Cintra stream is the major receiver of treated effluent from sanitary, hospital and research laboratory sewage from the UNESP campus in Botucatu, Rubião Júnior District, São Paulo state; and it has shown evidence of diffuse contamination by metals along it. Analyses of the most toxic metals (Pb, Cu, Fe, Ni and Zn) were carried out by using an atomic absorption spectrometer AA-6.800 - Perkin Elmer, analyst 700. ANOVA followed by the Tukey test were used for data analyses at 5% significance level. The main objective of this study was to compare current data on metal levels with data from other studies, in the same area, and of the same nature, conducted before and after implementation of chemical residue management by the university in 2008. Also, it aimed at identifying and quantifying possible diffuse and punctual contamination. Sites S1 and S2 showed punctual contamination, while diffuse sources were observed between S4 and S5. The quality of water in the Cintra Stream improved after establishment of the program of laboratory chemical residue management in the Campus of UNESP.

Two of the major problems caused by construction activity are the production of construction and demolition waste (CDW) and the exploitation of mineral resources, causing big impacts on the environment. Therefore, the recycling has been shown as an alternative to mitigate the harmful effects of waste on the urban environment and prevent the exploitation of new raw materials. This course work aims to study the behavior of recycled aggregates from Vale do Paraíba in concrete and mortar. Initially, it presents the definitions of recycled aggregates according to CONAMA Resolution No. 307/2002, the aggregate settings for concrete and mortar (such as the grain size, its origin and density, and the characterization parameters according to ABNT), and the definition of ACI method of concrete mix design. Afterwards, it presents the characterization of materials separated by assays. After that, it shows the theoretical concrete proportioning applying the ACI method and experimental concrete proportioning. Then, the analysis of results is performed to finally conclude that the materials provided can't be used to replace natural aggregates because they cannot have the same performance. With the studies, it could be observed that the recycled aggregate presents a great complexity and diversity in origin, therefore the form how the material should be handled requires great care

O propósito deste artigo é compartilhar com o leitor algumas noções conceituais de publicidade contra-intuitiva e observar seus possíveis efeitos na reavaliação, desconstrução de crenças e estereótipos sociais, na estrutura cognitiva do indivíduo receptor. Destaca-se entre os reflexos provavelmente gerados o irônico efeito ricochete, segundo a teoria desenvolvida por Daniel M. Wegner. A aplicação para se discutir o cruzamento da narrativa contra-intuitiva e o efeito ricochete será, neste primeiro momento, pela exemplificação do filme Motorista, peça integrante da campanha publicitária da Fiat do Brasil “Reveja seus conceitos”, para o lançamento do automóvel Palio 2002. Um experimento laboratorial está sendo desenvolvido pelos autores para se mensurar de maneira consistente a apresentação teórico-conceitual exposta

Imbalanced matrix metalloproteinase (MMP) expression, including MMP-2, has been demonstrated in pre-eclampsia. However, little is known about the effect of polymorphisms in MMP-2 gene on hypertensive disorders of pregnancy. We examined whether two functional MMP-2 polymorphisms (g.-1306C>T and g.-735C>T) are associated with pre-eclampsia and/or gestational hypertension and whether these polymorphisms affect therapeutic responses in women with these conditions. We studied 216 healthy pregnant women (HP), 185 patients with gestational hypertension (GH) and 216 patients with pre-eclampsia (PE). They were stratified as responsive or non-responsive to antihypertensive therapy according to clinical and laboratorial parameters of therapeutic responsiveness. Genomic DNA was extracted from whole blood and genotypes for g-1306C>T and g.-735C>T polymorphisms were determined by real-time PCR using Taqman allele discrimination assays. Haplotype frequencies were inferred using the PHASE 2.1 program. The distributions of MMP-2 genotypes and haplotypes were similar in HP, GH and PE patients (p > 0.05). In addition, we found no significant differences in MMP-2 genotype or haplotype frequencies when GH or PE patients were classified as responsive or non-responsive to antihypertensive therapy (p > 0.05). Our results suggest that MMP-2 polymorphisms do not affect the susceptibility to hypertensive disorders of pregnancy. In parallel, MMP-2 polymorphisms apparently do not affect the responsiveness to antihypertensive therapy of women with these hypertensive disorders of pregnancy.

Objective: To evaluate the prevalence of risk factors for cardiovascular disease in Japanese-Brazilian subjects. Subjects and methods: One hundred thirty-one residents of the Mombuca community were studied. Statistical analysis was based on the X-2 test, Fisher's Exact test, Student's t test, and ANOVA, at a 5% significance level. Results: The average age was 56.7 years-old; 76.3% had dyslipidemia, 24.4% pre-diabetes (PDM), 10.7% type 2 diabetes mellitus (T2DM), 46.6% hypertension, 52.7% abdominal obesity, and 35.8% metabolic syndrome (MS). There were significant correlations between HOMA-IR and MS diagnosis and obesity, while HOMA-beta levels were decreased in T2DM and PDM. The ankle-brachial index was positive for peripheral artery disease in 22.3% of the individuals. Electrocardiograms did not show increased evidence of myocardial ischemia. Conclusion: Subjects of this community are exposed to major cardiovascular risk factors, namely high prevalence of MS diagnoses and increased HOMA-IR. Arq Bras Endocrinol Metab. 2012; 56(9): 608-13

The objective of this study was to analyze the nurses' perceptions regarding tuberculosis control, according to the theoretical axis of comprehensive healthcare and the concepts of attachment and teamwork. This qualitative study involved 13 nurses from the Family Health Strategy of a priority city in the metropolitan region of Joao Pessoa, Paraiba, Brazil. Data were collected in focal groups and subjected to thematic content analysis. Factors that strengthen tuberculosis control were: supervised treatment, free medication and the provision of supplies. Weakening factors were: worker's turnover, the lack of encouragement for patients and incipient educational actions. The factors that, according to the nurses, weaken tuberculosis patient care should be reviewed by administrators, workers, users and educators with a view to redefining healthcare activities that strengthen attachment, comprehensive healthcare and teamwork.

PURPOSE: To evaluate the nutritional status of patients in the late postoperative period of pancreaticoduodenectomy (PD) and compare the long-term outcome according to pylorus-preserving (PPPD) or the standard technique (SPD) in which the pylorus is resected. METHODS: This prospective study was conducted twelve months prior or more in patients who had underwent PD (PD Group, n=15) and health volunteers (Control Group, n=15). At a post hoc analysis, the PD Group was divided in PPPD Subgroup (n=9) and SPD Subgroup (n=6), according to the PD techniques. Gastrointestinal complaints and nutritional status were evaluated, apart from a biochemical assessment; Student t-test or Mann-Whitney test were used. RESULTS: The patients recovered their body weight and the gastrointestinal complaints were uncommon. The PD Group showed higher energy and protein intake even though BMI was lower than in Control Group. There were no differences in laboratorial data, except for higher glycemia, serum alkaline phosfatase and C-reactive protein in PD Group. There was no difference in the various parameters evaluated when the Subgroups (PPPD and SPD) were compared. CONCLUSION: For long-term pancreaticoduodenectomy, the gastrointestinal symptoms are minimal and the patients had the clinical and nutritional status preserved, regardless of pylorus preservation.

Introduction: Modern life has imposed to people sedentary habits and excessive food consumption, what results into an increase of the incidence of metabolic diseases, which promote the development of atherosclerosis. Objectives: We aimed to evaluate the metabolic profile of diabetic patients assisted by the endocrinology service from Ceara Federal University, located in Barbalha, Brazil. Methods: This is a transversal and retrospective study, based on the analysis of patient records. 119 records were reviewed and 35 of them were selected, according to their registers about anthropometric and laboratorial measuring. Results and discussion: Among the selected records, 65.71% were female patients. It was observed a positive relationship between age and the level of triglycerides, between LDL-cholesterol and the use of tobacco and between blood glucose and glycated hemoglobin. Conclusion: The superposition of risk factor in this group shows the necessity of an integrated assistance and a follow-up about their metabolic profile, aiming to mitigate or retard serious circulatory pathologies.

Maturity-onset diabetes of the young (MODY) is characterized by an autosomal dominant mode of inheritance, early onset of hyperglycemia, and defects of insulin secretion. MODY subtypes described present genetic, metabolic, and clinical differences. MODY 2 is characterized by mild asymptomatic fasting hyperglycemia, and rarely requires pharmacological treatment. Hence, precise diagnosis of MODY is important for determining management and prognosis. We report two heterozygous GCK mutations identified during the investigation of short stature. Case 1: a prepubertal 14-year-old boy was evaluated for constitutional delay of growth and puberty. During follow-up, he showed abnormal fasting glucose (113 mg/dL), increased level of HbA1c (6.6%), and negative beta-cell antibodies. His father and two siblings also had slightly elevated blood glucose levels. The mother had normal glycemia. A GCK heterozygous missense mutation, p.Arg191Trp, was identified in the proband. Eighteen family members were screened for this mutation, and 11 had the mutation in heterozygous state. Case 2: a 4-year-old boy investigated for short stature revealed no other laboratorial alterations than elevated glycemia (118 mg/dL); beta-cell antibodies were negative. His father, a paternal aunt, and the paternal grandmother also had slightly elevated glycemia, whereas his mother had normal glycemia. A GCK heterozygous missense mutation, p.Glu221Lys, was identified in the index patient and in four family members. All affected patients had mild elevated glycemia. Individuals with normal glycemia did not harbor mutations. GCK mutation screening should be considered in patients with chronic mild early-onset hyperglycemia, family history of impaired glycemia, and negative beta-cell antibodies. Nonalcoholic fatty liver disease (NAFLD) is one of the comorbidities related to obesity. Liver biopsy has been used as the "gold standard" for the diagnosis, grading, and prognosis of obese patients. The objective of the present study was to evaluate clinical predictors of more advanced stages of NAFLD. In this retrospective study we assessed several physical and laboratorial factors, including some cytokines, in morbidly obese patients submitted to Roux-en-Y gastric bypass that could be related to the diagnosis and staging of NAFLD. Fragments of the livers were obtained from wedge biopsies during operation. The medical records of 259 patients were studied. The patients were divided into four groups: normal hepatic biopsy, steatosis, mild nonalcoholic steatohepatitis (NASH), and moderate and severe NASH. There were no differences in cytokine levels among groups. The triglyceride levels were the only variable that could stratify the grades of NAFLD and also differentiate from normal livers in the female patients. Also in this group, the aminotransferases and GGT levels and fasting glucose were predictors of the more advanced stages of NASH, while BMI and weight were predictors of the more advanced stages of NASH in male patients. There are no available markers in clinical practice to detect the initial stages of NAFLD. It is very important to perform a liver biopsy in all patients submitted to bariatric surgery and in obese patients with no indication to be operated in the presence of elevated blood levels of aminotransferases, GGT, and fasting glucose.

Patient, 75 years-old, with free wall rupture of the right ventricle, corrected with prolene 3.0 points anchored in bovine pericardium patch, promoting the closure of the rupture. The patient was discharged on the 59th day after surgery in good clinical ans laboratorial conditions.
