Drug resistance is one of the principal obstacles blocking worldwide malaria control. In Colombia, malaria remains a major public health concern and drug-resistant parasites have been reported. In vitro drug susceptibility assays are a useful tool for monitoring the emergence and spread of drug-resistant Plasmodium falciparum. The present study was conducted as a proof of concept for an antimalarial drug resistance surveillance network based on in vitro susceptibility testing in Colombia. Sentinel laboratories were set up in three malaria endemic areas. The enzyme linked immunosorbent assay-histidine rich protein 2 and schizont maturation methods were used to assess the susceptibility of fresh P. falciparum isolates to six antimalarial drugs. This study demonstrates that an antimalarial drug resistance surveillance network based on in vitro methods is feasible in the field with the participation of a research institute, local health institutions and universities. It could also serve as a model for a regional surveillance network. Preliminary susceptibility results showed widespread chloroquine resistance, which was consistent with previous reports for the Pacific region. However, high susceptibility to dihydroartemisinin and lumefantrine compounds, currently used for treatment in the country, was also reported. The implementation process identified critical points and opportunities for the improvement of network sustainability strategies.

Here we present a comprehensive review of the literature on the vectorial importance of the major Anopheles malaria vectors in Colombia. We provide basic information on the geographical distribution, altitudinal range, immature habitats, adult behaviour, feeding preferences and anthropophily, endophily and infectivity rates. We additionally review information on the life cycle, longevity and population fluctuation of Colombian Anopheles species. Emphasis was placed on the primary vectors that have been epidemiologically incriminated in malaria transmission: Anopheles darlingi, Anopheles albimanus and Anopheles nuneztovari. The role of a selection of local, regional or secondary vectors (e.g., Anopheles pseudopunctipennis and Anopheles neivai) is also discussed. We highlight the importance of combining biological, morphological and molecular data for the correct taxonomical determination of a given species, particularly for members of the species complexes. We likewise emphasise the importance of studying the bionomics of primary and secondary vectors along with an examination of the local conditions affecting the transmission of malaria. The presence and spread of the major vectors and the emergence of secondary species capable of transmitting human Plasmodia are of great interest. When selecting control measures, the anopheline diversity in the region must be considered. Variation in macroclimate conditions over a species' geographical range must be well understood and targeted to plan effective control measures based on the population dynamics of the local Anopheles species.

La operación de Collis-Nissen se propone como técnica de elección en pacientes con acortamiento esofágico. Se ha realizado un estudio prospectivo en 32 pacientes intervenidos consecutivamente de Collis-Nissen vía laparoscópica o abierta, con una edad media de 70 años y que presentaban grandes hernias hiatales o recidivadas. Se realizó seguimiento clínico, radiológico, endoscópico y de calidad de vida con cuestionarios validados, durante los primeros 12 meses postoperatorios. Los resultados obtenidos muestran un buen control de síntomas, así como mejora de la calidad de vida, manteniendo una aceptable recidiva anatómica del 12,5% a corto plazo, sin que ello afecte la morbi-mortalidad.

Malaria in La Guajira, the most northern state of Colombia, shows two different epidemiological patterns. Malaria is endemic in the municipality of Dibulla whereas in Riohacha it is characterised by sporadic outbreaks. This study aimed to establish whether differences in transmission patterns could be attributed to different vector species. The most abundant adult female species were Anopheles aquasalis, exclusive to Riohacha, and Anopheles darlingi, restricted to Dibulla. Anopheles mosquitoes were identified using morphology and the molecular markers internal transcribed spacer 2 and cytochrome c oxidase I. All specimens (n = 1,393) were tested by ELISA to determine natural infection rates with Plasmodium falciparum and Plasmodium vivax. An. darlingi was positive for P. vivax 210, with an infection rate of 0.355% and an entomological inoculation rate of 15.87 infective bites/person/year. Anopheles albimanus larvae were the most common species in Riohacha, found in temporary swamps; in contrast, in Dibulla An. darlingi were detected mainly in permanent streams. Distinctive species composition and larval habitats in each municipality may explain the differences in Plasmodium transmission and suggest different local strategies should be used for vector control.

Aims: To evaluate the impact on glycemic control and quality of life of a bolus calculator. Methods: Multicentre randomized prospective crosssectional study. Patients were randomized to control phase (3 months; calculation of prandial insulin according to insulinto-carbohydrate ratio and insulin sensitivity factor using a single strip meter) or intervention phase (3 months; calculation of prandial insulin with a bolus advisor), with a washout period (3 months). Patients wore a continuous glucosensor (7 days) and answered a quality of life questionnaire at the beginning and at the end of each phase. A questionnaire of satisfaction was obtained at the end of both phases. Inclusion criteria: Adults; T1DM> 1 year, HbA1c > 7.5%, basal-bolus therapy with insulin analogs, experience with carbohydrate Results: Data from the first 32 subjects with at least 1 ended phase (27 females, age 38 â 11 years, diabetes duration 16.8 â 7.5 years). Basal characteristics were comparable independently of the starting phase. No differences were found between phases in terms of mean blood glucose, standard deviation (from meter neither from sensor) and satisfaction. Conclusions: The use of a bolus calculator improves glycemic control and quality of life of T1DM subjects.

Gold-mining may play an important role in the maintenance of malaria worldwide. Gold-mining, mostly illegal, has significantly expanded in Colombia during the last decade in areas with limited health care and disease prevention. We report a descriptive study that was carried out to determine the malaria prevalence in gold-mining areas of Colombia, using data from the public health surveillance system (National Health Institute) during the period 2010-2013. Gold-mining was more prevalent in the departments of Antioquia, Córdoba, Bolívar, Chocó, Nariño, Cauca, and Valle, which contributed 89.3% (270,753 cases) of the national malaria incidence from 2010-2013 and 31.6% of malaria cases were from mining areas. Mining regions, such as El Bagre, Zaragoza, and Segovia, in Antioquia, Puerto Libertador and Montelíbano, in Córdoba, and Buenaventura, in Valle del Cauca, were the most endemic areas. The annual parasite index (API) correlated with gold production (R2 0.82, p < 0.0001); for every 100 kg of gold produced, the API increased by 0.54 cases per 1,000 inhabitants. Lack of malaria control activities, together with high migration and proliferation of mosquito breeding sites, contribute to malaria in gold-mining regions. Specific control activities must be introduced to control this significant source of malaria in Colombia.

BACKGROUND Health sector reforms taking place in Colombia during the Nineties included policies to promote social participation in the health system, which is considered essential to its functioning. The aim of this article is to analyse the meaning and the significance of participation in health for the different social actors involved in implementing policies in Colombia. METHODS A qualitative, descriptive, exploratory study was carried out using focal groups (FG) and semi-structured individual interviews (I) of the different social actors: 210 users (FG), 40 community leaders (FG), 3 policy makers (E) and 36 healthcare professionals (E). A carried out analysis was content up of the contents. The study area corresponded to the municipalities of Tulua and Palmira in Colombia. RESULTS The concept of participation was interpreted differently depending on the actor studied: for users and leaders the concept referred to contributing ideas, presence in social spaces, solidarity and frequently, and use of the health services. Healthcare professionals considered the activities carried out by institutions together with the community as social participation, the use of services and affiliation to the health system. Policy markers considered participation to concern evaluation and control of the health services by the community, to improve its quality. CONCLUSIONS The different concepts of participation reveal dif ferences between the content of the policy and how it is understood and interpreted by the different social actors in their interaction with the health services. These different perspectives must be taken into account to develop a link between society and the health services.

The Aedes aegypti vector for dengue virus (DENV) has been reported in urban and periurban areas. The information about DENV circulation in mosquitoes in Colombian rural areas is limited, so we aimed to evaluate the presence of DENV in Ae. aegypti females caught in rural locations of two Colombian municipalities, Anapoima and La Mesa. Mosquitoes from 497 rural households in 44 different rural settlements were collected. Pools of about 20 Ae. aegypti females were processed for DENV serotype detection. DENV in mosquitoes was detected in 74% of the analysed settlements with a pool positivity rate of 62%. The estimated individual mosquito infection rate was 4.12% and the minimum infection rate was 33.3/1,000 mosquitoes. All four serotypes were detected; the most frequent being DENV-2 (50%) and DENV-1 (35%). Two-three serotypes were detected simultaneously in separate pools. This is the first report on the co-occurrence of natural DENV infection of mosquitoes in Colombian rural areas. The findings are important for understanding dengue transmission and planning control strategies. A potential latent virus reservoir in rural areas could spill over to urban areas during population movements. Detecting DENV in wild-caught adult mosquitoes should be included in the development of dengue epidemic forecasting models.

BACKGROUND Health-related quality of life (HRQoL) is gaining importance as a valuable outcome measure in oral cancer area. The aim of this study was to assess the general and oral HRQoL of oral and oropharyngeal cancer patients 6 or more months after treatment and compare them with a population free from this disease. METHODS A cross-sectional study was carried out with patients treated for oral cancer at least 6 months post-treatment and a gender and age group matched control group. HRQoL was measured with the 12-Item Short Form Health Survey (SF-12); oral HRQoL (OHRQoL) was evaluated using the Oral Health Impact Profile (OHIP-14) and the Oral Impacts on Daily Performances (OIDP). Multivariable regression models assessed the association between the outcomes (SF-12, OHIP-14 and OIDP) and the exposure (patients versus controls), adjusting for sex, age, social class, functional tooth units and presence of illness. RESULTS For patients (nâ=â142) and controls (nâ=â142), 64.1% were males. The mean age was 65.2 (standard deviation (sd): 12.9) years in patients and 67.5 (sd: 13.7) years in controls. Patients had worse SF-12 Physical Component Summary scores than controls even in fully the adjusted model [β-coefficientâ=â-0.11 (95% CI: -5.12-(-0.16)]. The differences in SF-12 Mental Component Summary were not statistically significant. Regarding OHRQoL patients had 11.63 (95% CI: 6.77-20.01) higher odds for the OHIP-14 and 21.26 (95% CI: 11.54-39.13) higher odds for OIDP of being in a worse category of OHRQoL compared to controls in the fully adjusted model. CONCLUSION At least 6 months after treatment, oral cancer patients had worse OHRQoL, worse physical HRQoL and similar psychological HRQoL than the general population.

Este documento es la plasmación de una propuesta ambiciosa e innovadora para la mejora en la eficiencia y en el control de la calidad de un departamento hospitalario de oftalmología. La intención del texto es resumir el funcionamiento de un modelo nuevo de asistencia oftalmológica que, manteniendo o incluso mejorando la calidad de la asistencia, sea capaz de un ahorro en personal muy significativo. La propuesta se basa en la aplicación de la gestión clínica de la siguiente forma: ⢠Motivación del personal asistencial, con énfasis de los médicos especialistas ⢠Protocolización de la asistencia médica y quirúrgica ⢠Diferenciación por niveles de complejidad ⢠Reasignación de roles y competencias ⢠Formación del personal asociado y control de la formación continuada ⢠Control de la actividad, la eficiencia y la calidad mediante indicadores El documento explica como se ha calculado la necesidad de recursos humanos y como se establece la comparación con un departamento en funcionamiento. De la misma forma se ofrece el ahorro en horas de personal que después puede extenderse a unidades monetarias. El documento incorpora la propuesta de procedimientos, planes de formación del personal, de formación continuada, de calidad y de implementación. También la estructura física básica para llevarlo a cabo. No me cabe duda de que lo convincente de la propuesta, por audaz que pueda parecer, resultará del agrado de las gerencias y direcciones generales por generar un ahorro muy significativo, por la necesidad imperante de implantar medidas que incrementen la productividad y la competitividad y por tratarse de un plan que es factible. El trabajo presentado en esta comunicación se enmarca en un proyecto financiado por la Secretaría de Estado de Universidades, Ministerio de Educación Cultura y Deporte en su Programa de Estudios y Análisis, mejora de la calidad de la enseñanza superior y de la actividad del profesorado universitario en el Eje de Nuevos avances y determinación del impacto de las misiones universitarias, Modalidad de Trabajos de análisis, lideradopor la Universitat Pompeu Fabra y en el que participan docentes de las universidades de Valladolid, Cádiz, Las Palmas de Gran Canaria, Sevilla, Autónoma de Barcelona, Carlos III de Madrid y Universidat Oberta de Catalunya.En el proyecto se ha realizado un trabajo de análisis de las prácticas programadas en las distintas universidades para velar por la calidad de los trabajos fin de grado, actualmente proyectos fin carrera. Se han recopilado diferentes procedimientos de evaluación llevados a cabo en diferentes Universidades que van desde plantillas de verificación para los tribunales de los proyectos a guías de procedimiento completas que verifican elestablecimiento y cumplimiento de hitos intermedios.En esta comunicación se muestra la aplicación de algunas de las prácticas propuestas en el caso real de proyectos fin de carrera en la titulación de Ingeniería Técnica en Diseño Industrial de la Escuela de Ingenierías Industriales. Se pone de manifiesto la utilidad del uso de dichas prácticas frente a enfoques más clásicos o simplistas.

El treball que presentem a continuació pretén oferir una perspectiva general sobre les investigacions realitzades entorn a la qualitat televisiva per tal d'extreure una sèrie de criteris que es puguin aplicar en una anàlisi d'informatius de televisió. Per a això, presentem una panoràmica general de les investigacions que s'han dut a terme sobre qualitat televisiva i destaquem les principals valoracions del concepte "qualitat". D'aquesta manera, aplicant els criteris estudiats a la mostra seleccionada, podem extreure unes conclusions més precises sobre l'anàlisi de qualitat en els noticiaris
