Diplomityössä selvitettiin Andritz Oy:n Pulp & Paper Mill Services -divisioonan myynnin analysointiin tarkoitettuja työkaluja ja tunnuslukuja varaosa- ja projektiliiketoiminnassa. Diplomityön aikana toteutetut haastattelut ja sähköpostin välityksellä lähetettävä kyselytutkimus kohdennettiin ennen kaikkea myynnistä vastuussa oleville ihmisille, kuten myyntipäälliköille, divisioonan johtohenkilöille ja tuoteryhmäjohtajille. Diplomityön aikana rakennettiin myynnin analysoinnissa käytettävät raportit SAP Business Intelligence -tietojärjestelmään. Tulevaisuudessa kohdeorganisaatio pystyy diplomityön aikana rakennettujen myynnin analysointiraporttien avulla seuraamaan ja analysoimaan myyntiä. Tutkimus oli luonteeltaan kvalitatiivinen, jossa ensin luotiin teoreettinen viitekehys tutkimalla Business Intelligence:ä, varaosa- ja projektiliiketoimintaa sekä myynnin analysoinnissa käytettäviä työkaluja. Tämän jälkeen suoritettiin kysely- ja haastattelututkimukset, joiden pohjalta saatiin tietoa kuinka Andritz:lla tällä hetkellä myyntiä analysoidaanja kuinka sitä halutaan tulevaisuudessa analysoida. Työn tuloksena saatiin teoriaa sekä haastattelu- ja kyselytutkimusta yhdistämällä rakennettua SAP Business Intelligence -tietojärjestelmään myynnin analysoinnissa käytettävät raportit. Raportit ovat myynnin perusraportti sisältäen myynnin keskei-simmät tunnusluvut, abc-analyysi myynnistä asiakkaittain ja abc-analyysi myynnistä nimikkeittäin. Työn lopussa on myös pohdittu, mitä SAP Business Intelligence -tietojärjestelmään kannattaa tulevaisuudessa rakentaa raporttien osalta ja kuinka raportteja kannattaa lähteä hyödyntämään myynnin analysoinnin näkökulmasta.

Leukocytes are cells of defense. Their main function is to protect our body against invading microorganisms. Some leukocytes, in particular, polymorphonuclear and monocytes, accumulate at sites of infection and neutralize pathogens through innate mechanisms. The blood and lymphatic vascular system are essential partners in this defensive reaction: Activated endothelial cells promote leukocyte recruitment at inflammatory sites; new blood vessel formation, a process called angiogenesis, sustains chronic inflammation, and lymphatic vessels transport antigens and antigen-presenting cells to lymph nodes, where they stimulate naive T and B lymphocytes to elicit an antigen-specific immune response. In contrast, leukocytes and lymphocytes are far less efficient in protecting us from cancer, the "enemy from within." Worse, cancer can exploit inflammation to its advantage. The role of angiogenesis, leukocytes, and inflammation in tumor progression was discussed at the second Monte Verità Conference, Tumor Host Interaction and Angiogenesis: Basic Mechanisms and Therapeutic Perspectives, held in Ascona, Switzerland, October 1-5, 2005. (Conference chairs were K. Alitalo, M. Aguet, C. Rüegg, and I. Stamenkovic.) Eight articles reporting about topics presented at the conference are featured in this issue of the Journal of Leukocyte Biology.

Tämän pro gradu -tutkielman tavoitteena on selvittää, millainen prosessi on yksityisliikkeen muuttaminen osakeyhtiöksi. Tutkielmassa tarkastellaan yksityisliikettä ja osakeyhtiötä sekä niiden välistä yritysmuodonmuutosta yhtiö- ja vero-oikeudellisen normiston ja case-yrityksen tilinpäätösma-teriaalin perusteella. Tutkielmassa hyödynnetään kvalitatiivisia tutkimusmenetelmiä ja case-tutkimusta. Tutkielman teoriaosa luo pohjan empiiriselle osalle, jossa case-yrityksen tilinpäätöstietojen perusteella tehdään esimerkkiverolaskelmia. Tutkimuksen tulokset osoittavat, että mitään yleispätevää tilannekatsausta sellaisesta yksityisliikkeestä, jonka jatkossa pitäisi toimia osakeyhtiönä, ei ole. Yritysmuodon muuttamispäätökseen vaikuttaa usein verotukselliset tekijät tai syy voi olla hyvinkin tapauskohtainen. Tutkimuksessa kuitenkin nousi esille muutamia syitä toimintamuodonmuuttamispäätökselle kuten osakeyhtiön verotukselliset edut sekä yrittäjän mahdollisuudet tehdä erilaisia toimia osakeyhtiön kanssa. Yksityisliikettä voidaan pitää verotuksellisesti kannattava, kun ansiotulosta syntyvä kokonaisvero on yhteisöverokannan 26 prosenttia suuruinen tai sen alle. Varsinainen yhtiömuodonmuutos tapahtuu yksinkertaisesti niin, että perustetaan osakeyhtiö, jonne yksityisliikkeen omaisuus siirretäänapporttina.

ABSTRACT:¦BACKGROUND: The Spiritual Distress Assessment Tool (SDAT) is a 5-item instrument developed to assess unmet spiritual needs in hospitalized elderly patients and to determine the presence of spiritual distress. The objective of this study was to investigate the SDAT psychometric properties.¦METHODS: This cross-sectional study was performed in a Geriatric Rehabilitation Unit. Patients (N = 203), aged 65 years and over with Mini Mental State Exam score ≥ 20, were consecutively enrolled over a 6-month period. Data on health, functional, cognitive, affective and spiritual status were collected upon admission. Interviews using the SDAT (score from 0 to 15, higher scores indicating higher distress) were conducted by a trained chaplain. Factor analysis, measures of internal consistency (inter-item and item-to-total correlations, Cronbach α), and reliability (intra-rater and inter-rater) were performed. Criterion-related validity was assessed using the Functional Assessment of Chronic Illness Therapy-Spiritual well-being (FACIT-Sp) and the question "Are you at peace?" as criterion-standard. Concurrent and predictive validity were assessed using the Geriatric Depression Scale (GDS), occurrence of a family meeting, hospital length of stay (LOS) and destination at discharge.¦RESULTS: SDAT scores ranged from 1 to 11 (mean 5.6 ± 2.4). Overall, 65.0% (132/203) of the patients reported some spiritual distress on SDAT total score and 22.2% (45/203) reported at least one severe unmet spiritual need. A two-factor solution explained 60% of the variance. Inter-item correlations ranged from 0.11 to 0.41 (eight out of ten with P < 0.05). Item-to-total correlations ranged from 0.57 to 0.66 (all P < 0.001). Cronbach α was acceptable (0.60). Intra-rater and inter-rater reliabilities were high (Intraclass Correlation Coefficients ranging from 0.87 to 0.96). SDAT correlated significantly with the FACIT-Sp, "Are you at peace?", GDS (Rho -0.45, -0.33, and 0.43, respectively, all P < .001), and LOS (Rho 0.15, P = .03). Compared with patients showing no severely unmet spiritual need, patients with at least one severe unmet spiritual need had higher odds of occurrence of a family meeting (adjOR 4.7, 95%CI 1.4-16.3, P = .02) and were more often discharged to a nursing home (13.3% vs 3.8%; P = .027).¦CONCLUSIONS: SDAT has acceptable psychometrics properties and appears to be a valid and reliable instrument to assess spiritual distress in elderly hospitalized patients.

Cancer-related inflammation has emerged in recent years as a major event contributing to tumor angiogenesis, tumor progression and metastasis formation. Bone marrow-derived and inflammatory cells promote tumor angiogenesis by providing endothelial progenitor cells that differentiate into mature endothelial cells, and by secreting pro-angiogenic factors and remodeling the extracellular matrix to stimulate angiogenesis though paracrine mechanisms. Several bone marrow-derived myelonomocytic cells, including monocytes and macrophages, have been identified and characterized by several laboratories in recent years. While the central role of these cells in promoting tumor angiogenesis, tumor progression and metastasis is nowadays well established, many questions remain open and new ones are emerging. These include the relationship between their phenotype and function, the mechanisms of pro-angiogenic programming, their contribution to resistance to anti-angiogenic treatments and to metastasis and their potential clinical use as biomarkers of angiogenesis and anti-angiogenic therapies. Here, we will review phenotypical and functional aspects of bone marrow-derived myelonomocytic cells and discuss some of the current outstanding questions.

Purpose: To assess the diagnostic accuracy of the Heidelberg Retinal Tomograph 3 (HRT3) as a screening device in comparison with the reference standard of Octopus standard automated perimetry results (SAP) combined with clinical findings. Methods: All patients underwent screening examinations and investigations within a single day. Abnormal screening results were classified as follows: The HRT3: Either "borderline" or "outside normal limits" using the global Moorfields classification (MFC); SAP and clinical exam: A mean defect > 2.4 dB or "outside normal limits" clear text analysis of SAP; and one of the following i) IOP > 21 mmHg, ii) Van Herrick < ¼, iii) cup disc ratio > 0.55, iv) optic nerve head abnormality, v) narrow iridocorneal angle or vi) evidence of peripheral anterior synechiae on gonioscopy. Results: The mean age of the participants was 59.9 years (± 14.8 [21, 91]). Twenty-three subjects (16 %) were classified as abnormal on SAP and clinical exam. The HRT3 classification had a sensitivity of 30 % (95 % CI [16 %, 51 %]) with associated specificity of 58 % (95 % CI [49 %, 66 %]). Of the sixty subjects classified as borderline or outside normal limits with the HRT MFC global result, seven subjects were also abnormal according to SAP and clinical exam. Conclusion: The results suggest that the HRT3 may not be suitable as a sole screening device; however, further investigation is necessary.

Huntington's disease is a rare neurodegenerative disease caused by a pathologic CAG expansion in the exon 1 of the huntingtin (HTT) gene. Aggregation and abnormal function of the mutant HTT (mHTT) cause motor, cognitive and psychiatric symptoms in patients, which lead to death in 15-20 years. Currently, there is no treatment for HD. Experimental approaches based on drug, cell or gene therapy are developed and reach progressively to the clinic. Among them, mHTT silencing using small non-coding nucleic acids display important physiopathological benefit in HD experimental models.
L'ultrasonographie endorectale dans les cancers du rectum [Endorectal ultrasound of rectal cancers].

Endorectal ultrasonography has become the preferred exam to assess the local extent of rectal cancers. From 1990 to 1992, we have examined 28 patients with a rectal cancer. The tumours were classified according to the TNM. The objective of this exam is to identify patients whose tumours have invaded the perirectal fat. These patients are first treated in our clinic by an accelerated hyperfractionated radiotherapy and then operated. The preoperative staging made with the endorectal ultrasound was then compared with the anatomopathologic staging. The depth of the invasion was assessed precisely in 78.5% of cases. The exam's sensitivity to detect the invasion of the perirectal fat was 96% and its specificity 75%. Lymph node involvement was accurately identified in 67.8% of cases with a sensitivity of 81% and a specificity of 50%. This short retrospective study confirms that endorectal ultrasonography is a highly accurate tool for the staging of rectal carcinoma prior to operation and hence to select the patients that can benefit from preoperative irradiation.

Congenital heart defect (CHD) has a major influence on affected individuals as well as on the supportive and associated environment such as the immediate family. Unfortunately, CHD is common worldwide with an incidence of approximately 1% and consequently is a major health concern. The Arab population has a high rate of consanguinity, fertility, birth, and annual population growth, in addition to a high incidence of diabetes mellitus and obesity. All these factors may lead to a higher incidence and prevalence of CHD within the Arab population than in the rest of the world, making CHD of even greater concern. Sadly, most Arab countries lack appropriate public health measures directed toward the control and prevention of congenital malformations and so the importance of CHD within the population remains unknown but is thought to be high. In approximately 85% of CHD patients, the multifactorial theory is considered as the pathologic basis. The genetic risk factors for CHD can be attributed to large chromosomal aberrations, copy number variations (CNV) of particular regions in the chromosome, and gene mutations in specific nuclear transcription pathways and in the genes that are involved in cardiac structure and development. The application of modern molecular biology techniques such as high-throughput nucleotide sequencing and chromosomal array and methylation array all have the potential to reveal more genetic defects linked to CHD. Exploring the genetic defects in CHD pathology will improve our knowledge and understanding about the diverse pathways involved and also about the progression of this disease. Ultimately, this will link to more efficient genetic diagnosis and development of novel preventive therapeutic strategies, as well as gene-targeted clinical management. This review summarizes our current understanding of the molecular basis of normal heart development and the pathophysiology of a wide range of CHD. The risk factors that might account for the high prevalence of CHD within the Arab population and the measures required to be undertaken for conducting research into CHD in Arab countries will also be discussed.

Congenital tracheal lesions are rare, but important, causes of morbidity in infants and children. Consequently, experience in their management is limited and dispersed. Given its small diameter, the juvenile trachea is obstructed easily by various natural causes, or following a surgical intervention. The diagnosis of a congenital, tracheal, obstructive anomaly is based on a high degree of suspicion in infants and children with respiratory distress accompanied by retraction. In this article, the authors discuss the various causes of these conditions, their diagnostic features, and the treatment possibilities.

Johtavana ideana on tutkia, kuinka uusi tietojärjestelmään vaikuttaa varastointiyritykseen. Muutoksia on tutkittu niin käytännön kuin tietojärjestelmätasolla. Ongelmia ja niiden ratkaisuja on tutkittu molempien osalta erikseen. Uuteen järjestelmään siirtymisessä oli kaksi eri vaihetta. Tavoitteena on myös antaa kokonaiskuva varastonhallinnasta. Ensimmäisen käyttöönoton osalta tällä tutkielmalla oli 3kolme eri tehtävää: testata, kouluttaa ja tutkia käyttöönoton onnistumista. Tästä osiosta oli tuloksena mm. vastaanottohenkilöstön SAP-R3 käyttöohje ja vastaanottohenkilöstön koulutus. Testauksen tarkoituksena oli käyttöönoton ongelmien ennaltaehkäisy. Koulutuksen tavoitteena oli kouluttaa vastaanottohenkilöstö käyttämään R3:a siltä osin kun he sitä tarvitsevat. Tämä vaihe ei edellyttänyt vielä kerääjien koulutusta, koska heitä muutokset eivät vielä koskeneet. Työnjohto koulutettiin jo etukäteen.Toisen käyttöönoton osalta tehtäviä oli kaksi: tutkia käyttöönoton onnistumista ja tutkia, jäikö käyttöönoton jäljiltä vielä ongelmia. Lisäksi tutkielman tekijä oli mukana kouluttamassa keräilijöitä. Tämä työ on myös käyttöönottoprojektin yhteenveto. Ongelmia kappaleessa on esitetty puutteita toiminnassa ja tietojärjestelmässä. Lisäksi kappaleessa on ratkaisuehdotukset näihin pulmiin. Teoria osassa kerrotaan varastonhallinnan käytännön taustalla olevat teoriat.

Endothelial cell release of nitric oxide (NO) is a defining characteristic of nondiseased arteries, and abnormal endothelial NO release is both a marker of early atherosclerosis and a predictor of its progression and future events. Healthy coronaries respond to endothelial-dependent stressors with vasodilatation and increased coronary blood flow (CBF), but those with endothelial dysfunction respond with paradoxical vasoconstriction and reduced CBF. Recently, coronary MRI and isometric handgrip exercise (IHE) were reported to noninvasively quantify coronary endothelial function (CEF). However, it is not known whether the coronary response to IHE is actually mediated by NO and/or whether it is reproducible over weeks. To determine the contribution of NO, we studied the coronary response to IHE before and during infusion of N(G)-monomethyl-l-arginine (l-NMMA, 0.3 mg·kg(-1)·min(-1)), a NO-synthase inhibitor, in healthy volunteers. For reproducibility, we performed two MRI-IHE studies ∼8 wk apart in healthy subjects and patients with coronary artery disease (CAD). Changes from rest to IHE in coronary cross-sectional area (%CSA) and diastolic CBF (%CBF) were quantified. l-NMMA completely blocked normal coronary vasodilation during IHE [%CSA, 12.9 ± 2.5 (mean ± SE, placebo) vs. -0.3 ± 1.6% (l-NMMA); P < 0.001] and significantly blunted the increase in flow [%CBF, 47.7 ± 6.4 (placebo) vs. 10.6 ± 4.6% (l-NMMA); P < 0.001]. MRI-IHE measures obtained weeks apart strongly correlated for CSA (P < 0.0001) and CBF (P < 0.01). In conclusion, the normal human coronary vasoactive response to IHE is primarily mediated by NO. This noninvasive, reproducible MRI-IHE exam of NO-mediated CEF promises to be useful for studying CAD pathogenesis in low-risk populations and for evaluating translational strategies designed to alter CAD in patients.

Lactate has been shown to offer neuroprotection in several pathologic conditions. This beneficial effect has been attributed to its use as an alternative energy substrate. However, recent description of the expression of the HCA1 receptor for lactate in the central nervous system calls for reassessment of the mechanism by which lactate exerts its neuroprotective effects. Here, we show that HCA1 receptor expression is enhanced 24 hours after reperfusion in an middle cerebral artery occlusion stroke model, in the ischemic cortex. Interestingly, intravenous injection of L-lactate at reperfusion led to further enhancement of HCA1 receptor expression in the cortex and striatum. Using an in vitro oxygen-glucose deprivation model, we show that the HCA1 receptor agonist 3,5-dihydroxybenzoic acid reduces cell death. We also observed that D-lactate, a reputedly non-metabolizable substrate but partial HCA1 receptor agonist, also provided neuroprotection in both in vitro and in vivo ischemia models. Quite unexpectedly, we show D-lactate to be partly extracted and oxidized by the rodent brain. Finally, pyruvate offered neuroprotection in vitro whereas acetate was ineffective. Our data suggest that L- and D-lactate offer neuroprotection in ischemia most likely by acting as both an HCA1 receptor agonist for non-astrocytic (most likely neuronal) cells as well as an energy substrate.

Background Maternal mortality is a major public-health problem in developing countries. Extreme differences in maternal mortality rates between developed and developing countries indicate that most of these deaths are preventable. Most information on the causes of maternal death in these areas is based on clinical records and verbal autopsies. Clinical diagnostic errors may play a significant role in this problem and might also have major implications for the evaluation of current estimations of causes of maternal death. Methods and Findings A retrospective analysis of clinico-pathologic correlation was carried out, using necropsy as the gold standard for diagnosis. All maternal autopsies (n ¼ 139) during the period from October 2002 to December 2004 at the Maputo Central Hospital, Mozambique were included and major diagnostic discrepancies were analyzed (i.e., those involving the cause of death). Major diagnostic errors were detected in 56 (40.3%) maternal deaths. A high rate of false negative diagnoses was observed for infectious diseases, which showed sensitivities under 50%: HIV/AIDS-related conditions (33.3%), pyogenic bronchopneumonia (35.3%), pyogenic meningitis (40.0%), and puerperal septicemia (50.0%). Eclampsia, was the main source of false positive diagnoses, showing a low predictive positive value (42.9%). Conclusions Clinico-pathological discrepancies may have a significant impact on maternal mortality in sub-Saharan Africa and question the validity of reports based on clinical data or verbal autopsies. Increasing clinical awareness of the impact of obstetric and nonobstetric infections with their inclusion in the differential diagnosis, together with a thorough evaluation of cases clinically thought to be eclampsia, could have a significant impact on the reduction of maternal mortality.
