PURPOSE: To clarify the risk parameters measured by anterior segment optical coherence tomography (AS-OCT) for elevated intraocular pressures (IOP) provoked by the darkroom test and to provide recommendations for its clinical usage. METHODS: Subjects aged >40 years and whose peripheral anterior chambers were ≤1/4 corneal thickness were recruited. The anterior segment of the eye was imaged in sitting position and under both light and dark conditions and biometry was performed using anterior segment optical coherence tomography. The analyzed parameters were: (1) central anterior chamber depth (ACD); (2) anterior chamber width; (3) pupil diameter; (4) iris curvature; (5) lens thickness; and (6) number of meridians with closed angles (NCA). Then the darkroom test was performed and a positive provocative test result was defined as a rise in IOP ≥8 mm Hg after the test. Statistical analyses included: (1) the difference in parameters between positive and negative eyes; (2) the association between posttest IOP and the parameters; and (3) the difference in parameters between the 2 eyes in subjects with the asymmetric results. RESULTS: A total of 70 subjects were recruited. ACD (P=0.022), NCA in light (P<0.001), and NCA in dark (P<0.001) were different significantly between eyes with positive and negative results. There was a strong association between NCA in dark (r=0.755, P<0.001) and the posttest IOP. Among subjects with asymmetric results between the 2 eyes, the ACD was shallower and the lens thickness was larger in the positive eye. CONCLUSIONS: The posttest IOP is determined by the extent of functionally closed angles in the dark. The test may be useful in the early diagnosis of primary angle closure. At the same time, angle configuration should be evaluated to remove false positive result.

Introduction Institutionalization in a nursing home restricts autonomy, most notably free will, free choice, and free action. Decreased physical activity and fitness are predictive of disability and dependence (Rikli & Jones, 2013; Tak, Kuiper, Chorus, & Hopman-Rock, 2014); however little is known about the impact of these factors on institutionalization. Thus, this study aimed to analyze the impact of physical activity and fitness and on the risk of elderly people without cognitive impairment become institutionalized. Methods This cross-sectional study involved 195 non-institutionalized (80.14.4yrs) and 186 institutionalized (83.85.2yrs) participants. Cognitive impairment was assessed using Mini-Mental State Examination, physical activity was assessed using the International Physical Activity Questionnaire, and measures of physical fitness were determined by the Senior Fitness Test. Results: Multivariate binary logistic analysis selected 4 main predictors of institutionalization in both genders. The likelihood of becoming institutionalized increased by +18.6% for each additional year of age, while it decreased by -24.8% by each fewer kg/m2 in BMI, by -0.9% for each additional meter performed in the aerobic endurance test and by -2.0% for each additional 100MET-min/wk of physical activity expenditure (p<0.05). Values ≤50th percentile (age ≥81yrs, BMI≥26.7kg/m2, aerobic endurance ≤367.6m, and physical activity ≤693MET-min/wk) were computed using Receiver Operating Characteristics analysis as cut-offs discriminating institutionalized from non-institutionalized elderly people. Conclusion The performance of physical activity, allied to an improvement in physical fitness (mainly BMI and aerobic endurance) may prevent the institutionalization of elderly people without cognitive impairment only if they are above the 50th percentile; the following is highly recommend: expending ≥693MET-min/wk on physical activity, being ≤26.7kg/m2 on BMI, and being able to walk ≥367.6m in the aerobic endurance test, especially above the age of 80 years. The discovery of this trigger justifies the development of physical activity programs targeting the pointed cut-offs in old, and very old people. References Rikli, R., & Jones, C. (2013). Este trabalho pretende analisar a relação entre o envolvimento da família e o sucesso educativo das crianças. A hipótese principal foi construída para testar as teorias de Ana Henderson (1987), Davies (1989) e Epstein (2002). A primeira parte deste trabalho é constituída pelo enquadramento teórico, onde se abordam a problemática da relação escola/família, da participação e do sucesso e insucesso dos alunos. A segunda parte é constituída pela metodologia, análise e interpretação dos dados e análise e discussão dos resultados, onde estão incluídas as conclusões e as sugestões. O estudo foi feito nas escolas do 1° Ciclo do Agrupamento Vertical de Colos, relativamente aos alunos do 3° e 4° ano, no ano lectivo de 2004/2005. A amostra foi constituída por 67 pais e a recolha de informação foi feita através de um questionário. O tratamento de dados foi feito de forma estatística utilizando o SPSS, Os resultados do presente estudo levam-nos a concluir que os pais têm uma fraca participação na escola e que o sucesso educativo dos alunos não depende da participação dos pais na sua vida académica, não se confirmando a hipótese formulada para dar resposta à pergunta inicial. Concluímos ainda que, quanto maior é a distância entre a casa dos alunos e a escola frequentada, melhores resultados e menos retenções estes exibem ao longo do seu percurso escolar. /ABSTRACT - This work intends to analyse the relation between the role of the family and the educational success of children. The main assumption was built to test the theories of Ana Anderson (1987), Davies (1989) and Epstein (2002). The first part of the essay includes the theoretical framing where the subject of the relation school/family is approached. The second part contains the methodology, analysis and interpretation of data and the analysis and discussion of the results, which includes the conclusions and the suggestions. This survey was made in the Primary Schools of “Agrupamento Vertical de Colos”, among students of the third and fourth grades in the school year of 2004/2005. The sample was made of sixty-seven parents and the data was gathered by a questionnaire. The treatment of this data was made statistically by using SPSS. The results of this survey take us to the conclusion that parents have a low participation in school and that the educative success of students does not depend on their parents’ participation in their academic life. So, the hypothesis formulated to answer the initial question is not confirmed. We can also conclude that, the longer the distance between the students' home and the school they attend, the better the results and fewer times they fail in their school life.

Cachexia is a complex syndrome characterized by severe weight loss frequently observed in cancer patients and associated with poor prognosis. Cancer cachexia is also related to modifications in cardiac muscle structure and metabolism leading to cardiac dysfunction. In order to better understand the cardiac remodeling induced by bladder cancer and the impact of exercise training after diagnosis on its regulation, we used an animal model of bladder cancer induced by exposition to N-butyl-N-(4-hydroxybutyl)-nitrosamine (BBN) in the drinking water. Healthy animals and previously BBN exposed animals were submitted to a training program in a treadmill at a speed of 20m/min, 60 min/day, 5 days/week during 13 weeks. At the end of the protocol, animals exposed to BBN presented a significant decrease of body weight, in comparison with control groups, supporting the presence of cancer cachexia. Morphological analysis of the cardiac muscle sections revealed the presence of fibrosis and a significant decrease of cardiomyocyte’s cross-sectional area, suggesting the occurrence of cardiac dysfunction associated with bladder cancer. These modifications were accompanied by heart metabolic remodeling characterized by a decreased fatty acid oxidation given by diminished levels of ETFDH and of complex II subunit  from the respiratory chain. Exercise training promoted an increment of connexin 43, a protein involved in cardioprotection, and of c-kit, a protein present in cardiac stem cells. These results suggest an improved heart regenerative capacity induced by exercise training. In conclusion, endurance training seems an attractive non-pharmacological therapeutic option for the management of cardiac dysfunction in cancer cachexia.

Neste estudo, procuro compreender quais as atitudes e emoções de alunos do 4.º ano do ensino básico na resolução de desafios matemáticos, num ambiente extra-curricular. Para focar o problema de investigação, formulei as seguintes questões: i) Quais as atitudes de alunos do 4.º ano perante desafios matemáticos fora do contexto de sala de aula?; ii) Que tipo de emoções manifestam os alunos no âmbito ‘não escolar’?; iii) Que tipo de interacções se desenvolvem neste ambiente? e iv) Qual a relação entre estas interacções e as atitudes e emoções dos alunos fora da sala de aula? O quadro teórico ressalta a importância de considerarmos o domínio afectivo nas relações dos alunos com a Matemática e, consequentemente, no seu desempenho escolar. Mostra a complexidade de se atingir um consenso na definição de conceitos, na determinação dos conceitos a ter em conta e das fronteiras que os separem. Por último, esclareço o que entendo por atitude, emoção e desafio matemático. A experiência educacional desenvolveu-se uma vez por semana durante aproximadamente uma hora em horário extra-curricular, entre os meses de Outubro e Fevereiro de 2008/2009. Foram trabalhados treze desafios matemáticos, num total de dezassete sessões, com treze alunos voluntários do 4.º ano de escolaridade. Optei por uma metodologia de estudo de caso, dentro do paradigma de investigação qualitativa. A recolha de dados foi feita através de uma entrevista, dois questionários, observação participante e gravação áudio das sessões e registo das emoções expressas pelos alunos. Da análise dos dados, concluí que a principal preocupação dos alunos não foi apenas solucionar cada desafio, mas sim resolvê-lo no menor tempo possível, mostrando-se os participantes bastante competitivos entre si. O contentamento e o fascínio surgiram, na maioria dos casos, associados a emoções que viveram no meio e no fim da actividade. Os alunos mostraram-se empolgados quando lhes foram apresentados novos desafios e desanimados quando tinham de dar continuidade a uma tarefa já iniciada e para a qual ainda não tinham obtido êxito. Sentiram uma certa frustração quando falharam e alguns revelaram-se pouco autónomos e pouco confiantes. As emoções, tal como foram declaradas pelos alunos, não parecem traduzir fielmente as suas dificuldades, pelo contrário, sugerem um certo agrado dos alunos por definir livremente estratégias, testá-las e tirar conclusões.

Background: Providing an effective exercise prescription process for patients with non-specific chronic low back pain (NSCLBP) is a challenging task. Emerging research has indicated that partnership in care and shared decision making are important for people with NSCLBP and calls for further investigation into the approaches used to prescribe exercise. Objective: To explore how shared decision making and patient partnership are addressed by physiotherapists in the process of exercise prescription for patients with NSCLBP. Design: A qualitative study using a philosophical hermeneutic approach. Methods: Eight physiotherapists were each observed on three occasions undertaking their usual clinical activities (total n=24 observations). They conducted brief interviews after each observation and a later in depth semi-structured interview. Iterative hermeneutic strategies were used to interpret the texts and identify the characteristics and processes of exercise prescription for patients with NSCLBP. Findings: The findings revealed how physiotherapy practice often resulted in unequal possibilities for patient participation which were in turn linked to the physiotherapists? assumptions about the patients, clinical orientation, cognitive and decision making processes. Three linked themes emerged: (1) I want them to exercise, (2) Which exercise? - the tension between evidence and everyday practice and (3) Compliance-orientated more than concordance based. Conclusions: This research, by focusing on a patient-centred approach, makes an important contribution to the body of evidence relating to the management of NSCLBP. It challenges physiotherapists to critically appraise their approaches to the prescription of exercise therapy in order to improve outcomes for these patients.

Introdução: Apesar dos muitos estudos sobre a temática da sensação retardada de desconforto muscular, atualmente, ainda se discute a explicação dos mecanismos subjacentes a esta condição clínica, bem como, a sua prevenção e tratamento. A literatura sugere a massagem como uma das formas de terapia, contudo, os estudos têm mostrado resultados controversos. Objetivo: Verificar se a massagem aplicada 2 horas após um protocolo de exercício excêntrico tem influência na sensação retardada de desconforto muscular, bem como, se o seu efeito varia dependendo do tempo de aplicação. Métodos: 21 participantes (23,62±1,32 anos; 76,95±12,17 kg; 174,71±4,78 cm; 25,25±4,26 Kg/m 2) foram divididos em três grupos. Foi avaliada a dor, força muscular e a perimetria antes, e 2h, 24h, 48h, e 72h após um protocolo de exercício constituído por três séries de dez repetições de contrações excêntricas dos isquiotibiais do membro dominante, com 80% da força máxima, a uma velocidade constante de 60º/s, numa amplitude entre 0º e 80º, utilizando o dinamómetro isocinético Biodex System 4. A massagem foi efetuada 2 horas após o exercício em dois grupos experimentais com durações diferentes, sendo o terceiro grupo de controlo. Para identificar diferenças entre os grupos no momento inicial e na variável diferença entre o momento inicial e os restantes momentos, recorreu-se ao teste de Kruskal-Wallis, seguido de uma análise Post-Hoc através do teste de Dunn com um nível de significância de 0,05. Resultados: Verificou-se que a massagem teve efeito na redução da dor e na perimetria. Relativamente à força não foram encontradas alterações significativas. Conclusão: Os resultados mostraram que a massagem aplicada 2 horas após o exercício excêntrico, independentemente da duração utilizada, teve efeito na redução da dor, mas não na força muscular. Na perimetria apesar de haver alterações, estas não foram consideradas relevantes.

Introdução – O efeito de êmbolo é um dos principais problemas relacionados com a eficácia de uma prótese. Uma diminuição do mesmo pode levar a uma marcha mais natural através do aumento da propriocetividade. Objetivos – Verificar se existe diferença de valores do efeito de êmbolo entre vários sistemas de suspensão para próteses transtibiais com a utilização de Liners e testar a aplicação de testes de imagiologia na análise da melhor solução protésica para um determinado indivíduo. Metodologia – Foi obtida uma radiografia da prótese em carga na posição ortostática, mantendo o peso do indivíduo igualmente distribuído pelos dois pés. Seguidamente foi realizada outra radiografia no plano sagital com o joelho com 30° de flexão, com a prótese suspensa e um peso de 5kg aplicado na extremidade distal da mesma durante 30 seg. Através destes dois exames efetuaram-se as medições do êmbolo para cada tipo de sistema de suspensão. Resultados – Dos quatro sistemas estudados apenas três apresentam valores de êmbolo, visto que um dos sistemas não criou suspensão suficiente para suportar o peso colocado na extremidade distal da prótese. Através das medições realizadas nos exames imagiológicos dos três sistemas pudemos encontrar variações de efeito de êmbolo que vão dos 47,91mm aos 72,55mm. Conclusão – Através da realização do estudo imagiológico verificaram-se diferenças a nível do efeito de êmbolo nos vários sistemas de suspensão, provando que esta é uma ferramenta viável na avaliação do mesmo. Também através da análise dos resultados ficou notório que o sistema de suspensão Vacuum Assisted Suspention System (VASS) é o que apresenta menos êmbolo.

While the IEEE 802.15.4/Zigbee protocol stack is being considered as a promising technology for low-cost low-power Wireless Sensor Networks (WSNs), several issues in the standard specifications are still open. One of those ambiguous issues is how to build a synchronized multi-hop cluster-tree network, which is quite suitable for ensuring QoS support in WSNs. In fact, the current IEEE 802.15.4/Zigbee specifications restrict the synchronization in the beacon-enabled mode (by the generation of periodic beacon frames) to star-based networks, while it supports multi-hop networking using the peer-to-peer mesh topology, but with no synchronization. Even though both specifications mention the possible use of cluster-tree topologies, which combine multihop and synchronization features, the description on how to effectively construct such a network topology is missing. This paper tackles this problem, unveils the ambiguities regarding the use of the cluster-tree topology and proposes a synchronization mechanism based on Time Division Beacon Scheduling to construct cluster-tree WSNs. We also propose a methodology for an efficient duty cycle management in each router (cluster-head) of a cluster-tree WSN that ensures the fairest use of bandwidth resources. The feasibility of the proposal is clearly demonstrated through an experimental test bed based on our own implementation of the IEEE 802.15.4/Zigbee protocol.

Debugging electronic circuits is traditionally done with bench equipment directly connected to the circuit under debug. In the digital domain, the difficulties associated with the direct physical access to circuit nodes led to the inclusion of resources providing support to that activity, first at the printed circuit level, and then at the integrated circuit level. The experience acquired with those solutions led to the emergence of dedicated infrastructures for debugging cores at the system-on-chip level. However, all these developments had a small impact in the analog and mixed-signal domain, where debugging still depends, to a large extent, on direct physical access to circuit nodes. As a consequence, when analog and mixed-signal circuits are integrated as cores inside a system-on-chip, the difficulties associated with debugging increase, which cause the time-to-market and the prototype verification costs to also increase. The present work considers the IEEE1149.4 infrastructure as a means to support the debugging of mixed-signal circuits, namely to access the circuit nodes and also an embedded debug mechanism named mixed-signal condition detector, necessary for watch-/breakpoints and real-time analysis operations. One of the main advantages associated with the proposed solution is the seamless migration to the system-on-chip level, as the access is done through electronic means, thus easing debugging operations at different hierarchical levels.

Chronic Low Back Pain (CLBP) is a public health problem and older women have higher incidence of this symptom, which affect body balance, functional capacity and behavior. The purpose of this study was to verifying the effect of exercises with Nintendo Wii on CLBP, functional capacity and mood of elderly. Thirty older women (68 ± 4 years; 68 ± 12 kg; 154 ± 5 cm) with CLBP participated in this study. Elderly individuals were divided into a Control Exercise Group (n = 14) and an Experimental Wii Group (n = 16). Control Exercise Group did strength exercises and core training, while Experimental Wii Group did ones additionally to exercises with Wii. CLBP, balance, functional capacity and mood were assessed pre and post training by the numeric pain scale, Wii Balance Board, sit to stand test and Profile of Mood States, respectively. Training lasted eight weeks and sessions were performed three times weekly. MANOVA 2 x 2 showed no interaction on pain, siting, stand-up and mood (P = 0.53). However, there was significant difference within groups (P = 0.0001). ANOVA 2 x 2 showed no interaction for each variable (P > 0.05). However, there were significant differences within groups in these variables (P < 0.05). Tukey's post-hoc test showed significant difference in pain on both groups (P = 0.0001). Wilcoxon and Mann-Whitney tests identified no significant differences on balance (P > 0.01). Capacity to Sit improved only in Experimental Wii Group (P = 0.04). In conclusion, physical exercises with Nintendo Wii Fit Plus additional to strength and core training were effective only for sitting capacity, but effect size was small.
