But: La perte unilatérale du cortex visuel postérieur engendre une cécité corticale controlatérale à la lésion, qu’on appelle hémianopsie homonyme (HH). Celle-ci est notamment accompagnée de problèmes d’exploration visuelle dans l’hémichamp aveugle dus à des stratégies oculaires déficitaires, qui ont été la cible des thérapies de compensation. Or, cette perte de vision peut s’accompagner d’une perception visuelle inconsciente, appelée blindsight. Notre hypothèse propose que le blindsight soit médié par la voie rétino-colliculaire extrastriée, recrutant le colliculus supérieur (CS), une structure multisensorielle. Notre programme a pour objectif d’évaluer l’impact d’un entraînement multisensoriel (audiovisuel) sur la performance visuelle inconsciente des personnes hémianopsiques et les stratégies oculaires. Nous essayons, ainsi, de démontrer l’implication du CS dans le phénomène de blindsight et la pertinence de la technique de compensation multisensorielle comme thérapie de réadaptation. Méthode: Notre participante, ML, atteinte d’une HH droite a effectué un entraînement d’intégration audiovisuel pour une période de 10 jours. Nous avons évalué la performance visuelle en localisation et en détection ainsi que les stratégies oculaires selon trois comparaisons principales : (1) entre l’hémichamp normal et l’hémichamp aveugle; (2) entre la condition visuelle et les conditions audiovisuelles; (3) entre les sessions de pré-entraînement, post-entraînement et 3 mois post-entraînement. Résultats: Nous avons démontré que (1) les caractéristiques des saccades et des fixations sont déficitaires dans l’hémichamp aveugle; (2) les stratégies saccadiques diffèrent selon les excentricités et les conditions de stimulations; (3) une adaptation saccadique à long terme est possible dans l’hémichamp aveugle si l’on considère le bon cadre de référence; (4) l’amélioration des mouvements oculaires est liée au blindsight. Conclusion(s): L’entraînement multisensoriel conduit à une amélioration de la performance visuelle pour des cibles non perçues, tant en localisation qu’en détection, ce qui est possiblement induit par le développement de la performance oculomotrice.

EXECUTIVE SUMMARY All observers agree that energy efficiency must be the cornerstone of any serious EU energy strategy. In this general context, the EU building sector is critical. It represents about 40% of EU final energy consumption (residential houses, public/private offices, commercial buildings, etc.) and approximately 36% of EU CO2 emissions. This is massive. The EU has certainly not been inactive in this field. The Energy Performance in Buildings Directive 2002/91/EC (EPBD) was the first and the main instrument to address the problem of the energy performance of buildings. It has established numerous principles: a reliable methodology which enables the calculation and rating of the energy performance of buildings; minimum energy performance standards for new buildings and existing buildings under major renovation; energy performance certificates; regular inspection of heating and air-conditioning systems; and, finally, quality standards for inspections and energy performance certificates. They were strengthened in 2010 by the recast Directive 2010/31/EU. This directive also introduces a decisive concept for the development of the building sector: ‘nearly zeroenergy buildings’. In 2012, the new Energy Efficiency Directive 2012/27/EU dealt with other aspects. In the building sector, three of them are particularly important. They concern: (1) the establishment of long-term strategies for mobilizing investment in the renovation of the national building stocks; (2) the introduction of energy saving schemes for ‘designated’ energy companies with a view to reducing consumption among ‘final consumers’ by 1.5% annually; and (3), as an option, the setting up of an Energy Efficiency National Fund to support energy efficiency initiatives. This paper also briefly examines the different instruments put in place to disseminate information and consultation, and the EU funding for energy efficiency in buildings. Results, however, have remained limited until now. The improvement of the energy performance of buildings and the rhythm of renovation remain extremely weak. Member States’ unwillingness to timely and properly transpose and implement the Directives continues despite the high degree of flexibility permitted. The decentralized approach chosen for some specific aspects and the differentiation in the application of EPBD standards between Member States do not appear optimal either. Adequate financial schemes remain rare. The permanent deficit of qualified and trained personnel and the inertia of public authorities to make the public understand the stakes in this domain remain problematic. Hence the need to take new initiatives to reap the benefits that the building sector is meant to bring.

The improvement and performance of a micellar electrokinetic capillary chromatography assay for cefepime in human serum and plasma with a 50 μm id fused-silica capillary elongated from 40 to 60 cm is reported. Sample preparation with dodecylsulfate protein precipitation at pH 4.5, the pH 9.1 separation medium and the applied voltage were as reported previously[16]. The change resulted in a significant lower current, higher resolution and increased detection time intervals. The performance of the assay with multi-level internal calibration was assessed with calibration and control samples. Quality assurance data of a two year period assessed under the new conditions demonstrated the robustness of the assay. In serum samples of patients who received both cefepime and sulfamethoxazole, cefepime could not be detected due to the inseparability of the two compounds. The presence of an interference can be recognized by an increased peak width (width > 0.2 min), the appearance of a shoulder or an unresolved double peak. The patient data gathered during a three year period reveal that introduction of therapeutic drug monitoring led to a 50% reduction of the median drug level. The data suggest that therapeutic drug monitoring can help to minimize the risk of major adverse reactions and to increase drug safety on an individual basis. This article is protected by copyright. A tese estuda e analisa as Tecnologias da Informação e Comunicação (TIC) disponíveis em hospitais no Brasil, associados à ANAHP – Associação Nacional de Hospitais Privados que patrocinou a coleta de dados e apoiou a pesquisa com dois temas centrais: uso e custo; integração e interoperabilidade das TIC. A Organização para a Cooperação e Desenvolvimento Econômico (OECD), em estudo sobre TIC em saúde, afirmou que, se implantadas de maneira eficiente, as TIC podem resultar em melhoria da qualidade na prestação de serviços de saúde, aumento da segurança no atendimento ao paciente e custos menores. Para permitir a comparação de resultados e o uso de modelos de pesquisa confiáveis, este estudo adaptou o Modelo de Pesquisa de Uso de TI utilizado pelo GVcia - Centro de Tecnologia da Informação Aplicada da FGV – EAESP e realizou uma complementação dos instrumentos de pesquisa para levantar dados específicos da integração das TIC em hospitais utilizando com referência uma pesquisa aplicada a hospitais norte-americanos. A coleta de dados foi realizada para o período 2009 a 2015 em duas etapas. Na primeira, foi enviado o questionário eletrônico, validado por executivos de hospitais e apresentado no apêndice A. Na segunda etapa, os dados enviados foram validados e aspectos da integração foram esclarecidos por meio de entrevistas. O estudo de custo e uso das TIC resultou no cálculo de diversos indicadores. Gastos e investimentos com TIC em hospitais privados atingiu a 3,5% do faturamento anual dos hospitais da amostra enquanto que o setor da saúde, por sua vez, gastou e investiu 6,4% do faturamento anual com as TIC que implementou. Outro indicador inédito e importante para futuros estudos no setor é o custo anual das TIC por leito hospitalar que permaneceu estável em torno de US$ 39.000 entre 2010 e 2014 e caiu para US$ 36.000 em 2015, valor equivalente a R$ 120.000 de dezembro de 2015. Outras estatísticas e indicadores apresentados no estudo ajudam a entender a evolução e o desempenho das TIC na prestação de serviços de saúde e poderão ser úteis para decidir se as TIC ajudarão a melhorar a segurança e atenção ao paciente, aos profissionais de saúde no acesso a dados do paciente, e a necessária sintonia com o Modelo da Saúde Digital (e-Saúde), a custos adequados. Os resultados do presente estudo são comparáveis aos americanos semelhantes e permitem inferir que o parque tecnológico em uso nos hospitais privados brasileiros poderá alavancar a integração de sistemas existentes, permitindo a sintonia com Modelos de Saúde Digital, com a melhoria de desempenho da cadeia estendida da saúde na atenção ao cidadão que poderá ser assistido onde estiver, a qualquer momento, com segurança. Ação gerencial e investimentos são os tópicos centrais que envolvem a integração.

Heme oxygenase-1 (HO-1) is an enzyme induced by hypoxia and reperfusion injury, and is associated with organ dysfunction in critically ill patients. Patients resuscitated from out-of-hospital cardiac arrest (OHCA) are subjected to hypoxemia, brain injury, and organ dysfunction. Accordingly, we studied HO-1 among these patients. A total of 143 OHCA patients resuscitated from a shockable initial rhythm and admitted to an ICU were included, with plasma HO-1 measured at ICU admission and at 24 h. We analyzed the associations between plasma HO-1 and time to return of spontaneous circulation (ROSC), 90-day mortality, and 12-month Cerebral Performance Category (CPC). HO-1 plasma concentrations were higher after OHCA compared with controls. HO-1 concentrations at admission and on day 1 associated with ROSC (P = 0.002 to P = 0.003). Admission and day 1 HO-1 plasma concentrations were higher in 90-day non-survivors than in survivors (P = 0.017, 0.026). In addition, poor neurological outcome (CPC 3-5) was associated with higher HO-1 plasma levels at admission (P = 0.024). Admission plasma HO-1 levels had an AUC of 0.623 to predict 90-day mortality and an AUC of 0.611 to predict CPC 3 to 5. In conclusion, we found that higher HO-1 plasma levels are associated with longer ROSC and poor long-term outcome.

AIM To assess whether the established cardiovascular biomarker N-terminal pro-B-type natriuretic peptide (NT-proBNP) provides prognostic information in patients with out-of-hospital cardiac arrest due to ventricular tachycardia or fibrillation (OHCA-VT/VF). METHODS We measured NT-proBNP levels in 155 patients with OHCA-VT/VF enrolled into a prospective multicenter observational study in 21 ICUs in Finland. Blood samples were drawn <6h of OHCA-VT/VF and later after 24h, 48h, and 96h. The end-points were mortality and neurological outcome classified according to Cerebral Performance Category (CPC) after one year. NT-proBNP levels were compared to high-sensitivity troponin T (hs-TnT) levels and established risk scores. RESULTS NT-proBNP levels were higher in non-survivors compared to survivors on study inclusion (median 1003 [quartile (Q) 1-3 502-2457] vs. 527 [179-1284]ng/L, p=0.001) and after 24h (1913 [1012-4573] vs. 1080 [519-2210]ng/L, p<0.001). NT-proBNP levels increased from baseline to 96h after ICU admission (p<0.001). NT-proBNP levels were significantly correlated to hs-TnT levels after 24h (rho=0.27, p=0.001), but not to hs-TnT levels on study inclusion (rho=0.05, p=0.67). NT-proBNP levels at all time points were associated with clinical outcome, but only NT-proBNP levels after 24h predicted mortality and poor neurological outcome, defined as CPC 3-5, in models that adjusted for SAPS II and SOFA scores. hs-TnT levels did not add prognostic information to NT-proBNP measurements alone. CONCLUSION NT-proBNP levels at 24h improved risk assessment for poor outcome after one year on top of established risk indices, while hs-TnT measurements did not further add to risk prediction.

BACKGROUND The application of therapeutic hypothermia (TH) for 12 to 24 hours following out-of-hospital cardiac arrest (OHCA) has been associated with decreased mortality and improved neurological function. However, the optimal duration of cooling is not known. We aimed to investigate whether targeted temperature management (TTM) at 33 ± 1 °C for 48 hours compared to 24 hours results in a better long-term neurological outcome. METHODS The TTH48 trial is an investigator-initiated pragmatic international trial in which patients resuscitated from OHCA are randomised to TTM at 33 ± 1 °C for either 24 or 48 hours. Inclusion criteria are: age older than 17 and below 80 years; presumed cardiac origin of arrest; and Glasgow Coma Score (GCS) <8, on admission. The primary outcome is neurological outcome at 6 months using the Cerebral Performance Category score (CPC) by an assessor blinded to treatment allocation and dichotomised to good (CPC 1-2) or poor (CPC 3-5) outcome. Secondary outcomes are: 6-month mortality, incidence of infection, bleeding and organ failure and CPC at hospital discharge, at day 28 and at day 90 following OHCA. Assuming that 50 % of the patients treated for 24 hours will have a poor outcome at 6 months, a study including 350 patients (175/arm) will have 80 % power (with a significance level of 5 %) to detect an absolute 15 % difference in primary outcome between treatment groups. A safety interim analysis was performed after the inclusion of 175 patients. DISCUSSION This is the first randomised trial to investigate the effect of the duration of TTM at 33 ± 1 °C in adult OHCA patients. We anticipate that the results of this trial will add significant knowledge regarding the management of cooling procedures in OHCA patients. TRIAL REGISTRATION NCT01689077.

THE AIM OF THE STUDY There are limited data on blood pressure targets and vasopressor use following cardiac arrest. We hypothesized that hypotension and high vasopressor load are associated with poor neurological outcome following out-of-hospital cardiac arrest (OHCA). METHODS We included 412 patients with OHCA included in FINNRESUSCI study conducted between 2010 and 2011. Hemodynamic data and vasopressor doses were collected electronically in one, two or five minute intervals. We evaluated thresholds for time-weighted (TW) mean arterial pressure (MAP) and outcome by receiver operating characteristic (ROC) curve analysis, and used multivariable analysis adjusting for co-morbidities, factors at resuscitation, an illness severity score, TW MAP and total vasopressor load (VL) to test associations with one-year neurologic outcome, dichotomized into either good (1-2) or poor (3-5) according to the cerebral performance category scale. RESULTS Of 412 patients, 169 patients had good and 243 patients had poor one-year outcomes. The lowest MAP during the first six hours was 58 (inter-quartile range [IQR] 56-61) mmHg in those with a poor outcome and 61 (59-63) mmHg in those with a good outcome (p<0.01), and lowest MAP was independently associated with poor outcome (OR 1.02 per mmHg, 95% CI 1.00-1.04, p=0.03). During the first 48h the median (IQR) of the TW mean MAP was 80 (78-82) mmHg in patients with poor, and 82 (81-83) mmHg in those with good outcomes (p=0.03) but in multivariable analysis TWA MAP was not associated with outcome. Vasopressor load did not predict one-year neurologic outcome. CONCLUSIONS Hypotension occurring during the first six hours after cardiac arrest is an independent predictor of poor one-year neurologic outcome. High vasopressor load was not associated with poor outcome and further randomized trials are needed to define optimal MAP targets in OHCA patients.

Wetlands are extremely valuable natural features that have decreased significantly in number over time in Illinois and the United States ... Their important functions include flood protection, water conservation in times of drought, groundwater recharge, improvement of water quality through sediment reduction and contaminant removal, and providing habitat for native animals and plants, including many sensitive and state-listed threatened and endangered species ... Due to a federal "no net loss" policy on wetlands adopted through executive order by President George H. Bush in 1990, as well as a prevailing heightened interest in conservation in general, there is currently considerable interest in the restoration and creation of wetlands. Both Section-404 of the Clean Water Act of 1972 and the Swampbuster Provision of the Food Securities Act of 1985 require compensation or mitigation for the loss of wetlands. A number of federal and state programs such as Section 319 of the Clean Water Act and the Conservation Reserve Program within the Natural Resources Conservation Service (NRCS) encourage wetland restoration and creation. In addition, various conservation organizations, such as The Nature Conservancy and Ducks Unlimited are very active in wetland restoration. Despite wetland restoration efforts and the national goal of no net loss, wetlands and wetland functions continue to be lost due to degradation of existing wetlands ... Unfortunately, no reliable information exists on the quality of existing wetlands or on trends in wetland quality over time ... The functional quality of existing wetlands is likely decreasing in many areas due to the combined effects of habitat fragmentation, alteration of hydrology, invasive species, and continued input of nutrients and pollutants. Furthermore, it is still debatable whether created or restored wetlands can adequately replace the suite of ecological functions provided by natural wetlands ... and the failure of many wetland compensation projects contributes to a continued national net loss of wetland functions ... The need for post-construction site monitoring and assessment of created and restored wetlands to evaluate functional success is widely recognized. ...
