Descripció dels passos a seguir per realitzar un pla de conservació de la flora vascular a Girona, més concretament es tracten les àrees del Parc Natural dels Aiguamolls de l’Empordà i de les basses de l’Albera, i que sigui atribuïble a d’altres indrets de Catalunya per així obtenir una major consolidació i coordinació entre la xarxa d’espais protegits i no protegits.La metodologia seguida ha estat la realització d’una baremació dels tàxons prioritzats segons els espais escollits, l’elaboració de les actuacions de conservació per a les espècies prioritàries, l’establiment de la distribució geogràfica dels tàxons prioritzats dels quals es crearà un cronograma que recollirà totes les actuacions que es realitzaran durant un període de temps determinat, depenent dels impactes que els hi afectin i, per últim, es pressupostarà el cost genèric de les actuacions realitzades durant el programa

Background: Mortality from invasive meningococcal disease (IMD) has remained stable over the last thirty years and it is unclear whether pre-hospital antibiotherapy actually produces a decrease in this mortality. Our aim was to examine whether pre-hospital oral antibiotherapy reduces mortality from IMD, adjusting for indication bias. Methods: A retrospective analysis was made of clinical reports of all patients (n = 848) diagnosed with IMD from 1995 to 2000 in Andalusia and the Canary Islands, Spain, and of the relationship between the use of pre-hospital oral antibiotherapy and mortality. Indication bias was controlled for by the propensity score technique, and a multivariate analysis was performed to determine the probability of each patient receiving antibiotics, according to the symptoms identified before admission. Data on in-hospital death, use of antibiotics and demographic variables were collected. A logistic regression analysis was then carried out, using death as the dependent variable, and prehospital antibiotic use, age, time from onset of symptoms to parenteral antibiotics and the propensity score as independent variables. Results: Data were recorded on 848 patients, 49 (5.72%) of whom died. Of the total number of patients, 226 had received oral antibiotics before admission, mainly betalactams during the previous 48 hours. After adjusting the association between the use of antibiotics and death for age, time between onset of symptoms and in-hospital antibiotic treatment, pre-hospital oral antibiotherapy remained a significant protective factor (Odds Ratio for death 0.37, 95% confidence interval 0.15–0.93). Conclusion: Pre-hospital oral antibiotherapy appears to reduce IMD mortality.

BACKGROUND The dementias are a cause of mortality have increased over the last years. Therefore is important to analyze the variables more related to its development in Andalusia between 1999 and 2010. METHODS With the deaths of 60 and over by dementia from Andalusia Statistical Institute and the populations from corresponding years, are estimated crude mortality rates, standardized and age-specific; by joinpoint regression was calculated percentages annual change; and also, with the population estimates by marital status and deaths was calculated crude rates and standardized for age, sex and marital status. RESULTS The standardized mortality rates increased from 124.8 to 161.0 deaths per 100,000 in women and 110.3 to 147.7 in men, the annual increase was 4.2% and 3.8% in women and men. The women died more than men with a standardized rate ratio between 1.08 and 1.29. Age was the variable that determined mortality. CONCLUSIONS Mortality from dementia in Andalusia has increased over the past 12 years and will continue to increase with the consequent social and health impacts posed by these diseases, configured as a major health problem.

Andorra és un microestat situat al bell mig Pirineus, històricament la dinàmica de la població i els seus costums han estat lligats a l’entorn i als recursos que aquest els ofereix. A partir dels anys 50 però, Andorra pateix un increment demogràfic i un canvi en les activitats que s’hi desenvolupen, amb un descens del pes del sector primari i un increment de les activitats del terciari. Aquest fet porta a l'expansió dels nuclis de població i un augment de l’oferta de serveis per a potenciar el turisme i el comerç , augmentant així la pressió sobre el medi, que es veu incrementada per la manca de control i de regulació en l’àmbit de l’ordenament territorial. En els últims anys però, s’han realitzat diferents actuacions que han intentat esmenar aquestes mancances, creant, per exemple, legislació d’ordenació del territori i urbanisme (2001), i els Parcs Naturals de la Vall de Sorteny (1999) i del Comapedrosa (2006). Tot i això, és evident que en l’actualitat existeixen encara moltes mancances que cal resoldre urgentment. Per aquest motiu es fa la la proposta d’una xarxa d’espais naturals protegits a Andorra i la creació d’unes directrius que en facilitin la seva gestió. Situant Andorra a l’alçada d’altres països europeus en matèria de protecció del medi natural, i proporcionant unes bases que permetin la gestió unificada d’aquesta xarxa.

The purpose of the study was to know about the expectations had by chronic patients within th e Public Healthcare System in Andalusi a, concerning commu nication and making deci sions with the med ical professionals who take care of them duri ng the entire treatment process. A qualitative study was carried out in Andalusia, based on 19 i n-depth interviews and 17 focus groups with 15 4 chronic patients and their relatives, suffering from seven different types of illness, of which the content was later analyzed based on the type of disease (pathologies), and the level of care (Primary, Sp eci aliz ed or Em ergen cy Ser vices). The study shows that patients i ncl uded in all healthcare processes want information about their illness. In Primary Care, they value continued attention, closeness and trust as well as being referred to a proper specialist. In Specialized Care, they expect to receive trustworthy information, proper tests and a correct diagnosis, friendly treatment and to be listened to. In the Emergency Room, lack of tim e and overcrowding d i sru pt proper communic ation. The type of chronic illne ss i nfluenc es patients’ expectations in terms of communication and decisionmaking. Communic ation between patients and medical personnel and the decision-making process related to chronic illness are crucial for the improvement of the qualit y o f healthcare. El projecte Torre Iberdrola s’engloba en les actuacions urbanístiques de Bilbao per a regenerar espais industrials no utilitzats situats al Paseo de la Ribera, la Avenida Abandoibarra i la passarel•la Pedro Arrupe. La Torre Iberdrola, situada a la parcel•la 204 del PERI de Abandoibarra (veure plànol nº2), en el districte de Abando (Bilbao), és un gratacels d’oficines de 41 plantes amb una altura total de 165 metres i una superfície construïda total d’aproximadament 62.100 m2 sobre rasant. A més, l’edifici a construir consta de 5 plantes sota rasant amb una superfície total construïda d’aproximadament 32.100 m2. L’objecte del present projecte és presentar la solució acordada conjuntament entre l’arquitecte Pelli Clarke Pelli Architects, les consultories Buro Happold i IDOM, i BELLAPART per a l’execució del revestiment del vestíbul d’entrada de la Torre Iberdrola

En aquest projecte es presenta una Proposta de Parc Rural a la Vall de Llémena, que ha de servir per definir el marc gestor del desenvolupament d’aquest espai rural. Es pretén dinamitzar el sector agroramader, forestal i de serveis per tal de millorar la situació socioeconòmica de la Vall de Llémena. Això ha d’ajudar a fixar la població al territori, i tenir en compte la integració de les accions a les potencialitats i requeriments del medi. Amb aquesta finalitat, s’han definit una sèrie de línies estratègiques on s’han proposat accions per la Producció Agrària Ecològica i la Integració del turisme a l’activitat agrària. Introduction The Andalusian Public Health System Virtual Library (Biblioteca Virtual del Sistema Sanitario Público de Andalucía, BV-SSPA) was set up in June 2006. It consists of a regional government action with the aim of democratizing the health professional access to quality scientific information, regardless of the professional workplace. Andalusia is a region with more than 8 million inhabitants, with 100,000 health professionals for 41 hospitals, 1,500 primary healthcare centres, and 28 centres for non-medical attention purposes (research, management, and educational centres). Objectives The Department of Development, Research and Investigation (R+D+i) of the Andalusian Regional Government has, among its duties, the task of evaluating the hospitals and centres of the Andalusian Public Health System (SSPA) in order to distribute its funding. Among the criteria used is the evaluation of the scientific output, which is measured using bibliometry. It is well-known that the bibliometry has a series of limitations and problems that should be taken into account, especially when it is used for non-information sciences, such us career, funding, etc. A few years ago, the bibliometric reports were done separately in each centre, but without using preset and well-defined criteria, elements which are basic when we need to compare the results of the reports. It was possible to find some hospitals which were including Meeting Abstracts in their figures, while others do not, and the same was happening with Erratum and many other differences. Therefore, the main problem that the Department of R+D+i had to deal with, when they were evaluating the health system, was that bibliometric data was not accurate and reports were not comparable. With the aim of having an unified criteria for the whole system, the Department of R+D+i ordered the BV-SSPA to do the year analysis of the scientific output of the system, using some well defined criteria and indicators, among whichstands out the Impact Factor. Materials and Methods As the Impact Factor is the bibliometric indicator that the virtual library is asked to consider, it is necessary to use the database Web of Science (WoS), since it is its owner and editor. The WoS includes the databases Science Citation Index (SCI), Social Sciences Citation Index (SSCI) and Arts & Humanities Citation Index. To gather all the documents, SCI and SSCI are used; to obtain the Impact Factor and quartils, it is used the Journal Citation Reports, JCR. Unlike other bibliographic databases, such us MEDLINE, the bibliometric database WoS includes the address of all the authors. In order to retrieve all the scientific output of the SSPA, we have done general searches, which are afterwards processed by a tool developed by our library. We have done nine different searches using the field ‘address’; eight of them including ‘Spain’ and each one of the eight Andalusian Regions, and the other one combining ‘Spain’ with all those cities where there are health centres, since we have detected that there are some authors that do not use the region in their signatures. These are some of the search strategies: AD=Malaga and AD=Spain AD=Sevill* and AD=Spain AD=SPAIN AND (AD=GUADIX OR AD=BAZA OR AD=MOTRIL) Further more, the field ‘year’ is used to determine the period. To exploit the data, the BV-SSPA has developed a tool called Impactia. It is a web application which uses a database to store the information of the documents generated by the SSPA. Impactia allows the user to automatically process the retrieved documents, assigning them to their correspondent centres. In order to do the classification of documents automaticaly, it was necessary to detect the huge variability of names of the centres that the authors use in their signatures. Therefore, Impactia knows that if an author signs as “Hospital Universitario Virgen Macarena”, “HVM” or “Hosp. Virgin Macarena”, he belongs to the same centre. The figure attached shows the variability found for the Empresa Publica Hospital de Poniente. Besides the documents from WoS, Impactia includes the documents indexed in Scopus and in other databases, where we do bibliographic searches using similar strategies to the later ones. Aware that in the health centres and hospitals there is a lot of grey literature that is not gathered in databases, Impactia allows the centres to feed the application with these documents, so that all the SSPA scientific output is gathered and organised in a centralized place. The ones responsible of localizing this gray literature are the librarians of each one of the centres. They can also do statements to the documents and indicators that are collected and calculated by Impactia. The bulk upload of documents from WoS and Scopus into Impactia is monthly done. One of the main issues that we found during the development of Impactia was the need of dealing with duplicated documents obtained from different sources. Taking into account that sometimes titles might be written differently, with slashes, comas, and so on, Impactia detects the duplicates using the field ‘DOI’ if it is available or comparing the fields: page start, page end and ISSN. Therefore it is possible to guarantee the absence of duplicates. Results The data gathered in Impactia becomes available to the administrative teams and hospitals managers, through an easy web page that allows them to know at any moment, and with just one click, the detailed information of the scientific output of their hospitals, including useful graphs such as percentage of document types, journals where their scientists usually publish, annual comparatives, bibliometric indicators and so on. They can also compare the different centres of the SSPA. Impactia allows the user to download the data from the application, so that he can work with this information or include them in their centres’ reports. This application saves the health system many working hours. It was previously done manually by forty one librarians, while now it is done by only one person in the BV-SSPA during two days a month. To sum up, the benefits of Impactia are: It has shown its effectiveness in the automatic classification, treatment and analysis of the data. It has become an essential tool for all managers to evaluate quickly and easily the scientific production of their centers. It optimizes the human resources of the SSPA, saving time and money. It is the reference point for the Department of R+D+i to do the scientific health staff evaluation.

Temporo-mandibular joint disc disorders are highly prevalent in adult populations. Autologous chondrocyte implantation is a well-established method for the treatment of several chondral defects. However, very few studies have been carried out using human fibrous chondrocytes from the temporo-mandibular joint (TMJ). One of the main drawbacks associated to chondrocyte cell culture is the possibility that chondrocyte cells kept in culture tend to de-differentiate and to lose cell viability under in in-vitro conditions. In this work, we have isolated human temporo-mandibular joint fibrochondrocytes (TMJF) from human disc and we have used a highly-sensitive technique to determine cell viability, cell proliferation and gene expression of nine consecutive cell passages to determine the most appropriate cell passage for use in tissue engineering and future clinical use. Our results revealed that the most potentially viable and functional cell passages were P5-P6, in which an adequate equilibrium between cell viability and the capability to synthesize all major extracellular matrix components exists. The combined action of pro-apoptotic (TRAF5, PHLDA1) and anti-apoptotic genes (SON, HTT, FAIM2) may explain the differential cell viability levels that we found in this study. These results suggest that TMJF should be used at P5-P6 for cell therapy protocols.

INTRODUCTION Metastases are detected in 20% of patients with solid tumours at diagnosis and a further 30% after diagnosis. Radiation therapy (RT) has proven effective in bone (BM) and brain (BrM) metastases. The objective of this study was to analyze the variability of RT utilization rates in clinical practice and the accessibility to medical technology in our region. PATIENTS AND METHODS We reviewed the clinical records and RT treatment sheets of all patients undergoing RT for BM and/or BrM during 2007 in the 12 public hospitals in an autonomous region of Spain. Data were gathered on hospital type, patient type and RT treatment characteristics. Calculation of the rate of RT use was based on the cancer incidence and the number of RT treatments for BM, BrM and all cancer sites. RESULTS Out of the 9319 patients undergoing RT during 2007 for cancer at any site, 1242 (13.3%; inter-hospital range, 26.3%) received RT for BM (n = 744) or BrM (n = 498). These 1242 patients represented 79% of all RT treatments with palliative intent, and the most frequent primary tumours were in lung, breast, prostate or digestive system. No significant difference between BM and BrM groups were observed in: mean age (62 vs. 59 yrs, respectively); gender (approximately 64% male and 36% female in both); performance status (ECOG 0-1 in 70 vs. 71%); or mean distance from hospital (36 vs. 28.6 km) or time from consultation to RT treatment (13 vs. 14.3 days). RT regimens differed among hospitals and between patient groups: 10 × 300 cGy, 5 × 400 cGy and 1x800cGy were applied in 32, 27 and 25%, respectively, of BM patients, whereas 10 × 300cGy was used in 49% of BrM patients. CONCLUSIONS Palliative RT use in BM and BrM is high and close to the expected rate, unlike the global rate of RT application for all cancers in our setting. Differences in RT schedules among hospitals may reflect variability in clinical practice among the medical teams.

Aquesta investigació s’engloba dins del camp de la resolució de problemes en educació matemàtica i se centra en estudiar de quina manera els alumnes de primer curs de Batxillerat de dos instituts públics de Catalunya resolen alguns problemes d’extrems per mètodes elementals, és a dir, sense utilitzar les tècniques del càlcul diferencial. Tanmateix, s’estudia el punt de partida d’aquests alumnes davant dels problemes de màxims i mínims i es detecten algunes estratègies i diversos errors que cometen a l’hora de resoldre’ls. L’instrument de recollida de dades és un qüestionari individual que combina problemes d’extrems de resposta tancada amb altres problemes de resposta oberta. Es realitza una anàlisi inductiva – deductiva de les dades, la qual permet determinar que una part important dels participants en la recerca confonen les relacions entre l’àrea i el volum d’algunes figures geomètriques. A més, la majoria dels alumnes d’aquesta investigació resolen els problemes d’extrems plantejats fent ús de l’estratègia de conjecturar. Finalment, es detecta que diverses respostes dels participants presenten errors causats per l’ús de teoremes i/o de definicions matemàtiques equivocades. Aquest és el cas d’una aplicació errònia del teorema de Pitàgores, la confusió del perímetre d’un quadrat amb la longitud del seu costat i la determinació incorrecta de l’altura de diversos paral·lelograms.
