Many older adults with hearing impairment continue to have substantial communication difficulties after being fitted with hearing aids, and many do not choose to wear hearing aids. Two group communication education programs aimed at such older people are described. The 'Keep on Talking' program has a health promotion focus, and is aimed at maintaining communication for older adults living in the community. An experimental group (n=120) attended the program, and a control group (n=130) received a communication assessment but no intervention. Significant improvements were found in the experimental participants in terms of knowledge about communication changes with age and about strategies to maintain communication skills. At the follow-up evaluation at 1 year, 45% of the experimental group, compared to 10% of the control group, had acted to improve their communication skills. The 'Active Communication Education' program focuses on the development of problem-solving strategies to improve communication in everyday life situations. Preliminary outcomes have been assessed on a small scale (n=14) to date. It is concluded that communication programs represent an important adjunct to, or supplement for, the traditional approach that focuses on hearing aid fitting.

The aim of the research project was to identify the efficacy of the family psychoeducation program as a strategy for reducing the hospital admissions of young people. It also aimed to determine if the family psychoeducation program had an impact on the experience of caregiving and knowledge and satisfaction of services provided by the mental health service. A retrospective chart audit compared readmission history of 27 clients whose families attended a psychoeducation program with readmission history of a matched group of young people whose families did not attend the program. A telephone survey was conducted for both groups of families to investigate knowledge and understanding of services and burden of care. The results indicated that family participation in a brief multiple family psychoeducation program did not reduce the number or duration of admissions of the young people. There was no impact on the level of care for families who attended the psychoeducation program, however, this group showed some evidence of increased knowledge and understanding of services as compared to the control group.

Background: There is ample evidence of important symptomatic efficacy of tumour necrosis factor alpha (TNFalpha) inhibition in ankylosing spondylitis (AS). Moreover, studies suggest that anti-TNF could be considered as the first disease controlling antirheumatic treatment (DC-ART) for AS. Objective: To determine precisely which patients with AS are most likely to benefit from anti-TNFalpha treatment because of the cost and possible long term side effects of such treatment. Methods: Assessment in Ankylosing Spondylitis (ASAS) members were asked to use a Delphi technique to name the characteristics of patients with AS for whom they would start DC-ART, in three different clinical presentations (isolated axial involvement, peripheral arthritis, enthesitis). Results: Among the 62 invited ASAS members, more than 50% actively participated in the four phases of definition according to the Delphi technique. For each of the three clinical presentations, a combination of five to six domains was proposed, with an evaluation instrument and a cut off point defining a minimum level of activity for each domain. Conclusion: This study provides a profile for a patient with AS for considering initiation of biological agents that reflects the opinion of the ASAS members, using a Delphi exercise. Further studies are required to assess their relevance and their consistency with clinical practice.

Knowledge, especially scientific and technological knowledge, grows according to knowledge trajectories and guideposts that make up the prior knowledge of an organization. We argue that these knowledge structures and their specific components lead to successful innovation. A firm's prior knowledge facilitates the absorption of new knowledge, thereby renewing a firm's systematic search, transfer and acquisition of knowledge and capabilities. In particular, the exponential growth in biotechnology is characterized by the convergence of disparate scientific and technological knowledge resources. This paper examines the shift from protein-based to DNA-based diagnostic technologies as an example, to quantify the value of a firm's prior knowledge using relative values of knowledge distance. The distance between core prior knowledge and the rate of transition from one knowledge system to another has been identified as a proxy for the value a firm's prior knowledge. The overall difficulty of transition from one technology paradigm to another is discussed. We argue this transition is possible when the knowledge distance is minimal and the transition process has a correspondingly high value of absorptive capacities. Our findings show knowledge distance is a determinant of the feasibility, continuity and capture of scientific and technological knowledge. Copyright © 2003 John Wiley & Sons, Ltd.

VCAM-1 (vascular cell adhesion molecule-1) and Sox18 are involved in vascular development. VCAM-1 is an important adhesion molecule that is expressed on endothelial cells and has a critical role in endothelial activation, inflammation, lymphatic pathophysiology, and atherogenesis. The Sry-related high mobility group box factor Sox18 has previously been implicated in endothelial pathologies. Mutations in human and mouse Sox18 leads to hypotrichosis and lymphedema. Furthermore, both Sox18 and VCAM-1 have very similar spatio-temporal patterns of expression, which is suggestive of crosstalk. We use biochemical techniques, cell culture systems, and the ragged opossum (RaOP) mouse model with a naturally occurring mutation in Sox18 to demonstrate that VCAM-1 is an important target of Sox18. Transfection, site-specific mutagenesis, and gel shift analyses demonstrated that Sox18 directly targeted and trans-activated VCAM-1 expression. Importantly, the naturally occurring Sox18 mutant attenuates the expression and activation of VCAM-1 in vitro. Furthermore, in vivo quantitation of VCAM-1 mRNA levels in wild type and RaOP mice demonstrates that RaOP animals show a dramatic and significant reduction in VCAM-1 mRNA expression in lung, skin, and skeletal muscle. Our observation that the VCAM-1 gene is an important target of SOX18 provides the first molecular insights into the vascular abnormalities in the mouse mutant ragged and the human hypotrichosis-lymphedematelangiectasia disorder.

Background: A knowledge of energy expenditure in infancy is required for the estimation of recommended daily amounts of food energy, for designing artificial infant feeds, and as a reference standard for studies of energy metabolism in disease states. Objectives: The objectives of this study were to construct centile reference charts for total energy expenditure (TEE) in infants across the first year of life. Methods: Repeated measures of TEE using the doubly labeled water technique were made in 162 infants at 1.5, 3, 6, 9 and 12 months. In total, 322 TEE measurements were obtained. The LMS method with maximum penalized likelihood was used to construct the centile reference charts. Centiles were constructed for TEE expressed as MJ/day and also expressed relative to body weight (BW) and fat-free mass (FFM). Results: TEE increased with age and was 1.40,1.86, 2.64, 3.07 and 3.65 MJ/day at 1.5, 3, 6, 9 and 12 months, respectively. The standard deviations were 0.43, 0.47, 0.52, 0.66 and 0.88, respectively. TEE in MJ/kg increased from 0.29 to 0.36 and in MJ/day/kg FFM from 0.36 to 0.48. Conclusions: We have presented centile reference charts for TEE expressed as MJ/day and expressed relative to BW and FFM in infants across the first year of life. There was a wide variation or biological scatter in TEE values seen at all ages. We suggest that these centile charts may be used to assess and possibly quantify abnormal energy metabolism in disease states in infants.

This paper delineates the development of a prototype hybrid knowledge-based system for the optimum design of liquid retaining structures by coupling the blackboard architecture, an expert system shell VISUAL RULE STUDIO and genetic algorithm (GA). Through custom-built interactive graphical user interfaces under a user-friendly environment, the user is directed throughout the design process, which includes preliminary design, load specification, model generation, finite element analysis, code compliance checking, and member sizing optimization. For structural optimization, GA is applied to the minimum cost design of structural systems with discrete reinforced concrete sections. The design of a typical example of the liquid retaining structure is illustrated. The results demonstrate extraordinarily converging speed as near-optimal solutions are acquired after merely exploration of a small portion of the search space. This system can act as a consultant to assist novice designers in the design of liquid retaining structures.

This paper describes a coupled knowledge-based system (KBS) for the design of liquid-retaining structures, which can handle both the symbolic knowledge processing based on engineering heuristics in the preliminary synthesis stage and the extensive numerical crunching involved in the detailed analysis stage. The prototype system is developed by employing blackboard architecture and a commercial shell VISUAL RULE STUDIO. Its present scope covers design of three types of liquid-retaining structures, namely, a rectangular shape with one compartment, a rectangular shape with two compartments and a circular shape. Through custom-built interactive graphical user interfaces, the user is directed throughout the design process, which includes preliminary design, load specification, model generation, finite element analysis, code compliance checking and member sizing optimization. It is also integrated with various relational databases that provide the system with sectional properties, moment and shear coefficients and final member details. This system can act as a consultant to assist novice designers in the design of liquid-retaining structures with increase in efficiency and optimization of design output and automated record keeping. Industrial relations research that attempts to grapple with individuals' union-related sentiments and activities often draws on one of two traditions of psychological research—the individual-level factors tradition (for example, personality and attitude-behaviour relations) and the social context tradition (for example, frustration-aggression and relative deprivation). This paper provides an overview of research conducted from within these traditions to explain union-related phenomena and identifies some of the limitations that arise as a consequence of a shared tendency to treat people in an atomistic fashion. The paper argues for an understanding of the psychological processes that underpin group-based action. To this end, it elaborates a theoretical framework based on social identity theory and self-categorisation theory that would allow us to examine the dynamic interplay between the individual, their cognitions and their environment. The paper concludes with a brief discussion of a specific case of union mobilisation, to indicate how this theoretical framework might aid empirical analysis.

Inagaki and Hatano (2002) have argued that young children initially understand biological phenomena in terms of vitalism, a mode of construal in which life or life-force is the central causal-explanatory concept. This study investigated the development of vitalistic reasoning in young children's concepts of life, the human body and death. Sixty preschool children between the ages of 3 years, 7 months and 5 years, 11 months participated. All children were initially given structured interviews to assess their knowledge of (1) human body function and (2) death. From this sample 40 children in the Training group were taught about the human body and how it functions to maintain life. The Control group (n = 20) received no training. All 60 children were subsequently reassessed on their knowledge of human body function and death. Results from the initial interviews indicated that young children who spontaneously appealed to vitalistic concepts in reasoning about human body functioning were also more sophisticated in their understanding of death. Results from the posttraining interviews showed that children readily learned to adopt a vitalistic approach to human body functioning, and that this learning coincided with significant development in their understanding of human body function, and of death. The overall pattern of results supports the claim that the acquisition of a vitalistic causal-explanatory framework serves to structure children's concepts and facilitates learning in the domain of biology. O estudo em quest??o visa apresentar um conjunto de crit??rios representantes do ciclo de desenvolvimento do conhecimento organizacional e, mediante an??lise comparada entre sete escolas de governo internacionais e uma nacional, a Funda????o Escola Nacional de Administra????o P??blica (Enap), elaborar o desenho de uma estrutura organizacional que maximize o desenvolvimento do conhecimento organizacional em escolas de governo.

Este estudo analisa os processos de circulação e apropriação das representações sobre os saberes da educação escolarizada difundidos pela Revista de Educação do Espírito Santo, entre os anos de 1934 e 1937. Como referencial teórico baseamo- nos em Chartier (1990) acerca do conceito de representação, em Balandier (1982) em relação ao conceito de encenação de poder instituído que assume visibilidade quando circula na Revista, concedendo publicização aos feitos políticos realizados por dado grupo social, e em Julia (2001), junto ao conceito de cultura escolar. A partir desse arcabouço teórico, empreendemos metodologia de pesquisa a partir da análise histórica da fonte, dialogando, para tanto, com diferentes documentos e registros que configuram uma série de dados que constituem nossa fonte. Trata-se da análise das representações travadas no debate sobre a formação, divulgação e apropriação do conjunto de práticas e saberes pedagógicos dirigido aos professores, por parte de um grupo de intelectuais locais que se apresentava como portador do projeto de modernização do Espírito Santo inserido no contexto nacional. A Revista de Educação/ES tinha entre seus principais objetivos o de (in)formar os professores, ou seja, enquadrar suas práticas às novas demandas educacionais. Deste modo, a Revista esteve atrelada a um projeto educacional em que os intelectuais corroboravam a ampla circulação de um conjunto de representações sobre a modernidade, utilizando a Revista como suporte, visto que, desde as capas, são expostos “monumentos de modernização”, como os prédios escolares e todo um complexo arranjo de artefatos simbólicos, traduzidos muitas vezes em festas e rituais escolares que evocavam “um novo tempo para a Educação do Estado”, ou seja, fazendo da educação escolarizada um espetáculo.

Este trabalho é resultado da investigação que teve como objetivo analisar o processo de construção dos Projetos Políticos Pedagógicos dos cursos do Proeja no contexto do Ifes campus Vitória. O problema de pesquisa buscou captar os movimentos e as experiências desencadeados nesse processo. No percurso metodológico, com ênfase na pesquisa qualitativa, foi necessário entrelaçar duas abordagens: a etnografia escolar e a pesquisa-ação em função da atuação profissional da pesquisadora no lócus de estudo. Variados instrumentos foram utilizados para levantamento e a produção dos dados, dentre os quais: questionários, entrevistas, diário de campo das observações, pesquisa bibliográfica e documental. Participaram da pesquisa aproximadamente 80 pessoas, entre docentes, alunos e gestores, abordados em contextos específicos: o grupo de formação continuada, a comissão dos projetos, o encontros dos alunos, a reunião intermediária dentre outros. O referencial teórico-metodológico pautado na perspectiva do materialismo histórico dialético embasou toda a trajetória investigativa, em coerência com a base da produção das pesquisas sobre trabalho e educação e por se constituir a referência que fundamenta os princípios estruturantes do currículo integrado na perspectiva da formação humana. Por meio da metáfora dos observatórios, focamos nossas lentes sobre as questões que desafiaram a construção dos projetos políticos pedagógicos e sua coerência com os princípios epistemológicos, políticos e pedagógicos do Proeja. Nesse movimento, diversos olhares foram captados, possibilitando-nos levantar os seguintes resultados: o percurso de construção dos projetos foi marcado por contradições que perpassam todo o processo e que constituíram um debate profícuo que tenciona a gestão pedagógica, administrativa e financeira do Ifes campus Vitória. O movimento se constituiu também em um processo de construção, partilha de saberes e experiências, impulsionado pela busca da apreensão dos sentidos da integração, que contraditoriamente não alcançou seus objetivos, embora não se possa negar os resultados positivos do processo no interior da Instituição. Dessa forma, os desafios para efetivação da formação integrada no Ifes persistem. Ganha centralidade nessa discussão os sujeitos a quem o programa se volta e suas demandas de formação, bem como as condições materiais de oferta dos cursos e de forma especial as condições de envolvimento dos professores com o programa e as reflexões sobre suas práticas.
