BackgroundThere are no published reports on studies comparing itraconazole (ITC), sulfamethoxazole-trimethoprim (cotrimoxazole, CMX), and ITC followed by CMX (ITC/CMX) in the treatment of paracoccidiodomycosis. This study aimed to compare the efficacy, effectiveness, safety and time to clinical and serologic cure in paracoccidioidomycosis patients treated with ITC or CMX, the antifungal agents most widely used.MethodologyA quasi-experimental study was performed in 177 patients with a confirmed or probable diagnosis of paracoccidioidomycosis. Treatment was divided into two stages: 1) initial, which was continued until clinical cure was achieved and the erythrocyte sedimentation rate decreased to normal values; 2) complementary, which was continued until serologic cure was achieved. Medians were compared via the Mann-Whitney test, and frequencies were compared via the chi-squared test. The assessment of variables as a function of time was performed using Kaplan-Meier curves and Cox regression. The significance level was established as p <= 0.05.Principal FindingsNo difference was found in the efficacy and effectiveness of the initial treatment of 47 individuals given ITC and 130 individuals given CMX; however, the time to clinical cure was shorter in the former compared with the latter group (105 vs. 159 days; p = 0.001), specifically in patients with the chronic form. Efficacy and effectiveness of the three regimens were similar in the complementary treatment; however, the time to serologic cure was shorter when ITC (161 days) or CMX (495 days) was used compared with ITC/CMX (881 days) [p = 0.02]. The independent predictors of a shorter time to serologic cure were treatment with ITC [risk ratio = 6.61 (2.01-21.75)] or with CMX [risk ratio = 5.11 (1.91-13.67)]). The prevalence of side effects was lower with ITC (6.4%) than with CMX (20.0%; p = 0.03).ConclusionsSince ITC induced earlier clinical cure and was better tolerated than CMX, such triazole should be considered the first-choice for PCM treatment.

Soil CO2 emission (F-CO2) is influenced by chemical, physical and biological factors that affect the production of CO2 in the soil and its transport to the atmosphere. F-CO2 varies in time and space depending on environmental conditions, including the management of the agricultural area. The aim of this study was to investigate the spatial variability structure of F-CO2 and soil attributes in a mechanically harvested sugarcane area (green harvest) using fractal dimension (D-F) derived from isotropic variograms at different scales (fractograms). F-CO2 showed an overall average of 1.51 mu mol CO2 m(-2) s(-1) and correlated significantly (P < 0.05) with soil physical attributes, such as soil bulk density, air-filled pore space, macroporosity and microporosity. Topologically significant DF values were obtained from the characterization of F-CO2 at medium and large scales (above 20 m), with values of 2.92 and 2.90, respectively. The variations in D-F with scales indicate that the spatial variability structure of F-CO2 was similar to that observed for soil temperature and total pore volume and was the inverse of that observed for other soil attributes, such as soil moisture, soil bulk density, microporosity, air-filled pore space, silt and clay content, pH, available phosphorus and the sum of bases. Thus, the spatial variability structure of F-CO2 presented a significant relationship with the spatial variability structure for most soil attributes, indicating the possibility of using fractograms as a tool to better describe the spatial dependence of variables along the scale. Rocuronium (ROC) is a neuromuscular blocking agent used in surgical procedures which is eliminated primarily by biliary excretion. A liquid chromatography-tandem mass spectrometry (LC-MS/MS) method was developed and validated for analysis of ROC in human plasma. Separation of ROC and IS (verapamil) was performed using an endcapped C-18 column and a mixture of water:acetonitrile:trifluoracetic acid (50:50:0.1, v/v) as mobile phase. Aliquots of 100 mu L of human plasma were extracted at pH 3, using dichloromethane. The lower limit of quantification of 5 ng/mL shows the high sensitivity of this method. Intra- and inter-assay precision (as relative standard deviation) was all <= 14.2% and accuracy (as relative standard error) did not exceed 10.1%. The validated method was successfully applied to quantify ROC concentrations in patients under surgical procedures up to 6 h after the administration of the 0.4-0.9 mg/kg ROC. The pharmacokinetic parameter estimations of ROC showed AUC/dose of 563 mu g min/mL, total clearance of 2.5 mL/min/kg, volume of distribution at steady state of 190 mL/kg and mean residence time of 83 min. A new derivation of Euler's Elastica with transverse shear effects included is presented. The elastic potential energy of bending and transverse shear is set up. The work of the axial compression force is determined. The equation of equilibrium is derived using the variation of the total potential. Using substitution of variables an exact solution is derived. The equation is transcendental and does not have a closed form solution. It is evaluated in a dimensionless form by using a numerical procedure. Finally, numerical examples of laminates made of composite material (fiber reinforced) and sandwich panels are provided.

This experiment was conducted to evaluate the effect of grazing heights on daytime behavioral activities of Nellore beef cattle in the rainy season. The experimental area was 12 hectares divided into paddocks of one hectare each. The treatments consisted of four defoliation heights (15, 30, 45 and 60 cm) in pastures of Brachiaria brizantha cv. Xaraes with three replicates each. It was used the continuos grazing method, with variable stocking rate. Forage samples collected on the plots were sent to the laboratory for separation of the botanical components, weighing and determination of dry matter, with the material collected by simulated grazing. The variables: grazing time, idle time and ruminating time were evaluated for 12 consecutive hours on days 15 and 16 February 2011, considering the morning and afternoon periods. It was used a completely randomized design. The height of the canopy significantly influenced the daily grazing time and ruminating time, with a quadratic response as a function of time of defoliation. The bite rate decreased as a function of heights studied. However the chemical composition of the material collected by simulated grazing did not differ between treatments. Xaraes grass swards grazed at around 45 cm height provide greater ease of apprehension by grazing cattle.
