As micoses estão incluídas entre as doenças infecciosas mais ubíquas em todo o mundo, afectando todos os estratos sociais e todos os grupos etários numa variedade de manifestações superficiais, cutâneas, subcutâneas e sistémicas. Um diagnóstico rápido e eficaz destas doenças, com uma correcta identificação da espécie fúngica responsável pela infecção, é essencial para um planeamento do tratamento mais eficaz para o doente infectado. Tem-se registado um aumento da incidência das infecções fúngicas também em consequência do aumento do número de casos de doentes imunodeprimidos, devido particularmente à crescente utilização de terapêuticas imunossupressivas, procedimentos médicos invasivos, prescrição de tratamentos prolongados, entre outros aspectos. O aparecimento da epidemia da Imunodeficiência Humana no início da década de 80, causada pelo vírus VIH, contribuiu também decisivamente para o aumento das infecções fúngicas oportunistas. A Criptococose é uma infecção fúngica, predominantemente oportunista e com uma distribuição epidemiológica mundial, causada por leveduras encapsuladas do género Cryptococcus. A espécie clinicamente mais relevante é Cryptococcus neoformans, cujas estirpes têm sido tradicionalmente classificadas em cinco serotipos relacionados com os antigénios da respectiva cápsula polissacárida: A (C. neoformans var. grubii); D (C. neoformans var. neoformans); B e C (actualmente reconhecidos como uma espécie distinta mas filogeneticamente próxima, C. gattii); e AD (estirpes híbridas). O presente trabalho teve como principal objectivo determinar retrospectivamente os tipos moleculares de uma colecção alargada de estirpes de C. neoformans, isoladas e mantidas durante os últimos 18 anos no Laboratório de Micologia do IHMT/UNL. A maioria das estirpes foi isolada de doentes imunodeprimidos com criptococose, existindo também algumas estirpes de origem ambiental. Foi utilizada a técnica de PCR-RFLP do gene URA5 para diferenciar as estirpes de Cryptococcus neoformans, tendo sido detectados quatro tipos moleculares: VN1 e VN2 (relacionados com C. neoformans var. grubii, serotipo A); VN3 (relacionado com as estirpes híbridas de serotipo AD); e VN4 (relacionado com C. neoformans var. neoformans, serotipo D). Não foram encontrados entre os isolados de origem clínica perfis de restrição correspondentes aos tipos moleculares VG1, VG2, VG3 e VG4 (relacionados com C. gattii, serotipos B e C). O tipo molecular VN1 foi o mais abundante entre os isolados de origem clínica (45% dos isolados), seguindo-se o grupo de estirpes híbridas do tipo molecular VN3 (31%). Os tipos moleculares menos abundantes entre os isolados foram o VN2 (12%) e VN4 (12%). A estandardização do método de tipagem molecular utilizado neste trabalho permite comparar os resultados obtidos com os de outros estudos epidemiológicos semelhantes, realizados noutras regiões do globo e publicados em anos recentes, contribuindo para um conhecimento melhorado da epidemiologia global deste importante fungo patogénico. Em Portugal obteve-se uma percentagem mais elevada de isolados dos grupos moleculares VN1 e VN3 em relação a outros países da Europa e América Latina, em que os tipos mais abundantes são VN1 e VN2. Nestas mesmas regiões, o tipo molecular VN4, relacionado com as estirpes de C. neoformans var. neoformans do serotipo D, é muito raro. Esta variedade é mais comum em zonas mediterrâneas e está muito associada a casos clínicos de infecções cutâneas associadas a infecções do Sistema Nervoso Central. Este tipo molecular parece ser também significativamente mais abundante no nosso país.

Yeasts of the genus Candida have been recognized as important microorganisms responsible for nosocomial fungemia. Six blood-stream and two intravenous central catheter C. albicans strains were isolated from eight patients and studied by electrophoretic karyotyping of chromosomal DNA by pulsed-field gel electrophoresis. Seven chromosomal DNA profiles were identified. Two patients showed isolates with the same profile, suggesting nosocomial transmission. Karyotyping of C. albicans revealed an excellent discriminatory power among the isolates and may therefore be useful in the study of nosocomial candidemia.

Scorpion stings were surveyed in the Montes Municipality of the State of Sucre, Venezuela, aiming to extend the information on these poisonous accidents by characterizing their geographic distribution. From 1980 to 1990, 184 cases of scorpion stings were recorded with an incidence rate of 38.6 cases per 10,000 inhabitants. The locality of San Fernando presented the highest incidence (68.3(0)/000) of poisonous accidents. The highest percentages of severe cases were recorded in the towns of Arenas (27%), San Lorenzo (21%), and Cocollar (19%), which are located at the foot of the Turimiquire Mountains. This region is a dispersion area of scorpions of the Tityus genus. Our results show that this region of the State of Sucre is endemic for scorpion stings which are an important public health problem.

A revision of literature was done with the objective of tracing an epidemiologic profile of neurocysticercosis (NCC) in Brazil. The prevalence was 0.12-9% in autopsies. The frequency was 0.03-7.5% in clinical series and 0.68-5.2% in seroepidemiological studies. The disease corresponds to 0.08-2.5% of admissions to general hospitals. Patient origin was rural in 30-63% of cases. The most involved age range (64-100%) was 11 to 60 years, with a predominance (22-67%) between 21 and 40 years. The male sex was the most affected (51-80%). In the severe forms there was a predominance of urban origin (53-62%) and of the female sex (53-75%). The period of hospitalization ranges from 1 to 254 days and 33 to 50% of patients suffer 1.7 ± 1.4 admissions. The clinical picture was variable, with a predominance of epileptic syndrome (22-92%) and intracranial hypertension (19-89%). Psychiatric manifestations were associated in 9-23% of patients. Lethality was 0.29% in terms of all diseases in general and 4.8-25.9% in terms of neurologic diseases. The asymptomatic form was detected in 6% of patients in clinical serie and in 48.5% of case from autopsies. The racemose form and ventricular localization also was observed as asymptomatic form. Among the patients with cutaneous cysticercosis 65% of them showed neurologic manifestations.

Epidemiological aspects and the antimicrobial susceptibility profile of the Bacteroides fragilis group isolated from clinical and human intestinal specimens were examined in this study. B. fragilis group strains were isolated from 46 (37%) of 124 clinical specimens and the source of the samples was: Blood culture (3), intraabdominal infection (27), brain abscess (2), soft tissue infection (17), respiratory sinus (3), pleural aspirate (9), breast abscess (3), surgical infected wound (22), pelvic inflammatory disease (22), chronic otitis media (9) and miscellaneous (7). Intraabdominal and soft tissue infections were responsible for more than half of the clinical isolates. Susceptibility to penicillin, cefoxitin, tetracycline, metronidazole, chloramphenicol and clindamycin was examined. All isolates were susceptible to metronidazole and chloramphenicol. For clindamycin and cefoxitin the resistance rates observed were 21.7% and 10.9% respectively. Susceptibility profiles varied among the different species tested. A total of 37 species of B. fragilis group isolated from intestinal microbiota of individuals who had no antimicrobial therapy for at least 1 month before the sampling was also examined. All strains were also susceptible to chloramphenicol and motronidazole and the resistance rates to clindamycin and cefoxitin were 19.4% and 5.4% respectively. A few institutions, in Brazil, have monitored the antimicrobial susceptibility of B. fragilis group strains isolated from anaerobic infections. The resistance rates to cefoxitin and clindamycin and the variation in susceptibility patterns among the species isolated in this study emphasize the need for monitoring of susceptibility patterns of B. fragilis group organisms isolated, especially at our University Hospitals.

Systematics is the study of diversity of the organisms and their relationships comprising classification, nomenclature and identification. The term classification or taxonomy means the arrangement of the organisms in groups (rate) and the nomenclature is the attribution of correct international scientific names to organisms and identification is the inclusion of unknown strains in groups derived from classification. Therefore, classification for a stable nomenclature and a perfect identification are required previously. The beginning of the new bacterial systematics era can be remembered by the introduction and application of new taxonomic concepts and techniques, from the 50’s and 60’s. Important progress were achieved using numerical taxonomy and molecular taxonomy. Molecular taxonomy, brought into effect after the emergence of the Molecular Biology resources, provided knowledge that comprises systematics of bacteria, in which occurs great evolutionary interest, or where is observed the necessity of eliminating any environmental interference. When you study the composition and disposition of nucleotides in certain portions of the genetic material, you study searching their genome, much less susceptible to environmental alterations than proteins, codified based on it. In the molecular taxonomy, you can research both DNA and RNA, and the main techniques that have been used in the systematics comprise the build of restriction maps, DNA-DNA hybridization, DNA-RNA hybridization, sequencing of DNA sequencing of sub-units 16S and 23S of rRNA, RAPD, RFLP, PFGE etc. Techniques such as base sequencing, though they are extremely sensible and greatly precise, are relatively onerous and impracticable to the great majority of the bacterial taxonomy laboratories. Several specialized techniques have been applied to taxonomic studies of microorganisms. In the last years, these have included preliminary electrophoretic analysis of soluble proteins and isoenzymes, and subsequently determination of deoxyribonucleic acid base composition and assessment of base sequence homology by means of DNA-RNA hybrid experiments beside others. These various techniques, as expected, have generally indicated a lack of taxonomic information in microbial systematics. There are numberless techniques and methodologies that make bacteria identification and classification study possible, part of them described here, allowing establish different degrees of subspecific and interspecific similarity through phenetic-genetic polymorphism analysis. However, was pointed out the necessity of using more than one technique for better establish similarity degrees within microorganisms. Obtaining data resulting from application of a sole technique isolatedly may not provide significant information from Bacterial Systematics viewpoint

To evaluate the prevalence of antibodies against hepatitis A in two socioeconomically distinct populations, 101 and 82 serum samples from high and low socioeconomic groups, respectively, were analysed for the presence of IgG anti-HAV using a commercial ELISA. The prevalence in low socioeconomic level subjects was 95.0%, whereas in high socioeconomic subjects was only 19.6% (p<0.001). These data show a duality in Brazil: anti-HAV prevalence in low socioeconomic subjects is similar to that of developing countries, while in high socioeconomic subjects, a pattern typical of developed countries is found. The control of this infection in our country is primarily related to the improvement of sanitation, but especially for high socioeconomic level populations, the use of vaccination against hepatitis A is strongly advisable to avoid the occasional appearance of this disease in adults.

Drug development represents a highly complex, inefficient and costly process. Over the past decade, the widespread use of nuclear imaging, owing to its functional and molecular nature, has proven to be a determinant in improving the efficiency in selecting the candidate drugs that should either be abandoned or moved forward into clinical trials. This helps not only with the development of safer and effective drugs but also with the shortening of time-to-market. The modern concept and future trends concerning molecular imaging will assumedly be hybrid or multimodality imaging, including combinations between high sensitivity and functional (molecular) modalities with high spatial resolution and morphological techniques.

This study firstly describes the epidemiology of malaria in Roraima, Amazon Basin in Brazil, in the years from 1991 to 1993: the predominance of plasmodium species, distribution of the blood slides examined, the malaria risk and seasonality; and secondly investigates whether population growth from 1962 to 1993 was associated with increasing risk of malaria. Frequency of malaria varied significantly by municipality. Marginally more malaria cases were reported during the dry season (from October to April), even after controlling for by year and municipality. Vivax was the predominant type in all municipalities but the ratio of plasmodium types varied between municipalities. No direct association between population growth and increasing risk of malaria from 1962 to 1993 was detected. Malaria in Roraima is of the "frontier" epidemiological type with high epidemic potential.
