A dermatoscopia constitui uma técnica de diagnóstico não-invasiva, in vivo, que permite complementar a observação clínica de lesões cutâneas não-pigmentadas de etiologia diversa. Nos tumores cutâneos não-pigmentados, a dermatoscopia facilita a observação de estruturas vasculares, aumentando a acuidade no seu diagnóstico, distinguindo tumores melanocíticos e não-melanocíticos, benignos e malignos. Na parte I deste artigo são discutidos os princípios básicos da avaliação dermatoscópica dos tumores cutâneos não-pigmentados, incluindo a morfologia vascular e os padrões de distribuição dos vasos, assim como de outras pistas adicionais.

Os autores escrevem a experiência obtida no Diagnóstico Laboratorial de Varíola, durante o primeiro ano de funcionamento de uma unidade montada para servir à Campanha de Erradicação de Varíola no Brasil. O exame de 71 materiais de crostas e de 16 de líquido de vesícula ou pústula, forneceu 28 e 9 amostras de vírus da varíola, respectivamente. As provas de precipitação em agar-gel foram positivas em 21% e 25% dos espécimens respectivamente (Tabela 1). A coleta foi muitas vêzes realizada em regiões distantes e os espécimens levaram algumas vêzes até mais de 15 dias vara chegar ao laboratório, o que influenciou sensivelmente os resultados obtidos (Tabelas 2 e 3), reduzindo a percentagem de isolamentos. A reação de precipitação diretamente a partir do espécímen, não foi útil quando eram disponíveis quantidades reduzidas de material; a reação foi, no entanto, usada com sucesso para informar a presença de vírus em membranas cório-alantóicas, como confirmação do diagnóstico. Próximamente serão apresentados e analisados comparativamente os resultados obtidos nos meses seguintes de trabalho.

During July and August of 1968, a Health survey was conducted on the Ilha da Conceição, an area of Niterói containing approximately one thousand households. The survey was conducted by students from the Universidade Federal Fluminense and the University of Maryland, and was under the supervision of faculty of the Department of Tropical Medicine at U.F.F. and from the Department of Preventive Medicine at the University of Maryland, Baltimore, Maryland, U.S.A. The survey was focused on a 25 percent random sample of the households on the island. Information was obtained from a responsible adult at each Household for completion of a Health questionnaire. Physical measurements, as well as laboratory study information were obtained from, all children in these households. A number of environmental sanitation problems were identified on the Ilha da Conceição. In addition, the survey indicated that approximately half the children had not been adequately immunized against diphteria, pertussis and typhoid. Preventable communicable diseases were the major cause of reported deaths which had occurred in infants ou Household members. The Health of the population on the Ilha da Conceição could well be enhanced by the development of an intelligence system indicating the immunization status of all children in the area. In addition a Health education program for the residents could well be beneficial for improvement of sanitary conditions on the island, as well as maternity and well baby care.

The literature on the thermosensitive properties of strains or species of Leishmania and of other miercorganisms is revised. Cutaneous or mucocutaneous strains that infect animais in the coldest areas of the skin or mucosa in general can not grow in tissue culture at 37°C or higher temperatures and their respiratory metabolism decreases at these temperatures. These facts suggest a thermosensitive event in some important metabolism phase of the organisme. The strains or species that are able to produce visceral leishmaniasis were probably originated from cutaneous strains after genetioally determined physiological adaptation, to warmer temperatures. These strains can not only visceralize in animais and man but will also grow in tissue culture at 36-37°C and the respiratory metabolism will be higher at such temperatures. There are reasons to believe that intermediate strains, i. e., with properties of both groupsí do exist. A thermosensitive physiological event is a more general phenomenon and examples of it can also be found in the fields of virology, bacteriology and mycology. It has practical applications since some of the diseases produced by these agents can be cured by treatments with heat or artificial fever. Experiments along these line were performed on hamsters with a Costa Rican strain of L. braziliensis as an experimental model. Even after intraperitoneal inoculation lesions appear in the nose, ears, paws and tail with a subcutaneous temperature bellow 33°C at 22-24°C. Healing of the lesión is accomplished by increasing room temperature. A good lesión is produced in the rump of the animal if the area is depilated (comercial cream depilatory) previously and the naked skin cooled artificially. Elevated temperature, or the growing back of the hair will tend to diminish or cure the lesion.

O objectivo geral do estágio de Neonatologia (Iª parte) integrado no internato complementar de Pediatria Médica é proporcionar ao interno, em regime tutelado, as oportunidades de prática clínica para a resolução dos problemas correntes do recém-nascido saudável ou com patologia não requerendo terapia intensiva. Em educação médica torna-se fundamental proceder, não só à avaliação da aprendizagem dos formandos, mas também à avaliação, pelos próprios formandos, do treino ministrado pelos formadores. Utilizando um inquérito anónimo integrando 15 questões de resposta aberta e entregue aos internos (n=30) para preenchimento no último dia do estágio, procurámos, ao longo de um período de 7 1/2 anos conhecer as impressões daqueles sobre a formação que lhes fora propiciada, tendo cada parâmetro sido cotado de 1 a 10 pontos. Relativamente à impressão geral/organização e apoio dado pelos orientadores, foram obtidas médias respectivamente de 9,2 e 9,3. Quanto à impressão do estágio por sectores, as pontuações médias oscilaram entre 7,4 (bloco de partos) e 9,1 (sector de cuidados especiais). No que respeita ao período no bloco de partos, o aspecto mais negativo relacionou-se com as oportunidades perdidas para aquisição de competência em entubação traqueal. As acções de formação mais cotadas foram a discussão de casos clínicos (média: 8,6). Conclui-se que os internos consideraram globalmente o estágio relevante (média: 9,3), registando-se a mais baixa satisfação no âmbito do treino propiciado no bloco de partos. Quanto a sugestões, ressalta a que se relaciona com o alargamento do período do estágio.

The colors that are seen in dermoscopy depend on the anatomic level of the skin at which the chromophores are seen. Blue color can be found in a variety of melanocytic and nonmelanocytic lesions. An 89-year-old man presented with a 3-year history of a slow-growing, hyperpigmented patch located on the distal third of the right arm. Dermoscopy showed an atypical network, irregularly distributed globules, pigmented internal streaks and a milky-red area. Based on these findings a diagnosis of slow-growing malignant melanoma was made. Simultaneously, a well-defined blue papule was seen on the proximal third of the same arm. Dermoscopy disclosed a homogeneous blue pattern. After clinical and dermoscopic correlation our differential diagnosis for this blue lesion included cutaneous melanoma metastasis, blue nevus and foreign body reaction. The patient recalled its onset 75 years ago after a grenade explosion. We also discuss the blue lesion appearance under reflectance confocal microscopy and high-definition optical coherence tomography. Histopathological examination after excision of the hyperpigmented patch and blue papule revealed a melanoma in situ and a foreign body reaction, respectively. The diagnostic evaluation of a blue lesion should always rely on the integration of all data, especially clinical and dermoscopic features. Other non-invasive techniques, like reflectance confocal microscopy and high-definition optical coherence tomography can also be important aids for its differential diagnosis.

The type I interferon system is integral to human antiviral immunity. However, inappropriate stimulation or defective negative regulation of this system can lead to inflammatory disease. We sought to determine the molecular basis of genetically uncharacterized cases of the type I interferonopathy Aicardi-Goutières syndrome, and of other patients with undefined neurological and immunological phenotypes also demonstrating an upregulated type I interferon response. We found that heterozygous mutations in the cytosolic double-stranded RNA receptor gene IFIH1 (MDA5) cause a spectrum of neuro-immunological features consistently associated with an enhanced interferon state. Cellular and biochemical assays indicate that these mutations confer a gain-of-function - so that mutant IFIH1 binds RNA more avidly, leading to increased baseline and ligand-induced interferon signaling. Our results demonstrate that aberrant sensing of nucleic acids can cause immune upregulation.
