Com um mercado automóvel cada vez mais competitivo e com os construtores automóveis à procura de atingir os zero defeitos nos seus produtos, a Bosch Car Multimédia Portugal S.A, fabricante de sistemas multimédia para o mercado automóvel, tem como objetivo a qualidade perfeita dos seus produtos. Tal perfeição exige processos de fabrico cada vez mais evoluídos e com melhores sistemas de auxílio à montagem. Nesse sentido, a incorporação de sistemas de visão artificial para verificação da montagem correta dos componentes em sistemas multimédia tem vindo a crescer largamente. Os sistemas de inspeção visual da Cognex tornaram-se o standard da Bosch para a verifi-cação da montagem de componentes por serem sistemas bastante completos, fáceis de con-figurar e com um suporte técnico bastante completo. Estes sistemas têm vindo a ser inte-grados em diversas máquinas (postos) de montagem e nunca foi desenvolvida uma ferra-menta normalizada para integração destes sistemas com as máquinas. A ideia principal deste projeto passou por desenvolver um sistema (uma aplicação informá-tica) que permita controlar os indicadores de qualidade destes sistemas de visão, garantir o seguimento dos produtos montados e, ao mesmo tempo, efetuar cópias de segurança de todo o sistema para utilização em caso de avaria ou de troca de equipamento. Tal sistema foi desenvolvido recorrendo à programação de uma Dynamic Link Library (DLL), através da linguagem VisualBasic.NET, que permite às aplicações dos equipamen-tos (máquinas) da Bosch Car Multimédia comunicarem de uma forma universal e transpa-rente com os sistemas de inspeção visual da marca Cognex. Os objetivos a que o autor se propôs no desenvolvimento deste sistema foram na sua maioria alcançados e o projeto encontra-se atualmente implementado e em execução nas linhas de produção da Bosch Car Multimédia.

Os Transformadores de potência são máquinas de elevada importância ao nível dos Sistemas Elétricos de Energia (SEE) uma vez que são estas máquinas que possibilitam a interligação dos diferentes níveis de tensão da rede e a transmissão de energia elétrica em Corrente Alternada (CA). Geralmente, estas máquinas são de grandes dimensões e de elevado nível de complexidade construtiva. Caracterizam-se por possuírem períodos de vida útil bastante elevados (vinte a trinta anos) e preços elevados, o que conduz a um nível de exigência de fiabilidade muito elevada, uma vez que não e viável a existência de muitos equipamentos de reserva nos SEE. Com o objetivo de tentar maximizar o período de vida útil dos transformadores de potência e a sua fiabilidade, tenta-se, cada vez mais, implementar conceitos de manutenção preventiva a este tipo de máquinas. No entanto, a gestão da sua vida útil e extremamente complexa na medida em que, estas máquinas têm vários componentes cruciais e suscetiveis de originar falhas e, quase todos eles, encontram-se no interior de uma cuba. Desta forma, não e possível obter uma imagem do seu estado, em tempo real, sem colocar o transformador fora de serviço, algo que acarreta custos elevados. Por este motivo, desenvolveu-se uma técnica que permite obter uma indicação do estado do transformador, em tempo real, sem o retirar de serviço, colhendo amostras do óleo isolante e procedendo a sua análise físico-química e Analise Gases Dissolvidos (DGA). As análises aos óleos isolantes tem vindo a adquirir uma preponderância muito elevada no diagnóstico de falhas e na analise do estado de conservação destes equipamentos tendo-se desenvolvido regras para interpretação dos parâmetros dos óleos com carácter normativo. Considerando o conhecimento relativo a interpretação dos ensaios físico-químicos e DGA ao oleol, e possível desenvolver ferramentas capazes de otimizar essas mesmas interpretações e aplicar esse conhecimento no sentido de prever a sua evolução, assim como o surgimento de possíveis falhas em transformadores, para assim otimizar os processos de manutenção. Para garantir que um equipamento opere com segurança e fiabilidade durante o seu ciclo de vida, desde a sua instalação ao desmantelamento, devem ser realizadas inspeções e/ou monitorizações que, dependendo dos dados recolhidos, podem implicar avaliações Fitness- For-Service (FFS) que definirão a necessidade de reparação ou alteração do equipamento ou das condições processuais. A combinação de inspeção ou monitorização com os melhores procedimentos e técnicas de avaliação atuais fazem sobressair insuficiências dos procedimentos mais antigos. Usando métodos mais avançados de avaliação, validados e suportados através de uma vasta experiência de campo, pode-se agora avaliar defeitos nos ativos (equipamentos) e determinar a adequação ao serviço com uma análise FFS. As análises FFS tornaram-se cada vez mais aceites em toda a indústria ao longo dos últimos anos. A norma API 579 - 1/ASME FFS-1: 2007 fornece diretrizes para avaliar os tipos de danos que afetam os equipamentos processuais e a norma API RP 571: 2011 descreve os mecanismos de degradação que afetam os equipamentos nas petroquímicas e refinarias, que incluem os danos por corrosão, desalinhamentos, deformações plásticas, laminações, fissuras, entre outros. Este trabalho consiste na análise de Integridade Estrutural de uma Flare Industrial que surgiu da necessidade real de análise do equipamento no âmbito da atividade profissional do candidato. O estudo realizado a nível profissional é de grande abrangência, incluindo a inspeção do equipamento, identificação da falha ou dano, recolha e registo de dados, definição de estratégia de atuação e seleção de técnicas de avaliação da condição para posterior alteração, reparação ou desmantelamento. Na presente dissertação de mestrado em Engenharia Mecânica, Ramo Construções Mecânicas, pretende-se efetuar o estudo aprofundado de uma das etapas de projeto, nomeadamente estudar a causa, avaliar a falha e implicações estruturais ou processuais devido à degradação interior do riser de uma flare, com base numa análise FFS, assumindo a operabilidade em segurança e garantindo resolutas condições de funcionamento. A presente análise FFS tem como finalidade validar ou não a integridade atual (avaliação técnica quantitativa) de modo a conhecer se o item em questão é seguro e confiável para continuar a operar em condições específicas durante um período de tempo determinado, tendo em consideração as condições verificadas no ato de inspeção e especificações de projeto, segundo os itens de verificação de segurança estabelecidos na norma EN 1990:2002/A1:2005/AC:2010 e com base na norma API 579-1/ASME FFS-1: 2007. No âmbito do presente trabalho foram realizadas as seguintes tarefas: - Identificar e analisar os mecanismos de degradação com base na norma API RP 571: 2011, face às condições processuais e geométricas que o equipamento em análise está sujeito de modo a perceber a evolução da degradação estrutural e quais as implicações na sua longevidade. - Avaliar e propor soluções de reparação e ou alterações processuais e ou geométricas do equipamento em análise de modo a permitir a continuidade em funcionamento sem afetar as condições de segurança e por conseguinte minimizar ou evitar o elevado custo económico associado a um novo equipamento e tempo de paragem processual. - Assumir limites estruturais face às condições processuais e ações externas ou adjacentes. - Garantir a segurança e qualidade da vida humana ou meio que o rodeia, do equipamento, das instalações, das infraestruturas envolventes e evitar o colapso económico, quer por motivos processuais, quer indeminizações ou agravamento do prémio do seguro. Verificou-se que os resultados serão a base para as alterações inerentes, tais como reforço estrutural, alteração da geometria do defletor, ajuste no tensionamento dos cabos e controlo da espessura mínima, de modo a alargar o período de vida útil da flare com segurança.

As empresas nacionais deparam-se com a necessidade de responder ao mercado com uma grande variedade de produtos, pequenas séries e prazos de entrega reduzidos. A competitividade das empresas num mercado global depende assim da sua eficiência, da sua flexibilidade, da qualidade dos seus produtos e de custos reduzidos. Para se atingirem estes objetivos é necessário desenvolverem-se estratégias e planos de ação que envolvem os equipamentos produtivos, incluindo: a criação de novos equipamentos complexos e mais fiáveis, alteração dos equipamentos existentes modernizando-os de forma a responderem às necessidades atuais e a aumentar a sua disponibilidade e produtividade; e implementação de políticas de manutenção mais assertiva e focada no objetivo de “zero avarias”, como é o caso da manutenção preditiva. Neste contexto, o objetivo principal deste trabalho consiste na previsão do instante temporal ótimo da manutenção de um equipamento industrial – um refinador da fábrica de Mangualde da empresa Sonae Industria, que se encontra em funcionamento contínuo 24 horas por dia, 365 dias por ano. Para o efeito são utilizadas medidas de sensores que monitorizam continuamente o estado do refinador. A principal operação de manutenção deste equipamento é a substituição de dois discos metálicos do seu principal componente – o desfibrador. Consequentemente, o sensor do refinador analisado com maior detalhe é o sensor que mede a distância entre os dois discos do desfibrador. Os modelos ARIMA consistem numa abordagem estatística avançada para previsão de séries temporais. Baseados na descrição da autocorrelação dos dados, estes modelos descrevem uma série temporal como função dos seus valores passados. Neste trabalho, a metodologia ARIMA é utilizada para determinar um modelo que efetua uma previsão dos valores futuros do sensor que mede a distância entre os dois discos do desfibrador, determinando-se assim o momento ótimo da sua substituição e evitando paragens forçadas de produção por ocorrência de uma falha por desgaste dos discos. Os resultados obtidos neste trabalho constituem uma contribuição científica importante para a área da manutenção preditiva e deteção de falhas em equipamentos industriais.

RESUMO: Enthesitis is the hallmark of spondyloarthritis (SpA), and is observed in all subtypes. Wide information on SpA abnormalities, including synovitis, tendinitis and enthesitis, can be efficiently perceived by Doppler ultrasound. Furthermore, several studies on imaging of enthesis showed that imaging techniques are better than clinical examination to detect enthesis alterations; and vascularized enthesitis detected by Doppler ultrasound appears to be a valuable diagnostic tool to confirm SpA diagnosis. However, data published until now concerning entheseal elementary alterations that characterize SpA enthesitis (enthesis inflammatory activity) or enthesopathy (permanent structural changes) reflect rather the authors’ empiric opinion than a methodological validation process. In this sense it seems crucial to identify elementary entheseal lesions associated with activity or damage, in order to improve monitoring and treatment response in SpA patients. The development of better assessment tools is today a challenge and a need in SpA. The first study of this thesis focused on the analysis of the reliability of inter-lector and inter-ultrasonography equipment of Madrid sonography enthesitis index (MASEI). Fundamental data for the remaining unrolling project validity. In the second and third studies we concerned about two entheseal elemental lesions: erosions and bursa. In literature erosions represent a permanent structural damage, being useful for monitoring joint injury, disease activity and therapeutic response in many rheumatic diseases; and to date, this concept has been mostly applied in rheumatoid arthritis (RA). Unquestionably, erosion is a tissue-related damage and a structural change. However, the hypothesis that we decided to test was if erosions represent a permanent structural change that can only grow and worsen over time, as occurs in RA, or a transitory alteration. A longitudinal study of early SpA patients was undertaken, and the Achilles enthesis was used as a model. Our results strongly suggested that previously detected erosions could disappear during the course of the disease, being consistent with the dynamic behavior of erosion over time. Based on these striking results it seems reasonable to suggest that the new-bone formation process in SpA could be associated with the resolution of cortical entheseal erosion over time. These results could also be in agreement with the apparent failure of anti-tumor necrosis factor (TNF) therapies to control bone proliferation in SpA; and with the relation of TNF-α, Dickkopf-related protein 1 (Dkk-1) and the regulatory molecule of the Wnt signaling pathway in the bone proliferation in SpA. In the same model, we then proceeded to study the enthesis bursa. Interestingly, the Outcome Measures in Rheumatology Clinical Trials (OMERACT) enthesopathy definition does not include bursa as an elementary entheseal lesion. Nonetheless, bursa was included in 46% of the enthesis studies in a recently systematic literature review, being in agreement with the concept of “synovio-entheseal complex” that includes the link between enthesitis and osteitis in SpA. It has been clarified in recent data that there is not only a close functional integration of the enthesis with the neighboring bone, but also a connection between enthesitis and synovitis. Therefore, we tried to assess the prevalence and relevance of the bursa-synovial lesion in SpA. Our findings showed a significant increase of Achilles bursa presence and thickness in SpA patients compared to controls (healthy/mechanical controls and RA controls). These results raise awareness to the need to improve the enthesopathy ultrasonographic definition. In the final work of this thesis, we have explored new perspectives, not previously reported, about construct validity of enthesis ultrasound as a possible activity outcome in SpA. We performed a longitudinal Achilles enthesis ultrasound study in patients with early SpA. Achilles ultrasound examinations were performed at baseline, six- and twelve-month time periods and compared with clinical outcome measures collected at basal visit. Our results showed that basal erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) are higher in patients with Doppler signal in enthesis, and even that higher basal ESR, CRP and Ankylosing Spondylitis Disease Activity Score (ASDAS) predicted a higher Doppler signal (an ultrasound alteration accepted as representative of inflammation) six months later. Patients with very high disease activity assessed by ASDAS (>3.5) at baseline had significantly higher Achilles total ultrasound score verified at the same time; and ASDAS <1.3 predicted no Doppler signal at six and twelve months. This seems to represent a connection between classical biomarkers and clinical outcomes associated with SpA activity and Doppler signal, not only at the same time, but also for the following months. Remarkably, patients with inactive disease (ASDAS < 1.3) at baseline had no Doppler signal at six and twelve months. These findings reinforce the potential use of ultrasound related techniques for disease progression assessment and prognosis purposes. Intriguingly, Ankylosing Spondylitis Disease Activity Index (BASDAI) didn’t show significant differences between different cut-offs concerning ultrasound lesions or Doppler signal, while verified with ASDAS. These results seem to indicate that ASDAS reflects better than BASDAI what happens in the enthesis. The work herein discussed clearly shows the potential utility of ultrasound in enthesis assessment in SpA patients, and can be important for the development of ultrasound activity and structural damage scores for diagnosis and monitoring purposes. Therefore, local promotion of this technique constitutes a medical intervention that is worth being tested in SpA patients for diagnosis, monitoring and prognosis purposes.

The following case study depicts the bitter transfiguration of Portugal Telecom, SGPS (PT), a multinational telecommunications company that was once an honourable flag of innovation, corporate governance standards in Portugal and overseas. It scrutinises the controversial episodes that paved the way for the pitiful condition in which PT is nowadays: a company that carries the weight of a ruinous €897 million investment in a defaulted company and no more than a 25.6% stake in a heavily indebted Brazilian carrier. The free-fall is made, ironically, of a complete disregard for best corporate governance practices, allowing for PT’s major shareholders to take over the helm of the company, using it selfishly as a cash cow.

Acute and chronic respiratory failure is one of the major and potentially life-threatening features in individuals with myotonic dystrophy type 1 (DM1). Despite several clinical demonstrations showing respiratory problems in DM1 patients, the mechanisms are still not completely understood. This study was designed to investigate whether the DMSXL transgenic mouse model for DM1 exhibits respiratory disorders and, if so, to identify the pathological changes underlying these respiratory problems. Using pressure plethysmography, we assessed the breathing function in control mice and DMSXL mice generated after large expansions of the CTG repeat in successive generations of DM1 transgenic mice. Statistical analysis of breathing function measurements revealed a significant decrease in the most relevant respiratory parameters in DMSXL mice, indicating impaired respiratory function. Histological and morphometric analysis showed pathological changes in diaphragmatic muscle of DMSXL mice, characterized by an increase in the percentage of type I muscle fibers, the presence of central nuclei, partial denervation of end-plates (EPs) and a significant reduction in their size, shape complexity and density of acetylcholine receptors, all of which reflect a possible breakdown in communication between the diaphragmatic muscles fibers and the nerve terminals. Diaphragm muscle abnormalities were accompanied by an accumulation of mutant DMPK RNA foci in muscle fiber nuclei. Moreover, in DMSXL mice, the unmyelinated phrenic afferents are significantly lower. Also in these mice, significant neuronopathy was not detected in either cervical phrenic motor neurons or brainstem respiratory neurons. Because EPs are involved in the transmission of action potentials and the unmyelinated phrenic afferents exert a modulating influence on the respiratory drive, the pathological alterations affecting these structures might underlie the respiratory impairment detected in DMSXL mice. Understanding mechanisms of respiratory deficiency should guide pharmaceutical and clinical research towards better therapy for the respiratory deficits associated with DM1.

BACKGROUND & AIMS: Hepatitis C virus (HCV) induces chronic infection in 50% to 80% of infected persons; approximately 50% of these do not respond to therapy. We performed a genome-wide association study to screen for host genetic determinants of HCV persistence and response to therapy. METHODS: The analysis included 1362 individuals: 1015 with chronic hepatitis C and 347 who spontaneously cleared the virus (448 were coinfected with human immunodeficiency virus [HIV]). Responses to pegylated interferon alfa and ribavirin were assessed in 465 individuals. Associations between more than 500,000 single nucleotide polymorphisms (SNPs) and outcomes were assessed by multivariate logistic regression. RESULTS: Chronic hepatitis C was associated with SNPs in the IL28B locus, which encodes the antiviral cytokine interferon lambda. The rs8099917 minor allele was associated with progression to chronic HCV infection (odds ratio [OR], 2.31; 95% confidence interval [CI], 1.74-3.06; P = 6.07 x 10(-9)). The association was observed in HCV mono-infected (OR, 2.49; 95% CI, 1.64-3.79; P = 1.96 x 10(-5)) and HCV/HIV coinfected individuals (OR, 2.16; 95% CI, 1.47-3.18; P = 8.24 x 10(-5)). rs8099917 was also associated with failure to respond to therapy (OR, 5.19; 95% CI, 2.90-9.30; P = 3.11 x 10(-8)), with the strongest effects in patients with HCV genotype 1 or 4. This risk allele was identified in 24% of individuals with spontaneous HCV clearance, 32% of chronically infected patients who responded to therapy, and 58% who did not respond (P = 3.2 x 10(-10)). Resequencing of IL28B identified distinct haplotypes that were associated with the clinical phenotype. CONCLUSIONS: The association of the IL28B locus with natural and treatment-associated control of HCV indicates the importance of innate immunity and interferon lambda in the pathogenesis of HCV infection.

Purpose: Diabetic myocardium is particularly vulnerable to develop heart failure in response to chronic stress conditions including hypertension or myocardial infarction. We have recently observed that angiotensin II (Ang II)-mediated downregulation of the fatty acid oxidation pathway favors occurrence of heart failure by myocardial accumulation of lipids (lipotoxicity). Because diabetic heart is exposed to high levels of circulating fatty acid, we determined whether insulin resistance favors development of heart failure in mice with Ang II-mediated myocardial remodeling.Methods: To study the combined effect of diabetes and Ang II-induced heart remodeling, we generated leptin-deficient/insulin resistant (Lepob/ob) mice with cardiac targeted overexpression of angiotensinogen (TGAOGN). Left ventricular (LV) failure was indicated by pulmonary congestion (lung weight/tibial length>+2SD of wild-type mice). Myocardial metabolism and function were assessed during in vitro isolated working heart perfusion.Results: Forty-eight percent of TGAOGN mice without insulin resistance exhibited pulmonary congestion at the age of 6 months associated with increased myocardial BNP expression (+375% compared with WT) and reduced LV power (developed pressure x cardiac output; -15%). The proportion of mice presenting heart failure was markedly increased to 71% in TGAOGN mice with insulin resistance (TGAOGN/Lepob/ob). TGAOGN/Lepob/ob mice with heart failure exhibited further increase of BNP compared with failing non-diabetic TGAOGN mice (+146%) and further reduction of cardiac power (-59%). Mice with insulin resistance alone (Lepob/ob) did not exhibit signs of heart failure or LV dysfunction. Myocardial fatty acid oxidation measured during in vitro perfusion was markedly increased in non-failing hearts from Lepob/ob mice (+380% compared with WT) and glucose oxidation decreased (-72%). In contrast, fatty acid and glucose oxidation did not differ from Lepob/ob mice in hearts from TGAOGN/Lepob/ob mice without heart failure. However, both fatty acid and glucose oxidation were markedly decreased (-47% and -48%, respectively, compared with WT/Lepob/+) in failing hearts from TGAOGN/Lepob/ob mice. Reduction of fatty acid oxidation was associated with marked reduction of protein expression of a number of regulatory enzymes implied in fatty acid oxidation.Conclusions: Insulin resistance favors the progression to heart failure during chronic exposure of the myocardium to Ang II. Our results are compatible with a role of Ang II-mediated downregulation of fatty acid oxidation, potentially promoting lipotoxicity.

In 1903, the eastern slope of Turtle Mountain (Alberta) was affected by a 30 M m3-rockslide named Frank Slide that resulted in more than 70 casualties. Assuming that the main discontinuity sets, including bedding, control part of the slope morphology, the structural features of Turtle Mountain were investigated using a digital elevation model (DEM). Using new landscape analysis techniques, we have identified three main joint and fault sets. These results are in agreement with those sets identified through field observations. Landscape analysis techniques, using a DEM, confirm and refine the most recent geology model of the Frank Slide. The rockslide was initiated along bedding and a fault at the base of the slope and propagated up slope by a regressive process following a surface composed of pre-existing discontinuities. The DEM analysis also permits the identification of important geological structures along the 1903 slide scar. Based on the so called Sloping Local Base Level (SLBL) an estimation was made of the present unstable volumes in the main scar delimited by the cracks, and around the south area of the scar (South Peak). The SLBL is a method permitting a geometric interpretation of the failure surface based on a DEM. Finally we propose a failure mechanism permitting the progressive failure of the rock mass that considers gentle dipping wedges (30°). The prisms or wedges defined by two discontinuity sets permit the creation of a failure surface by progressive failure. Such structures are more commonly observed in recent rockslides. This method is efficient and is recommended as a preliminary analysis prior to field investigation.

Background: Leptin is produced primarily by adipocytes. Although originally associated with the central regulation of satiety and energy metabolism, increasing evidence indicates that leptin may be an important factor for congestive heart faire (CHF). In the study, we aimed to test the hypothesis that leptin may influence CHF pathophysiology via a pathway of increasing body mass index (BMI). Methods: We studied 2,389 elderly participants aged 70 and older (M; 1161, F: 1228) without CHF and with serum leptin measures at the Health Aging, and Body Composition study. We analyzed the association between serum leptin level and risk of incident CHF using Cox hazard proportional regression models. Elevated leptin level was defined as more than the highest quartile (Q4) of leptin distribution in the total sample for each gender. Adjusted-covariates included demographic, behavior, lipid and inflammation variables (partially-adjusted models), and further included BMI (fully-adjusted models). Results: In a mean 9-year follow-up, 316 participants (13.2%) developed CHF. The partially-adjusted models indicated that men and women with elevated serum leptin levels (>=9.89 ng/ml in men and >=25 ng/ml in women) had significantly higher risks of developing CHF than those with leptin level of less than Q4. The adjusted hazard ratios (95%CI) for incident CHF was 1.49 (1.04 -2.13) in men and 1.71 (1.12 -2.58) in women. However, these associations became non-significant after adjustment for including BMI for each gender. The fully-adjusted hazard ratios (95%CI) were 1.43 (0.94 -2.18) in men and 1.24 (0.77-1.99) in women. Conclusion: Subjects with elevated leptin levels have a higher risk of CHF. The study supports the hypothesis that the influence of leptin level on risk of CHF may be through a pathway related to increasing BMI.

Terminal heart failure can be the cause or the result of major dysfunctions of the organisms. Although, the outcome of the natural history is the same in both situations, it is of prime importance to differentiate the two, as only heart failure as the primary cause allows for successful mechanical circulatory support as bridge to transplantation or towards recovery. Various objective parameters allow for the establishment of the diagnosis of terminal heart failure despite optimal medical treatment. A cardiac index <2.0 l/min, and a mixed venous oxygen saturation <60%, in combination with progressive renal failure, should trigger a diagnostic work-up in order to identify cardiac defects that can be corrected or to list the patient for transplantation with/without mechanical circulatory support.

PURPOSE: To analyze final long-term survival and clinical outcomes from the randomized phase III study of sunitinib in gastrointestinal stromal tumor patients after imatinib failure; to assess correlative angiogenesis biomarkers with patient outcomes. EXPERIMENTAL DESIGN: Blinded sunitinib or placebo was given daily on a 4-week-on/2-week-off treatment schedule. Placebo-assigned patients could cross over to sunitinib at disease progression/study unblinding. Overall survival (OS) was analyzed using conventional statistical methods and the rank-preserving structural failure time (RPSFT) method to explore cross-over impact. Circulating levels of angiogenesis biomarkers were analyzed. RESULTS: In total, 243 patients were randomized to receive sunitinib and 118 to placebo, 103 of whom crossed over to open-label sunitinib. Conventional statistical analysis showed that OS converged in the sunitinib and placebo arms (median 72.7 vs. 64.9 weeks; HR, 0.876; P = 0.306) as expected, given the cross-over design. RPSFT analysis estimated median OS for placebo of 39.0 weeks (HR, 0.505, 95% CI, 0.262-1.134; P = 0.306). No new safety concerns emerged with extended sunitinib treatment. No consistent associations were found between the pharmacodynamics of angiogenesis-related plasma proteins during sunitinib treatment and clinical outcome. CONCLUSIONS: The cross-over design provided evidence of sunitinib clinical benefit based on prolonged time to tumor progression during the double-blind phase of this trial. As expected, following cross-over, there was no statistical difference in OS. RPSFT analysis modeled the absence of cross-over, estimating a substantial sunitinib OS benefit relative to placebo. Long-term sunitinib treatment was tolerated without new adverse events.
