Drugs and surgery can interfere with the biodistribution of radiopharmaceuticals and data about the effect of splenectomy on the metabolism of phytate-Tc-99m are scarce. This study aimed at evaluating the interference of splenectomy on phytate-Tc-99m biodistribution and liver function in rats. The SP group rats (n=6) underwent splenectomy. In group C (control) the animals were not operated on. After 15 days, all rats were injected with 0.1mL of Tc-99m-phytate via orbital plexus (0.66MBq). After 30 minutes, liver samples were harvested, weighed and the percentage of radioactivity per gram (%ATI-g) was determined by a Wizard Perkin-Elme gama counter. The ATI%-g in splenectomized rats (0.99±0.02) was significantly higher than in controls (0.4±0.02), (p=0.034). ALT, AST and HDL were significantly lower in SP rats (p= 0.001) and leukocytosis was observed in SP rats. In conclusion, splenectomy in rats changed the hepatic biodistribution of Tc-99m-phytate and liver enzimatic activity

Les enfants atteints de déficience motrice cérébrale (DMC) intégrés en milieu scolaire ordinaire vivent davantage de difficultés relatives à la compétence sociale que les autres enfants. En lien avec le modèle SOCIAL de Beauchamp et Anderson (2010) (The socio-cognitive integration of abilities model), deux questions sont abordées afin de comprendre ces difficultés. Dans le premier article, les attributions d’intentions (AI) hostiles sont mesurées par le Home Interview with Child (HIWC), un ensemble de vignettes représentants des situations sociales ambigües. Les résultats montrent que l’acceptation sociale, plus que le statut de naissance, est associé à l’hostilité des AI à 15 ans. Les enfants rejetés et populaires ont un niveau d’AI hostiles supérieur aux enfants de la catégorie d’acceptation sociale moyenne. Il est proposé que l’acceptation sociale soit un médiateur important à considérer dans le développement d’un biais d’AI hostile chez les enfants atteints de DMC. Dans le deuxième article, la dominance sociale (i.e., une forme de leadership) est évaluée dans une situation de résolution de problème en équipe. Les comportements relatifs à la dominance sociale (i.e., les comportements prosociaux et coercitifs) sont codifiés à l’aide d’une grille créée et validée pour cette étude. Les résultats révèlent que, sans égard au score d’acceptation sociale, les enfants atteints de DMC effectuent significativement moins de comportements prosociaux et coercitifs que les enfants sans DMC au cours de la résolution de problème. De plus, leur équipe de travail émet un nombre de comportements prosociaux et coercitifs inférieur à celui observé dans l’équipe des deux enfants témoins. Les difficultés de compétence sociale observées chez les jeunes atteints de DMC pourraient être en partie expliquées par une faible dominance sociale lors d’activités sollicitant les fonctions exécutives. Enfin, les deux articles présentés permettent d’explorer de nouvelles avenues pour comprendre et soutenir le développement de la compétence sociale chez les enfants et les adolescents atteints de DMC. Les interventions auprès de cette clientèle devraient avoir lieu tôt et devraient cibler autant les habiletés motrices de ces jeunes que leur compétence sociale, avant que des signaux clairs ne compromettent déjà leur expérience sociale globale en milieu scolaire ordinaire.

To evaluate the biodistribution of sodium pertecnetate (Na99mTcO4) in organs and tissues, the morphometry of remnant intestinal mucosa and ponderal evolution in rats subjected to massive resection of the small intestine. Methods: Twenty-one Wistar rats were randomly divided into three groups of 7 animals each. The short bowel (SB) group was subjected to massive resection of the small intestine; the control group (C) rats were not operated on, and soft intestinal handling was performed in sham rats. The animals were weighed weekly. On the 30th postoperative day, 0.l mL of Na99mTcO4, with mean activity of 0.66 MBq was injected intravenously into the orbital plexus. After 30 minutes, the rats were killed with an overdose of anesthetic, and fragments of the liver, spleen, pancreas, stomach, duodenum, small intestine, thyroid, lung, heart, kidney, bladder, muscle, femur and brain were harvested. The biopsies were washed with 0.9% NaCl.,The radioactivity was counted using Gama Counter WizardTM 1470, PerkinElmer. The percentage of radioactivity per gram of tissue (%ATI-g) was calculated. Biopsies of the remaining jejunum were analysed by HE staining to obtain mucosal thickness. Analysis of variance (ANOVA) and the Tukey test for multiple comparisons were used, considering p<0.05 as signifi cant. Results: There were no signifi cant differences in %ATI-g of the Na99mTcO4 in the organs of the groups studied (p>0.05). An increase in the weight of the SB rats was observed after the second postoperative week. The jejunal mucosal thickness of the SB rats was signifi cantly greater than that of C and sham rats (p<0.05). Conclusion: In rats with experimentally-produced short bowel syndrome, an adaptive response by the intestinal mucosa reduced weight loss. The biodistribution of Na99mTcO4 was not affected by massive intestinal resection, suggesting that short bowel syndrome is not the cause of misleading interpretation, if an examination using this radiopharmaceutical is indicated

Background: Acute lower extremity compartment syndrome (CS) is a condition that untreated causes irreversible nerve and muscle ischemia. Treatment by decompression fasciotomy without delay prevents permanent disability. The use of intracompartmental pressure (iCP) measurement in uncertain situations aids in diagnosis of severe leg pain. As an infrequent complication of lower extremity trauma, consequences of CS include chronic pain, nerve injury, and contractures. The purpose of this study was to observe the clinical and functional outcomes for patients with lower extremity CS after fasciotomy. Methods: Retrospective chart analysis for patients with a discharge diagnosis of CS was performed. Physical demographics, employment status, activity at time of injury, injury severity score, fracture types, pain scores, hours to fasciotomy, iCP, serum creatine kinase levels, wound treatment regimen, length of hospital stay, and discharge facility were collected. Lower extremity neurologic examination, pain scores, orthopedic complications, and employment status at 30 days and 12 months after discharge were noted. Results: One hundred twenty‑four patients were enrolled in this study. One hundred and eight patients were assessed at 12 months. Eighty‑one percent were male. Motorized vehicles caused 51% of injuries in males. Forty‑one percent of injuries were tibia fractures. Acute kidney injury occurred in 2.4%. Mean peak serum creatine kinase levels were 58,600 units/ml. Gauze dressing was used in 78.9% of nonfracture patients and negative pressure wound vacuum therapy in 78.2% of fracture patients. About 21.6% of patients with CS had prior surgery. Nearly 12.9% of patients required leg amputation. Around 81.8% of amputees were male. Sixty‑seven percent of amputees had associated vascular injuries. Foot numbness occurred in 20.5% of patients and drop foot palsy in 18.2%. Osteomyelitis developed in 10.2% of patients and fracture nonunion in 6.8%. About 14.7% of patients underwent further orthopedic surgery. At long‑term follow‑up, 10.2% of patients reported moderate lower extremity pain and 69.2% had returned to work. Conclusion: Escalation in leg pain and changes in sensation are the cardinal signs for CS rather than reliance on assessing for firm compartments and pressures. The severity of nerve injury worsens with the delay in performing fasciotomy. Standardized diagnostic protocols and wound treatment strategies will result in improved outcomes from this complication.

L’encéphalopathie néonatale (EN) affecte plus de 0.8% des nouveau-nés à terme et est la deuxième cause d’handicap neurologique durant l’enfance. Le traitement des enfants atteints d’EN consiste en une prise en charge symptomatique et en l’hypothermie (HT). Néanmoins, plus de 50% des patients traités par HT gardent des séquelles neurologiques majeures. Les données provenant du Canadian Cerebral Palsy Registry montrent que l’HT prévient seulement 4% des cas de paralysie cérébrale (PC). Le fait que l’HT ait des effets neuroprotecteurs chez certains nouveau-nés, mais pas chez tous les nouveau-nés atteints d’EN n’est pas encore totalement élucidé. Il a été rapporté que les nouveau-nés exposés à l’inflammation (sepsis néonatal, infections placentaires) et à l’hypoxie-ischémie (HI) semblent avoir moins d’effets bénéfiques de l’HT que ceux qui ont été seulement exposés à l’HI. Des études précliniques ont établi que l’HT avait des effets neuroprotecteurs dans les cas d’encéphalopathies hypoxique-ischémiques chez les nouveau-nés à terme ou peu prématurés (plus de 35 semaines d’âge gestationnel). Cependant, peu d’études ont investigué les effets de l’HT lorsque l’EN résulte de la combinaison d’HI et d’infection/inflammation, alors qu’il s’agit du scénario pathophysiologique le plus fréquemment rencontré chez les nouveau-nés humains atteints d’EN. Ceci nous a amené à tester les effets neuroprotecteurs de l’HT dans un modèle animal (rat) d’EN induite par l’inflammation et l’HI. Des ratons Lewis au jour postnatal 12 sont injectés avec du lipopolysaccharide (LPS) d’E.coli afin de recréer l’inflammation, puis sont soumis à l’ischémie (ligature de la carotide commune droite) et à l’hypoxie (8% O[indice inférieur 2], 1h30). Les ratons sont ensuite traités ou non par l’HT (32 ± 0.5°C, 4 h). L‘étendue des lésions cérébrales ainsi que les cascades inflammatoires et oxydatives ont été étudiées. Nos résultats montrent que l’HT prévient l’étendue des lésions dans la composante de pénombre ischémique (néocortex et hippocampe). Cet effet neuroprotecteur est indépendant d’un effet anti-inflammatoire cérébral de l’HT portant sur le système de l’interleukine-1. Nos données indiquent que l’HT module l’expression des enzymes responsables de l’élimination des réactifs de l’oxygène (ROS). En conclusion, nos résultats montrent que l'HT a des effets neuroprotecteurs dans un modèle d’EN induite par l’exposition combinée au LPS et à l’HI. Ce projet devrait ouvrir des perspectives translationnelles pour prévenir la PC au-delà de l'hypothermie seule, comme par exemple celle visant à tester le bénéfice de la combinaison thérapeutique d’HT jointe au blocage de cytokines pro-inflammatoires.

Aim. Reoperative thyroid surgery is an uncommon operation associated with a higher complication rate; we reviewed our series of patients on whom reoperative thyroid surgery was performed.Method. 106 patients had a thyroid reoperation for recurrent multinodular goiter (93 patients), recurrent thyrotoxicosis (3) or suspected malignancy (10); bilateral completion thyroidectomy was performed in 68 cases, lobectomy in 36, removal of a mediastinal recurrence and of a pyramidal remnant in 1 patient respectively. Results. Temporary hypoparathyroidism occurred in 41 patients (38.67%), definitive in 7 (6.6%), transient recurrent laryngeal nerve palsy in 5 (4.71%), permanent nerve palsy in 1 (0.94%); in 3 cases (2.83%) surgical revision of haemostasis was necessary for postoperative haemorrhage. After monolateral surgery we had 13 cases of transient hypoparathyroidism (34.21%), 2 of definitive (5.26%) and 1 transient recurrent laryngeal nerve palsy (2.63%); after bilateral surgery we had 29 cases of transient hypoparathyroidism (42.64%), 5 of definitive (7.35%), 4 of transient recurrent laryngeal nerve palsy (5.88%), 1 of definitive (1.47%) and 3 of postoperative bleeding (4.41%). Conclusions. Reoperative thyroid surgery is a technical challenge with a high incidence of complications. Scarring, edema and friability of the tissues together with distortion of the landmarks make reoperative surgery azardous. A higher risk of complications is described when previous surgery has been performed on both sides. Total thyroidectomy should be considered the procedure of choice for benign multinodular goiter eliminating the potential of a reoperation. Whenever necessary, reoperative hyroidectomy may be performed safely with little morbidity in experienced hands.

Arnold–Chiari malformation is defined as downward displacement of the brainstem and cerebellum through the foramen magnum. It has different clinical presentations and four subtypes. It is known that downward migration of posterior fossa components through the foramen magnum and associated lower cranial nerve palsy and brainstem compression can cause respiratory failure. Acute respiratory failure could mark the onset of the disease. Posterior fossa decompression performed to treat primary disease can improve the central sleep abnormalities. As respiratory failure is rarely seen, this paper presents two cases of Arnold–Chiari malformation with respiratory failure.

The biodistribution of sodium pertechnetate, the most used radiopharmaceutical in nuclear medicine, has not been studied in details after bariatric surgery. The objective was to investigate the effect of Roux-en-Y gastric bypass (RYGB) on biodistribution of sodium pertechnetate (Na99mTc-) in organs and tissues of rats. Methods: Twelve rats were randomly divided into two groups of 6 animals each. The RYGB group rats were submitted to the Roux-en-Y gastric bypass and the control group rats were not operated. After 15 days, all rats were injected with 0.1mL of Na99mTc- via orbital plexus with average radioactivity of 0.66 MBq. After 30 minutes, liver, stomach, thyroid, heart, lung, kidney and femur samples were harvested, weighed and percentage of radioactivity per gram (%ATI/g) of each organ was determined by gama counter Wizard Perkin-Elmer. We applied the Student t test for statistical analysis, considering p<0.05 as significant. Results: Significant reduction in mean %ATI/g was observed in the liver, stomach and femur in the RYGB group animals, compared with the control group rats (p<0.05). In other organs no significant difference in %ATI/g was observed between the two groups. Conclusion: This work contributes to the knowledge that the bariatric surgery RYGB modifies the pattern of biodistribution of Na99mTc

Purpose: Many patients with metastatic bone disease have to use radiopharmaceuticals associated with chemotherapy to relieve bone pain. The aim of this study was to assess the influence of docetaxel on the biodistribution of samarium-153-EDTMP in bones and other organs of rats. Methods: Wistar male rats were randomly allocated into 2 groups of 6 rats each. The DS (docetaxel/samarium) group received docetaxel (15 mg/kg) intraperitoneally in two cycles 11 days apart. The S (samarium/control) group rats were not treated with docetaxel. Nine days after chemotherapy, all the rats were injected with 0.1ml of samarium-153-EDTMP via orbital plexus (25μCi). After 2 hours, the animals were killed and samples of the brain, thyroid, lung, heart, stomach, colon, liver, kidney and both femurs were removed. The percentage radioactivity of each sample (% ATI/g) was determined in an automatic gamma-counter (Wizard-1470, Perkin-Elmer, Finland). Results: On the 9th day after the administration of the 2nd chemotherapy cycle, the rats had a significant weight loss (314.50±22.09g) compared (p<0.5) to pre-treatment weight (353.66± 22.8). The % ATI/g in the samples of rats treated with samarium-153-EDTMP had a significant reduction in the right femur, left femur, kidney, liver and lungs of animals treated with docetaxel, compared to the control rats. Conclusion: The combination of docetaxel and samarium-153-EDTMP was associated with a lower response rate in the biodistribution of the radiopharmaceutical to targeted tissues. Further investigation into the impact of docetaxel on biodistribution of samarium-153-EDTMP would complement the findings of this study
