Apresenta aspectos semânticos, morfossintáticos e morfológicos das palavras descritivas da língua Apurinã (Aruák), isto é, palavras que comumente são traduzidas como adjetivos nas línguas européias. Algumas dessas palavras recebem a marca de sujeito pronominal (ex. unatxitapeka 'ela está com fome'); outras recebem a marca de objeto (ex. ere-ru 'ela é bonita'); e outras podem receber tanto uma como a outra (ex. ny-pĩkareta eu estou com medo vs. papĩkare-nu eu sou medroso). A primeira questão aqui tratada foi quanto ao lugar das palavras descritivas nas partes do discurso Apurinã (são nomes, são verbos ou formam uma classe independente?). Utilizando evidências internas da língua, inicialmente estabelecemos uma classificação gramatical para essas palavras a partir de suas propriedades morfológicas em comparação aos nomes e verbos na língua, de modo a nos permitir responder a essa questão. A segunda questão foi sobre os correlatos semânticos das palavras descritivas. Considerando a divisão interna das palavras descritivas em Apurinã (subjetivas vs. objetivas), apresentamos as propriedades semânticas associadas a cada grupo e, a partir disso, apresentamos uma tentativa de motivar o subagrupamento de conceitos descritivos na língua com base nas noções aspectuais de transitoriedade e permanência. Finalmente, o fenômeno gramatical descrito é contextualizado dentro da tipologia de sistemas de intransitividade cindida descrito para outras línguas (PAYNE: 1997), e a descrição de suas propriedades semânticas é situada em relação à tipologia de aspectos lexicais ou aktionsarten (COMRIE: 1976, FRAWLEY: 1992).

In this essay, we sustain the idea that structuralist thinking is part of spontaneous criticism against the reductionisms that surround psychology. We depart from the radical split-up between the scientific viewpoint and that of metaphysics, expressed in the end-19th century scientific psychology projects. Next, we highlight the importance of the structuralist perspective in the review of the antinomic relations between the subjective and objective, operated at the heart of psychology throughout the 20th century. We show that the rejection of unilineal causality in favor of network causality curbed the advancement of unilateral or reductionist theories in psychology. Moreover, we consider the idea of structure as a point of convergence between psychology and philosophy. More than its explanatory nature, the notion of structure reveals an epistemological register capable of re-approximating psychology to the relativization of the ideal of scientific neutrality. The importance of structuralist thinking in psychology makes us consider the history of psychological knowledge as a type of research that belongs to cultural history.

This research aims to present a new method to get real attenuation of hearing protection devices, with good reproducibility and a small standard deviation, without relying on skills and cooperation individual. Thus the authors performed tests in 10 individuals without protection and after with two kinds of ear protections (ear plug and ear muffs), to get the threshold limit value in each of the 3 stages. For this, the research used an electrophysical exam, normally used in speech therapy, named ASSR (auditory steady-state response). The principle of this exam is put on individual’s head 3 electrodes, to capture electrical signs directly in auditory nerve. In summary, the authors presented the final results. The method proposed by ANSI (American National Standards Institute) indicated an attenuation of 27.6 dB for ear plugs, while the value found in this work was 16 dB; and for ear muffs, the ANSI method indicated 29.8 dB while the value found here was 28.5 dB.

This study aimed to evaluate the sleep quality and impact of thoracentesis on sleep in patients with a large pleural effusion. Patients with large unilateral pleural effusion were evaluated by the Pittsburgh Sleep Quality Index (PSQI) questionnaire and dyspnea Borg scale. Full polysomnography (PSG) was performed on the night before and 36 h after thoracentesis. We studied 19 patients, 11 males and 8 females, age 55 +/- 18 years and body mass index of 26 +/- 5 kg/m(2). The baseline sleep quality was poor (PSQI = 9.1 +/- 3.5). Thoracentesis removed 1.624 +/- 796 mL of pleural fluid and resulted in a significant decrease in dyspnea Borg scale (2.3 +/- 2.1 vs. 0.8 +/- 0.9, p < 0.001). The PSG before and after thoracentesis showed no significant change in apnea-hypopnea index and sleep time with oxygen saturation < 90%. There was a significant improvement in sleep efficiency (76% vs. 81%, p = 0.006), decrease percent sleep stage 1 (16% vs. 14%, p = 0.002), and a trend improvement in total sleep time (344 +/- 92 vs. 380 +/- 69 min, p = 0.056) and percentage of rapid eye movement sleep (15% vs. 20%, p = 0.053). No significant changes occurred in six patients that performed two consecutive PSG before thoracentesis. The improvement in sleep quality was not associated with the volume of pleural fluid withdrawn or changes in dyspnea. Patients with large pleural effusion have poor subjective and objective sleep quality that improves after thoracentesis.

Proximal row carpectomy (PRC) is a well-accepted procedure for the treatment of early post-traumatic degenerative disease of the wrist. Much less frequently, PRC has been advocated as an emergency procedure for irreparable fracture-dislocation of the wrist. Our objective was to compare the results of PRC in patients having undergone this procedure in the two contexts. We conducted a retrospective analysis of the clinical and radiographic results of six patients treated by emergency PRC as compared to six patients who underwent elective PRC. The mean follow-up was 36 months. Both the patient's satisfaction and the grasp of the wrist joint were significantly better in patients who underwent PRC emergency as compared to those having undergone elective PRC. Quick DASH score, radiographic results, and return to work were also more favourable in these patients, but the difference between the two groups was not significant. This study confirms that PRC is a valuable salvage technique indicated in early posttraumatic wrist collapse. Moreover, when performed in emergency, the procedure shows even better subjective and objective results, allowing a majority of patients to return to their previous job.

BACKGROund: Patient-oriented medicine is an emerging concept, encouraged by the World Health Organization, to greater involvement of the patient in the management of chronic diseases. The Patient-Oriented SCORing Atopic Dermatitis (PO-SCORAD) index is a self-assessment score allowing the patient to comprehensively evaluate the actual course of atopic dermatitis (AD), using subjective and objective criteria derived mainly from the SCORAD, a validated AD severity clinical assessment tool.

The present paper describes standardized procedures within clinical sleep medicine. As such, it is a continuation of the previously published European guidelines for the accreditation of sleep medicine centres and European guidelines for the certification of professionals in sleep medicine, aimed at creating standards of practice in European sleep medicine. It is also part of a broader action plan of the European Sleep Research Society, including the process of accreditation of sleep medicine centres and certification of sleep medicine experts, as well as publishing the Catalogue of Knowledge and Skills for sleep medicine experts (physicians, non-medical health care providers, nurses and technologists), which will be a basis for the development of relevant educational curricula. In the current paper, the standard operational procedures sleep medicine centres regarding the diagnostic and therapeutic management of patients evaluated at sleep medicine centres, accredited according to the European Guidelines, are based primarily on prevailing evidence-based medicine principles. In addition, parts of the standard operational procedures are based on a formalized consensus procedure applied by a group of Sleep Medicine Experts from the European National Sleep Societies. The final recommendations for standard operational procedures are categorized either as 'standard practice', 'procedure that could be useful', 'procedure that is not useful' or 'procedure with insufficient information available'. Standard operational procedures described here include both subjective and objective testing, as well as recommendations for follow-up visits and for ensuring patients' safety in sleep medicine. The overall goal of the actual standard operational procedures is to further develop excellence in the practice and quality assurance of sleep medicine in Europe.

BACKGROUND: To use amifostine concurrently with radiochemotherapy (CT-RT) or radiotherapy (RT) alone in order to prevent dry eye syndrome in patients with malignancies located in the fronto-orbital region. METHODS: Five patients (2 males, 3 females) with diagnosed malignancies (Non-Hodgkin B-cell Lymphoma, neuroendocrine carcinoma) involving the lacrimal gland, in which either combined CT-RT or local RT were indicated, were prophylactically treated with amifostine (500 mg sc). Single RT fraction dose, total dose and treatment duration were individually adjusted to the patient's need. Acute and late adverse effects were recorded using the RTOG score. Subjective and objective dry eye assessment was performed for the post-treatment control of lacrimal gland function. RESULTS: All patients have completed CT-RT or RT as indicated. The median total duration of RT was 29 days (range, 23 - 39 days) and the median total RT dose was 40 Gy (range, 36 - 60 Gy). Median lacrimal gland exposure was 35.9 Gy (range, 16.8 - 42.6 Gy). Very good partial or complete tumor remission was achieved in all patients. The treatment was well tolerated without major toxic reactions. Post-treatment control did not reveal in any patient either subjective or objective signs of a dry eye syndrome. CONCLUSION: The addition of amifostine to RT/CT-RT of patients with tumors localized in orbital region was found to be associated with absence of dry eye syndrome.

BACKGROUND Neuropsychological deficits (NPD) are common in patients with aneurysmal subarachnoid haemorrhage (aSAH). NPD are one of the major limiting factors for patients with an otherwise acceptable prognosis for sustained quality of life. There are only a few studies reporting outcome after aSAH, which used a standardized neuropsychological test battery as a primary or secondary outcome measure. Aim of this study was to determine the current practice of reporting NPD following aSAH in clinical studies. METHODS A MEDLINE analysis was performed using the search term "subarachnoid haemorrhage outcome". The latest 1,000 articles were screened. We recorded study design, number of patients, and the presence of neuropsychological outcome report. Additionally, the time of testing after aSAH, the neuropsychological tests administered, as well as the percentage of patients with NPD were analyzed. RESULTS A total of 324 publications between 2009 and 2012 were selected for further review. Of those, 21 studies (6.5%) reported neuropsychological outcome, in 2,001 of 346,666 patients (0.6%). The assessment of NPD differed broadly using both subjective and objective cognitive evaluation, and a large variety of tests were used. CONCLUSION Neuropsychological outcome is underreported, and there is great variety in assessment in currently published clinical articles on aSAH. Prospective randomized trials treating aSAH may benefit from implementing more comprehensive and standardized neuropsychological outcome measures. This approach might identify otherwise unnoticed treatment effects in future interventional studies of aSAH patients.

BACKGROUND Intraoperatively fabricated polymethylmethacrylate (PMMA) implants based on computer-designed moulds were used to improve cosmetic results after hard tissue replacement. To assess the implant's cosmetic and functional results we performed both subjective and objective assessments. METHODS This retrospective analysis was performed using a cohort of 28 patients who received PMMA implants between February 2009 and March 2012. The cosmetic and functional results were assessed using a patient questionnaire. Furthermore an objective volumetric subtraction score (0-100) was applied and implant thickness, as well as gaps and tiers, were measured. RESULTS Patients mainly judged their cosmetic result as "good". Two of the 28 patients found their cosmetic result unfavourable. The functional result and stability was mainly judged to be good. Measurements of implant thickness showed a very high correlation with the thickness of the contralateral bone. Volumetric subtraction led to a median quality of 80 on a scale from 0 to 100. Median gaps around the margins of the implant were 1.5 mm parietally, 1.7 mm frontally and 3.5 mm fronto-orbitally, and median tiers were 1.2 mm, 0 mm and 0 mm respectively. The overall rate of surgical revisions was 10.7 % (three patients). Two patients suffered from wound healing disturbances (7.1 %). The overall complication rate was comparable to other reports in the literature. CONCLUSIONS Implantation of intraoperatively fabricated patient-specific moulded implants is a cost-effective and safe technique leading to good clinical results with a low complication rate.

El presente artículo aborda la política de protección social en Chile como una política pública, inaugurada por los gobiernos posrégimen militar, que busca cambiar el paradigma, desde la tradicional visión asistencialista coyuntural por parte del Estado, a una perspectiva que persigue reducir las vulnerabilidades de manera sustentable y con una fuerte participación social en la elaboración de las políticas basado en tres pilares: equidad, integración y cohesión social.

La tesis analiza dos objeciones corrientes a la interpretación del problema de las reglas por el Wittgenstein de Kripke: el PROBLEMA DE LA OBJETIVIDAD DE LA REGLA, ¿puede un individuo estar acertado en contra de la opinión comunitaria consensuada?; y el PROBLEMA DE LAS RELACIONES INTERNAS entre reglas y casos, presuntamente violentada por el rol de la noción de acuerdo en la misma. La estrategia general de la tesis es examinar las vías de ataque al lenguaje privado (LP) previas a la de Kripke. El contraste con las mismas permite conceptualizar adecuadamente la propuesta de Kripke, para mostrar que las objeciones mencionadas no se le aplican. En el marco de la tesis, comprendemos por LP a cualquier tesis que sostenga que las condiciones de significatividad del lenguaje pueden ser provistas por estados mentales subjetivos, p.e. creencias, de ahí el solapamiento de la tesis del lenguaje privado con el escepticismo epistemológico y con posiciones solipsistas, y la convicción de que la refutación del LP acarrea la ruina del escepticismo epistemológico. Hay dos versiones clásicas del argumento del LP. Las versiones epistemológicas (Malcom, Fogelin son los representantes que examinamos) derivan la imposibilidad del LP de la imposibilidad de establecer la verdad/corrección de un uso en el contexto de privacidad. La crítica a estas versiones (Ayer, thomson) muestra un círculo justificatorio entre oraciones subjetivas y objetivas. Las versiones semánticas (Kenny, Canfield, Tugendhat) intentan evitar el círculo, elucidando las presuposiciones que le subyacen. Esta respuesta no es satisfactoria ya que o bien regenera el círculo en un nuevo nivel, o bien no logran derrocar al círculo de la justificación. Con Stroud identificamos el fallo común de estas estrategias en el hecho de que en su intento de derrotar al escéptico-privatista, requieren o bien una PREMISA FÁCTICA que indica que conocemos, o bien la especificación del conocimiento como una NOTA DEFINICIONAL de los criterios/condiciones de significatividad que el argumento trascendental elucida en la pregunta del escéptico. Kripke impone un cambio de rumbo en el argumento, al plantear el desafío como una forma de ESCEPTICISMO SEMÁNTICO ONTOLÓGICO, el cual ya no se basa en las limitaciones cognitivas de la privacidad, sino justamente en las presuntas ventajas que presenta. El desafío de Kripke pregunta por las razones que podemos aportar para sostener que no ha ocurrido un cambio en el uso, lo cual equivale a preguntar ¿cómo sabes que tu uso actual CORRESPONDE con tu intención/significado previo? De la imposibilidad de responder a la cuestión clave del realismo clásico (correspondencia) en el caso del significado, Kripke concluye que no puede haber condiciones de verdad para las oraciones semánticas. Esto motiva una paradoja escéptica y una SOLUCIÓN ESCÉPTICA de la paradoja en términos de condiciones de aseverabilidad. Tener en cuenta el abandono de las condiciones de verdad es la clave para responder al problema de la objetividad, mientras que la forma del desafío y el funcionamiento de la concordancia en los juicios como base de atribución, es la clave para la solución del problema de las relaciones internas.
