Hallux rígidus (HR) affects the first metatarsophalangeal joint (MTPJ) between 35% and 60% of the population over 65 years and there are multiple ways of treatment. Depending on the radiological stage where you find the deformity determines the procedure to be performed; in the early stages cheilectomy techniques and corrective osteotomy is performed while the more advanced ratings, the surgeon chooses destructive techniques considered as arthrodesis and arthroplasty. This final of degree project aims to focus on 1 MTPJ destructive techniques to clarify which of the procedures generates better results by a number of parameters; outcomes of the American Orthopaedic Foot scale and Ankle Society Hallux metatarsophalangeal Interphalangeal-scale (AOFAS), range of motion (ROM) of the 1ºAMTF, radiological classification. As for the implant arthroplasty technique, this article offers information on material and design that generates better relating to patient characteristics such as age, inflammatory joint diseases, viability and durability of the implant results. The conclusion from this review is that the values obtained in the arthrodesis according AOFAS decrease due to loss of mobility, but both techniques have similar values of effectiveness and concludes with the decision that the technique used is determined taking into account various factors and patient characteristics. Ce rapport est en accord avec de nombreux travaux sur l'enseignement des sciences qui insistent sur l'importance de partir des connaissances de l'enfant. Le projet d'apprentissage des sciences au primaire qui est le plus élaboré et qui part des idées des enfants est néo-zélandais (Harlen, 1988). Dans ce projet, les guides du maître ont été réalisés après une période de recherche sur les problèmes de l'enseignement et de l'apprentissage des sciences dans les classes primaires. Les impératifs mis de l'avant par ce projet étaient de préparer des activités en classe correspondant davantage au questionnement de l'enfant; de permettre à l'enfant de poser ses propres questions et de mener ses propres recherches; enfin, de ne pas laisser l'enfant seul pour tirer des conclusions et de ne pas l'obliger à accepter des conclusions non rattachées à son expérience personnelle. L'enseignement des sciences doit parvenir à éveiller l'élève, à le rendre plus mûr affectivement et intellectuellement en développant son esprit critique. Cet enseignement doit amener un savoir réinvestissable dans la vie quotidienne, car c'est cette absence de culture scientifique, qui aggrave le sentiment d'impuissance, que l'enfant éprouve une fois adulte devant la situation du monde qui l'environne et qui limite ses capacités d'assimilation et d'adaptation (Astolfi et al. 1978). Cet enseignement doit aussi faire naître chez l'élève le désir d'explorer, d'expérimenter et de laisser libre cours à sa créativité personnelle par le développement d'attitudes. Ces considérations générales nous ont conduit à élaborer (figure 1) dans un premier chapitre une problématique où nous tracerons les grandes lignes des problèmes posés par notre sujet de recherche et la méthodologie retenue. L'apprentissage pose des difficultés tant au niveau de l'élève que de l'enseignant et du savoir. Nous tenterons de cerner ces difficultés et de les expliquer dans un second chapitre. Un troisième chapitre nous amènera à développer brièvement deux approches de la créativité et à définir celle que nous choisissons et pourquoi. Une fois les problèmes circonscris par rapport à la réalité existant dans l'apprentissage scolaire, le dernier chapitre traitera de notre modèle théorique d'intervention didactique où la créativité intervient au niveau de l'objet d'apprentissage, de l'élève et de l'enseignant.

We tested the prediction that, if hoverflies are Batesian mimics, this may extend to behavioral mimicry such that their numerical abundance at each hour of the day (the daily activity pattern) is related to the numbers of their hymenopteran models. After accounting for site, season, microclimatic responses and for general hoverfly abundance at three sites in north-west England, the residual numbers of mimics were significantly correlated positively with their models 9 times out of 17, while 16 out of 17 relationships were positive, itself a highly significant non-random pattern. Several eristaline flies showed significant relationships with honeybees even though some of them mimic wasps or bumblebees, perhaps reflecting an ancestral resemblance to honeybees. There was no evidence that good and poor mimics differed in their daily activity pattern relationships with models. However, the common mimics showed significant activity pattern relationships with their models, but the rarer mimics did not. We conclude that many hoverflies show behavioral mimicry of their hymenopteran models.

Microbial inhabitants of soils are important to ecosystem and planetary functions, yet there are large gaps in our knowledge of their diversity and ecology. The ‘Biomes of Australian Soil Environments’ (BASE) project has generated a database of microbial diversity with associated metadata across extensive environmental gradients at continental scale. As the characterisation of microbes rapidly expands, the BASE database provides an evolving platform for interrogating and integrating microbial diversity and function.

Purpose: Individuals with generalized joint hypermobility (GJH) are reported, in the clinical setting, to be at greater risk of developing musculoskeletal related joint pain, joint dislocations and tendinopathies. It is hypothesized that impaired static and dynamic neuromuscular movement control in those with GJH is responsible for contributing to an increased risk of injury and subsequent knee osteoarthritis (OA). Yet, to date, it remains unproven if there is an association between GJH and knee OA.

Purpose: affecting approximately one in four adults over the age of 40 years in the UK, knee pain is the most common presenting feature of osteoarthritis (OA). Despite the plethora of studies that have investigated the factors associated with the onset of knee pain in the sedentary population, relatively little is known about the prevalence and occupational factors associated with knee pain in an athletic sporting population. This study aimed to determine in Great Britain’s (GB) Olympians, aged 40 years and older, (1) the individual factors.

Objectives: We report the unusual case of a patient with a thyrotropinoma, discovered after a hemithyroidectomy for a suspicious thyroid nodule, and its therapeutic challenges. Materials and methods: In a patient who underwent hemithyroidectomy for cold thyroid nodule, hyperthyroid symptoms persisted, despite stopping levothyroxine treatment. Further investigation was carried out through the following laboratory tests: thyroid-stimulating hormone (TSH) test; free thyroxine (fT4) test; and the thyrotropin releasing hormone (TRH) test. A pituitary magnetic resonance imaging (MRI) scan and genetic analysis was also carried out. The test results confirmed the diagnosis of a thyrotropinoma. Results: Treatment with long-acting somatostatin analogues normalised thyroid hormones and symptoms of hyperthyroidism. Conclusion: The diagnostic approach to the thyroid nodule should include a detailed clinical and biochemical examination. Initial biochemical evaluation by TSH alone does not allow detecting inappropriate TSH secretion that may increase the risk of thyroid malignancy. In case of a thyrotropinoma, the ideal treatment consists of combined care of central and peripheral thyroid disease.

Following its inception in 1994, the certification of European Registered Toxicologists (ERT) by EUROTOX has been recognized as ensuring professional competence as well as scientific integrity and credibility. Criteria and procedures for registration are contained in the ERT "Guidelines for Registration 2012". The register of ERT currently has over 1900 members. In order to continue the harmonisation of requirements and processes between national registering bodies as a prerequisite for official recognition of the ERT title as a standard, and to take account of recent developments in toxicology, an update of the ERT Guidelines has been prepared in a series of workshops by the EUROTOX subcommittees for education and registration, in consultation with representatives of national toxicology societies and registers. The update includes details of topics and learning outcomes for theoretical training, and how these can be assessed. The importance of continuing professional development as the cornerstone of re-registration is emphasised. To help with the process of harmonisation, it is necessary to collate and share best practices of registration conditions and procedures across Europe. Importantly, this information can also be used to audit compliance with the EUROTOX standards. As recognition of professionals in toxicology, including specialist qualifications, is becoming more important than ever, we believe that this can best be achieved based on the steps for harmonisation outlined here together with the proposed new Guidelines.
