancer diagnosis is a death sentence'). SES was indexed by education. Analyses controlled for sex, ethnicity, marital status, age, self-rated health, and cancer experience. There were few education-level differences for the positive statements, and overall agreement was high (all>90%). In contrast, there were strong differences for negative statements (all Ps<0.001). Among respondents with lower education levels, 57% agreed that 'treatment is worse than cancer', 27% that cancer is 'a death sentence' and 16% 'would not want to know if I have cancer'. Among those with university education, the respective proportions were 34, 17 and 6%. Differences were not explained by cancer experience or health status. In conclusion, positive statements about cancer outcomes attract near-universal agreement. However, this optimistic perspective coexists alongside widespread fears about survival and treatment, especially among less-educated groups. Health education campaigns targeting socioeconomically disadvantaged groups might benefit from a focus on reducing negative attitudes, which is not necessarily achieved by promoting positive attitudes.

The impending and increasing threat of antimicrobial resistance has led to a greater focus into developing alternative therapies as substitutes for traditional antibiotics for the treatment of multi-drug resistant infections.1 Our group has developed a library of short, cost-effective, diphenylalanine-based peptides (X1-FF-X2) which selective eradicate (viability reduced >90% in 24 hours) the most resistant biofilm forms of a range of Gram-positive and negative pathogens including: methicillin resistant and sensitive Staphyloccoccus aureus and Staphyloccoccus epidermidis; Pseudomonas aeruginosa, Proteus mirabilis and Escherichia coli. They demonstrate a reduced cell cytotoxic profile (NCTC929 murine fibroblast) and limited haemolysis.2 Our molecules have the ability respond to subtle changes in pH, associated with bacterial infection, self-assembling to form β-sheet secondary structures and supramolecular hydrogels at low concentrations (~0.5%w/v). Conjugation of variety of aromatic-based drugs at the X1 position, including non-steroidal anti-inflammatories (NSAIDs), confer further pharmacological properties to the peptide motif enhancing their therapeutic potential. In vivo studies using waxworms (Galleria mellonella) provide promising preliminary results demonstrating the low toxicity and high antimicrobial activity of these low molecular weight gelators in animal models. This work shows biofunctional peptide-based nanomaterials hold great promise for future translation to patients as antimicrobial drug delivery and biomaterial platforms.3 [1] G. Laverty, S.P. Donor lymphocyte infusions (DLI) have been shown to enhance the graft-versus-leukaemia (GVL) effect and induce haematological and molecular remission in patients with relapsed CML following allogeneic bone marrow transplantation (BMT). The potent donor cell-mediated cytolysis following DLI may lead to a short period of aplasia before the re-establishment of donor haematopoiesis. The absence of detectable donor cells in patients prior to DLI infusion may result in permanent aplasia in certain patients. We report on four patients who relapsed 1, 3, 6.5 and 7 years post-BMT for chronic phase CML and were treated with DLI from their original BMT donor. Polymorphic short tandem repeats (STRs) were used to assess haematological chimaerism both prior to and following DLI. At the time of relapse, STR-PCR indicated the presence of donor cells in all four patients, at levels ranging from 1-40%. A clinical and molecular response was seen in 4/4 patients following a short period of cytopenia and all patients remain in clinical remission with a follow-up of 2 months-3 years post-DLI. STR-PCR indicated that a response was occurring during the period of pancytopenia when metaphase analysis was unsuccessful. Lineage-specific analysis of the cellular response to DLI was monitored using STR-PCR of peripheral blood (PB) and bone marrow (BM) lymphocyte-enriched fractions and CD2-positive and -negative T cell fractions. In one patient BM and PB CD34-positive and -negative fractions were also assessed. A change in the ratio of donor:recipient cells in the PB lymphocyte fraction was the earliest molecular indication of an anti-leukaemic response. Subsequent conversion to donor chimaerism occurred in the other lineages and the granulocyte fraction was the last lineage to convert. In conclusion, lineage-specific STR-PCR permits detailed monitoring of subtle changes in donor/recipient cell dynamics in specific lineages following DLI during the crucial pancytopenic phase and may be a useful predictor of haematological response to DLI therapy.

Analysing public sentiment about future events, such as demonstration or parades, may provide valuable information while estimating the level of disruption and disorder during these events. Social media, such as Twitter or Facebook, provides views and opinions of users related to any public topics. Consequently, sentiment analysis of social media content may be of interest to different public sector organisations, especially in the security and law enforcement sector. In this paper we present a lexicon-based approach to sentiment analysis of Twitter content. The algorithm performs normalisation of the sentiment in an effort to provide intensity of the sentiment rather than positive/negative label. Following this, we evaluate an evidence-based combining function that supports the classification process in cases when positive and negative words co-occur in a tweet. Finally, we illustrate a case study examining the relation between sentiment of twitter posts related to English Defence League and the level of disorder during the EDL related events.

BACKGROUND: Although pneumonia is a common cause of death in children in Malawi, healthcare staff frequently encounter patients or carers who refuse oxygen therapy. This qualitative study documents factors that influence acceptance or refusal of oxygen therapy for children in Malawi.

METHODS: Nine group interviews involving 86 participants were held in community and hospital settings in rural and urban Malawi. Eleven in-depth interviews of healthcare staff providing oxygen were held in a central hospital. Thematic analysis of transcripts of the audio recordings was carried out to identify recurring themes.

RESULTS: Similar ideas were identified in the group interviews and in-depth staff interviews. Past experiences of oxygen use (direct and indirect, positive and negative) had a strong influence on views of oxygen. A recurrent theme was fear of oxygen, often due to a perceived association between death and recent oxygen use. Fears were intensified by a lack of familiarity with equipment used to deliver oxygen, distrust of medical staff and concerns about cost of oxygen.

CONCLUSIONS: This study identifies reasons for refusal of oxygen therapy for children in a low-income country. Findings from the study suggest that training of healthcare staff to address fears of parents, and information, education and communication (IEC) approaches that improve public understanding of oxygen and provide positive examples of its use are likely to be helpful in improving uptake of oxygen therapy in Malawi.

Este estudo visa identificar as imagens do processo de envelhecimento e da velhice, as imagens relacionadas com a preparação de um envelhecimento bem sucedido e compreender a relação entre as mesmas. Pretende-se ainda identificar possíveis linhas de intervenção que permitam modificar imagens da velhice e da sua preparação, capazes de promover activamente um envelhecimento bem sucedido. Optou-se por um estudo de desenho de carácter transversal, descritivocorrelacional, com recurso à técnica de análise quantitativa para correlações entre diferentes variáveis sócio-demográficas. A amostra compreende 800 sujeitos com idades compreendidas entre os 13 e os 87 anos, analisadas as seguintes variáveis sociodemográficas: idade, género, escolaridade, estado civil, ter ou não filhos, se trabalhou ou não com pessoas idosas e freguesia de residência (urbana ou rural). Os instrumentos utilizados foram duas escalas, a ImAge e a ImPreVe. Os principais resultados indicam que: a) os sujeitos que manifestam imagens positivas tendem a preocupar-se com a preparação da velhice, a viver no presente mas também a planear o seu futuro; b) os sujeitos que manifestam imagens negativas da velhice e do envelhecimento também tendem a preocupar-se com a preparação da velhice, a focalizar-se no futuro; c) os sujeitos que manifestam imagens ambivalentes (positivas e negativas, onde há ganhos e perdas em simultâneo) tendem a considerar a preparação como pouco importante; d) são os sujeitos mais velhos e com menor escolaridade quem tende a manifestar imagens negativas e a valorizar a preparação da velhice. A promoção de um envelhecimento bem sucedido deve incidir nas dimensões individual, familiar, social e institucional, com vista à alteração de imagens irreais e baseadas em estereótipos ou preconceitos (sejam positivas ou negativas), ajustadas à realidade do normal processo de envelhecimento. Impõe-se uma mudança dessas imagens, desmistificando a sua conjugação com o envelhecimento patológico e promover activamente estratégias adequadas a cada perfil para a promoção de um envelhecimento bem sucedido.

Esta tese incide sobre as dinâmicas do turismo rural e as respetivas implicações em termos de desenvolvimento rural. Para tal recorreu-se à mobilização de um conjunto de conhecimentos, nomeadamente da área do turismo no espaço rural, do desenvolvimento rural e do marketing dos destinos rurais. Após uma revisão teórica minuciosa acerca de cada uma das temáticas anteriores, passámos à definição das hipóteses de investigação e do modelo de análise. As hipóteses de investigação permitiram-nos, efetivamente, testar algumas relações que considerámos, desde logo, relevantes no que diz respeito à problemática da investigação. Estas centraram-se nos promotores da oferta e na população rural. Se por um lado pretendíamos analisar a relação entre o perfil dos promotores, motivações de criação do empreendimento turístico e atividades de marketing seguidas, por outro, assumia-se como fundamental, observar as repercussões da oferta turística proporcionada junto da população local. Assumimos que esta se manifesta não só nos benefícios pessoais usufruídos, mas também a nível das perceções positivas e negativas. Após termos integrado as hipóteses de investigação, o modelo foi testado em duas regiões rurais que apresentam particularidades de regiões pobres, mas, ao mesmo tempo, um potencial turístico enorme, sendo que, inclusivamente, parte de uma dessas regiões é Património Mundial da Humanidade. Tomando em consideração esta última constatação, foram também apresentadas hipóteses complementares relativas à existência de diferenças (ao nível dos promotores e residentes) entre as regiões. Estas tarefas obrigaram à recolha de dados primários e secundários. Após a recolha da informação primária, os dados foram tratados e analisados à luz das perspetivas teóricas entretanto fornecidas. Os resultados obtidos com o estudo realizado permitiram identificar relações significativas entre o perfil do promotor, motivações de abertura do empreendimento, objetivos económicos e atividades de marketing desenvolvidas no empreendimento. Por outro lado, confirmámos ainda a existência de relações significativas entre benefícios pessoais auferidos e perceções positivas e negativas desenvolvidas pelos residentes e entre estas e a satisfação e apoio à atividade turística. Considera-se ainda que os resultados obtidos pela investigação devem ser utilizados em prol do desenvolvimento das respetivas regiões rurais. Por fim, sublinha-se o facto do modelo desenvolvido nesta investigação ser passível de aplicar a outras regiões rurais.

Apple is undoubtedly a sui generis and remarkable brand in what concerns to its branding and brand experience, and remains an indisputable reference in the consumer electronics and computer industries. The aim of this study encompasses the development and empirical testing of two conceptual models that evaluate and allow to draw a comparison between Apple clients and non-clients’ perceptions, regarding brand experience, brand perceived value, price perceptions and brand trust; and that determine the antecedents of clients’ loyalty to the brand. Therefore, the contribution of this study to the literature relies on the research of the impact of brand experience on brand perceived value and on price perceptions, as well as the influence of price perceptions (positive and negative) on trust, satisfaction, and commitment to the brand. Two surveys were used to test each conceptual model, through a sample collected across the portuguese academic community. The findings prove the strong influence of the sensory and affective aspects of brand experience on both clients and non-clients. They also reveal that the negative role of price is neutralized in the presence of strong brand experience, and that the fact that Apple is viewed as an “expensive brand” may highlight its association to higher quality and prestige. Also, this study reinforces the important role of satisfaction and commitment in building customer loyalty, corroborating the existent literature. Managerial implications derived from the findings are also discussed.
