BACKGROUND: Increasing incidence of head and neck cancer (HNC) in young adults has been reported. We aimed to compare the role of major risk factors and family history of cancer in HNC in young adults and older patients. METHODS: We pooled data from 25 case-control studies and conducted separate analyses for adults ≤45 years old ('young adults', 2010 cases and 4042 controls) and >45 years old ('older adults', 17 700 cases and 22 704 controls). Using logistic regression with studies treated as random effects, we estimated adjusted odds ratios (ORs) and 95% confidence intervals (CIs). RESULTS: The young group of cases had a higher proportion of oral tongue cancer (16.0% in women; 11.0% in men) and unspecified oral cavity / oropharynx cancer (16.2%; 11.1%) and a lower proportion of larynx cancer (12.1%; 16.6%) than older adult cases. The proportions of never smokers or never drinkers among female cases were higher than among male cases in both age groups. Positive associations with HNC and duration or pack-years of smoking and drinking were similar across age groups. However, the attributable fractions (AFs) for smoking and drinking were lower in young when compared with older adults (AFs for smoking in young women, older women, young men and older men, respectively, = 19.9% (95% CI = 9.8%, 27.9%), 48.9% (46.6%, 50.8%), 46.2% (38.5%, 52.5%), 64.3% (62.2%, 66.4%); AFs for drinking = 5.3% (-11.2%, 18.0%), 20.0% (14.5%, 25.0%), 21.5% (5.0%, 34.9%) and 50.4% (46.1%, 54.3%). A family history of early-onset cancer was associated with HNC risk in the young [OR = 2.27 (95% CI = 1.26, 4.10)], but not in the older adults [OR = 1.10 (0.91, 1.31)]. The attributable fraction for family history of early-onset cancer was 23.2% (8.60% to 31.4%) in young compared with 2.20% (-2.41%, 5.80%) in older adults. CONCLUSIONS: Differences in HNC aetiology according to age group may exist. The lower AF of cigarette smoking and alcohol drinking in young adults may be due to the reduced length of exposure due to the lower age. Other characteristics, such as those that are inherited, may play a more important role in HNC in young adults compared with older adults.

El principal objetivo de este proyecto es realizar un aplicativo web que utilice API’s para que, a partir de un conjunto de rutas almacenadas en el sistema, éste sea capaz de generar nuevas rutas a partir de éstas, de modo que dicho sistema no se comporte como un mero "almacén" de rutas introducidas por los usuarios para poder ser utilizadas por los demás posteriormente, sino que podamos ser nosotros mismos los que decidamos ciertos aspectos de la ruta que deseamos que el sistema nos proporcione. Así, el aplicativo desarrollado, partiendo de un conjunto previo de rutas, será capaz de generar nuevas rutas pasando lo más cerca posible de un conjunto de puntos definidos por el usuario, pudiendo ser calculadas siguiendo diferentes parámetros de optimización tales como el menor número de rutas utilizadas en el cálculo o el menor número de intersecciones entre ellas. Estas rutas, posteriormente, pueden visualizarse en la aplicación mediante el API de Google Maps, o pueden ser descargadas para utilizarlas en nuestro dispositivo GPS portátil.

OBJECTIVES: Perioperative fluid accumulation determination is a challenge for the clinician. Bioelectrical impedance analysis (BIA) is a noninvasive method based on the electrical properties of tissues, which can assess body fluid compartments. The study aimed at assessing their changes in three types of surgery (thoracic, abdominal, and intracranial) requiring various regimens of fluid administration. DESIGN: Prospective descriptive trial. PATIENTS: A total of 26 patients scheduled for elective surgery were separated into three groups according to site of surgery: thoracic (n = 8), abdominal aortic (n = 8), and brain surgery (n = 10). SETTING: University teaching hospital. INTERVENTION: None. MEASUREMENTS: Whole body, segmental (arm, trunk, and legs) BIA at multiple frequency (0.5, 50, 100 kHz) was used to assess perioperative fluid accumulation after surgery. The fluid balances were calculated from the charts. RESULTS: The patients were aged 62+/-4 yrs. Fluid balances were 4.8+/-1.0 L, 4.1+/-0.5 L, and 1.9+/-0.3 L, respectively, in the three groups. In trunk surgery patients, fluid accumulation was detected as a drop in impedance in the operated area at all frequencies. In the operated area, there was an expansion of both intra- and extracellular compartments. A reduction in high frequencies' impedance in the legs was only detected after aortic surgery. Fluid accumulation and trunk impedance changes were strongly correlated. Neurosurgery only induced minor body fluid changes. CONCLUSIONS: Segmental BIA is able to detect and localize perioperative fluid accumulation. It may become a bedside tool to quantify and to localize fluid accumulation.

Oenocytes of adult workers and queens of Apis mellifera L. were studied in different ages or life stages, by means of morphometric and histologic techniques. In workers, the oenocytes were found in the head, near the mandibles and in the abdomen, immersed in the parietal fat body mainly below the sterna, close to the wax gland. In queens, two populations of oenocytes different in size and localization were found within the parietal and visceral fat body, respectively. The oenocytes of workers and queens show the presence of acid lipids and acid phosphatase. The role of these cells in the castes differences is discussed.

The authors revalidate the genus Mepraia, described in 1940 by Mazza, Gajardo & Jörg. The genus is monotypic and represented by the species M. spinolai (Porter, 1934). Based on the characteres described below the authors considered the separation of the genus Triatoma Laporte, 1832 and the return of the genus Mepraia. Strongly alar polymorphism, the abdomen of female with connexivum like in genus Dipetalogaster Usinger 1939, seven testicular follicles, like in Triatoma, however with a different relation of lenghts; external male genitalia showing a big and foliar process of the conjunctiva, vesica with an arch-shape, struts and endosoma process absent.

A male of Triatoma ryckmani Zeledón & Ponce, 1972, was studied by scanning electron microscopy. Only few specimens of this species are known. In this paper, some structures from the head, thorax, abdomen and distal region of the second leg are shown. Some of them could have taxonomic importance, as the oculo-ocellar region, the buccula, the anterolateral angle of the collar, the scutellum with the process longer than the main body, the stridulatory sulcus with an unusual backward vermiform area, and the tibia-tarsal articulation, with a spongy fossula. The last structure was absent in specimens previously studied (Lent & Wygodzinsky 1979). Differences between this specimen and others previously described by several authors are dicussed.

AIM: To determine the prevalence and characteristics of pain in Thai human immunodeficiency virus-infected children. METHODS: A cross-sectional study was performed at the HIV/AIDS outpatient clinic at the Queen Sirikit National Institute of Child Health, Bangkok, Thailand from November 2002 to January 2003. Sixty-one human immunodeficiency virus-infected patients aged 4 to 15 y, an equal number of age-matched children with no chronic disease and their caregivers participated. We interviewed children and their caregivers using a structured questionnaire on pain. The main outcome measure was the percentage of human immunodeficiency virus-infected children reporting pain. RESULTS: Forty-four percent of the human immunodeficiency virus-infected children reported pain compared to 13% of the children with no chronic disease (odds ratio, OR = 5.3; 95% CI: 2.0-14.3). Seven percent of the infected children experienced chronic pain. Children in human immunodeficiency virus clinical categories B and C reported more pain than children in categories N and A (OR = 4.0, 95% CI: 1.1-14.7). Pain in infected children tended to occur in the abdomen, lower limbs or head. Only 44 percent of the infected children experiencing pain received analgesic medication. CONCLUSION: Despite being a common experience, pain is insufficiently taken into account and treated in Thai children with HIV/AIDS. Therefore, adequate pain identification, assessment and management should be systemically considered in their routine care.

Triatoma jurbergi n. sp. is described based on nine specimens of both sexes deposited in the Rodolfo Carcavallo Collection in the Oswaldo Cruz Institute Entomological Collection. The new species can be separated from the closely related Triatoma guazu Lent & Wygodzinsky, 1979 by several characters. The most important are longer anteocular region; thin and pointed juga; the shape of the eyes without concavity in the posterior edge; much longer second rostral segment, passing the posterior edge of eye; the absence of a ventral longitudinal depression on the abdomen; the general color redish, brown and orange and the male genitalia, mainly in the vesica lightly chitinized and smaller, the phallosome with apical projection and the pointed apex of the endosome process.

Aquest treball pretén valorar la utilitat del model de risc d’evisceració desenvolupat per Van Ramshorst et al. entre els pacients sotmesos a cirurgia abdominal per laparotomia mitja en el Servei de Cirurgia General i de l’Aparell Digestiu de l’Hospital de Sabadell – Corporació Sanitària i Universitària Parc Taulí – Barcelona. El model de risc inclou dades postoperatòries que fan perdre capacitat pronòstica clínica, per aquest motiu es proposa una modificació d’aquest model (Van Ramshorst modificat), tenint en compte només les variables preoperatòries. Podem concloure que mentre el model de risc d’evisceració de Van Ramshorst et al. és útil en la nostra mostra de pacients, la modificació proposada necessitaria retocs per a millorar la seva capacitat pronòstica.

Introducció: El fracàs renal agut és una complicació que afecta entre el 6 i el 25% dels pacients crítics. Un 72% d’aquests malalts necessitaran de tècniques de depuració extrarenal contínues (TCDER) o intermitents en algun moment de l’ingrés. Actualment la dosi d’ultrafiltrat a cada situació clínica és objecte d’estudi i publicacions. Objectius: Analitzar les diferències entre les dosis pautades i administrades en les TCDER. Comparar les diferències entre la dosi en funció del tipus de TCDER: hemofiltració venovenosa contínua (HVVC) i hemodiafiltració venovenosa contínua. Descriure la tassa de recuperació de la funcionalitat renal o bé la necessitat d’hemodiàlisis convencional a l’alta d’UCI en els malalts que s’han sotmès a TCDER. Determinar la mortalitat Material i mètodes: Estudi observacional de 40 pacients crítics sotmesos a TCDER, 20 pacients en HVVC i 20 pacients en HDFVVC. S’han analitzat les dades mitjançant proves no paramètriques U de Mann Withney, test exacte de Fischer i Chi quadrat. Resultats: L’edat mitjana de la mostra fou 63 ± 13 anys, amb un 57% d’homes. La incidència d’hipertensió arterial fou del 60%, de diabetis mellitus del 30%, de dislipèmia del 25% i de malaltia renal crònica del 27%. La mediana de puntuació de SOFA fou de 10, i d’AKIN de 3. Un 85% dels pacients van precisar ventilació mecànica i/o amines durant l’ingrés. Vam registrar la dosi d’ultrafiltrat pautada pel facultatiu, essent la mitjana de la mostra de 34,34 mL/kg/h (34,73 en HVVC i 33,95 en HDFVVC), mentre que la dosi administrada fou de 27,93 mL/kg/h (26,85 en HVVC i 29 en HDFVVC), essent la diferència entre ambdues estadísticament significativa. L’estada mitjana a UCI fou de 23,7 dies, i la mortalitat de la sèrie del 65%. Conclusió: Les dosis d’ultrafiltrat rebudes en les TCDER són menors a les dosis pautades independentment del tipus de tècnica. La mortalitat de la nostra sèrie fou similar a les sèries publicades.

El fracàs renal agut constitueix una de les complicacions més greus en els pacients de les unitats de cures intensives, amb una mortalitat segons publicacions del 42%. Un nombre important d’aquests pacients requereixen tècniques continues de depuració extrarrenal (TCDE). Hem realitzat un estudi en el que s’ha inclós 29 pacients ingressats durant l’any 2008 a la UCI del Hospital Germans Trias i Pujol, que foren sotmesos a TCDE. Analitzem les possibles variables associades a la mortalitat. Les variables relacionades amb la mortalitat són el tipus de patologia motiu d'ingrés en UCI, l'edat i els dies de tractament amb TCDE.

La preeclàmpsia greu és una malaltia multisistemàtica que es caracteritza per tenir presents alguns dels següents símptomes: PA ≥160 / 110, proteïnúria & 5 g/ 24 h, creatinina plasmàtica elevada, oligúria & 500 cc / 24 h, plaquetes & 100.000 /L, elevació de las transaminases, hemòlisis, dolor epigàstric o hipocondri dret, cefalea, alteracions visuals, mentals, edema agut de pulmó., Sd. Hellp, RCIU o oligoamnis; que apareixen durant l’embaràs. Els factors de risc més comuns entre les pacients estudiades a l’àrea matern infantil de l’ Hospital La Vall d´Hebron de Barcelona són la nuliparidat , l’obesitat i l’ edat &35 anys. Hi ha també uns altres factors menys freqüents com la HTA crònica i la gestació múltiple. En la majoria de les gestants es va seguir el protocol de l’ hipertensió arterial d’aquesta àrea. Totes van ser tractades amb sulfato de magnesi per a prevenir les convulsions i antihipertensiu. Sobre el 70% de les pacients va rebre també un tractament per a l’hipertensió amb labetalol e.v. i hidralacina e.v. Aquests fàrmacs són eficaços per al tractament de l’hipertensió en la preeclàmpsia severa. Es va comprovar que les pacients que van rebre el tractament amb hidralacina van presentar un major nombre de manifestacions clíniques i complicacions. L’anestèsia regional és el tipus d’anestèsia escollida. L’anestèsia intradural produeix el major descens de la TA en les pacients amb preeclàmpsia greu tractades amb anestèsia peridural Per altra banda, la cefalea és més freqüent desprès de l’anestèsia intradural que de l’epidural.

Estudi observacional de 60 pacients ingressats a la UCI per patologia extraabdominal, que demostra alta incidència de HIA (90%) i que el valor de PIA màxima és marcador pronòstic del SCA però no prediu mortalitat. Els graus més elevats d’HIA s’associen amb VMI, shock, SDRA, insuficiència renal e ili. La PIA màxima del pacient crític no neurològic (sèptic, traumatològic i respiratori) és més elevada que la del no neurològic, 20 i 16 mmHg respectivament. El valor de PIA màxima que millor prediu insuficiència renal al pacient no neurològic és 18,5mmHg. La PIA màxima es correlaciona amb l’estada a UCI i hospitalària.

Natural Orifice Transluminal Endoscopic Surgery (NOTES) is a novel, potentially less invasive alternative to laparoscopic surgery. However, the problems of transluminal access and closure represent significant obstacles to its successful introduction in humans. Objective: to evaluate the feasibility and safety of a novel device designed for transluminal access and closure in a survival porcine model. Subjects: Four adult female Yorkshire pigs were used in the study. Interventions: While under general anesthesia, the animals were prepared with multiple tap water enemas followed by instillation of an antibiotic suspension and povidone-iodine lavage. At a distance of 15 to 20 cm from the anus, the prototype device (LSI Solutions, Victor, NY, USA) deployed a circumscribing purse-string suture around the planned incision site and subsequently used a blade mechanism to create a 2.5-cm linear incision. The transcolonic incision was then closed by cinching and securing the purse-string suture with a titanium knot by use of a separate hand-activated suture-locking device. Main Outcome Measurements: The animals were monitored daily for signs of peritonitis and sepsis and were survived for 14 days. The peritoneal cavity was examined for peritonitis, and the colonic incision site was examined for wound dehiscence, pericolic abscess formation, and gross adhesions. Tissue samples from both incisional and random peritoneal sites were obtained for histologic examination. Results: Transcolonic incision and closure were successful in all 4 animals. The device performed in a rapid and reproducible fashion. All animals recovered without septic complications. At necropsy, there was no evidence of peritonitis, abscesses, or wound dehiscence. Salpingocolonic and colovesicular adhesions were noted in 3 of 4 animals. Histologic examination revealed microabscesses at the incision site in all animals. Conclusions: The prototype incision and closure device represents a promising solution to the problems of transluminal access for NOTES. The presence of incision-related adhesions and microabscesses signal the need for further refinement in aseptic technique.

The larva, pupa, male and female of Simulium tergospinosum n. sp. are described and illustrated. The adults of this new species share many characters with species in the subgenus Psaroniocompsa, where it is placed. The larva of this species bears dorsal and lateral triangular tubercles on the abdomen and multiply branched scale-like setae on the body, suggesting that it belongs to the S. siolii species group. S. tergospinosum n.sp. was collected along the Juma River, Apuí county, in the southern part of the State of Amazonas, Brazil. The Juma, a black-water river, is a tributary of the Aripuanã River in the Madeira River hydrographic basin, on the southern part of Amazonas. Females were collected biting humans along the water courses during the sampling period (in the dry season).
