A serological survey for Chagas' disease was carried out in school children in the Rio de Janeiro State, a zone considered as non-endemic for the infection. A total of 168 schools in 20 municipalities have been visited and 13,254 blood samples were obtained. The blood eluates were screened by the indirect fluorescence test (IFT), and all positive samples were checked and confirmed in sera by the complement fixation test (CFT). AH serologically positive children were subject to a clinical scrutiny, and the houses where the children lived have been searched for triatomine bugs. Only in two municipalities, Magé and Araruama, there was a significant number of children found positive. The total number of reactive samples by IFT and CFT from 13,004 blood samples screened was 143 (1.00 per cent). No serious clinicai symptoms suggestive of Chagas' disease have been found in any of the positive children, and no triatomine bugs were discovered in the dwellings where the children lived. The overall small percentage of children with positive serology postulates that the infection is not a serious health problem in the area investigated. It is recommended, however, to carry out a more detailed study in Magé and Araruama to find the reason for the relatively high percentage of serologically positive children encountered in these two municipalities.

The authors report a case of a male patient from Bacabal, MA with diffuse cutaneous leishmaniasis (DCL), for at least nine years, with 168 lesions on his body. These were tumour-like nodules with some ulceratmi. He usedpentavalent antimonial (glucantime®) and an association of gamma interferon plus glucantime with improvement of the lesions but relapsed later. Recently, pentamidine isethionate (pentacarinat®) was given a dosage of 4mg/kg/weight/day on alternate days for 20 applications. After 3 months a similar course of 10 application was given 2 times. Later he developed diabetic signs with weight loss of 10kg, polydypsia, polyuria and xerostomia. The lower limbs lesions showed signs of activity. Blood glucose levels normalised and remain like this at moment. Attention is drawn to the fact that pentamidine isethionate should be used as a therapy option with care, obeyng rigorous laboratory controls including a glucose tolerance test.

RESUMO - Objetivo: Quantificar a variação da procura ocorrida no Serviço de Urgência Hospitalar (SUH) de um hospital na zona de Lisboa face ao aumento de preço da taxa moderadora da urgência em janeiro de 2012. Metodologia: O presente trabalho recorre à análise de micro dados sobre a utilização do Serviço de Urgência do Hospital Garcia de Orta (HGO) em dois períodos: 1 de janeiro de 2011 a 30 de junho de 2011 e 1 de janeiro de 2012 a 30 de junho de 2012. A amostra é constituída por 156.654 idas ao SUH do HGO. Aferiu-se ainda a elasticidade da procura face ao preço por sexo, escalão etário, proveniência, local e causa da admissão e destino dos utentes. Resultados: Existiram 80.344 episódios de urgência em 2011 e 76.310 em 2012 (-5%).Em relação aos utentes não isentos, houve uma redução de 12% no total de episódios de urgência (26.168 em 2011 e 23.037 em 2012). O preço da urgência aumentou 108% para os indivíduos não isentos (€9,6 para €20). Os valores obtidos para a elasticidade da procura face ao preço são próximos de zero para o total da procura bem como para as restantes variáveis. Conclusões: Conclui-se que a procura de cuidados de urgência é inelástica face ao aumento do preço no hospital analisado. Embora se tenha verificado uma redução dos cuidados procurados (12%), esta foi muito inferior ao aumento ocorrido no preço (108%).

Mannans (linear mannan, glucomannan, galactomannan and galactoglucomannan) are the major constituents of the hemicellulose fraction in softwoods and show great importance as a renewable resource for fuel or feedstock applications. As complex polysaccharides, mannans can only be degraded through a synergistic action of different mannan-degrading enzymes, mannanases. Microbial mannanases are mainly extracellular enzymes that can act in wide range of pH and temperature, contributing to pulp and paper, pharmaceutical, food and feed, oil and textile successful industrial applications. Knowing and controlling these microbial mannan-degrading enzymes are essential to take advantage of their great biotechnological potential. The genome of the laboratory 168 strain of Bacillus subtilis carries genes gmuA-G dedicated to the degradation and utilization of glucomannan, including an extracellular -mannanase. Recently, the genome sequence of an undomesticated strain of B. subtilis, BSP1, was determined. In BSP1, the gmuA-G operon is maintained, interestingly, however, a second cluster of genes was found (gam cluster), which comprise a second putative extracellular β-mannanase, and most likely specify a system for the degradation and utilization of a different mannan polymer, galactoglucomannan. The genetic organization and function of the gam cluster, and whether its presence in BSP1 strain results in new hemicellulolytic capabilities, compared to those of the laboratory strain, was address in this work. In silico and in vivo mRNA analyses performed in this study revealed that the gam cluster, comprising nine genes, is organized and expressed in at least six different transcriptional units. Furthermore, cloning, expression, and production of Bbsp2923 in Escherichia coli was achieved and preliminary characterization shows that the enzyme is indeed a β-mannanase. Finally, the high hemicellulolytic capacity of the undomesticated B. subtilis BSP1, demonstrated in this work by qualitative analyses, suggests potential to be used in the food and feed industries.

Relata-se os resultados de uma investigação sobre a possibilidade da existência de memória em flebotomíneos em área endêmica de leishmaniose tegumentar americana, no Estado do Paraná. Capturaram-se flebotomíneos no Recanto Marista, Doutor Camargo, Estado do Paraná, Brasil, com armadilhas de Falcão, de 15/11/2007 a 26/11/2007, em dois galinheiros (G1 e G2). Foram capturados 2.080 flebotomíneos (1.000 em G1 e 1.080 em G2), que foram marcados e soltos. Nyssomyia neivai foi a espécie mais (90,5%) freqüente. Do total solto, recapturaram-se 168 (8%) flebotomíneos e a recaptura no G2 foi significativa. Os resultados evidenciam que é possível a existência de memória espacial, olfativa e/ou a fidelidade ao hospedeiro e que isto orientem os flebotomíneos no reconhecimento dos locais onde há disponibilidade de fontes de sangue.

INTRODUCTION: Acute bacterial meningitis (ABM) remains a public health problem in Brazil. To evaluate the epidemiology of ABM cases at Giselda Trigueiro Hospital, Rio Grande do Norte, a descriptive retrospective survey was conducted covering 2005 to 2008. METHODS: Clinical and laboratory data were collected from the epidemiology department of the hospital and analyzed. RESULTS: Out of 168 ABM cases, 24.4%, 10.7%, and 2.4% were, respectively, caused by Streptococcus pneumoniae, Neisseria meningitidis and Haemophilus influenza b, and 5.4% by other bacteria. The mean age was 22.48 ± 18.7 years old. CONCLUSIONS: Streptococcus pneumoniae was the main causative pathogen in the young urban population.

RESUMO - Introdução: A saúde oral é uma componente essencial na saúde geral e no bem-estar dos indivíduos. Sabe-se que os problemas de saúde oral afectam predominantemente os elementos de níveis socioeconómicos mais baixos, evidenciando a influência dos determinantes sociais da saúde na saúde oral das populações. Os objectivos deste estudo são caracterizar os comportamentos de rotinas diárias de higiene oral, frequências de idas a consultas de saúde oral, auto-avaliação do estado de saúde oral e percepção de dor na cavidade oral em crianças de 12 anos em Portugal e analisar a associação entre estes e os factores sociodemográficos. Métodos: Foi realizado um estudo observacional, transversal e analítico, abrangendo 1309 jovens e baseado em informação recolhida no III Estudo Nacional de Prevalência de Doenças Orais (ENPDO). Para além das estatísticas descritivas usuais, as estatísticas inferenciais basearam-se predominantemente em modelos de regressão logística binária. Resultados: Dos participantes, 70.6% (n=924) escova “duas ou mais vezes por dia” com associação com todas as variáveis sociodemográficas. Na análise multivariada, o género masculino (OR=2.088; IC95%: 1.574-2.770, em relação ao género feminino), a área de residência predominantemente rural ou mediamente urbana (OR= 1.800; IC95%: 2.587; OR=1.516; IC95%: 1.093-2.103, em relação a zonas predominantemente urbanas), a escolaridade da mãe ser o ensino básico (OR= 2.112; IC95%: 1.408-3.168, em relação ao ensino superior) e a actividade laboral do pai ser desempregado (OR= 1.938; IC95%: 1.280-2.934, em relação a ser trabalhador) foram as variáveis com mais impacto para a adopção de comportamentos de escovagem potencialmente inadequados (p<0.05). A maioria dos inquiridos (94.2%; n=1247) já tinham ido a uma consulta de saúde oral e 74.5% (n=860) nos últimos 12 meses, 95.5% (n=1250) encontram-se satisfeitos com a saúde oral e 44.5% (n=578) afirma ter tido algum tipo de dor na cavidade oral nos últimos 12 meses. Conclusão: Os resultados obtidos estão de acordo com a literatura em termos de factores de associação. Desta forma, a saúde oral nos jovens de 12 anos em Portugal, nos diversos contextos aqui analisados, pode ser considerada como satisfatória. A única excepção relevante é a componente da dor, com valores alarmantes embora de natureza mais subjectiva. A influência dos factores sociodemográficos sugere que futuras abordagens para a promoção da saúde oral tenham em conta os determinantes de saúde no delineamento de estratégias quer a nível individual quer a nível comunitário.

INTRODUCTION: In this study, we evaluated the seroprevalence of Helicobacter pylori infection among chagasic and non-chagasic subjects as well as among the subgroups of chagasic patients with the indeterminate, cardiac, digestive, and cardiodigestive clinical forms. METHODS: The evaluated subjects were from the Triângulo Mineiro region, Minas Gerais, Brazil. Chagasic patients showed positive reactions to the conventional serological tests used and were classified according to the clinical form of their disease. Immunoglobulin G antibodies specific to H. pylori were measured using a commercial enzyme-linked immunosorbent assay kit. RESULTS: The overall H. pylori prevalence was 77.1% (239/310) in chagasic and 69.1% (168/243) in non-chagasic patients. This difference was statistically significant even after adjustment for age and sex (odds ratio = 1.57; 95% confidence interval, 1.02-2.42; p = 0.04) in multivariate analysis. The prevalence of infection increased with age in the non-chagasic group (p = 0.007, χ2 for trend), but not in the chagasic group (p = 0.15, χ2 for trend). H. pylori infection was not associated with digestive or other clinical forms of Chagas disease (p = 0.27). CONCLUSIONS: Our findings demonstrate that chagasic patients have a higher prevalence of H. pylori compared to non-chagasic subjects; a similar prevalence was found among the diverse clinical forms of the disease. The factors contributing to the frequent co-infection with H. pylori and Trypanosoma cruzi as well as its effects on the clinical outcome deserve further study.

INTRODUCTION: Infection by Neisseria meningitidis, termed as meningococcal disease, can cause meningococcal meningitis and septicemia with or without meningitis. Meningococcal disease is endemic in Brazil and has a high potential to cause large-scale epidemics; therefore, it requires the immediate notification of cases to the Information System for Notifiable Diseases (SINAN) in Brazil. The aim of this study was to describe an epidemiological profile using data from notified and confirmed cases in the State of Minas Gerais, Brazil, from January 2000 to December 2009, obtained from the investigation records of individuals with meningitis registered with SINAN. METHODS: This was a retrospective, population-based study. Descriptive analysis of the data was made using the simple and relative frequencies of the categorical variables in the investigation records. RESULTS: There were 1,688 confirmed patients in Minas Gerais of which 45.5% lived in the Central, North, and Triângulo Mineiro regions. The highest frequencies of cases were in the 1-4-years age group (26.3%), males (54.7%), caucasian (36.4%), and lived in an urban area (80%). In the patients with specified education, 650 (60.9%) patients had secondary education. Serogrouping of meningococci had been performed in 500 (29.6%) patients by age and gender; 285 (57%) belonged to serogroup C, 67 (13.4%) were in the 1-to 4-years age group, and 168 (33.6%) were male. CONCLUSIONS: The epidemiological profiles of patients in the Central, North, and Triângulo Mineiro regions were not significantly different from the profile of patients in Minas Gerais.

Introduction This study compares virulence markers of Helicobacter pylori isolated from patients in 2 cities in the Brazilian Amazon. Methods The study analyzed 168 patients with chronic gastritis from Belém and 151 from Bragança, State of Pará, Brazil. Levels of bacterial DNA associated with cagA and vacA alleles were checked by PCR, and hematoxylin-eosin staining was used for histologic diagnosis. Results In Bragança 87% of patients were genotype s1m1 cagA-positive (s1m1 cagA+), compared with 76% in Belém. In samples from patients in both cities, there was an association between s1m1 cagA+ strains and gastric mucosal damage. Conclusions Both cities have a high frequency of s1m1 cagA+ strains of H. pylori.
