Enterococci are Gram-positive cocci saprophyte of the human gastrointestinal tract, diners who act as opportunistic pathogens. They can cause infections in patients hospitalized for a long time or who have received multiple antibiotic therapy. Enterococcus faecalis and Enterococcus faecium are the most common species in human infections. To evaluate the possibility of rapid detection of these species and their occurrence in the blood of newborns with suspected nosocomial infection, blood samples were collected from 50 newborns with late infections, admitted to the Neonatal Care Unit of the University Hospital Federal de Mato Grosso do Sul (UFMS-HU), from September 2010 to January 2011. The samples were subjected to conventional PCR and real time PCR (qPCR) to search for Enterococcus faecium and Enterococcus faecalis, respectively. The PCR results were compared with respective blood cultures from 40 patients. No blood cultures were positive for Enterococci, however, eight blood samples were identified as genomic DNA of Enterococcus faecium by qPCR and 22 blood samples were detected as genomic DNA of Enterococcus faecalis by conventional PCR. These findings are important because of the clinical severity of the evaluated patients who were found positive by conventional PCR and not through routine microbiological methods.

O carcinoma foi a principal causa de morte para cerca de 8,2 milhões de habitantes no ano de 2012 a nível mundial. Deste valor, estima-se que o carcinoma colo retal foi responsável por 694.000 mortes nesse mesmo ano. O tratamento eficaz deste carcinoma carece de uma deteção precoce de determinadas patologias associadas, nomeadamente, à presença de hemorragia ou pólipo na mucosa intestinal. Essa deteção precoce passa pela realização frequente de determinados exames clínicos invasivos, como a endoscopia e a colonoscopia, que poderão ser demasiado invasivos para o Homem para serem realizados regularmente. Assim surgiu a capsula endoscópica (CE) como método de diagnóstico pouco invasivo, confortável, seguro e com a funcionalidade de permitir a visualização de todo o trato gastrointestinal (TGI), uma vez que, com os métodos tradicionais de diagnóstico (endoscopia e colonoscopia) isso não acontece. Técnicas computacionais de processamento e análise de imagem automáticas, tais como filtros de suavização, remoção de ruído, deteção de contorno ou segmentação de zonas de interesse, podem ser utilizadas para facilitar a deteção destas patologias e homogeneizar a resposta entre diferentes clínicos, uma vez que, por cada exame de endoscopia por capsula são recolhidas cerca de 57 600 imagens. As imagens recolhidas a partir da CE passam por uma série de passos de processamento de imagem a m de clarificar a existência ou ausência de patologias no interior do TGI. Essa classificação pretende simplificar e auxiliar o clínico no diagnóstico precoce relativamente às patologias em causa, assim como reduzir o seu cansaço, aumentar a sua performance e aumentar a sua eficiência na análise de dados. Neste contexto e em parceria com a empresa INOVA+, esta tese está integrada no projeto PhotonicPill cofinanciado pelo QREN (Quadro de Referência Estratégico Nacional). Este projeto visa desenvolver um conjunto de módulos baseados em fotónica para incorporar numa CE, a m de possibilitar um diagnóstico mais preciso e atempado de diversas patologias, nomeadamente a presença de pólipos e hemorragias, assim como a possibilidade de terapêutica em locais do trato gastrointestinal de difícil acesso, como é o caso do intestino delgado. Um dos módulos baseados em fotónica assenta na tecnologia narrow band imaging (NBI). A contribuição desta tese no projeto prendeu-se no desenvolvimento de 3 métodos de deteção automática. O primeiro direcionado para a deteção de hemorragia, baseou-se na identificação dos valores mínimos e máximos dos canais de R,G,B para criar um valor de threshold duplo. De seguida, complementa-se o método através de operações morfológicas e operações locais. O segundo método de deteção automática é direcionado para a deteção de pólipo e baseou-se na aplicação da transformada de watershed juntamente com o cálculo de medidas associadas à forma típica de um pólipo. Por último, desenvolveu-se um método de deteção de vascularização na mucosa intestinal recorrendo essencialmente à deteção de valores máximos para cada canal do modelo RGB, definindo um valor de threshold máximo para cada um dos três canais. Uma vez testados os algoritmos e obtendo uma percentagem de especificidade e sensibilidade média superior a 70% em todos os métodos, desenvolveu-se um protótipo de uma interface gráfica para este sistema de apoio à decisão clínica que engloba os três parâmetros em análise: deteção de hemorragia, deteção de pólipo e deteção de vascularização. Esta interface fará toda a gestão do processo, ou seja, fara de forma automática a deteção e classificação das patologias a detetar, lançando uma mensagem de alerta ao clínico a informar se o paciente é ou não portador de alguma das anomalias em análise.

Background: Approximately 5% of the population donates blood each year in developed countries. Recruiting and maintaining a pool of altruistic and healthy blood donors is a challenging task. Blood donation as a dynamic process must naturally co-exist with the arguably essential deferrals. Aims: To analyse a 11-year cohort of donors and blood donations in order to determine the profile of the average donor and the typical donation. Characterize the donor’s population in terms of gender, age, number of donations, most common causes for deferral and exclusion and the possible relationships between them. Establish the tendency flow of donations per year. Methods: Analysis of 95861 blood donations from 31550 donors collected between 2000 and 2010 (11 years) in the Immunohemotherapy Department of the ‘‘Centro Hospitalar Lisboa Central - Hospital de Sa˜o Jose´’’ (Lisboa, Portugal). Prior to blood donation, donors were required to fill out a form of informed consent, a questionnaire of 21 ‘‘yes or no’’ questions and were submitted to a clinical assessment and physical examination including: measurement of weight, blood pressure, pulse and capillary hemoglobin levels. Post-donation, the collected blood was tested for ALT elevation and blood-borne viral agents (HBV, HCV, HIV 1 and 2 and HTLV 1 and 2) and other infections (Treponema pallidum). Blood donors and donations were registered in a database and statistically studied in terms of: gender and age distribution, number of donations, most common causes for deferral and exclusion. The frequency of blood donations throughout the period of observation was analyzed and statistically significant relationships between the collected variables were investigated. Results: From the population of 31550 donors 61% were male and a mean age of 41.5 years (± 12.5 years) was found. From the total of 95682 blood donations collected 78% were successful while the most common causes for deferral were: donation incompatible hemoglobin levels (5% of the blood donations and 22% of deferrals), ALT elevation (3% and 14%), positive blood screening test for Treponema pallidum (1% and 6%), medication (1% and 4%), positive serological blood markers for HBV (1% and 4%), endoscopy in the previous 12 months (1% and 3%), arterial hypertension (1% and 3%), infectious conditions (1% and 3%), influenza or influenza-like symptoms (1% and 2%) and positive serological blood markers for HCV (1% and 2%). Summary/Conclusions: Several factors may have contributed to a limited number of new regular donors in the population, namely: ageing population, the alienation of the individual from the community induced by modern lifestyles and job precariousness. It is of the utmost importance to refine our blood donation campaigns according to the existing population of donors. The optimization of the blood donation potential of a population of donors must be achieved through the development of reliable and consistent screening methods. In order to appeal to new donors it is important to promote blood donations considering the profile of the regular and healthy blood donor of the existing population.

This study aimed to determine the existence of blood vessels within ganglia of the myenteric plexus of the human esophagus and colon. At necropsy, 15 stillborns, newborns and children up to two years of age, with no gastrointestinal disorders, were examined. Rings of the esophagus and colon were analyzed and then fixed in formalin and processed for paraffin. Histological sections were stained by hematoxylin-eosin, Giemsa and immunohistochemistry for the characterization of endothelial cells, using antibodies for anti-factor VIII and CD31. Blood vessels were identified within the ganglia of the myenteric plexus of the esophagus, and no blood vessels were found in any ganglia of the colon. It was concluded that the ganglia of the myenteric plexus of the esophagus are vascularized, while the ganglia of the colon are avascular. Vascularization within the esophageal ganglia could facilitate the entrance of infectious agents, as well as the development of inflammatory responses (ganglionitis) and denervation, as found in Chagas disease and idiopathic achalasia. This could explain the higher frequency of megaesophagus compared with megacolon.

BACKGROUND: Upper gastrointestinal bleeding is the severe complication of stress-related mucosal disease in hospitalized patients. In intensive care units (ICU), risk factors are well defined and only mechanical ventilation and coagulopathy proved to be relevant for significant bleeding. On the contrary, in non-ICU settings there is no consensus about this issue. Nevertheless, omeprazole is still widely used in prophylaxis of bleeding. The objective of our study was to evaluate the relevance of stress-related mucosal disease bleeding in patients admitted to an internal medicine ward, and the role of omeprazole in its prophylaxis. METHODS: We conducted a retrospective study in which we analysed consecutive patients who were admitted to our ward over a year. We recorded demographic characteristics of the patients, potential risk factors for stress-related mucosal disease (clinical data, laboratory, and medication), administration of prophylactic omeprazole, and total cost of this prophylaxis. Patients with active gastrointestinal bleeding on the admission were excluded. We recorded every upper gastrointestinal bleeding event with clinical relevance. RESULTS: Five hundred and thirty-five patients, mean age 70 years, mean length of stay 9.6+/-7.7 days; 140 (26.2%) patients were treated with 40 mg of omeprazole intravenously, 193 (36.1%) with 20mg of omeprazole orally, and 202 (37.8%) patients had no prophylaxis. There was only one episode (0.2%) of clinically relevant bleeding. CONCLUSION: In patients admitted to an internal medicine ward, incidence of upper gastrointestinal bleeding as a complication of stress-related mucosal disease is low. We found that there is no advantage in prophylaxis with omeprazole.

The yeasts of the genus Candida infect skin, nails, and mucous membranes of the gastrointestinal and the genitourinary tract. The aim of this study was to determine the prevalence of dermatomycoses caused by Candida spp., and their etiological aspects in the metropolitan area of Porto Alegre, Brazil. A retrospective study with data obtained from tertiary hospital patients, from 1996 to 2011, was performed. The analyzed parameters were date, age, gender, ethnicity, anatomical region of lesions, and the direct examination results. For all the statistical analyses, a = 0.05 was considered. Among positive results in the direct mycological examination, 12.5% of the total of 4,815 cases were positive for Candida spp. The angular coefficient (B) was -0.7%/ year, showing a decrease over the years. The genus Candida was more prevalent in women (15.9% of women versus 5.84% of men), and in addition, women were older than men (54 versus 47 years old, respectively). There was no difference between ethnic groups. The nails were more affected than the skin, with 80.37% of the infections in the nails (72.9% in fingernails and 7.47% in toenails). Our study corroborates the literature regarding the preference for gender, age, and place of injury. Moreover, we found a decrease in infection over the studied period.

INTRODUCTION: Primary small bowel malignancy is unusual and accounts for 1-3% of all gastrointestinal tract neoplasms. Adenocarcinoma is one of the most common histologic types, but its frequency decreases with more distal locations. Its clinical presentation is nonspecific and is usually associated with advanced disease, which contributes to delayed diagnosis. PRESENTATION OF CASE: A 66-year-old woman was admitted to the hospital with a 6-day history of progressively worsening abdominal pain localized in the right lower quadrant, nausea, and vomiting. Investigation revealed an inflammatory appendiceal tumor. The patient underwent surgery and an unexpected tumor involving the distal ileal segment and ileocecal appendix was found. Right radical hemicolectomy with en bloc resection of the distal ileum was performed. Histopathological examination revealed adenocarcinoma of the ileum. DISCUSSION: This rare entity is associated with a nonspecific clinical presentation that contributes to delayed diagnosis and treatment, and consequently to a worse prognosis. Approximately half of the cases are only diagnosed at surgery. Primary treatment consists of wide resection with locoregional lymphadenectomy. The role of adjuvant chemotherapy has yet to be determined. CONCLUSION: This case demonstrates an unusual condition characterized by late and challenging diagnosis. We highlight the importance of an earlier diagnosis and optimal treatment for improved patient outcomes.

A enterocolite necrosante (ECN) constitui o problema gastrointestinal mais grave e mais frequente no recém-nascido (RN) de baixo peso. A melhoria na taxa de sobrevivência tem sido atribuída ao diagnóstico mais precoce e à experiência adquirida no tratamento do recém-nascido pré-termo em estado crítico. Desde 1977 que a drenagem peritoneal como actuação prioritária nos quadros de ECN tem sido preconizada nos recém- -nascidos de peso inferior a 1500 g com perfuração intestinal, e nos de peso superior a 1500 g com instabilidade hemodinâmica. Neste artigo relata-se o caso de um recém-nascido, com 1473 g de peso e 30 semanas de idade gestacional, ECN, sinais de perfuração intestinal e de instabilidade hemodinâmica, o qual foi submetido a drenagem peritoneal com evolução favorável e sem sequelas. Na discussão faz-se referência especial, de acordo com dados de literatura, aos mecanismos que explicam os bons resultados do procedimento em cerca de 2/3 dos casos de ECN com perfuração, os quais estão relacionados com as características particulares da cicatrização nos tecidos imaturos. Em conclusão, admite-se que a drenagem peritoneal deverá constituir a forma de actuação prioritária nos casos de ECN com perfuração e instabilidade hemodinâmica em RN pré-termo de muito baixo peso.

Intramural dissecting hematoma is an unusual esophageal condition with a threatening presentation but excellent prognosis when managed conservatively.We report the case of an 88-year-old woman who developed an intramural hematoma of the esophagus after intravenous thrombolysis for an acute ischemic stroke. Before thrombolysis, nasogastric intubation was attempted unsuccessfully. She was kept on nil by mouth, intravenous hydration, proton pump inhibitor, antiemetics,and an antibiotic initiated 2 days before for periodontal disease. The esophageal hematoma regressed, and she resumed oral diet asymptomatically.To our knowledge, this is the first report of this type of lesion after thrombolysis for an ischemic stroke. A brief discussion and literature review are presented.

Although the standard of care for cow’s milk (CM) allergy is strict food avoidance, oral immunotherapy (OIT) is being widely investigated as an alternative management option in certain cases. Immediate adverse reactions to OIT have been described, but its long-term effects are much less often reported. We present the case of a girl diagnosed with IgE-mediated CM allergy that was proposed for our CM OIT protocol at the age of 3 years. The first sessions (dose escalation up to 5 ml) were well tolerated, however eight hours after her daily morning dose of 5ml CM the child developed late episodes of vomiting. No other symptoms, particularly immediately after CM ingestion, were reported. These episodes became progressively worse and on the third day she presented mild dehydration and blood eosinophilia. After OIT interruption, a progressive clinical improvement was observed. An esophageal endoscopy was performed, showing signs of eosinophilic esophagitis (EoE) with peak 20 eosinophils/hpf. After treatment with topical swallowed fluticasone (500 mcg bid) and a CM-free diet for 4 months, the child was asymptomatic and endoscopy and biopsy findings were normal. The long-term effects of milk OIT are still in part unknown. We hypothesize that eosinophilic esophagitis may have been a consequence of OIT in this case. The findings seem to indicate that food allergy may play a role in the pathogenesis of esophageal eosinophilia and stress the importance of a well programmed long-term follow-up of patients that have undergone milk OIT.

A colecistite aguda acalculosa é uma entidade pouco frequente na idade pediátrica e uma complicação rara da enterocolite por salmonelas não tifóides. A co-infecçãocom o virus influenza A (H1N1)v nunca foi previamente descrita. Caso Clinico: Rapaz de 10 anos de idade, previamente saudável, com febre elevada, cefaleias , mialgias, vómitos e diarreia. a RT-PCR para virus influenza A, subtipo H1N1v foi postiva no exsudado nasofaríngeo mas não foi prescrito oseltamivir. Ao quinto dia da doença por persistência do quadro clinico com agravamento da sintomatologia gastrointestinal recorreu à urgência. Foi internado com sinais de desidratação grave, insuficiência pré-renal aguda e oligúria, tendo-se procedido a analgesia , antipiréticos e hidratação endovenosa. Foi isolada Salmonella Enteriditis nas coproculturas. Ao 9º dia de doença houve agravamento da dor abdominal associando-se sinal de Murphy. A ecografia abdominal revelou sinais de colecistite aguda e adenopatia a comprimir infundíbulo da vesicula biliar tendo-se iniciado antibioterapia. Os sintomas persisitiram durante cinco dias, verificando-se posterior evolução clinica favorável. Discussão: A etiologia da colecistite acalculosa foi provavelmente multifatorial para a qual contribuíram a febre, a desidratação, os analgésicos, o jejum prolongado e a infecção por salmonela, cujo ciclo envolve a via biliar. A compressão pelos gânglios hipertrofiados na sequência da infecção H1N1v foi provavelmente um fator adicional, sendo questionável se a terapêutica prévia com oseltamivir teria modificado o prognóstico.
