BACKGROUND: Pneumonia is the biggest cause of deaths in young children in developing countries, but early diagnosis and intervention can effectively reduce mortality. We aimed to assess the diagnostic value of clinical signs and symptoms to identify radiological pneumonia in children younger than 5 years and to review the accuracy of WHO criteria for diagnosis of clinical pneumonia. METHODS: We searched Medline (PubMed), Embase (Ovid), the Cochrane Database of Systematic Reviews, and reference lists of relevant studies, without date restrictions, to identify articles assessing clinical predictors of radiological pneumonia in children. Selection was based on: design (diagnostic accuracy studies), target disease (pneumonia), participants (children aged <5 years), setting (ambulatory or hospital care), index test (clinical features), and reference standard (chest radiography). Quality assessment was based on the 2011 Quality Assessment of Diagnostic Accuracy Studies (QUADAS-2) criteria. For each index test, we calculated sensitivity and specificity and, when the tests were assessed in four or more studies, calculated pooled estimates with use of bivariate model and hierarchical summary receiver operation characteristics plots for meta-analysis. FINDINGS: We included 18 articles in our analysis. WHO-approved signs age-related fast breathing (six studies; pooled sensitivity 0·62, 95% CI 0·26-0·89; specificity 0·59, 0·29-0·84) and lower chest wall indrawing (four studies; 0·48, 0·16-0·82; 0·72, 0·47-0·89) showed poor diagnostic performance in the meta-analysis. Features with the highest pooled positive likelihood ratios were respiratory rate higher than 50 breaths per min (1·90, 1·45-2·48), grunting (1·78, 1·10-2·88), chest indrawing (1·76, 0·86-3·58), and nasal flaring (1·75, 1·20-2·56). Features with the lowest pooled negative likelihood ratio were cough (0·30, 0·09-0·96), history of fever (0·53, 0·41-0·69), and respiratory rate higher than 40 breaths per min (0·43, 0·23-0·83). INTERPRETATION: Not one clinical feature was sufficient to diagnose pneumonia definitively. Combination of clinical features in a decision tree might improve diagnostic performance, but the addition of new point-of-care tests for diagnosis of bacterial pneumonia would help to attain an acceptable level of accuracy. O objetivo deste trabalho foi identificar a variabilidade química de frutos de jenipapeiro com potencial econômico para o Recôncavo Baiano. Foram identificadas 100 árvores de jenipapeiro distribuídas em seis municípios do Recôncavo Baiano, onde se coletaram 10 frutos por planta para realização das análises químicas. As variáveis estudadas foram: pH, teor de sólidos solúveis (SS), ácido ascórbico (AA), acidez titulável (AT), relação entre sólidos solúveis e acidez titulável (SS/AT), açúcares redutores (AR), não-redutores (ANR) e totais (AST). Para a interpretação dos resultados, utilizaram-se análise descritiva e coeficiente de Correlação de Pearson. As análises dos frutos nas safras de 2004/2005 apresentaram valores médios iguais a 3,44 e 3,39 para o pH; 1,40% e 1,42% de AT; 17,18 ºBrix e 16,8 ºBrix para SS; 2,76 mg.100g-1 e 2,65 mg.100g-1 de ácido ascórbico; 9,26% e 8,95% de AR; 3,39% e 3,31% de ANR; 12,61% e 12,28% de AST; 12,37 e 12,00 para SS/AT. Os resultados permitiram concluir que existe variabilidade para os caracteres analisados, possibilitando a exploração econômica dos frutos para o consumo in natura e industrialização; que o SS contribui para a maioria dos caracteres, com exceção da vitamina C, e os genótipos JP12, JP39, JP41, JP59, JP73, JP79, JP80, JP83, JP89, JP90 e JP99 podem ser recomendados para utilização nas condições agroecológicas do Recôncavo Baiano.

O objetivo deste estudo foi investigar a interação entre o fungo Botryosphaeria dothidea e maçãs cv. Fuji por meio da técnica de Differential Display RT-PCR. O cDNA de frutos infectados e não infectados pelo fungo foi amplificado com uma combinação de 15 oligonucleotídeos iniciadores. Foram isolados 400 fragmentos de cDNA diferencialmente expressos, dos quais 120 foram sequenciados e comparados com sequências disponíveis no GenBank, por meio do programa BLASTX. As sequências obtidas foram similares à metalotioninas, profilina alergênica, proteína de resistência e fosfatase.

Current standard treatments for metastatic colorectal cancer (CRC) are based on combination regimens with one of the two chemotherapeutic drugs, irinotecan or oxaliplatin. However, drug resistance frequently limits the clinical efficacy of these therapies. In order to gain new insights into mechanisms associated with chemoresistance, and departing from three distinct CRC cell models, we generated a panel of human colorectal cancer cell lines with acquired resistance to either oxaliplatin or irinotecan. We characterized the resistant cell line variants with regards to their drug resistance profile and transcriptome, and matched our results with datasets generated from relevant clinical material to derive putative resistance biomarkers. We found that the chemoresistant cell line variants had distinctive irinotecan- or oxaliplatin-specific resistance profiles, with non-reciprocal cross-resistance. Furthermore, we could identify several new, as well as some previously described, drug resistance-associated genes for each resistant cell line variant. Each chemoresistant cell line variant acquired a unique set of changes that may represent distinct functional subtypes of chemotherapy resistance. In addition, and given the potential implications for selection of subsequent treatment, we also performed an exploratory analysis, in relevant patient cohorts, of the predictive value of each of the specific genes identified in our cellular models.

Background: Previous studies have presented contradictory data concerning obstructive sleep apnoea syndrome (OSAS), lipid oxidation and nitric oxide (NO) bioavailability. This study was undertaken to (1) compare the concentration of 8-isoprostane and total nitrate and nitrite (NOx) in plasma of middle-aged men with OSAS and no other known co-morbidity and healthy controls of the same age, gender and body mass index; and (2) test the hypothesis that nasal continuous positive airway pressure (CPAP) therapy attenuates oxidative stress and nitrate deficiency. Methods: A prospective, randomised, placebo controlled, double-blind, crossover study was performed in 31 consecutive middle-aged men with newly diagnosed OSAS and 15 healthy control subjects. Patients with OSAS were randomised to receive sham CPAP or effective CPAP for 12 weeks. Blood pressure, urinary catecholamine levels and plasma 8-isoprostane and NOx concentrations were obtained before and after both treatment modalities. Results: Patients with OSAS had significantly higher 8-isoprostane levels (median (IQR) 42.5 (29.2-78.2) vs 20.0 (12.5-52.5) pg/ml, p = 0.041, Mann-Whitney test) and lower NOx levels (264 (165-650) vs 590 (251- 1465) mmol/l, p = 0.022) than healthy subjects. Body mass index, blood pressure and urinary catecholamines were unchanged by CPAP therapy, but 8-isoprostane concentrations decreased (38.5 (24.2-58.7) pg/ml at baseline vs 22.5 (16.2-35.3) pg/ml on CPAP, p = 0.0001) and NOx levels increased (280 (177-707) vs 1373 (981-1517) mmol/l, p = 0.0001) after CPAP. Conclusions: OSAS is associated with an increase in oxidative stress and a decrease in NOx that is normalised

Mercurialis canariensis Obbard & S. A. Harris is an endemic plant recently described from the Canary Islands (OBBARD et al., 2006b: 103). This taxon went undetected until recently, having been included within the variability of the Mercurialis annua sensu lato complex (HANSEN & SUNDING, 1993:170; STIERSTORFER & GAISBERG, 2006).

This study examines the relationship between floral structure and bud quality with the productivity and fruit shape of Gala, Fuji and Daiane apple cultivars under the mild winter conditions in Southern Brazil. Six different types of floral structures were characterized in field growing plants, according to their nature and bud size: spurs, short and long twigs with weak and vigorous buds. Variables related to the phenology and the productivity for these different structures were evaluated. Gala and Fuji cvs. showed earlier phenological development in the twigs, and cv. Daiane in the spurs. For the three cvs. the highest percentage of buds in each phenological phase was observed in the long twigs. The long twigs also showed the highest sprout and fruit set index, floral number per cluster, and leaf area in the three cvs., while the bud abortion was higher in the spurs than in the twigs. No difference was observed among the structures in cvs. Gala and Fuji regarding to the fruit shape. In the cv. Daiane, however, a tendency to higher length diameter ratio of the fruits produced by the long twigs was observed.

The epidermis on leaves protects plants from pathogen invasion and provides a waterproof barrier. It consists of a layer of cells that is surrounded by thick cell walls, which are partially impregnated by highly hydrophobic cuticular components. We show that the Arabidopsis T-DNA insertion mutants of REDUCED WALL ACETYLATION 2 (rwa2), previously identified as having reduced O-acetylation of both pectins and hemicelluloses, exhibit pleiotrophic phenotype on the leaf surface. The cuticle layer appeared diffused and was significantly thicker and underneath cell wall layer was interspersed with electron-dense deposits. A large number of trichomes were collapsed and surface permeability of the leaves was enhanced in rwa2 as compared to the wild type. A massive reprogramming of the transcriptome was observed in rwa2 as compared to the wild type, including a coordinated up-regulation of genes involved in responses to abiotic stress, particularly detoxification of reactive oxygen species and defense against microbial pathogens (e.g., lipid transfer proteins, peroxidases). In accordance, peroxidase activities were found to be elevated in rwa2 as compared to the wild type. These results indicate that cell wall acetylation is essential for maintaining the structural integrity of leaf epidermis, and that reduction of cell wall acetylation leads to global stress responses in Arabidopsis.

Leprosy is a contagious and chronic systemic granulomatous disease caused by Mycobacterium leprae (Hansen"s bacillus). It is transmitted from person to person and has a long incubation period (between two and six years). The disease presents polar clinical forms (the"multibacillary" lepromatous leprosy and the"paucibacillary" tuberculoid leprosy), as well as other intermediate forms with hybrid characteristics. Oral manifestations usually appear in lepromatous leprosy and occur in 20-60% of cases. They may take the form of multiple nodules (lepromas) that progress to necrosis and ulceration. The ulcers are slow to heal, and produce atrophic scarring or even tissue destruction. The lesions are usually located on the hard and soft palate, in the uvula, on the underside of the tongue, and on the lips and gums. There may also be destruction of the anterior maxilla and loss of teeth. The diagnosis, based on clinical suspicion, is confirmed through bacteriological and histopathological analyses, as well as by means of the lepromin test (intradermal reaction that is usually negative in lepromatous leprosy form and positive in the tuberculoid form). The differential diagnosis includes systemic lupus erythematosus, sarcoidosis, cutaneous leishmaniasis and other skin diseases, tertiary syphilis, lymphomas, systemic mycosis, traumatic lesions and malignant neoplasias, among other disorders. Treatment is difficult as it must be continued for long periods, requires several drugs with adverse effects and proves very expensive, particularly for less developed countries. The most commonly used drugs are dapsone, rifampicin and clofazimine. Quinolones, such as ofloxacin and pefloxacin, as well as some macrolides, such as clarithromycin and minocyclin, are also effective. RESUMO A determinação do nível de ploidia é muito importante, principalmente em programas de melhoramento genético que envolvem poliploides, a fim de possibilitar a escolha adequada dos materiais vegetais com os quais se deseja trabalhar. A relação entre o conteúdo de DNA de acessos de bananeira e sua ploidia ainda permanece controversa na literatura; assim, o presente trabalho teve como objetivo avaliar o conteúdo de DNA de acessos de bananeira com diferentes níveis de ploidia. Foram avaliados sete acessos tetraploides, quatro triploides e quatro diploides. A determinação do conteúdo foi realizada pela técnica de citometria de fluxo. Foram trituradas entre 50-60 mg de folhas frescas, juntamente com o padrão interno (Pisum sativum) no tampão LB01, e, posteriormente, as amostras foram filtradas em gaze e filtro de 50 µm. Adicionaram-se 5 µL de RNase e 25 µL de iodeto de propídeo. Para cada amostra, foram analisados 10 mil núcleos, com três repetições. Os resultados obtidos para o conteúdo de DNA permitiram estimar o tamanho dos genomas A e B, sendo o primeiro cerca de 14% maior que o segundo. Os resultados apresentaram clara relação entre o conteúdo de DNA e o nível de ploidia dos materiais. O contéudo de DNA apresentou aumento médio de 30% nas cultivares diploides em relação às cultivares triploides avaliadas e de 25% nas cultivares triploides em relação às cultivares tetraploides. Apesar da diferença nos tamanhos dos genomas A e B, contribuições distintas desses dois genomas não foram diretamente relacionadas com alterações no conteúdo do DNA de cultivares tetraploides.

Neste estudo foi feita avaliação retrospectiva de 71 casos selecionados de pacientes pediátricos, maiores de um ano e menores de sete anos, que realizaram exame de tomografia computadorizada helicoidal dos seios paranasais no período de março de 1997 a abril de 1998, apresentando quadro clínico de sinusopatia inflamatória aguda recorrente e sinusopatia inflamatória crônica. Foram correlacionados os quadros clínicos com os achados da tomografia computadorizada helicoidal, avaliando a pneumatização dos seios paranasais, os complexos óstio-meatais, as variações anatômicas, o estado da superfície mucosa e a extensão lesional. Os achados tomográficos de maior prevalência foram o velamento total ou parcial de uma ou mais cavidades paranasais (92,9%), seguido da hipertrofia da mucosa de revestimento (67,6%). Houve, na maioria dos casos, associação entre a sinusopatia inflamatória e a obstrução dos complexos óstio-meatais (53,5%). As variações anatômicas foram identificadas já a partir de um ano, com predomínio dos desvios do septo nasal (14,1%), estando correlacionadas às sinusopatias inflamatórias em cerca de 71% dos pacientes.

Genome-wide association studies (GWAS) have identified more than 100 genetic variants contributing to BMI, a measure of body size, or waist-to-hip ratio (adjusted for BMI, WHRadjBMI), a measure of body shape. Body size and shape change as people grow older and these changes differ substantially between men and women. To systematically screen for age- and/or sex-specific effects of genetic variants on BMI and WHRadjBMI, we performed meta-analyses of 114 studies (up to 320,485 individuals of European descent) with genome-wide chip and/or Metabochip data by the Genetic Investigation of Anthropometric Traits (GIANT) Consortium. Each study tested the association of up to ~2.8M SNPs with BMI and WHRadjBMI in four strata (men ≤50y, men >50y, women ≤50y, women >50y) and summary statistics were combined in stratum-specific meta-analyses. We then screened for variants that showed age-specific effects (G x AGE), sex-specific effects (G x SEX) or age-specific effects that differed between men and women (G x AGE x SEX). For BMI, we identified 15 loci (11 previously established for main effects, four novel) that showed significant (FDR<5%) age-specific effects, of which 11 had larger effects in younger (<50y) than in older adults (≥50y). No sex-dependent effects were identified for BMI. For WHRadjBMI, we identified 44 loci (27 previously established for main effects, 17 novel) with sex-specific effects, of which 28 showed larger effects in women than in men, five showed larger effects in men than in women, and 11 showed opposite effects between sexes. No age-dependent effects were identified for WHRadjBMI. This is the first genome-wide interaction meta-analysis to report convincing evidence of age-dependent genetic effects on BMI. In addition, we confirm the sex-specificity of genetic effects on WHRadjBMI. These results may provide further insights into the biology that underlies weight change with age or the sexually dimorphism of body shape.

Tumores malignos das cavidades sinonasais são raros e freqüentemente diagnosticados em estágio avançado da doença. A extensão destes tumores para locais críticos como a órbita e o crânio gera dificuldades no tratamento destas lesões. Dez pacientes com neoplasia maligna sinonasal, sem qualquer tratamento prévio e com evidência radiológica de extensão órbito-craniana, foram estudados por tomografia computadorizada. Dos dez tumores, cinco (50%) foram neoplasias epiteliais, tendo sido a mais comum o carcinoma epidermóide (três casos). O sítio de origem tumoral mais comum foi o seio etmoidal, em quatro pacientes (40%), seguido pelo seio maxilar (30%) e pela fossa nasal (30%). Dezesseis órbitas foram comprometidas, já que seis pacientes (60%) apresentaram acometimento orbitário tumoral bilateral. Os tumores se estenderam mais freqüentemente para as órbitas através de erosão da parede medial e do soalho orbitários. A maioria das órbitas teve todos os compartimentos acometidos. Extensão dos tumores para a cavidade craniana foi mais comum através do teto etmoidal (70%) e teto orbitário (30%). A fossa craniana anterior foi acometida em oito casos (80%), seguida pela fossa craniana média (40%) e pelo lobo frontal (excluindo-se a fossa anterior) (30%). Trinta e sete regiões da face foram acometidas pelos dez tumores, excluindo-se o sítio de origem da neoplasia e a região órbito-craniana, corroborando a grande extensão loco-regional do tumor no momento do diagnóstico.

OBJECTIVE: This study aimed to assess the long-term outcome of functional endoscopic sinus surgery for Samter's triad patients using an objective visual analogue scale and nasal endoscopy. METHOD: Using a retrospective database, 33 Samter's triad patients who underwent functional endoscopic sinus surgery were evaluated pre- and post-operatively between 1987 and 2007 in Hospital of La Chaux-de-Fonds, Switzerland. RESULTS: A total of 33 patients participated in the study, and the mean follow-up period was 11.6 years (range 1.2-20 years). Patients were divided into two groups based on visual analogue scale scores of the five parameters with the greatest difference in intensity of symptoms between the beginning and end of follow up. Group 1 included patients with a mean visual analogue scale score of 6 and below at the end of follow up and group 2 included patients with a mean visual analogue scale score of more than 6. The only statistically significant difference noted between the two groups was the endonasal findings: stage III-IV polyposis was present in 1 out of 24 patients (4 per cent) in group 1 and in 5 out of 9 patients (56 per cent) in group 2. CONCLUSION: The results of our study indicate that functional endoscopic sinus surgery helps stabilise disease progression. Stage III-IV polyposis had a significant adverse effect on long-term outcome.
