Le stress oxydatif et la formation des radicaux libres sont impliqués dans plusieurs mécanismes de la mort neuronale qui caractérisent la maladie d'Alzheimer. Les antioxydants sont reconnus comme une source de protection contre le stress oxydatif et peuvent avoir un effet protecteur sur le développement de la maladie d’Alzheimer. Cette étude visait à évaluer le potentiel antioxydant, par le biais du score « oxygen radical absorbance capacity » (ORAC), de l’alimentation habituelle de personnes âgées atteintes de la maladie d’Alzheimer en comparaison avec des témoins appariés pour l’âge sans problèmes cognitifs. L’hypothèse stipulait que les patients atteints de la maladie d’Alzheimer ont une alimentation dont le potentiel antioxydant est inférieur à celui des témoins sans problèmes cognitifs. L’étude a consisté en des analyses secondaires de données provenant de l’étude « Nutrition-Mémoire » (NMS), durant laquelle quarante-deux patients avec une démence du type Alzheimer (DTA) probable et leurs aidants étaient suivis pendant une période de dix-huit mois. Pour la présente étude, les données provenaient de trois jours de collecte alimentaire, ont été colligées au début (T0) de l’étude NMS, selon la méthode « Multiple-Pass ». Le potentiel antioxydant de l’alimentation a été déterminé à l’aide de la description des aliments énumérés dans la base de données des valeurs ORAC de l’USDA. Les résultats de l’étude ont montré que les patients avaient une alimentation dont le potentiel antioxydant était inférieur à celui des témoins (13784,07 ± 7372,70 μmol TE/100g contre 23220,54 ± 10862,55 μmol TE/100g, patients et témoins, respectivement; p<0,0001). Les analyses de régression hiérarchique pas à pas montraient que l’IMC, l’éducation, et le groupe (patients, témoins) étaient des facteurs influençant le score ORAC total. La consommation des aliments riches en antioxydants est un comportement préventif à faible risque qui pourrait bénéficier des individus susceptibles de développer la maladie d'Alzheimer.

L’objectif de ce travail de thèse est d’évaluer le potentiel de la musique comme support mnémotechnique pour l’acquisition de nouvelles informations chez des personnes âgées saines et atteintes de la maladie d’Alzheimer (MA). Les bénéfices de la musique sur la cognition ont souvent été mis en évidence, y compris chez des populations âgées ou atteintes de démence. Parallèlement, chez des sujets jeunes, l’idée que la musique peut servir de support pour la mémoire a été largement débattue. Pourtant, très peu d’études ont posé cette question auprès de populations âgées ou dans la démence, malgré le besoin persistant de stratégies d’intervention dans ce domaine. Dans le présent travail, deux études sont menées dans une cohorte de 8 participants atteints d’un stade léger de la maladie d’Alzheimer, et 7 participants âgés sains appariés en âge et niveau de scolarité. La première étude porte sur la mémoire verbale, et compare l’apprentissage et la rétention de paroles (textes inconnus) présentées de manière récitée ou chantée. Lorsque les paroles sont chantées, différents degrés de familiarité de la mélodie sont contrastés. Aussi, l’action motrice étant intimement liée à l’écoute musicale, nous contrastons deux procédures d’apprentissage impliquant (ou non) la production synchronisée des paroles à mémoriser pendant l’encodage : le participant est invité à chanter à l’unisson avec un modèle (ou à écouter simplement sans chanter). Les résultats de cette étude sont présentés et discutés dans les deux premiers articles de la partie expérimentale. Ils suggèrent globalement que la musique n’aide pas l’apprentissage en rappel immédiat ; un effet délétère est même observé lorsque la mélodie utilisée est non familière. Par contre, la musique favorise la rétention à long terme des paroles, principalement pour les participants MA. Elle ne semble cependant pas interagir avec la procédure d’apprentissage impliquant le chant à l’unisson. La seconde étude porte sur l’apprentissage de séquences de gestes. Suivant la même logique que dans la première étude, nous explorons l’influence d’un accompagnement musical (versus apprentissage en silence) et d’une procédure d’apprentissage avec production synchronisée (versus observation) des gestes durant l’encodage. Les résultats (article 3) ne montrent pas non plus d’interaction entre l’accompagnement et la procédure d’apprentissage, mais différents effets de chaque composante sur les deux groupes de participants. Effectuer les gestes en synchronie avec un modèle lors de l’encodage est bénéfique pour les sujets Contrôles, mais plutôt délétère pour les participants MA. Par contre, l’accompagnement musical favorise davantage l’apprentissage chez les sujet MA que chez les Contrôles. En discussion générale, nous discutons les implications de ces résultats pour la neuropsychologie fondamentale et clinique, et proposons notamment différentes recommandations visant à maximiser ces effets et à les rendre pertinents pour l’usage thérapeutique en stimulation cognitive.

L'étude de la représentation de la maladie d'Alzheimer dans Le monde de Barney de Mordecai Richler, de Trois-Pistoles et les Basques, le pays de mon père de Victor-Lévy Beaulieu et de "Je ne suis pas sortie de ma nuit", d'Annie Ernaux permet d'esquisser les contours d'une spécificité littéraire de la maladie, bien au-delà du thème de l'oubli. N'envisager la maladie d'Alzheimer qu'en termes de dégénérescence cognitive, c'est évacuer sa dimension de matériau littéraire qui influence tant la construction que le style d'un texte. L'analyse de trois récits contemporains permet l'identification de procédés rhétoriques mais aussi de stratégies narratives et stylistiques qui servent à circonscrire l'empreinte littéraire propre à la maladie d'Alzheimer. La prise en charge du récit par un tiers parti explique la thématique de la filiation qui donne à voir la maladie de l'intérieur et de l'extérieur. L'oeuvre de création s'inscrit dans ce travail d'archivage de la mémoire familiale. Une petite-fille s'adresse indirectement à sa grand-mère qui souffre de la maladie d'Alzheimer. Nous suivons l'évolution de la maladie et sommes témoins de l'effritement des souvenirs et du discours de la grand-mère.

The goal of this study was to investigate the specific patterns of memory breakdown in patients suffering from early-onset Alzheimer’s disease (EOAD) and late-onset Alzheimer’s disease (LOAD). Twenty EOAD patients, twenty LOAD patients, twenty matched younger controls, and twenty matched older controls participated in this study. All participants underwent a detailed neuropsychological assessment, an MRI scan, an FDG-PET scan, and AD patients had biomarkers as supporting evidence of both amyloïdopathy and neuronal injury. Results of the neuropsychological assessment showed that both EOAD and LOAD groups were impaired in the domains of memory, executive functions, language, praxis, and visuoconstructional abilities, when compared to their respective control groups. EOAD and LOAD groups, however, showed distinct patterns of memory impairment. Even though both groups were similarly affected on measures of episodic, short term and working memory, in contrast semantic memory was significantly more impaired in LOAD than in EOAD patients. The EOAD group was not more affected than the LOAD group in any memory domain. EOAD patients, however, showed significantly poorer performance in other cognitive domains including executive functions and visuoconstructional abilities. A more detailed analysis of the pattern of semantic memory performance among patient groups revealed that the LOAD was more profoundly impaired, in tasks of both spontaneous recall and semantic recognition. Voxel-Based Morphometry (VBM) analyses showed that impaired semantic performance in patients was associated with reduced gray matter volume in the anterior temporal lobe region, while PET-FDG analyses revealed that poorer semantic performance was associated with greater hypometabolism in the left temporoparietal region, both areas reflecting key regions of the semantic network. Results of this study indicate that EOAD and LOAD patients present with distinct patterns of memory impairment, and that a genuine semantic impairment may represent one of the clinical hallmarks of LOAD.

The recent developments in neurobiology have rendered new prominence and potential to study about the structure and function of brain and related disorders. Human behaviour is the net result of neural control of the communication between brain cells. Neurotransmitters are chemicals that are used to relay, amplify and modulate electrical signals between neurons and/or another cell. It mediates rapid intercellular communication through the nervous system by interacting with cell surface receptors. These receptors often trigger second messenger signaling pathways that regulate the activity of ion channels. The functional balance of different neurotransmitters such as Acetylcholine (Ach), Dopamine (DA), Serotonin (5-HT), Norepinephrine (NE), Epinephrine (EPI), Glutamate and Gamma amino butyric acid (GABA) regulates the growth, division and other vital functions of a normal cell / organism (Sudha, 1998). Any change in neurotransmitters' functional balance will result in the failure of cell function and may lead to the occurrence of diseases. Abnormalities in the production or functioning of neurotransmitters have been implicated in a number of neurological disorders like Schizophrenia, Alzheimer's, Epilepsy, Depression and Parkinson's disease. Changes in central and peripheral neuronal signaling system is also noted in diabetes, cancer, cell proliferation, alcoholism and aging. Elucidation of neurotransmitters receptor interaction pathways and gene expression regulation by second messengers and transcriptional factors in health and disease conditions can lead to new small molecules for development of therapeutic agents to improve neurological disease conditions. Increased awareness of the global effects of neurological disorders should help health care planners and the neurological community set appropriate priorities in research, prevention, and management of these diseases.

Although in developing countries an apolipoprotein E4 (apoE4) genotype may offer an evolutionary advantage, as it has been shown to offer protection against certain infectious disease, in Westernised societies it is associated with increased morbidity and mortality, and represents a significant risk factor for cardiovascular disease, late-onset Alzheimer's disease and other chronic disorders. ApoE is an important modulator of many stages of lipoprotein metabolism and traditionally the increased risk was attributed to higher lipid levels in E4 carriers. However, more recent evidence demonstrates the multifunctional nature of the apoE protein and the fact that the impact of genotype on disease risk may be in large part due to an impact on oxidative status or the immunomodulatory/anti-inflammatory properties of apoE. An increasing number of studies in cell lines, targeted replacement rodents and human volunteers indicate higher oxidative stress and a more pro-inflammatory state associated with the F,4 allele. The impact of genotype on the antioxidant and immunomodulatory/anti-inflammatory properties of apoE is the focus of the current review. Furthermore, current information on the impact of environment (diet, exercise, smoking status, alcohol) on apoE genotype-phenotype associations are discussed with a view to identifying particular lifestyle strategies that could be adapted to counteract the 'at-risk' E4 genotype.

Nicotinic acetylcholine receptors (nAChRs) were studied in detail in the past regarding their interaction with therapeutic and drug addiction related compounds. Using fast kinetic whole-cell recording, we have now studied effects of tacrine, an agent used clinically to treat Alzheimer`s disease, on currents elicited by activation of rat alpha(3)beta(4) nAChR heterologously expressed in KX alpha(3)beta(4)R2 cells. Characterization of receptor activation by nicotine used as agonist revealed a K(d) of 23 +/- 0.2 mu M and 4.3 +/- 1.3 for the channel opening equilibrium constant, Phi(-1). Experiments were performed to investigate whether tacrine is able to activate the alpha(3)beta(4) nAChR. Tacrine did not activate whole-cell currents in KX alpha(3)beta(4)R2 cells but inhibited receptor activity at submicromolar concentration. Dose response curves obtained with increasing agonist or inhibitor concentration revealed competitive inhibition of nAChRs by tacrine, with an apparent inhibition constant, K(I), of 0.8 mu M. The increase of Phi(-1) in the presence of tacrine suggests that the drug stabilizes a nonconducting open channel form of the receptor. Binding studies with TCP and MK-801 ruled out tacrine binding to common allosteric sites of the receptor. Our study suggests a novel mechanism for action of tacrine on nAChRs besides inhibition of acetylcholine esterase.

Parkinson's disease (PD) is the second most common neurodegenerative disorder (after Alzheimer's disease) and directly affects upto 5 million people worldwide. The stages (Hoehn and Yaar) of disease has been predicted by many methods which will be helpful for the doctors to give the dosage according to it. So these methods were brought up based on the data set which includes about seventy patients at nine clinics in Sweden. The purpose of the work is to analyze unsupervised technique with supervised neural network techniques in order to make sure the collected data sets are reliable to make decisions. The data which is available was preprocessed before calculating the features of it. One of the complex and efficient feature called wavelets has been calculated to present the data set to the network. The dimension of the final feature set has been reduced using principle component analysis. For unsupervised learning k-means gives the closer result around 76% while comparing with supervised techniques. Back propagation and J4 has been used as supervised model to classify the stages of Parkinson's disease where back propagation gives the variance percentage of 76-82%. The results of both these models have been analyzed. This proves that the data which are collected are reliable to predict the disease stages in Parkinson's disease.

O gene da apolipoproteina E (APOE) possui três alelos com freqüências polimórficas. Esta apolipoproteína possui um importante papel no metabolismo de lipídeos, crescimento e regeneração neuronal, e parece estar relacionada com a doença de Alzheimer. No entanto, a magnitude destas influências difere de acordo com a população estudada, sugerindo uma interação genótipo/ambiente. No presente trabalho, foram estudadas seis tribos indígenas sul-americanas (n=186), 100 negróides e 466 caucasóides de Porto Alegre. Destes últimos, 343 foram investigados quanto à associação com níveis lipídicos e 23 quanto à associação com doença de Alzheimer. Todas as amostras foram amplificadas pela reação em cadeia da polimerase (PCR) e clivadas com a enzima de restrição Hha I. Os genótipos foram identificados após separação dos fragmentos de restrição por eletroforese em gel de agarose a 4% corado com brometo de etídeo. O presente estudo teve os seguintes objetivos específicos: 1)Determinar as freqüências gênicas e genotípicas da APOE nas populações negróides e caucasóides de Porto Alegre e de seis tribos indígenas da América do Sul; 2)Verificar se as associações entre os alelos da APOE e lipídeos séricos descritas em caucasóides também ocorrem em populações indígenas brasileiras; 3)Investigar a influência do polimorfismo do gene APOE em pacientes com hipercolesterolemia e hipertrigliceridemia, bem como em indivíduos normais da população de Porto Alegre e 4)Determinar a distribuição dos alelos da APOE em uma amostra de pacientes com Doença de Alzheimer.

The aim of the present study was to understand the feelings and the difficulties faced by the family caregiver in the care of the person affected by Alzheimer`s Disease (AD). It is a descriptive, exploratory study with a qualitative approach, using the oral life history proposed by Bom Meihy as the method. Data collection was conducted in the Basic Health Unit of Candelaria, located in Natal -RN, with five collaborators that carry out the role of family caregivers for people affected by Alzheimer`s disease (AD) and are members of the Group "Caring for those who Care". Caregi vers who resided with the affected family member for at least one year were selected for the study, and as a collection tool, it was opted to use semi-structured interviews via a script of open questions, recorded by permission of the collaborators, then t ranscribed and subsequently returned to respondents for checking the contents described. To analyze the results, the collaborators narrative technique was used in conjuction with the specific literature on the subject.The discussions were organized around five themes inherent to the guiding questions, and defined as follows: the incorporation of the role of the family caregiver; life before and after assuming the role of caregiver, the caregiver`s feelings and attitudes after assuming the care, difficulti es in caring, participation of the group as a foundation for caregivers. The stories showed many difficulties in the daily routine of the caregivers, and also that their participation in the group "Caring for those who Care" helps them in maintaining the q uality of their lives. The results open possibilities for the construction of new forms of approach and care for the people who fulfill the role of family caregiver contributing to strengthening of subsidies that help them better face the daily difficulti es.This study helped shed light on the fact that being a family caregiver of a person affected by AD is a suffered, exhausting and stressful condition involving much self-denial in one´s life. The situation experienced by these collaborators is considered a public health issue, and thus highlights the urgency for governmental political -social actions, besides the programs of care and health promotion for this target group.

The Brazilian Constitution maintains that care for elderly people is a responsibility shared by the state, the family and the society. The politics for the elderly corroborate this understanding and treats home as a privileged place for elderly care taking. This determines the participation of the familiar as a caregiver, but highlights the lack of strategic assistance for the needs of the relative caregiver who feels helplessly and unattended in their responsibility for elderly homecare. In recent years , despite the recently pursuit for health and life quality, there is an increasing incidence of elderly patients with dementia diseases that lead to disability, the most common among then is the Alzheimer´s disease. This disease affects seriously and irreversibly cognition, memory and independence of the elderly, making it dependent on others to perform basic activities of daily life, for all his life. The present study aims to evaluate the perceptions and feelings of family caregivers of elders with Alzheimer on the role of caregiver. This is a qualitative study conducted with family caregivers of seniors with Alzheimer´s, caregivers linked to the group of the Specialized Care Center of the Elderly´s Health, located in Natal / RN. Through semi-structured interview research sought to investigate the perceptions of family caregivers on the role of caregiver, the feelings and the changes in the caregiver´s life since they assumed this role. The data were organized into categories and units of semantic analysis and analyzed using thematic content analysis by Bardin. The reports originated three categories: the perception of the role of caregiver, feelings related to the caregving and consequences of the caregiver role. Perceptions of caregivers of elderly from the requirement of dedication to the care generates losses in personal and professional life for the familiar who assumes this responsibility. The lack of family and social support, aggravates the burden of care for the dependent elderly. Public health politics for the elderly recognize the importance and needs of family caregivers, but not enough to provide support and meet the needs and assist them in supporting their limitations. The research results show the urgent need to take measures to assist the caregivers of seniors with Alzheimer, recognizing them as an action of promotion quality of life and health of the elderly and protection the health of the caregiver
