Exercise training has an important role in the prevention and treatment of hypertension, but its effects on the early metabolic and hemodynamic abnormalities observed in normotensive offspring of hypertensive parents (FH+) have not been studied. We compared high-intensity interval (aerobic interval training, AIT) and moderate-intensity continuous exercise training (CMT) with regard to hemodynamic, metabolic and hormonal variables in FH+ subjects. Forty-four healthy FH+ women (25.0+/-4.4 years) randomized to control (ConFH+) or to a three times per week equal-volume AIT (80-90% of VO(2MAX)) or CMT (50-60% of VO(2MAX)) regimen, and 15 healthy women with normotensive parents (ConFH-; 25.3+/-3.1 years) had their hemodynamic, metabolic and hormonal variables analyzed at baseline and after 16 weeks of follow-up. Ambulatorial blood pressure (ABP), glucose and cholesterol levels were similar among all groups, but the FH+ groups showed higher insulin, insulin sensitivity, carotid-femoral pulse wave velocity (PWV), norepinephrine and endothelin-1 (ET-1) levels and lower nitrite/ nitrate (NOx) levels than ConFH- subjects. AIT and CMT were equally effective in improving ABP (P<0.05), insulin and insulin sensitivity (P<0.001); however, AIT was superior in improving cardiorespiratory fitness (15 vs. 8%; P<0.05), PWV (P<0.01), and BP, norepinephrine, ET-1 and NOx response to exercise (P<0.05). Exercise intensity was an important factor in improving cardiorespiratory fitness and reversing hemodynamic, metabolic and hormonal alterations involved in the pathophysiology of hypertension. These findings may have important implications for the exercise training programs used for the prevention of inherited hypertensive disorder. Objective We characterized the impact of the metabolic syndrome (MetS) and its components on cardiovascular adverse events in patients with symptomatic chronic multivessel coronary artery disease, which have been followed prospectively for 2 years. Methods Patients enrolled in the MASS II study were evaluated for each component of the MetS, as well as the full syndrome. Results The criteria for MetS were fulfilled in 52% of patients. The presence of MetS (P < 0.05), glucose intolerance (P=0.007), and diabetes (P=0.04) was associated with an increased mortality in our studied population. Moreover, despite a clear tendency for each of its components to increase the mortality risk, only the presence of the MetS significantly increased the risk of mortality among nondiabetic study participants in a multivariate model (P=0.03, relative risk 3.5, 95% confidence interval 1.1-6). Finally, MetS was still associated with increased mortality even after adjustment for diabetes status. These results indicate a strong and consistent relationship of the MetS with mortality in patients with stable coronary artery disease. Conclusion Although glucose homeostasis seems to be the major force driving the increased risk of MetS, the operational diagnosis of MetS still has information for stratifying patients when diabetes information is taken into account.

This study evaluated the effect of TAK-778 [(2R, 4S)-(-)-N-(4-diethoxyphosphorylmethylphenyl)-1,2,4,5-tetrahydro-4-methyl-7,8-methylenedioxy-5-oxo-3-benzothiepin-2-carboxamide)] on in vitro osteogenic events and on gene expression of osteoblastic cells derived from human alveolar bone and the participation of estrogen receptors (ERs) on such effect. Osteoblastic cells were subcultured, with or without TAK-778 (10(-5) M), to evaluate cell growth and viability, total protein content, and alkaline phosphatase (ALP) activity at 7, 14, and 21 days; bone-like formation at 21 days; and gene expression, using cDNA microarray, at 7 days. Also, osteoblastic cells were exposed to TAK-778 (10-5 M) combined to ICI182,780, a nonspecific ER antagonist (10(-6) M), and gene expression was evaluated by real-time polymerase chain reaction (PCR) at 7 days. TAK-778 induced a reduction in culture growth and an increase in cell synthesis, ALP activity, and bone-like formation. The cDNA microarray showed genes associated with cell adhesion and differentiation, skeletal development, ossification, and transforming growth factor-P receptor signaling pathway, with a tendency to be higher expressed in cells exposed to TAK-778. The gene expression of ALP, osteocalcin, Msh homeobox 2, receptor activator of NF-kappa B ligand, and intercellular adhesion molecule 1 was increased by TAK-778 as demonstrated by real-time PCR, and this effect was antagonized by ICI182,780. The present results demonstrated that TAK-778 acts at a transcriptional level to enhance the in vitro osteogenic process and that its effect on gene expression of osteoblastic cells is mediated, at least partially, through ERs. Objective: To test a conceptual model linking parental physical activity orientations, parental support for physical activity, and children's self-efficacy perceptions with physical activity participation. Participants and Setting: The sample consisted of 380 students in grades 7 through 12 (mean age, 14.0 +/- 1.6 years) and their parents. Data collection took place during the fall of 1996. Main Outcome Measures: Parents completed a questionnaire assessing their physical activity habits, enjoyment of physical activity, beliefs regarding the importance of physical activity, and supportive behaviors for their child's physical activity. Students completed a 46-item inventory assessing physical activity during the previous 7 days and a 5-item physical activity self-efficacy scale. The model was tested via observed variable path analysis using structural equation modeling techniques (AMOS 4.0). Results: An initial model, in which parent physical activity orientations predicted child physical activity via parental support and child self-efficacy, did not provide an acceptable fit to the data. Inclusion of a direct path from parental support to child physical activity and deletion of a nonsignificant path from parental physical activity to child physical activity significantly improved model fit. Standardized path coefficients for the revised model ranged from 0.17 to 0.24, and all were significant at the p < 0.0001 level. Conclusions: Parental support was an important correlate of youth physical activity, acting directly or indirectly through its influence on self-efficacy. Physical activity interventions targeted at youth should include and evaluate the efficacy of individual-level and community-level strategies to increase parents' capacity to provide instrumental and motivational support for their children's physical activity.

A large number of countries worldwide have legalized homosexual rights. But for 147 years, since when India was a British colony, Section 377 of the Indian Penal Code defines homosexuality as a crime, punishable by imprisonment. This outdated law violates the fundamental rights of homosexuals in India. Despite the fact that literature drawn from Hindu, Buddhist, Muslim, and modern fiction testify to the presence of same-sex love in various forms, homosexuality is still considered a taboo subject in India, by both the society and the government. In the present article, the continuation of the outdated colonial-era homosexuality law and its impact on the underprivileged homosexual society in India is discussed, as well as consequences to this group's health in relation to HIV infection.

OBJECTIVE: To investigate the impact of socioeconomic status on elderly health. METHODS: The study was based on cross-sectional data from Survey on Health, Well-Being, and Aging in Latin America and the Caribbean. The sample comprised 2,143 non-institutionalized elderly aged 60 years and older living in the urban area of São Paulo, southeastern Brazil. Linear regression models estimated the effect of socioeconomic status indicators (years of schooling completed, occupation and purchasing power) on each one of the following health indicators: depression, self-rated health, morbidity and memory capacity. A 5% significance level was set. RESULTS: There was a significant effect of years of education and purchasing power on self-rated health and memory capacity when controlled for the variables number of diseases during childhood, bed rest for at least a month due to health problems during childhood, self-rated health during childhood, living arrangements, sex, age, marital status, category of health insurance, intake of medicines. Only purchasing power had an effect on depression. Despite the bivariate association between socioeconomic status indicators and number of diseases (morbidity), this effect was no longer seen after including the controls in the model. CONCLUSIONS: The study results confirm the association between socioeconomic status indicators and health among Brazilian elderly, but only for some dimensions of socioeconomic status and certain health outcomes.

The goal of this article is to present a snapshot of an ongoing debate within epidemiology, pitching opposing sides in the struggle to define the path it should follow in the years to come. The debate among epidemiologists in the mid-90s pitted those who defended the idea that epidemiology should necessarily deal with a wide context against those who believed that science and public health are better served by focusing on the individual level. Ian Hacking’s concept of styles of reasoning was used as a theoretical tool. The literature was reviewed using a core set of articles as an entry point, seeking articles that cited them, and then back-tracking the citations of the resulting set in the Scopus database. The main arguments are presented according to levels (ontological, epistemological, axiological and pragmatic), in order to show an even deeper disagreement, in the very conception of science and its relation to social issues and public policy.

RESUMO: Objectivo: O exercício tem sido identificado como um factor de promoção da qualidade de vida em mulheres submetidas a cirurgia por cancro de mama. Mas os níveis de actividade física tendem a reduzir após o diagnóstico de cancro de mama e a manterem-se baixos após fim dos tratamentos. O objectivo deste estudo é verificar se um programa de exercício físico supervisionado, associado a estratégias motivacionais em mulheres submetidas a cirurgia por cancro da mama e que mantêm a intervenção usual praticada, é mais efectivo no aumento dos níveis de actividade física, na redução do índice de massa corporal e na melhoria da qualidade de vida, do que o tratamento usualmente praticado. Metodologia: Trata-se de um estudo piloto experimental, aleatorizado e controlado. Cumpriram os critérios de inclusão no estudo 37 utentes submetidas a cirurgia por cancro de mama no Hospital Fernando Fonseca, Amadora, e submetidos à intervenção usual da fisioterapia. Foram aleatoriamente distribuídas pelos grupos experimental (n=19) e de controlo (n=18). O contacto telefónico foi feito por um elemento externo, cego em relação à distribuição dos sujeitos. Alguns elementos desistiram ou não puderam participar no estudo. O número de sujeitos final foi de 11 para o grupo experimental e de 10 para o grupo de controlo, idênticos na média de idades. Todos os sujeitos mantiveram a intervenção usual da fisioterapia. Os sujeitos do grupo experimental foram ainda submetidos ao programa de exercício e promoção da actividade física. Todo o grupo foi avaliado no início do programa, no final do mesmo (12 semanas) e após 6 meses de follow-up, nos seguintes indicadores: nível de actividade física, pelo International Physical Activity Questionnaire (IPAQ), índice de massa corporal e qualidade de vida, pelos questionários: European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) e Supplementary Questionnaire Breast Cancer Module (QLQ-BR23). Resultados: O programa implementado demonstrou-se efectivo no aumento dos níveis de actividade física (p=0,017) e na qualidade de vida associada ao estado funcional (p=0,016) e ao estado de saúde global auto-percepcionado (p=0,030) no final do programa. Foi ainda possível identificar resultados positivos noutras variáveis estudadas, como por exemplo a fadiga auto-reportada, mas que não se demonstraram estatisticamente significativos, facto que pode atribuir-se à reduzida dimensão da amostra. Conclusões: Apesar de algumas limitações, este estudo é de grande a importância para a comunidade científica preocupada com crescente problemática do cancro de mama. Mais uma vez se demonstra que o exercício físico é uma estratégia importante para a melhoria do estado de saúde de pacientes confrontados com doença. Esta comprovação é de grande interesse para os profissionais de saúde e particularmente para os fisioterapeutas dedicados à promoção da saúde nesta população.---------------------ABSTRACT: Purpose: Exercise has been identified as a quality of life promoting factor, amongst women after breast cancer surgery. But the physical activity levels reduce significantly after a diagnosis of breast cancer and remain low after treatment is completed. The aim of this study is to determine whether a supervised group exercise program allied to motivating strategies in women after breast cancer surgery in conjunction with standard treatment, is more effective in improving physical activity levels, reducing body mass index and promoting quality of life, than standard treatment on its own. Methods: This is a pilot randomized controlled trial. Thirty-seven women, submitted to breast cancer surgery in the Hospital Fernando Fonseca, Amadora, and submitted to standard physiotherapy intervention, completed the inclusion criteria. They were randomly allocated into intervention group (n=19) and control group (n=18). The telephone contact was done by an external element, blind to the subjects’ allocation. Some elements declined or could not participate. The final intervention group was n=11 and the control n=10, similar in age. All subjects maintained the standard physiotherapy intervention. The experimental group was submitted to the exercise and health promotion program. The complete group was evaluated in the beginning of the program, at the final (after 12 weeks) and after 6 months follow-up, using as outcomes: physical activity index with the international Physical Activity Questionnaire (IPAQ), the body mass index and quality of life using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) and Supplementary Questionnaire Breast Cancer Module (QLQBR23). Results: The studied program demonstrated to be effective in improving the physical activity index (p=0,017) and quality of live related to physical functioning (p=0,016) and to global health status (p=0,030) at the end of the program. It was possible to find positive results in some other outcomes, such as fatigue, although the differences were not statistically significant. We believe that these results can be attributed to the small sample size. Conclusions: Although we can identify some methodological constrains, this is a very important study for scientific community working on the breast cancer subject. Once more, exercise was identified as an important strategy to improve global heath status in breast cancer patients. This represents an important contribution to the health professionals and mostly for physiotherapists working on health promotion subject.

introdução de drogas que assegurem o crescimento e a preservação das espécies, mas que eventualmente se espalham para o meio aquático envolvente, promovendo alterações da biodiversidade e entrar, directamente ou indirectamente, na cadeia alimentar. Quando estas drogas são agentes antimicrobianos de uso humano, tais como a amoxicilina, tetraciclina ou sulfonamidas, há um alto risco de aparecimento de espécies bacterianas resistentes, algo que constitui uma ameaça grave para a saúde pública. Esta introdução de agentes antimicrobianos no ambiente aquático através do sector das pescas pode ser reduzida através da monitorização regular ou contínua dos níveis de antibióticos no sistema de água, durante a execução, bem como antes da descarga para o meio aquático. Para isso, é necessário métodos analíticos que permitam uma frequência analítica elevada e continua, nos tanques de cultivos dos peixes. O presente trabalho descreve para este efeito, um sensor constituído por papel quimicamente modificado por reações em monocamadas, assumindo uma coloração típica após contacto com o antibiótico . A intensidade da coloração estava relacionada com a concentração desse antibiótico. A modificação do papel foi baseada na alteração química das unidades de glucose do papel por meio de uma reação covalente com reagentes apropriados. De seguida, criou-se uma camada de quitosano sobre o papel modificado onde se adsorveu a espécie metálica capaz de mudar de cor na presença de sulfadiazina. As modificações resultantes foram avaliadas em relação a vários parâmetros, com o intuito de provocar uma variação de cor intensa face à concentração de antibiótico. Os sensores preparados foram caracterizados do ponto de vista do seu desempenho analítico, efetuou-se a construção de uma gama de concentração que permitiu obter uma resposta previsível e transversal em relação a outros antibióticos, bem como a identificação de uma relação linear entre concentração e coordenadas de cor e a aplicação de sensores em amostra de água ambiental dopados com antibiótico. Generalizando, foi possível estabelecer um processo de modificação simples de papel capaz de medir a presença e quantidade de sulfadiazina

RESUMO - Enquadramento/Objectivos: As doenças oncológicas constituem a segunda causa de morte em Portugal, e têm um profundo impacto psicossocial, não só pela sua elevada incidência e mortalidade mas também pelos enormes custos envolvidos na sua prevenção, tratamento e reabilitação. De acordo com estudos anteriores, existem disparidades geográficas na incidência da doença oncológica. É por isso indispensável caracterizar e analisar as diferentes distribuições espaciais no tempo e no espaço, para controlar a doença e promover a saúde, contribuindo ao mesmo tempo para uma melhor compreensão da etiologia da doença. Este projecto compreende 3 objectivos principais que são: a caracterização de distribuição espacio-temporal do cancro do pulmão e do cancro do estômago, separadamente e em conjunto, na região sul de Portugal Continental (abrangida pelo ROR-Sul) no espaço temporal de 2000 a 2008, procurando identificar potenciais áreas de risco no desenvolvimento destes tumores. Metodologia: Numa primeira fase realizou-se um estudo descritivo das taxas de incidência dos tumores aqui retratados por idades, por sexo, por ano e por distritos. Posteriormente com o objectivo de identificar a presença de áreas de elevada incidência, procedeu-se à análise de clustering espacio-temporal das taxas de incidência ao nível dos concelhos na região do estudo, em 2000-2008. Resultados: Os resultados da análise descritiva revelaram que ambos os tumores são mais incidentes nos homens do que nas mulheres e que estes são igualmente mais incidentes em pessoas com mais de 75 anos. A análise de clustering espacio temporal permitiu verificar a existência um padrão geográfico heterogéneo da incidência de ambos os tumores, da qual resultaram 3 clusters para o cancro do estômago e 2 clusters para o cancro do pulmão (p <0,001). Os clusters do estômago pertencem maioritariamente à região do Alentejo e os clusters do cancro do pulmão à região da grande Lisboa. Conclusões: Os resultados da análise de clustering demonstraram um padrão heterogéneo da distribuição da incidência dos dois cancros na região e período temporal do estudo. As zonas identificadas de elevado risco são diferentes para ambos o tumores. A região que apresenta maior risco para o desenvolvimento do cancro do estômago é o Alentejo e do pulmão é o distrito de Lisboa.
