Fractures of the pelvic ring are comparatively rare with an incidence of 2-8 % of all fractures depending on the study in question. The severity of pelvic ring fractures can be very different ranging from simple and mostly "harmless" type A fractures up to life-threatening complex type C fractures. Although it was previously postulated that high-energy trauma was necessary to induce a pelvic ring fracture, over the past decades it became more and more evident, not least from data in the pelvic trauma registry of the German Society for Trauma Surgery (DGU), that low-energy minor trauma can also cause pelvic ring fractures of osteoporotic bone and in a rapidly increasing population of geriatric patients insufficiency fractures of the pelvic ring are nowadays observed with no preceding trauma.Even in large trauma centers the number of patients with pelvic ring fractures is mostly insufficient to perform valid and sufficiently powerful monocentric studies on epidemiological, diagnostic or therapeutic issues. For this reason, in 1991 the first and still the only registry worldwide for the documentation and evaluation of pelvic ring fractures was introduced by the Working Group Pelvis (AG Becken) of the DGU. Originally, the main objectives of the documentation were epidemiological and diagnostic issues; however, in the course of time it developed into an increasingly expanding dataset with comprehensive parameters on injury patterns, operative and conservative therapy regimens and short-term and long-term outcome of patients. Originally starting with 10 institutions, in the meantime more than 30 hospitals in Germany and other European countries participate in the documentation of data. In the third phase of the registry alone, which was started in 2004, data from approximately 15,000 patients with pelvic ring and acetabular fractures were documented. In addition to the scientific impact of the pelvic trauma registry, which is reflected in the numerous national and international publications, the dramatically changing epidemiology of pelvic ring fractures, further developments in diagnostics and the changes in operative procedures over time could be demonstrated. Last but not least the now well-established diagnostic and therapeutic algorithms for pelvic ring fractures, which could be derived from the information collated in registry studies, reflect the clinical impact of the registry.

Nuclear imaging is used for non-invasive detection, staging and therapeutic monitoring of tumors through the use of radiolabeled probes. Generally, these probes are used for applications in which they provide passive, non-specific information about the target. Therefore, there is a significant need for actively-targeted radioactive probes to provide functional information about the site of interest. This study examined endostatin, an endogenous inhibitor of tumor angiogenesis, which has affinity for tumor vasculature. The major objective of this study was to develop radiolabeled analogues of endostatin through novel chemical and radiochemical syntheses, and to determine their usefulness for tumor imaging using in vitro and in vivo models of vascular, mammary and prostate tumor cells. I hypothesize that this binding will allow for a non-invasive approach to detection of tumor angiogenesis, and such detection can be used for therapeutic monitoring to determine the efficacy of anti-angiogenic therapy. ^ The data showed that endostatin could be successfully conjugated to the bifunctional chelator ethylenedicysteine (EC), and radiolabeled with technetium-99m and gallium-68, providing a unique opportunity to use a single precursor for both nuclear imaging modalities: 99mTc for single photon emission computed tomography and 68Ga for positron emission tomography, respectively. Both radiolabeled analogues showed increased binding as a function of time in human umbilical vein endothelial cells and mammary and prostate tumor cells. Binding could be blocked in a dose-dependent manner by unlabeled endostatin implying the presence of endostatin receptors on both vascular and tumor cells. Animal biodistribution studies demonstrated that both analogues were stable in vivo, showed typical reticuloendothelial and renal excretion and produced favorable absorbed organ doses for application in humans. The imaging data provide evidence that the compounds quantitate tumor volumes with clinically-useful tumor-to-nontumor ratios, and can be used for treatment follow-up to depict changes occurring at the vascular and cellular levels. ^ Two novel endostatin analogues were developed and demonstrated interaction with vascular and tumor cells. Both can be incorporated into existing nuclear imaging platforms allowing for potential wide-spread clinical benefit as well as serving as a diagnostic tool for elucidation of the mechanism of action of endostatin. ^

4HPR is a synthetic retinoid that has shown chemopreventive and therapeutic efficacy against premalignant and malignant lesions including oral leukoplakia, ovarian and breast cancer, and neuroblastoma. 4HPR induces apoptosis in various cancer cells and production of reactive oxygen species (ROS) has been suggested as a possible cause underlying these effects. However, the mechanisms governing these effects by 4HPR are not fully elucidated. In this study, we explored the mechanisms of 4HPR-induced ROS increase and apoptosis in human cancer cells. ^ First, we identified genes modulated by 4HPR using oligonucleotide gene expression arrays and found that they fall into specific functional canonical pathways and gene networks using Ingenuity Pathways Analysis®. Further analysis has shown that 4HPR induced up-regulation of Endoplasmic Reticulum (ER)-related genes such as Heat shock proteins 70 and 90 and the transcriptional factor, GADD153. These findings were validated using quantitative real-time PCR. ^ Second, we found that 4HPR induced extensive ER stress evidenced by dilation of the ER and endoribonuclease-mediated splicing and activation of the transcriptional factor, XBP-1. In addition, 4HPR induced the up-regulation of various ER stress-related genes and their protein products, as well as cleavage and activation of the ER specific Caspase-4. Concomitantly with XBP-1 splicing, all of these effects were dependent on ROS generation by 4HPR. Furthermore, chemical inhibition and RNA interference studies revealed a novel pro-apoptotic role for HSP70/A1A in 4HPR-mediated apoptosis. ^ Third, we observed rapid activation of the small GTPase Rac by 4HPR which was upstream of ROS generation. Inhibition of Rac activity or silencing of its expression by RNA interference inhibited ROS generation and apoptosis induction by 4HPR. siRNA targeting PAK1 and expression of a dominant negative Rac, decreased 4HPR-mediated ROS generation, while expression of a constitutive active Rac increased basal and 4HPR-induced ROS generation and PARP cleavage. Furthermore, metastatic cancer cells exhibited higher Rac activation, ROS generation, and cell growth inhibition due to 4HPR exposure compared to their primary cancer cell counterparts. ^ These findings provide novel insights into 4HPR-mediated ROS generation and apoptosis induction and support the use of ROS inducing agents such as 4HPR against metastatic cancer cells. ^

Atherosclerosis is a complex disease resulting from interactions of genetic and environmental risk factors leading to heart failure and stroke. Using an atherosclerotic mouse model (ldlr-/-, apobec1-/- designated as LDb), we performed microarray analysis to identify candidate genes and pathways, which are most perturbed in changes in the following risk factors: genetics (control C57BL/6 vs. LDb mice), shearstress (lesion-prone vs. lesion-resistant regions in LDb mice), diet (chow vs. high fat fed LDb mice) and age (2-month-old vs. 8-month old LDb mice). ^ Atherosclerotic lesion quantification and lipid profile studies were performed to assess the disease phenotype. A microarray study was performed on lesion-prone and lesion-resistant regions of each aorta. Briefly, 32 male C57BL/6 and LDb mice (n =16/each) were fed on either chow or high fat diet, sacrificed at 2- and 8-months old, and RNA isolated from the aortic lesion-prone and aortic lesion-resistant segments. Using 64 Affymetrix Murine 430 2.0 chips, we profiled differentially expressed genes with the cut off value of FDR ≤ 0.15 for t-test, and q <0.0001 for the ANOVA. The data were normalized using two normalization methods---invariant probe sets (Loess) and Quantile normalization, the statistical analysis was performed using t-tests and ANOVA, and pathway characterization was done using Pathway Express (Wayne State). The result identified the calcium signaling pathway as the most significant overrepresented pathway, followed by focal adhesion. In the calcium signaling pathway, 56 genes were found to be significantly differentially expressed out of 180 genes listed in the KEGG calcium signaling pathway. Nineteen of these genes were consistently identified by both statistical tests, 11 of which were unique to the test, and 26 were unique to the ANOVA test, using the cutoffs noted above. ^ In conclusion, this finding suggested that hypercholesterolemia drives the disease progression by altering the expression of calcium channels and regulators which subsequently results in cell differentiation, growth, adhesion, cytoskeletal change and death. Clinically, this pathway may serve as an important target for future therapeutic intervention, and thus the calcium signaling pathway may serve as an important target for future diagnostic and therapeutic intervention. ^

This culminating experience was a practice based intervention conducted by an organization, utilizing an intervention mapping approach for the program planning. It took place summer 2010 through spring 2011 and included incorporating a community garden into the Gusto wellness program at The Women's Home. This organization offers long-term residential care, and therapeutic services. Literature relating to community gardens and nutrition behavior change was reviewed. Short-term objectives included: 1) Conducting a needs assessment using focus groups, 2) Designing gardening program components based on intervention mapping guidelines, 3) Constructing a garden bed at Midtown Community Garden for use of The Women's Home, 4) Planning and implementing gardening education, and 5) Assessing feasibility of the garden program. The target population included 24 residents living at the residential dormitory of The Women's Home at the time of this project. The major variables are intervention mapping constructs including: 1) Needs assessment, 2) Preparing matrices of change objectives, 3) Selecting theory-informed intervention methods and practical strategies, 4) Producing program components and materials, 5) Planning program adoption, implementation, and sustainability, and 6) Planning for evaluation. The specific focus was lack of access to fresh fruits and vegetables (FV) for this population. Focus group responses revealed interest in community garden participation. Matrices of change were developed for lack of FV access based on performance objectives for behavioral and environmental factors and related determinants and theory. Methods and strategies were developed to implement a community garden and encourage participation. Program components included initiating a garden club, networking activities, creating gardening curriculum, and participating at Midtown Community Garden. Adoption and implementation performance objectives were outlined, and many were carried out. Evaluation questions were designed and outcomes of the garden project were discussed. ^ Outcomes of the project included exposure of garden topics and activities for The Women's Home residents, focus group responses revealing an interest in gardening among this population, gardening program components designed based on intervention mapping steps, and a constructed garden bed that was used for planting vegetables and flowers through fall 2010. Limited resources and budget along with a lack of a residential coordinator at The Women's Home were the main limiting factors for this project. Future garden projects can be developed using the intervention mapping process.^

Many lines of clinical and experimental evidence indicate a viral role in carcinogenesis (1-6). Our access to patient plasma, serum, and tissue samples from invasive breast cancer (N=19), ductal carcinoma in situ (N=13), malignant ovarian cancer (N=12), and benign ovarian tumors (N=9), via IRB-approved and informed consent protocols through M.D. Anderson Cancer Center, as well as normal donor plasmas purchased from Gulf Coast Regional Blood Center (N=6), has allowed us to survey primary patient blood and tissue samples, healthy donor blood from the general population, as well as commercially available human cell lines for the presence of human endogenous retrovirus K (HERV-K) Env viral RNA (vRNA), protein, and viral particles. We hypothesize that HERV-K proteins are tumor-associated antigens and as such can be profiled and targeted in patients for diagnostic and therapeutic purposes. To test this hypothesis, we employed isopycnic ultracentrifugation, a microplate-based reverse transcriptase enzyme activity assay, reverse transcription – polymerase chain reaction (RT-PCR), cDNA sequencing, SDS-PAGE and western blotting, immunofluorescent staining, confocal microscopy, and transmission electron microscopy to evaluate v HERV-K activation in cancer. Data from large numbers of patients tested by reverse transcriptase activity assay were analyzed statistically by t-test to determine the potential use of this assay as a diagnostic tool for cancer. Significant reverse transcriptase enzyme activity was detected in 75% of ovarian cancer patients, 53.8% of ductal carcinoma in situ patient, and 42.1% of invasive breast cancer patient samples. Only 11.1% of benign ovarian patient and 16.7% of normal donor samples tested positive. HERV-K Env vRNA, or Env SU were detected in the majority of cancer types screened, as demonstrated by the results shown herein, and were largely absent in normal controls. These findings support our hypothesis that the presence of HERV-K in patient blood circulation is an indicator of cancer or pre-malignancy in vivo, that the presence of HERV-K Env on tumor cell surfaces is indicative of malignant phenotype, and that HERV-K Env is a tumor-associated antigen useful not only as a diagnostic screening tool to predict patient disease status, but also as an exploitable therapeutic target for various novel antibody-based immunotherapies.

Genome-wide association studies (GWAS) have successfully identified several genetic loci associated with inherited predisposition to primary biliary cirrhosis (PBC), the most common autoimmune disease of the liver. Pathway-based tests constitute a novel paradigm for GWAS analysis. By evaluating genetic variation across a biological pathway (gene set), these tests have the potential to determine the collective impact of variants with subtle effects that are individually too weak to be detected in traditional single variant GWAS analysis. To identify biological pathways associated with the risk of development of PBC, GWAS of PBC from Italy (449 cases and 940 controls) and Canada (530 cases and 398 controls) were independently analyzed. The linear combination test (LCT), a recently developed pathway-level statistical method was used for this analysis. For additional validation, pathways that were replicated at the P <0.05 level of significance in both GWAS on LCT analysis were also tested for association with PBC in each dataset using two complementary GWAS pathway approaches. The complementary approaches included a modification of the gene set enrichment analysis algorithm (i-GSEA4GWAS) and Fisher's exact test for pathway enrichment ratios. Twenty-five pathways were associated with PBC risk on LCT analysis in the Italian dataset at P<0.05, of which eight had an FDR<0.25. The top pathway in the Italian dataset was the TNF/stress related signaling pathway (p=7.38×10 -4, FDR=0.18). Twenty-six pathways were associated with PBC at the P<0.05 level using the LCT in the Canadian dataset with the regulation and function of ChREBP in liver pathway (p=5.68×10-4, FDR=0.285) emerging as the most significant pathway. Two pathways, phosphatidylinositol signaling system (Italian: p=0.016, FDR=0.436; Canadian: p=0.034, FDR=0.693) and hedgehog signaling (Italian: p=0.044, FDR=0.636; Canadian: p=0.041, FDR=0.693), were replicated at LCT P<0.05 in both datasets. Statistically significant association of both pathways with PBC genetic susceptibility was confirmed in the Italian dataset on i-GSEA4GWAS. Results for the phosphatidylinositol signaling system were also significant in both datasets on applying Fisher's exact test for pathway enrichment ratios. This study identified a combination of known and novel pathway-level associations with PBC risk. If functionally validated, the findings may yield fresh insights into the etiology of this complex autoimmune disease with possible preventive and therapeutic application.^

Because of its antiproliferative and differentiation-inducing properties, all-trans-retinoic acid (ATRA) has been used as a chemopreventive and therapeutic agent, for treatment various cancers including squamous cell carcinomas (SCCs). Long-term treatment with ATRA is associated with toxic effects in patients leading to acute or chronic hypervitaminosis syndrome. Moreover, prolonged treatment with oral ATRA leads to acquired resistance to the differentiation-inducing effects of the drug. This resistance is attributed to the induction of cytochrome P-450-dependent catabolic enzymes that lead to accelerated ATRA metabolism and decline in circulating levels. Most of these problems could be circumvented by incorporating ATRA in liposomes (L-ATRA) which results in sustained drug release, decrease in drug-associated toxicity, and protection of the drug from metabolism in the host. Liposomes also function as a solubilization matrix enabling lipophilic drugs like ATRA to be aerosolized and delivered directly to target areas in the aerodigestive tract and lungs. Of the 14 formulations tested, the positively-charged liposome, DPPC:SA (9:1, w/w) was found to be most effective in interacting with SCC cell lines. This, L-ATRA formulation was stable in the presence of serum proteins and buffered the toxic effects of the drug against several normal and malignant cell lines. The positive charge attributed by the presence of SA was critical for increased uptake and retention of L-ATRA by SCC cell lines and tumor spheroids. L-ATRA was highly effective in mediating differentiation in normal and transformed epithelial cells. Moreover, liposomal incorporation significantly reduced the rate of ATRA metabolism by cells and isolated liver microsomes. In vivo studies revealed that aerosol delivery is an effective way of administering L-ATRA, in terms of its safety and retention by lung tissue. The drug so delivered, is biologically active and had no toxic effects in mice. From these results, we conclude that liposome-incorporation is an excellent way of delivering ATRA to target tissues. The results obtained may have important clinical implications in treating patients with SCCs of the aerodigestive tract. ^

Objetivo: Determinar etiología, manifestaciones clínicas, morbimortalidad y recursos diagnósticos y terapéuticos utilizados en pacientes internados con pancitopenia. Material y métodos: Estudio protocolizado, descriptivo y observacional de 14 meses. Criterios de inclusión: pacientes internados con pancitopenia definida por hemoglobina (Hb) <12 g/dL; plaquetas <150.000/ mL y leucocitos <3.800/mL. Los datos fueron analizados con Epi Info 6.04. Resultados: Se diagnosticaron 54 casos de pancitopenia. Prevalencia: 22/1.000 egresos. Edad media: 48,72 años (DS±20,64); 29,63% fueron > 65 años y 53,70% hombres. Permanencia media: 17,13 días (DS±13,22) vs 7,25 días (DS±5,4) del Servicio (p<0.0001). Charlson medio: 7,16 (DS±2,96) y APACHEII medio: 12 (DS±5,04). El 83,33% (45/54, IC95%70,71-92,08) de las pancitopenias fueron secundarias a compromiso medular, 22 casos (40,74%; IC95%27,57-54,97) postquimioterapia (15 en neoplasias oncohematológicas y 7 en sólidas), 11 (20,37%; IC95%10,63-33,53) por mieloptisis y 4 casos (7,41%; IC95%2,06-17,89) por megaloblastosis. El 16,66% (9/54; IC95%7,92- 29,29) fue secundaria a hiperesplenismo y el 16,66% asociadas a infecciones (3 casos por SIDA). Se realizó estudio de médula ósea en 19 casos (35,18%). El 96,29% (IC95%87,25-99,55) presentó comórbidas. El síndrome de respuesta inflamatoria sistémica (85,19%), síndrome anémico (77,8%) y púrpura (50%) fueron las manifestaciones clínicas más frecuentes. Presentó sepsis el 81,48% (IC95%68,57-90,75) y el 29,63% (IC95%17,98-46,31) hemorragias. El 81,48% tuvo infecciones; el 50% de origen nosocomial y el 65,91% clínicamente documentadas. El 34,09% (IC95%20,49-49,92) tuvo aislamiento microbiológico, con hemocultivos positivos en 29,55%. El 51,85% (IC95%37,84-65,66) desarrolló neutropenia febril (75% postquimioterapia). El 64,81% recibió hemoderivados y factores estimulantes de colonias (G-CSF) el 46,34% (IC95%32,62-60,39). La mortalidad fue mayor a la media global del Servicio (16,66 vs 8,65%)(p=0.07).- Conclusiones: Las pancitopenias en pacientes hospitalizados se caracterizaron por ser secundarias a compromiso medular, hiperesplenismo e infecciones, asociarse a permanencia prolongada, altos índices de comorbilidad, complicaciones infecciosas, y mayor mortalidad que la media global del Servicio.

Los estudios de calidad de vida no sólo son usados para evaluar resultados de programas preventivos y terapéuticos, sino también para acercar a la comunidad científica las percepciones sociales y personales de los individuos estudiados. Por ello el presente proyecto desarrolló un estudio correlacional, descriptivo y transversal con el objetivo de llegar a conclusiones válidas acerca de la relación entre la percepción parental del impacto en la calidad de vida de la población de niños estudiada, y su experiencia de caries expresada a través del índice ceod. Se elaboró un cuestionario para padres de niños preescolares con el objeto de valorar la percepción parental de la salud bucal en la calidad de vida de sus hijos, que incluye 18 preguntas unidas en cuatro grupos de dominio: síntomas orales, limitaciones funcionales, bienestar emocional y bienestar social. Del análisis de los porcentajes discriminados de las respuestas se puede inferir que la percepción parental con respecto a la calidad de vida en relación a la salud bucal de sus hijos está distorsionada. La mayor actividad de caries no es percibida por los padres como un problema o como una disminución de la calidad de vida en relación a la salud bucal de sus hijos.

Primary-care pediatricians could play a key role in early detection of development disorders as quick as they might have enough time and knowledge for suitable screenings at clinical routine. This research paper focuses on the development and validation of a knowledge-based web tool whose aim is to support a smart detection of developmental disorders in early childhood. Thus, the use of the system can trigger the necessary preventive and therapeutic actions from birth until the age of six. The platform was designed on the basis of an analysis of significant 21 cases of children with language disorders that supported the creation of a specific knowledge base, its ontology and a set of description logic relations. The resulting system is being validated in a scalable approach with a team of seven experts from the fields of neonathology, pediatrics, neurology and language therapy.

Monitoring of neuro-evolutive development from birth until the age of six is a decisive factor in a child's quality of life. Early detection of development disorders in early childhood can facilitate necessary diagnosis and/or treatment. Primary-care pediatricians play a key role in early detection of development alterations as they can undertake the preventive and therapeutic actions necessary in the interest of a child's optimal development. The focus of this research paper is the construction of a Knowledge Base for smart screening aimed to assist pediatricians in processes of early referral in language disorders. The proposed model provides health professionals with a decision-making tool that supports referral processes. In this way, essential diagnostic and/or therapeutic actions are triggered for a comprehensive individual development. The resulting system was developed on the basis of an analysis and verification of 21 cases of children with language disorders.

Los nanomateriales han adquirido recientemente un gran interés debido a la gran variedad de aplicaciones que pueden llegar a tener en el ámbito de la biomedicina. Este trabajo recoge las posibilidades tanto diagnósticas como terapéuticas que presentan dos modalidades de nanomateriales: nanopartículas de óxido de hierro y nanopartículas de oro. Para ello, en una primera aproximación se ha llevado a cabo la caracterización de las nanopartículas desde el punto de vista de la biocompatibilidad asociada a su tamaño y al tiempo de contacto o circulación en células y tejidos, ensayada tanto in vitro como in vivo así como la cinética de acumulación de dichas nanopartículas en el organismo vivo. Posteriormente se ha realizado la biofuncionalización de los dos tipos de nanopartículas para reconocer dianas moleculares específicas y poder ser utilizadas en el futuro en dos aplicaciones biomédicas diferentes: diagnóstico de enfermedad de Alzheimer mediante imagen de resonancia magnética y destrucción selectiva de células tumorales mediante hipertermia óptica. ABSTRACT Nanomaterials have recently gained a great interest due to the variety of applications that can have in the field of biomedicine. This work covers both diagnostic and therapeutic possibilities that present two types of nanomaterials: iron oxide nanoparticles and gold nanoparticles. Therefore, in a first approximation it has performed the characterizing of nanoparticles from the standpoint of biocompatibility associated with their size and time of contact or movement in cells and tissues, tested both in vitro and in vivo as well as the kinetics of accumulation of the nanoparticles into the living organism. Subsequently the biofunctionalization of two types of nanoparticles was made to recognize specific molecular targets and can be used in the future in two different biomedical applications: diagnosis of Alzheimer's disease by magnetic resonance imaging and selective destruction of tumor cells by optical hyperthermia.

La nanotecnología es el estudio que la mayoría de veces es tomada como una meta tecnológica que nos ayuda en el área de investigación para tratar con la manipulación y el control en forma precisa de la materia con dimensiones comprendidas entre 1 y 100 nanómetros. Recordando que el prefijo nano proviene del griego vavoc que significa enano y corresponde a un factor de 10^-9, que aplicada a las unidades de longitud corresponde a una mil millonésima parte de un metro. Ahora sabemos que esta ciencia permite trabajar con estructuras moleculares y sus átomos, obteniendo materiales que exhiben fenómenos físicos, químicos y biológicos, muy distintos a los que manifiestan los materiales usados con una longitud mayor. Por ejemplo en medicina, los compuestos manométricos y los materiales nano estructurados muchas veces ofrecen una mayor eficacia con respecto a las formulaciones químicas tradicionales, ya que muchas veces llegan a combinar los antiguos compuestos con estos nuevos para crear nuevas terapias e inclusive han llegado a reemplazarlos, revelando así nuevas propiedades diagnósticas y terapéuticas. A su vez, la complejidad de la información a nivel nano es mucho mayor que en los niveles biológicos convencionales y, por tanto, cualquier flujo de trabajo en nano medicina requiere, de forma inherente, estrategias de gestión de información avanzadas. Muchos investigadores en la nanotecnología están buscando la manera de obtener información acerca de estos materiales nanométricos, para mejorar sus estudios que muchas veces lleva a probar estos métodos o crear nuevos compuestos para ayudar a la medicina actual, contra las enfermedades más poderosas como el cáncer. Pero en estos días es muy difícil encontrar una herramienta que les brinde la información específica que buscan en los miles de ensayos clínicos que se suben diariamente en la web. Actualmente, la informática biomédica trata de proporcionar el marco de trabajo que permita lidiar con estos retos de la información a nivel nano, en este contexto, la nueva área de la nano informática pretende detectar y establecer los vínculos existentes entre la medicina, la nanotecnología y la informática, fomentando así la aplicación de métodos computacionales para resolver las cuestiones y problemas que surgen con la información en la amplia intersección entre la biomedicina y la nanotecnología. Otro caso en la actualidad es que muchos investigadores de biomedicina desean saber y comparar la información dentro de los ensayos clínicos que contiene temas de nanotecnología en las diferentes paginas en la web por todo el mundo, obteniendo en si ensayos clínicos que se han creado en Norte América, y ensayos clínicos que se han creado en Europa, y saber si en este tiempo este campo realmente está siendo explotado en los dos continentes. El problema es que no se ha creado una herramienta que estime un valor aproximado para saber los porcentajes del total de ensayos clínicos que se han creado en estas páginas web. En esta tesis de fin de máster, el autor utiliza un mejorado pre-procesamiento de texto y un algoritmo que fue determinado como el mejor procesamiento de texto en una tesis doctoral, que incluyo algunas pruebas con muchos de estos para obtener una estimación cercana que ayudaba a diferenciar cuando un ensayo clínico contiene información sobre nanotecnología y cuando no. En otras palabras aplicar un análisis de la literatura científica y de los registros de ensayos clínicos disponibles en los dos continentes para extraer información relevante sobre experimentos y resultados en nano medicina (patrones textuales, vocabulario en común, descriptores de experimentos, parámetros de caracterización, etc.), seguido el mecanismo de procesamiento para estructurar y analizar dicha información automáticamente. Este análisis concluye con la estimación antes mencionada necesaria para comparar la cantidad de estudios sobre nanotecnología en estos dos continentes. Obviamente usamos un modelo de datos de referencia (gold standard) —un conjunto de datos de entrenamiento anotados manualmente—, y el conjunto de datos para el test es toda la base de datos de estos registros de ensayos clínicos, permitiendo distinguir automáticamente los estudios centrados en nano drogas, nano dispositivos y nano métodos de aquellos enfocados a testear productos farmacéuticos tradicionales.---ABSTRACT---Nanotechnology is the scientific study that usually is seen as a technological goal that helps us in the investigation field to deal with the manipulation and precise control of the matter with dimensions that range from 1 to 100 nanometers. Remembering that the prefix nano comes from the Greek word νᾶνος, meaning dwarf and denotes a factor of 10^-9, that applyied the longitude units is equal to a billionth of a meter. Now we know that this science allows us to work with molecular structures and their atoms, obtaining material that exhibit physical, chemical and biological phenomena very different to those manifesting in materials with a bigger longitude. As an example in medicine, the nanometric compounds and the materials in nano structures are often offered with more effectiveness regarding to the traditional chemical formulas. This is due to the fact that many occasions combining these old compounds with the new ones, creates new therapies and even replaced them, reveling new diagnostic and therapeutic properties. Even though the complexity of the information at nano level is greater than that in conventional biologic level and, thus, any work flow in nano medicine requires, in an inherent way, advance information management strategies. Many researchers in nanotechnology are looking for a way to obtain information about these nanometric materials to improve their studies that leads in many occasions to prove these methods or to create a new compound that helps modern medicine against powerful diseases, such as cancer. But in these days it is difficult to find a tool that searches and provides a specific information in the thousands of clinic essays that are uploaded daily on the web. Currently, the bio medic informatics tries to provide the work frame that will allow to deal with these information challenge in nano level. In this context, the new area of nano informatics pretends to detect and establish the existing links between medicine, nanotechnology and informatics, encouraging the usage of computational methods to resolve questions and problems that surge with the wide information intersection that is between biomedicine and nanotechnology. Another present case, is that many biomedicine researchers want to know and be able to compare the information inside those clinic essays that contains subjects of nanotechnology on the different webpages across the world, obtaining the clinic essays that has been done in North America and the essays done in Europe, and thus knowing if in this time, this field is really being exploited in both continents. In this master thesis, the author will use an enhanced text pre-processor with an algorithm that was defined as the best text processor in a doctoral thesis, that included many of these tests to obtain a close estimation that helps to differentiate when a clinic essay contains information about nanotechnology and when it does not. In other words, applying an analysis to the scientific literature and clinic essay available in both continents, in order to extract relevant information about experiments and the results in nano-medicine (textual patterns, common vocabulary, experiments descriptors, characterization parameters, etc.), followed by the mechanism process to structure and analyze said information automatically. This analysis concludes with the estimation, mentioned before, needed to compare the quantity of studies about nanotechnology in these two continents. Obviously we use a data reference model (Gold standard) – a set of training data manually annotated –, and the set of data for the test conforms the entire database of these clinic essay registers, allowing to distinguish automatically the studies centered on nano drugs, nano devices and nano methods of those focus on testing traditional pharmaceutical products.
