A dor é um fenômeno que induz alterações fisiológicas que alteram e que podem até mesmo invalidar os resultados de um experimento. Nos protocolos de cirurgia estereotáxica (SS) em roedores não é comum a utilização de analgésicos no pósoperatório, sob o argumento de que os mesmos interferem nos resultados. Nosso objetivo foi investigar o efeito da administração de analgésicos no pós-operatório de ratos submetidos ao modelo de SS. Os parâmetros avaliados foram mudanças na ingestão de ração, no peso dos animais, possíveis alterações comportamentais e a astrogliose reativa. A cirurgia foi realizada nas coordenadas referentes ao estriado esquerdo. Foram utilizados ratos Wistar machos. O tramadol foi administrado a cada 12 h e o carprofeno a cada 24 h, ambos até os animais completarem 72 h de pós-operatório. O peso da ração mostra que todos os animais que sofreram cirurgia comeram significativamente menos do que os grupos Ctrl nos tempos 48 e 72 h após a SS. Quando o peso dos animais foi avaliado, os grupos SS+TM e SS+CP perderam significativamente mais peso do que os grupos Ctrl. Os testes comportamentais não apresentaram diferença estatística. Nosso estudo mostra que, para os parâmetros avaliados, não houve prejuízo aos animais que receberam analgesia após a cirurgia. Assim, concluímos que é possível o uso desses fármacos na rotina dos animais submetidos ao modelo de SS

La lenteja de parto (LDP) supone una herramienta de ayuda para la consecución de un parto eutócico, puesto que favorece la movilidad de las articulaciones de la pelvis, la cadera y la columna, al tiempo que favorece el descenso fetal y la correcta alineación feto-pélvica. Esta herramienta presenta características similares a la pelota de parto, pudiendo ser utilizada como sustituta de ésta en ocasiones donde el uso de la pelota se puede ver limitado. No se ha demostrado ningún efecto perjudicial asociado al uso de la LDP para la madre o el recién nacido; por ello, los profesionales encargados de la atención de la gestante deben conocer las diferentes opciones y beneficios que ofrece la LDP, para ofertarla con seguridad a la mujer y optimizar sus efectos sobre el proceso del parto.

Background: Intrathecal adjuvants are added to local anaesthetics to improve the quality of neuraxial blockade and prolong the duration of analgesia during spinal anaesthesia. Used intrathecally, fentanyl improves the quality of spinal blockade as compared to plain bupivacaine and confers a short duration of post-operative analgesia. Intrathecal midazolam as an adjuvant has been used and shown to improve the quality of spinal anaesthesia and prolong the duration of post-operative analgesia. No studies have been done comparing intrathecal fentanyl with bupivacaine and intrathecal 2 mg midazolam with bupivacaine. Objective: To compare the effect of intrathecal 2 mg midazolam to intrathecal 20 micrograms fentanyl when added to 2.6 ml of 0.5% hyperbaric bupivacaine, on post-operative pain, in patients undergoing lower limb orthopaedic surgery under spinal anaesthesia. Methods: A total of 40 patients undergoing lower limb orthopaedic surgery under spinal anaesthesia were randomized to two groups. Group 1: 2.6mls 0.5% hyperbaric bupivacaine with 0.4mls (20micrograms) fentanyl Group 2: 2.6mls of 0.5% hyperbaric bupivacaine with 0.4mls (2mg) midazolam Results: The duration of effective analgesia was longer in the midazolam group (384.05 minutes) as compared to the fentanyl group (342.6 minutes). There was no significant difference (P 0.4047). The time to onset was significantly longer in midazolam group 17.1 minutes as compared to the fentanyl group 13.2 minutes (P 0.023). The visual analogue score at rescue was significantly lower in the midazolam group (5.55) as compared to the fentanyl group 6.35 (P - 0.043). Conclusion: On the basis of the results of this study, there was no significant difference in the duration of effective analgesia between adjuvant intrathecal 2 mg midazolam as compared to intrathecal 20 micrograms fentanyl for patients undergoing lower limb orthopaedic surgery.

Sulfated polysaccharides comprise a complex group of macromolecules with a range of several biological activities, including antiviral activity, anticoagulant, antiproliferative, antiherpética, antitumor, anti-inflammatory and antioxidant. These anionic polymers are widely distributed in tissues of vertebrates, invertebrates and algae. Seaweeds are the most abundant sources of sulfated polysaccharides in nature. The green algal sulfated polysaccharides are homo or heteropolysaccharides comprised of galactose, glucose, arabinose and/or glucuronic acid. They are described as anticoagulant, anti-inflammatory, antiviral, anti-angiogenic, antitumor compounds. However, there are few studies about elucidation and evaluation of biological/pharmacological effects of sulfated polysaccharides obtained from green algae, for example, there is only one paper reporting the antinociceptive activity of sulfated polysaccharides of these algae. Therefore this study aimed to obtain sulfated polysaccharides of green seaweed Codium isthmocladum and evaluates them as potential antinociceptive agents. Thus, in this study, the total extract of polysaccharides of green alga C. isthmocladum was obtained by proteolytic digestion, followed by fractionation resulting in five fractions (F0.3, F0.5, F0.7, F0.9 and F1.2) by sequential precipitation with acetone. Using the test of abdominal contractions we observed that the fraction F0.9 was the most potent antinociceptive aompound. F0.9 consists mainly of a sulfated heterogalactana. More specific tests showed that Fo.9 effect is dose and time dependent, reaching a maximum at 90 after administration (10 mg / kg of animal). F0.9 is associated with TRPV1 and TRPA1 receptors and inhibits painful sensation in animals. Furthermore, F0.9 inhibits the migration of lymphocytes induced peritonitis test. On the other hand, stimulates the release of NO and TNF-α. These results suggest that F0.9 has the potential to be used as a source of sulfated galactan antinociceptive and anti-inflammatory

Introduction: One of the most common strategies for pain control during and after surgical procedures is the use of local anesthetics. Prolonged analgesia can be safely achieved with drug delivery systems suitably chosen for each local anesthetic agent.Areas covered: This review considers drug delivery formulations of local anesthetics designed to prolong the anesthetic effect and decrease toxicity. The topics comprise the main drug delivery carrier systems (liposomes, biopolymers, and cyclodextrins) for infiltrative administration of local anesthetics. A chronological review of the literature is presented, including details of formulations as well as the advantages and pitfalls of each carrier system. The review also highlights pharmacokinetic data on such formulations, and gives an overview of the clinical studies published so far concerning pain control in medicine and dentistry.Expert opinion: The design of novel drug delivery systems for local anesthetics must focus on how to achieve higher uploads of the anesthetic into the carrier, and how to sustain its release. This comprehensive review should be useful to provide the reader with the current state-of-art regarding drug delivery formulations for local anesthetics and their possible clinical applications.

Ropivacaine (RVC) is an enantiomerically pure local anesthetic (LA) largely used in surgical procedures, which presents physico-chemical and therapeutic properties similar to those of bupivacaine (BPV), but associated to less systemic toxicity This study focuses on the development and pharmacological evaluation of a RVC in 2-hydroxypropyl-beta-cyclodextrin (HP-P-CD) inclusion complex. Phase-solubility diagrams allowed the determination of the association constant between RVC and HP-beta-CD (9.46 M-1) and showed an increase on RVC solubility upon complexation. Release kinetics revealed a decrease on RVC release rate and reduced hemolytic effects after complexation. (onset at 3.7 mM and 11.2 mM for RVC and RVCHP-beta-CD, respectively) were observed. Differential scanning calorimetry (DSC), scanning electron microscopy (SEM) and X-ray analysis (X-ray) showed the formation and the morphology of the complex. Nuclear magnetic resonance (NMR) and job-plot experiments afforded data regarding inclusion complex stoichiometry (1:1) and topology. Sciatic nerve blockade studies showed that RVCHP-beta-CD was able to reduce the latency without increasing the duration of motor blockade, but prolonging the duration and intensity of the sensory blockade (p < 0.001) induced by the LA in mice. These results identify the RVCHP-beta-CD complex as an effective novel approach to enhance the pharmacological effects of RVC, presenting it as a promising new anesthetic formulation. The aim of this work was to investigate the influence of the oily nucleus composition on physico-chemical properties and anesthetic activity of poly (lactide-co-glycolide) nanocapsules with benzocaine.Nanocapsules containing benzocaine were prepared with three different oily nucleus composition and characterized by mean diameter, polydispersivity, zeta potential, pH and stability were investigated as a function of time. In vitro release kinetics were performed in a system with two compartments separated by a cellulose membrane. Intensity and duration of analgesia were evaluated in rats by sciatic nerve blockade.The greatest stability, slower release profile and improvement in the local anesthetic activity of BZC were obtained with the formulation using USP mineral oil as component.Results from our study provide useful perspectives on selection of the primary materials needed to produce suspensions of polymeric nanocapsules able to act as carriers of BZC, with potential future application in the treatment of pain.

Objetivo: El análisis del pH de sangre arterial de cordón umbilical sigue siendo un criterio objetivo usado para determinar el estado metabólico del recién nacido tras el parto, y por tanto del bienestar fetal. El objetivo de este estudio fue identificar los factores perinatales asociados con los valores de sangre arterial de cordón umbilical. Material y métodos: Se realizó un estudio descriptivo y analítico entre Enero de 2010 y Enero de 2013 en un hospital de tercer nivel en el sur de España, con mujeres atendidas por parto. Los criterios de inclusión fueron: embarazo sin complicaciones y parto vaginal único, a término, con presentación cefálica. Las variables independientes con gran significación tras un análisis univariante fueron: edad, paridad, edad gestacional, analgesia epidural, plan de parto, episiotomía, duración de la primera fase del parto, y uso de oxitocina. Como variable dependiente se consideró: los valores de pH de sangre arterial de cordón umbilical (< = 7.24; > 7.24). El número total de mujeres fue de 165. El análisis estadístico se realizó mediante regresión logística múltiple. Resultados: La analgesia epidural y la edad gestacional mayor o igual a 41 semanas influyeron negativamente en el pH de cordón umbilical neonatal, mientras que haber presentado un plan de parto tuvo una influencia protectora. Conclusiones: Los hallazgos de este estudio proporcionan a los profesionales más evidencias sobre los elementos que pueden influenciar en el bienestar neonatal, con el fin de actuar en consecuencia, anticipándose a las situaciones de riesgo y aplicando una atención más eficaz.

Objective: To compare the eficacy and safety of 4 mg of ondansetron vs. 4 mg of nalbuphine for the treatment of neuraxial morphine-induced pruritus, in patients at the “Dr. José Eleuterio González” University Hospital from September 2012 to August 2013. Material and methods: A controlled, prospective, randomized study of 28 patients (14 per group) receiving neuraxial morphine analgesia was conducted, which was registered and approved by the ethics Committee of the Institution and patients agreed to participate in the study under informed consent. The results were segmented and contrasted (according to drug) by hypothesis testing; the association was determined by X2 with a 95% conidence interval (CI). Results: Pruritus was effectively resolved in both groups and no signiicant difference was found in the rest of the variables. An increase in the visual analogue scale (eVA) was observed at 6 and 12 hours for the ondansetron group, which was statistically signiicant (p≤0.05), however both groups had an eVA of less than 3. Conclusions: When comparing the eficacy and safety of ondansetron 4 mg vs. nalbuphine 4 mg for the treatment of neuraxial morphine induced pruritus, the only signiicant difference found was the mean eVA at 6 and 12 hours, favoring the ondansetron group. However, both groups scored less than 3 on the eVA. Therefore, we consider that both treatments are effective and safe in the treatment of pruritus caused by neuraxial morphine.

Objetivo: Evaluar la eficacia del tratamiento del dolor postoperatorio con el uso de la morfina en anestesia conductiva en pacientes con cirugía ginecológica, obstétrica. Fundación Pablo Jaramillo Crespo; septiembre- diciembre de 2011. Métodos: Estudio observacional, analítico, prospectivo y cuantitativo. Se incluyeron, con la clasificación de la Sociedad Americana de Anestesiología (ASA) I y II a 231 pacientes, entre 15 y 65 años, hospitalizadas en la Fundación Pablo Jaramillo Crespo, por cirugía ginecológica-obstétrica, realizadas desde septiembre a diciembre de 2011. Se administraron dosis de morfina: 2 mg para epidural y 0,2 mg para anestesia raquídea. El dolor fue evaluado a las 12 y 24 horas, con la escala visual analógica de la intensidad del dolor (EVA). Resultados: La morfina utilizada no tuvo preservantes; se observó que la anestesia raquídea muestra una ligera ventaja a la epidural, con mejor tolerancia al dolor a las 12 y 24 horas (p<0.05) No hubo relación entre la etiología de las cirugías ginecológicas y obstétricas con el dolor a las 12 y 24 horas (p>0.05). En 25.78 % de los casos, la analgesia de la morfina, por administración raquídea, se complementó con el uso de ketorolaco por vía intravenosa. Un 49.2% de las pacientes no mostró efectos secundarios; de estos, los más observados fueron: el prurito (13.14 %), retención urinaria (11.3%), náuseas (9.8%) y vómitos (6.9%). Conclusión: el uso de morfina en dosis: 2 mg para epidural y 0.2 mg para anestesia raquídea, proporciona una favorable recuperación del dolor en las pacientes con cirugía ginecológica y obstétrica. Los efectos secundarios provocados por su administración pueden manejarse de manera eficiente sin alterar la recuperación postoperatoria

Purpose: To develop and characterize an herbal gel prepared from methanol root extract of Urtica dioica (Urticaceae) (Stinging nettle) for the treatment of arthritis in mice. Methods: A methanol root extract from Urtica dioica was prepared, and a gel was then prepared using Carbopol 934. The prepared gel was subjected to various physical tests (color, appearance, pH, texture, viscosity) and in vivo evaluation, including primary skin irritation, analgesic, and anti-inflammatory tests, in arthritic mice and compared with 2 % indomethacin gel, which was used as standard. Results: The prepared herbal gel was of light gray color with a smooth texture. It showed a pH of 7.1 and a viscosity of 21.2 cps. The gel exhibited pseudoplastic rheology, as evidenced by shear thinning with increased shear rate. It was non-irritating to the skin in primary skin irritation test in mice and showed 55.05 % inhibition of paw edema in a carrageenan-induced hind rat paw edema model, comparable to that of the standard gel (53.93 %), after 24 h. The gel showed 58.21 % analgesia, versus 61.19 % analgesia for the indomethacin gel standard in writhing test. Conclusion: The topical gel from methanol root extract of U. dioica may be an efficacious and safe alternative to non-steroidal anti-inflammatory drugs in the treatment of rheumatoid arthritis but this requires further investigations to ascertain its safety and clinical efficacy.
