A hepatite C é um dos principais problemas de saúde pública no Brasil e no mundo, tendo alta prevalência em algumas populações específicas, inclusive em pacientes com insuficiência renal crônica, submetidos à hemodiálise. A disseminação do Vírus da Hepatite C (VHC) neste ambiente pode estar relacionada a diversos fatores, como transfusões sanguíneas freqüentes, tempo de duração do tratamento, compartilhamento de máquinas, cateteres e linhas de diálise, e à dificuldade do diagnóstico da infecção, sobretudo nas fases iniciais, quando ainda não ocorreu a soroconversão de anticorpos (anti-VHC). O objetivo deste trabalho foi descrever a soroprevalência de anti-VHC em pacientes renais crônicos submetidos à hemodiálise no Estado do Amapá, e correlacioná-los com os fatores de risco. Para este fim, foram avaliados 103 prontuários de pacientes do serviço de hemodiálise da unidade de nefrologia do Hospital de Clínicas Dr. Alberto Lima, em Macapá, Amapá, em dezembro de 2007. Os resultados das sorologias para o anti-HCV foram relacionados com os fatores de risco. Os resultados mostraram baixa prevalência de anti-VHC (4,8%), concluindo-se, estarem relacionados à baixa freqüência de transfusões sanguíneas, ao diagnóstico precoce da soroconversão, à não reutilização de máquinas e insumos, à adoção de “boas práticas” nos procedimentos e ao não compartilhamento de frações de injetáveis.

The present study reports a snakebite in a horse in the state of Pará, Brazil. At initial evaluation the animal was reluctant to walk and had tachycardia, tachypnea, severe lameness, bleeding on the pastern and swelling around the left hind leg. Blood samples from the bleeding sites, took on the first day, showed leukocytosis and neutrophilia, whereas biochemical values of urea and creatinine were significantly increased. The chosen treatment was snake antivenom, fluid therapy, antibiotics, anti-inflammatory agents and diuretic drugs. On the fourth day of therapy, the hematological values were within normal parameters. There was improvement related to the clinical lameness and swelling of the limb. However, a decrease in water intake and oliguria were observed. On the seventh day the animal died. Necropsy revealed areas of hemorrhagic edema in the left hind limb and ventral abdomen; the kidneys presented equimosis in the capsule, and when cut they were wet. Moreover, the cortex was pale, slightly yellow and the medullary striae had the same aspect. Based on these data, we concluded that the snakebite in the present study was caused by Bothrops spp. and that renal failure contributed to death.

Com o objetivo de desenvolver um modelo de hemodiálise (HD) em cães, foram estudados 18 animais, sem raça definida, machos, clinicamente sadios, com peso corporal variando entre sete e 14 kg. O acesso vascular foi obtido através de implantação do cateter de duplo lúmen em veia jugular externa. As sessões de HD, em número de cinco por animal, com até três horas de duração, foram realizadas em hemodialisadora de sistema proporcional com ultrafiltração (UF) controlada, com solução dialisante padrão e tampão bicarbonato. A UF foi ajustada para HD isovolêmica, utilizou-se perfil de sódio, e para anticoagulação heparina sódica. Os animais foram mantidos anestesiados com cloridrato de levomepromazina e propofol. Foram avaliados dados hematológicos, bioquímicos, hemogasometria, pressão arterial sistêmica e tempo de coagulação ativado. Foi observada diminuição do número global de hemácias, volume globular, hemoglobina e leucócitos. Em relação aos exames bioquímicos, houve manutenção nos níveis de sódio sérico, e quanto à hemogasometria, a manutenção da SO2. A pressão arterial sistêmica manteve-se constante. Os resultados obtidos no presente trabalho permitiram concluir que foi possível o desenvolvimento do modelo proposto e mostrou que a HD em cães é um método viável e seguro, que poderá contribuir para o tratamento clínico da insuficiência renal nesta espécie.

OBJECTIVE: Pneumoperitoneum during laparoscopy results in transient oliguria and decreased glomerular filtration and renal blood flow. The presence of oliguria and elevated serum creatinine is suggestive of acute renal injury. Serum cystatin C has been described as a new marker for the detection of this type of injury. In this study, our aim was to compare the glomerular filtration rate estimated using cystatin C levels with the rate estimated using serum creatinine in patients with normal renal function who were undergoing laparoscopic surgery. METHODS: In total, 41 patients undergoing laparoscopic cholecystectomy or hiatoplasty were recruited for the study. Blood samples were collected at three time intervals: first, before intubation (T1); second, 30 minutes after the establishment of pneumoperitoneum (T2); and third, 30 minutes after deflation of the pneumoperitoneum (T3). These blood samples were then analyzed for serum cystatin C, creatinine, and vasopressin. The Larsson formula was used to calculate the glomerular filtration rate based on the serum cystatin C levels, and the Cockcroft-Gault formula was used to calculate the glomerular filtration rate according to the serum creatinine levels. Although enzymuria tends to be associated to renal injury, there are no studies that have evaluated the presence of the enzyme gamma-glutamyl transpeptidase (GGT) spectrophotometry in the urine using a non-nephrotoxic agent (Nerium oleander) in order to evaluate the possibility of false positive results. The urinary GGT/urinary creatinine concentration ratio (uGGT/uCr) of 10 healthy dogs was calculated and posteriorly confronted with data from clinical evaluation, hematological and serum biochemical profiles, creatinine clearance (CrC), urinalysis, urine protein/creatinine ratio (UPC), electrocardiogram, systemic blood pressure (SBP) and light and electron microscopy. The results for kidney histology, SBP, UPC and CrC were not significantly different in any of the time-points analyzed. However, uGGT/uCr was significantly higher when measured 4 hours and 24 hours after administration of N. oleander. The measurement of the urinary GGT enzyme, as performed in many studies, yielded false positive results in dogs poisoned by a non-nephrotoxic agent.

Introduction: The Diabetic Nephropathy (DN) affects approximately 40% of patients diagnosed with DM is associated with increased mortality from cardiovascular phenomena and is considered the main cause of Chronic Renal Failure (CRF) in patients dialectics. Methods: Searches were performed on Medline, SciELO, Lilacs and Cochrane databases using the crossing between the key-words: “genetic polymorphism” and “diabetic nephropathy”. Results: The selected studies indicated that diabetic nephropathy is the leading cause of chronic renal failure, which significantly reduces the life expectancy of diabetics. Currently, some factors may have connection with DN. They are: genetic predisposition based on family history, hypertension, and cardiovascular events, quality of glycemic control and lipid levels and blood pressure and smoking. Conclusion: Studies constants are essential to add new elements in the literature for the definition (s) of factor (s) gene (s) specific (s) of diabetic nephropathy.

The uric gout has been often diagnosed in cockatiel, birds that has been popularized and frequently shown in the avian practice. This disease is a consequence of renal failure. In the Wildlife Clinic, a specialized facility in Sao Paulo, 15 cockatiel presenting unspecific signs of polyuria and polidipsia were followed and diagnosed as uric gout. These animals had as common feature being lutin breed and fed with commercial pellet diet. In the present work nutritional aspects and causes of this disease are discussed. The main application of laboratory tests in avian practice and uric gout treatment were exemplified
