The aim of this study was to adapt and initiate validation of an instrument of attitudes toward mathematics to sports competitions. So through a correlational study, we adapted the instrument counting on a total sample of 260 young athletes (n=260), all students, divided into three rounds, and the confirmatory purpose we apply a pre competitive stress scale in order to show mappable symptoms. An initial reliability of the instrument, Alpha 0.83 to negative attitudes, and 0.90 for positive attitudes, with the average results of 2.52 (±0.86) to negative attitudes and 4.66 (±0.84) was determined for attitudes positive; we conclude that the instrument performed well in this first test, and should be applied in a larger sample, by un mounting this case study that young athletes have positive attitudes toward sports competition, so they enjoy participating in the competition.

Heterotrophic bacterial and biomass, production, specific growth rates and growth efficiencies were studied in July 2001 and January 2002 during both spring and neap tides, along a tidal cycle, at three sites in a subtropical estuary. Major freshwater inputs located in the Northern region led to differences in both and bacterioplankton biomass and activity along the estuary. While in the Northern region is light-limited, with mean production (PP) between 1.1 and 1.9 mu g C l(-1) h(-1) and mean specific growth rates (PSG) between 0.14 and 0.16 d(-1), the Southern region registered values as high as 24.7 mu g C l(-1) h(-1) for PP and 2.45 d(-1) (mean PP between 3.4 and 7.3 mu g C l(-1) h(-1); mean PSG between 0.28 and 0.57 d(-1)). On the other hand, maximum bacterial production (BP: 63.8 mu g C l(-1) h(-1)) and specific growth rate (BSG: 32.26 d(-1)) were observed in the Northern region (mean BP between 3.4 and 12.8 mu g C l(-1) h(-1); mean BSG between 1.98 and 6.67 day(-1)). These bacterial activity rates are among the highest recorded rates in estuarine and coastal waters, indicating that this system can be highly heterotrophic, due to high loads of allochthonous carbon (mainly derived from mangrove forest). Our results also showed that, despite that BP rates usually exceeded PP, in the Southern region BP may be partially supported (similar to 45%) by PP, since a significant regression was observed between BP and PP (r = 0.455, P < 0.001).

Nutrient distributions observed at some depths along the continental shelf from 27 degrees 05`S (Brazil) to 39 degrees 31`S (Argentina) in winter, 2003 and summer, 2004 related to salinity and dissolved oxygen (mL L-1) and saturation (%) data showed remarkable influences of fresh water discharge over the coastal region and in front of the La Plata estuary. In the southern portion of the study area different processes were verified. Upwelling processes caused by ocean dynamics typical of shelf break areas, eddies related to surface dynamics and regeneration processes confirmed by the increase of nutrients and the decrease of dissolved and saturation oxygen data were verified. High silicate concentrations in the surface waters were identified related to low salinities (minimum of 21.22 in winter and 21.96 in summer), confirming the importance of freshwater inputs in this region, especially in winter. Silicate concentration range showed values between 0.00 and 83.52 mu M during winter and from 0.00 to 41.16 mu M during summer. Phosphate concentrations worked as a secondary trace of terrestrial input and their values varied from 0.00 to 3.30 mu M in winter and from 0.03 to 2.26 mu M in summer; however, in shallow waters, phosphate indicated more clearly the fresh water influence. The most important information given by nitrate concentrations was the presence of water from SACW upwelling that represents a new source of nutrients for marine primary production. Nitrate maximum values reached 41.96 M in winter and 33.10 mu M in summer. At a depth similar to 800m, high nitrate, phosphate and silicate concentrations were related to Malvinas Current Waters, Subantarctic Shallow Waters and Antarctic Atlantic Intermediate Waters (AAIW). Dissolved oxygen varied from 3.41 to 7.06 mL L-1 in winter and from 2.65 to 6.85 mL L-1 in summer. The percentage of dissolved oxygen saturation in the waters showed values between 48% and 113% in winter and from 46% to 135% in summer. The most important primary production was verified in the summer, and situations of undersaturation were mainly observed below 50 m depth and at some points near the coast. The anti-correlation between nutrients and dissolved oxygen which showed evident undersaturation also revealed important potential sites of remineralization processes. The nutrient behaviours showed some aspects of the processes that occur over the Southwestern South Atlantic continental shelf and in their land-sea interfaces between Mar del Plata and Itajai.

BACKGROUND Vorapaxar is a new oral protease-activated receptor 1 (PAR-1) antagonist that inhibits thrombin-induced platelet activation. METHODS In this multinational, double-blind, randomized trial, we compared vorapaxar with placebo in 12,944 patients who had acute coronary syndromes without ST-segment elevation. The primary end point was a composite of death from cardiovascular causes, myocardial infarction, stroke, recurrent ischemia with rehospitalization, or urgent coronary revascularization. RESULTS Follow-up in the trial was terminated early after a safety review. After a median follow-up of 502 days (interquartile range, 349 to 667), the primary end point occurred in 1031 of 6473 patients receiving vorapaxar versus 1102 of 6471 patients receiving placebo (Kaplan-Meier 2-year rate, 18.5010 vs. 19.9%; hazard ratio, 0.92; 95% confidence interval [CI], 0.85 to 1.01; P=0.07). A composite of death from cardiovascular causes, myocardial infarction, or stroke occurred in 822 patients in the vorapaxar group versus 910 in the placebo group (14.7% and 16.4%, respectively; hazard ratio, 0.89; 95% CI, 0.81 to 0.98; P=0.02). Rates of moderate and severe bleeding were 7.2% in the vorapaxar group and 5.2% in the placebo group (hazard ratio, 1.35; 95% CI, 1.16 to 1.58; P<0.001). Intracranial hemorrhage rates were 1.1% and 0.2%, respectively (hazard ratio, 3.39; 95% CI, 1.78 to 6.45; P<0.001). Rates of nonhemorrhagic adverse events were similar in the two groups. CONCLUSIONS In patients with acute coronary syndromes, the addition of vorapaxar to standard therapy did not significantly reduce the primary composite end point but significantly increased the risk of major bleeding, including intracranial hemorrhage. OBJETIVO: Analisar as características dos atendimentos decorrentes de quedas em serviços de urgência e emergência e identificar fatores associados. MÉTODOS: Estudo transversal com 12.617 atendimentos decorrentes de quedas registrados no Sistema de Vigilância de Violências e Acidentes, coletados em 23 capitais e Distrito Federal, de setembro a novembro de 2009, por meio de uma amostra por conglomerado. Foi utilizada a técnica de análise de correspondência, por permitir a observação conjunta de um grande número de variáveis qualitativas. RESULTADOS: A maior parte das vítimas foi do sexo masculino (56,5%), faixa etária de 0 a 19 anos (45,7%) e declarados não brancos (62,2%). A maioria das quedas ocorreu na residência (54,6%) e via pública (17,4%); 14,3% foram relacionadas ao trabalho. Os tipos predominantes foram "queda no mesmo nível" (57,0%) e "queda de escada/degrau" (15,6%). A maioria das lesões foi classificada como entorse, luxação, contusão, corte e laceração (68,3%). Quedas dentre as crianças associaram-se à ocorrência na residência; com os adolescentes na escola; e jovens na prática esportiva. Quedas em adultos estiveram associadas ao local de trabalho, queda de andaimes, telhados, escada/degrau e buracos e uso de álcool. As quedas no mesmo nível resultaram em lesões de menor gravidade, em membros inferiores e superiores, e as quedas de andaime e telhado se associaram com lesões de maior gravidade e internações. CONCLUSÕES: Os resultados mostram que estratégias para a prevenção das quedas devem ser implantadas particularmente em residências, escolas e ambientes de trabalho.

Background: The role of an impaired estimated glomerular filtration rate (eGFR) at hospital admission in the outcome of acute kidney injury (AKI) after acute myocardial infarction (AMI) has been underreported. The aim of this study was to assess the influence of an admission eGFR<60 mL/min/1.73 m(2) on the incidence and early and late mortality of AMI-associated AKI. Methods: A prospective study of 828 AMI patients was performed. AKI was defined as a serum creatinine increase of >= 50% from the time of admission (RIFLE criteria) in the first 7 days of hospitalization. Patients were divided into subgroups according to their eGFR upon hospital admission (MDRD formula, mL/min/1.73 m(2)) and the development of AKI: eGFR >= 60 without AKI, eGFR<60 without AKI, eGFR >= 60 with AKI and eGFR<60 with AKI. Results: Overall, 14.6% of the patients in this study developed AKI. The admission eGFR had no impact on the incidence of AKI. However, the admission eGFR was associated with the outcome of AMI-associated AKI. The adjusted hazard ratios (AHR, Cox multivariate analysis) for 30-day mortality were 2.00 (95% CI 1.11-3.61) for eGFR, 60 without AKI, 4.76 (95% CI 2.45-9.26) for eGFR >= 60 with AKI and 6.27 (95% CI 3.20-12.29) for eGFR, 60 with AKI. Only an admission eGFR of <60 with AKI was significantly associated with a 30-day to 1-year mortality hazard (AHR 3.05, 95% CI 1.50-6.19). Conclusions: AKI development was associated with an increased early mortality hazard in AMI patients with either preserved or impaired admission eGFR. Only the association of impaired admission eGFR and AKI was associated with an increased hazard for late mortality among these patients.

The main goal of this work is to describe the diurnal and seasonal variations of the radiation balance components at the surface in the city of So Paulo based on observations carried out during 2004. Monthly average hourly values indicate that the amplitudes of the diurnal cycles of net radiation (Q*), downwelling and upwelling shortwave radiation (SW(DW), SW(UP)), and longwave radiations (LW(DW), LW(UP)) in February were, respectively, 37%, 14%, 19%, 11%, and 5% larger than they were in August. The monthly average daily values indicate a variation of 60% for Q*, with a minimum in June and a maximum in December; 45% for SW(DW), with a minimum in May and a maximum in September; 50% for SW(UP), with a minimum in June and a maximum in September; 13% for LW(DW), with a minimum in July and a maximum in January; and 9% for LW(UP), with a minimum in July and a maximum in February. It was verified that the atmospheric broadband transmissivity varied from 0.36 to 0.57; the effective albedo of the surface varied from 0.08 to 0.10; and the atmospheric effective emissivity varied from 0.79 to 0.92. The surface effective emissivity remained approximately constant and equal to 0.96. The albedo and surface effective emissivity for So Paulo agreed with those reported for urban areas in Europe and North America cities. This indicates that material and geometric effects on albedo and surface emissivity in So Paulo are similar to ones observed in typical middle latitudes cities. On the other hand, it was found that So Paulo city induces an urban heat island with daytime maximum intensity varying from 2.6A degrees C in July (16:00 LT) to 5.5A degrees C in September (15:00 LT). The analysis of the radiometric properties carried out here indicate that this daytime maximum is a primary response to the seasonal variation of daily values of net solar radiation at the surface.

The presence of metastatic lymph nodes is a relevant prognostic factor in oral cancer. Objective: This paper aims to assess metastatic lymph node density (pN+) in patients with tongue and floor-of-mouth squamous cell carcinoma (SCC) and the association of this parameter with disease-free survival (DFS). Materials and Methods: A group of 182 patients seen between 1985 and 2007 was included, 169 of which were males. Five were on stage I, 35 on stage II, 56 on stage III, and 85 on stage IV. Median values were considered in lymph node density assessment, and the Kaplan-Meier curve was used to evaluate DFS; survival differences within the group were elicited through the log-rank test. Results: An average 3.2 metastatic lymph nodes were excised from the patients in the group. Density ranged from 0.009 to 0.4, with a mean value of 0.09. Five-year DFS rates were of 44% and 28% for the groups with lymph node densities below and above the median respectively (p = 0.006). Two-year local/regional control was achieved for 71% and 49% for the patients below and above the median density respectively (p = 0.01). In terms of pN staging, local/regional control was achieved in 70% and 54% of pN1 and pN2 patients respectively, albeit without statistical significance (0.20%). Conclusion: Lymph node density may be used as a prognostic indicator for tongue and floor-ofmouth SCC.

In the present study, a fast, sensitive and robust method to quantify dextromethorphan, dextrorphan and doxylamine in human plasma using deuterated internal standards (IS) is described. The analytes and the IS were extracted from plasma by a liquid-liquid extraction (LLE) using diethyl-ether/hexane (80/20, v/v). Extracted samples were analyzed by high performance liquid chromatography coupled to electrospray ionization tandem mass spectrometry (HPLC-ESI-MS/MS). Chromatographic separation was performed by pumping the mobile phase (acetonitrile/water/formic acid (90/9/1, v/v/v) during 4.0 min at a flow-rate of 1.5 mL min(-1) into a Phenomenex Gemini (R) C18, 5 mu m analytical column (150 x 4.6 mm id.). The calibration curve was linear over the range from 0.2 to 200 ng mL(-1) for dextromethorphan and doxylamine and 0.05 to 10 ng mL(-1) for dextrorphan. The intra-batch precision and accuracy (%CV) of the method ranged from 2.5 to 9.5%, and 88.9 to 105.1%, respectively. Method inter-batch precision (%CV) and accuracy ranged from 6.7 to 10.3%, and 92.2 to 107.1%, respectively. The run-time was for 4 min. The analytical procedure herein described was used to assess the pharmacokinetics of dextromethorphan, dextrorphan and doxylamine in healthy volunteers after a single oral dose of a formulation containing 30 mg of dextromethorphan hydrobromide and 12.5 mg of doxylamine succinate. The method has high sensitivity, specificity and allows high throughput analysis required for a pharmacokinetic study. (C) 2012 Elsevier B.V. All rights reserved.
